    E.2 Objective of the trial
    E.2.1Main objective of the trial
    The main hypothesis for the study is that prophylactic use of antibiotics (an "act first" approach) in acute stroke patients with swallowing problems on a bedside clinical assessment will be better than the current practice of monitoring for infection and treatment if necessary (a "wait and watch" approach) in reducing chest infections and their consequences in stroke patients
    E.2.2Secondary objectives of the trial
    1. Evaluate the effectiveness of prophylactic antibiotics given for 7 days to acute (<48 hours from onset) stroke patients with clinical dysphagia in reducing the frequency of chest infections, days lost to rehabilitation, duration of hospital stay, mortality and hospital costs up to 3 months after stroke onset. 2. Monitor the frequency of the occurrence of antibiotic related adverse events, including development of C difficile diarrhoea and antibiotic resistant infections associated with prophylactic use of antibiotics. 3. Assess the impact of this approach on psychosocial outcomes such as mood measured by the Hospital Anxiety and Depression Scale and quality of life measured by using Euroqol (EQ-5D and EQ-VAS) at 3 months.

    1. Ischaemic or hemorrhagic stroke patients
    2.  48 hours of symptom onset
    3. Unable to take a normal diet or fluids because of
    – impaired consciousness levels.
    – failed the clinical bedside swallowing assessment performed by a trained assessor.

    1) age <18 years
    2) pre-existing swallowing problems
    3) evidence of infection or pyrexia at the time of admission
    4) allergy to penicillin’s, macrolides, or other antibiotics
    5) pregnant and lactating women
    6) antibiotic treatment within the week prior to inclusion
    7) lack of consent from the patient or next of kin
    8) Life expectancy of less than 14 days

    E.5.1Primary end point(s)
    Clinical and costs indicators will be used as co-primary outcome measures. The clinical primary outcome measure is the incidence of chest infections in the first 14 days after stroke onset. The primary cost outcome measure will be the total hospital costs (acute and rehabilitation) for the initial episode of care, calculated as a product of costs per unit of each type of care (standardised NHS tariff) and amount used, using methodology validated previously.

    Adverse events related to antibiotic use will be monitored during the first 14 days and include antibiotic related side-effects, new onset of MRSA infection and C. difficile diarrhoea.

    Secondary outcomes will be measured at 3 months. These include mortality, institutionalisation, total duration of initial hospital stay, incident chest infections between 15-90 days and readmissions to hospital. Patients centred outcomes that will be measured at this point include: measures of activity (Barthel Index), participation (Rankin Scale), mood (Hospital Anxiety and Depression Scale) and quality of life (Euroqol 5D and VAS).

    At 14 days:

    Adverse events related to antibiotic use will be monitored for 14 days or until discharge from hospital and include antibiotic related side-effects, new onset of MRSA infection, C. difficile diarrhoea, concordance with protocol treatment, discontinuation of prophylaxis or use of additional antibiotics.

    NIH Stroke Scale, Barthel Index, Rankin Scale and Quality of Life assessments will be undertaken at 14 days of at the time of discharge if this is less than 14 days.

    At 3 months:

    Secondary outcomes at 3 months include mortality, institutionalisation, total duration of initial hospital stay, incident chest infections between 15-90 days and readmissions to hospital.

    Patients centred outcomes that will be measured at this point include: measures of activity (Barthel Index), participation (Rankin Scale), mood (Hospital Anxiety and Depression Scale) and quality of life (Euroqol 5D and VAS).

    The change in Barthel Index per week of hospital stay will be used to assess the rate of functional recovery in the hospital. The number of days of participation in programmed assessment or therapy activities, defined as sessions lasting for 30 minutes or more under the professional supervision of any therapy discipline, will be recorded. The time spent in each session will also be recorded.

    Secondary economic outcome measures will include incremental cost-effectiveness ratios (ICERs) and cost per quality-adjusted life-year (QALY) gained by the intervention.
