Evaluer la capacité à maintenir le succès virologique, c’est à dire l’absence d’échec virologique défini par la survenue de deux valeurs successives de charge virale (CV) > 400 copies/ml, d’une stratégie de monothérapie d’IP boosté avec le darunavir versus le maintien d’une trithérapie comprenant 2 INTI + darunavir/r, chez des patients infectés par le VIH-1, ayant une charge virale indétectable de façon prolongée |

- Comparer entre les 2 groupes :La proportion de patients ayant une CV< 50 cp/ml, entre 50 et 400 cp/ml et > 400 cp/ml ; le délai d’apparition d’échec viro ; le profil des mutations de résistances apparues au cours de l’essai ; la proportion de patients arrêtant la stratégie attribuée et le délai de cet arrêt ; l’évolution des lymphocytes CD4 ; l’incidence des effets indésirables ; l’évolution du métabolisme glucido-lipidique ; l’évaluation de la CV au sein du compartiment génital masculin (40 patients) ; l’évolution de la répartition des graisses (120 patients) ; l’évolution de l’observance et des symptômes ressentis par autoquestionnaires. - La proportion de patients ayant un ARN VIH < 50 cp/ml après réintroduction des INTI en cas d’échec virologique dans le bras monothérapie d’IP. - La recherche de facteurs prédictifs du taux de succès.

Phase I : Patient(e) VIH + ; >ou=18 ans ; trt ARV >ou=18 mois, comportant 2INTI+IP ou 2INTI+INNTI ou 3 INTI ; CV < 400 cp/ml, >ou=18 mois avant l’entrée dans l’étude avec au moins 4 CV < 400 cp/ml ; absence d’antécédent d’échec viro sous IP défini par 2 CV > 1000 cp/ml ; CV documentée avant mise sous trt ARV ; nadir des CD4 > 100/mm3 ; CV < 50 cp/ml à l’entrée dans l’étude ; CD4 > 200/mm3 >ou=6 mois ; patient n’ayant jamais reçu de darunavir ; béta HCG pour la femme en âge de procréer et utilisation d’une contraception mécanique lors des rapports sexuels ; affilié ou bénéficiaire d’un régime de sécurité sociale ; consentement signé Phase II :CV< 50 cp/ml à S-4, absence d’intolérance au darunavir/r pendant les 4 premières semaines d’administration

Phase I : Patient infecté par VIH2, ayant eu un évènement classant SIDA, nadir des CD4 inf ou égal à 100/mm3, CV non documentée avant mise sous traitement ARV, antécédents d’atteintes neurologiques liées à l’infection VIH, hépatite virale B chronique, C chronique nécessitant une mise au traitement spécifique, échec viro défini par 2 CV > 1000 cp/ml sous IP, suivi irrégulier pb observance >ou=18 mois précédant l’étude, trt par interféron, interleukine, ou toute autre immunothérapie, chimiothérapie en cours, trt d’attaque ou prophylaxie pour infection opportuniste ou tuberculose, toute condition susceptible de compromettre la tolérance du trt et/ou l’observance du patient, trts associés interagissant avec les cytochromes hépatiques. Phase II : intolérance au TMC114/r, survenue d’un événement opportuniste ou infectieux majeur, d’une complication clinique majeure ou tout événement nécessitant la suppression d’un traitement par IP.
