    E.2.1Main objective of the trial
    Comparer la survie globale chez les patients porteurs d’une carcinose péritonéale d’origine colorectale, ayant fait l’objet d’une résection chirurgicale complète macroscopiquement ou suboptimale (laissant en place un tissu tumoral résiduel ≤ 1mm) et bénéficiant d’un traitement maximal avec chimiohyperthermie (c’est-à-dire chirurgie maximale + chimiohyperthermie + chimiothérapie systémique pré ou postopératoire) ou d’un traitement maximal sans chimiohyperthermie (c’est-à-dire chirurgie maximale + chimiothérapie systémique pré ou postopératoire).

    -Etude sur la nutrition et chirurgie digestive : les objectifs sont :
    -Evaluation nutritionnelle des patients pris en charge pour une carcinose péritonéale avant
    et après la chirurgie à 3 mois,
    - Corrélation de l’état nutritionnel et des complications,
    - Mesurer la morbidité plus spécifiquement,
    - Etudier le devenir des patients.

    E.3Principal inclusion criteria
    1) Cancer colorectal histologiquement prouvé
    2) Carcinose péritonéale d’importance minime ou modérée avec une score d’extension péritonéal de Sugarbaker ≤ à 25 (annexe 9), (évaluation peropératoire)
    3) Obtention d’une réduction tumorale chirurgicale macroscopiquement complète R1 ou d’épaisseur résiduelle ne dépassant pas 1 mm (R2), (évaluation peropératoire)
    4) Absence de métastases extra péritonéales y compris les métastases hépatiques et pulmonaires (si possible confirmée par la pratique d’une TEP)
    5) Patient devant être traité pour sa carcinose par une chimiothérapie systémique
    6) 18 ≤ âge ≤ 70 ans
    7) Performance Status OMS ≤ 1
    8) Espérance de vie > 12 semaines
    9) Fonction hématologique : PNN ≥ 1,5x109/L, plaquettes ≥ 100x109/L
    10) Fonction hépatique : Bilirubine totale ≤1,5 x la limite supérieure de la normale, ASAT (SGOT) et ALAT (SGPT) ≤ 3 x la limite supérieure de la normale, Phosphatases alcalines ≤ 3 x la limite supérieure de la normale
    11) Créatinine plasmatique ≤ 1,25 x la limite supérieure de la normale
    12) Patients opérables
    13) Neuropathie périphérique de grade ≤ 2 (CTC AE v3.0 annexe 7)
    14) Information du patient et signature du consentement éclairé obtenu avant le début de toute procédure spécifique à l’étude
    15) Affiliation à un régime de sécurité sociale.
    E.4Principal exclusion criteria
    1) Les carcinoses d’autre origine que colorectale, en particulier les carcinomes appendiculaires
    2) Les carcinoses de volume important, avec une score d’extension péritonéal de Sugarbaker >25 (annexe 9), (évaluation peropératoire)
    3) Patient ayant déjà été traité par chimiohyperthermie
    4) Antécédents de cancer (sauf un carcinome baso-cellulaire cutané ou un carcinome in situ du col utérin) ayant récidivé dans les 5 années précédant l'entrée dans l'essai,
    5) Patient déjà inclus dans un autre essai thérapeutique de première ligne pour la maladie étudiée,
    6) Femme enceinte, susceptible de l’être ou en cours d'allaitement,
    7) Personnes privées de liberté ou sous tutelle,
    8) Impossibilité de se soumettre au suivi médical de l'essai pour des raisons géographiques, sociales ou psychiques.

    Le critère principal d'évaluation sera la prise en compte de la survie globale. La survie médiane est estimée à 30 mois dans le groupe contrôle. L’objectif est d’augmenter la survie médiane pour atteindre 48 mois par le traitement expérimental CHIP (hasard ratio = 0,625).
