    E.2.1Main objective of the trial
    El objetivo principal de este ensayo es comparar la eficacia broncodilatadora de 3 dosis (50 mcg, 100 mcg and 200 mcg) de BEA 2180 liberadas con el inhalador Respimat una vez al día con placebo y con bromuro de tiotropio liberado también con el inhalador Respimat en pacientes con EPOC.

    E.3Principal inclusion criteria
    1. Todos los pacientes deben firmar un consentimiento informado de acuerdo a las normativas ICH-BPC antes de participar en el ensayo, que incluya lavados de medicación y otras restricciones.
    2. Todos los pacientes deben haber sido diagnosticados de EPOC (P95 4381) y deben cumplir los siguientes criterios espirométricos:
    Los pacientes deben presentar una obstrucción de las vías respiratorias de moderada a grave relativamente estable con un FEV1 ≤70% de la FVC en las pruebas de Función Pulmonar de Visita 1 (selección)
    3. Hombres y Mujeres de 40 años o más
    4. Fumadores o ex-fumadores con un historial de fumador > 10 paquetes-año
    5. Los pacientes deben ser capaces de realizar Pruebas de Función Pulmonar y mediciones de flujo espiratorio máximo técnicamente aceptables así como de anotar correctamente el diario del paciente durante el periodo de estudio como requiere el protocolo.
    6. Los pacientes deben ser capaces de inhalar la medicación correctamente con el inhalador Respimat® (Apéndice I)
    E.4Principal exclusion criteria
    1. Pacientes con otra enfermedad significativa aparte de la EPOC; una enfermedad significativa se define como una enfermedad que, en opinión del investigador, puede hacer que la participación en el estudio constituya un riesgo para el paciente, influir en los resultados del estudio, o plantear dudas acerca de la capacidad del paciente para participar en el estudio.
    2. Pacientes que presenten resultados anormales de laboratorio en visita 1 clínicamente relevantes, si esta alteración define una enfermedad significativa tal como se explica en el criterio anterior.
    3. Pacientes con antecedentes recientes (un año o menos) de infarto de miocardio.
    4. Pacientes con cualquier arritmia cardiaca inestable o potencialmente mortal o pacientes que hayan sido hospitalizados por tales acontecimiento durante el pasado año.
    5. Pacientes que hayan sido hospitalizados por fallo cardiaco en los últimos 3 años.
    6. Pacientes con cáncer que hayan requerido resección, radioterapia o quimioterapia durante los últimos 5 años. Se permite la inclusión de pacientes con carcinoma basocelular tratado.
    7. Pacientes con hiperplasia prostática benigna sintomática conocida u obstrucción del cuello vesical acordes con el criterio de exclusión nº 1.
    8. Pacientes con glaucoma de ángulo estrecho conocido.
    9. Pacientes con asma o antecedentes de asma.
    10. Pacientes que usen regularmente oxigenoterapia diurna y en opinión del investigador no puedan prescindir de ella durante los días de pruebas.
    11. Pacientes que han sido tratados con medicamentos beta-bloqueantes no cardioselectivos (p. ej. propanolol) durante los 30 días antes de Visita 1.
    12. Pacientes que hayan tomado Spiriva® en los 30 días previos a Visita 1.
    13. Pacientes que usen corticoesteroides orales a dosis inestables (es decir, menos de 6 semanas con una dosis estable).
    14. Pacientes que hayan participado previamente en este estudio (que hayan recibido tratamiento aleatorizado).

    E.5.1Primary end point(s)
    El criterio principal de valoración en este estudio es la respuesta del FEV1 trough (valle) después de 24 semanas.
    El FEV1 trough (valle) se define como la media de dos mediciones del FEV1 registradas antes de tomar la dosis (-40 y -15 minutos) al final del intervalo de administración de 24 horas después de la toma del fármaco con el inhalador Respimat®. La respuesta del FEV1 trough (valle) se define como el cambio respecto al nivel basal en el FEV1 trough (valle). El FEV1 basal son los valores del FEV1 previos al tratamiento medidos en la visita 2 (día de prueba de función pulmonar 1) antes de la administración de la primera dosis de medicación del estudio. El valor basal será la media de las pruebas de función pulmonar (PFP) realizadas -40 y -15 minutos en la visita 2.
