    E.2.1Main objective of the trial
    Démontrer l'équivalence en terme de niveau d'activité inflammatoire de la PR sur 18 mois, de deux stratégies thérapeutiques chez des patients atteints de PR, traités par anti-TNF et en rémission ou en faible niveau d'activité inflammatoire depuis au moins six mois, évalué par le DAS 28:
    - Maintien du traitement à l'identique (Bras M)
    - Versus Décroissance progressive du traitement anti-TNF par espacement progressif des injections (Bras D)
    E.2.2Secondary objectives of the trial
    1- Comparer ces deux stratégies thérapeutiques en terme médico-économiques. Le critère de jugement sera le rapport coût-efficacité incrémentiel.
    2- Démontrer l'équivalence de ces deux stratégies en terme de:
    - Activité de la maladie mesurée par le DAS 44
    - Survie sans rechute à 18 mois
    - Incapacité fonctionnelle selon l'indice HAQ à 18 mois et sur 18 mois
    - Qualité de vie selon le SF-36 à 18 mois et sur la période des 18 mois
    3- Décrire la tolérence des traitements dans les 2 groupes, en particulier en terme de pathologies infectieuses.
    4- Vérifier que la progression structurale à 18 mois (score de Sharp modifié par van dre Heijde - SHS) n'est pas supérieure dans le groupe (D) que dans le groupe (M).
    5- Identifier des facteurs prédictifs de maintien de la rémission lors de la décroissance thérapeutique.
    6- Décrire la réponse thérapeutique à la réascension si une rechute est observée lors de la décroissance dans le groupe D.

    E.3Principal inclusion criteria
    - Age supérieur ou égal à 18 ans,
    - Polyarthrite rhumatoïde (PR) selon les critères ACR 1987 modifiés,
    - Traitement de la PR par anti-TNF en infection sous-cutané de type adalimumab ou etanercept depuis plus d'un an, à posologie usuelle (celles correspondant au libellé d'AMM) et stable depuis au moins 1 an. Si un traitement de fond conventionnel est associé à l'anti-TNF, traitement de type méthotexate ou leflonominde à posologie stabledepuis au moins 6 mois.
    - PR en rémission, cad DAS28 inférieur ou égal à 2,6 depuis au moins 6 mois,
    - Absence de progression radiographique dans l'année précédent l'inclusion dans l'étude basée sur l'avis de rhumatologue traitant,
    - Réalisation d'un examen médical préalable,
    - Consentement éclairé et écrit,
    - Capacité de se conformer aux recommandations du protocole.

    - Progression radiographique dans l'année précédant l'inclusion dans l'étude, selon l'avis du rhumatologue traitant,
    - Traitement par prednisone, prednisolone ou methylprednisolone,
    - Grossesse en cours,
    - Femme en âge de procréer sans contraception efficace,
    - Pathologie cancéreuse ou toute autre pathologir nécessitant un traitement pouvant interférer sur l'évolution de la PR,
    - Antécédent de la maladie inflammatoire aiguë dont le diagnostic est autre que celui de la polyarthrite rhumatoïde, par exemple, connectivites mixtes, lupus érythémateux systmique,
    - Patient devant subir une intervention chirurgicale pendant la durée de l'étude,
    - Non affiliation à un régime de sécurité sociale (bénéficiare ou ayant droit).



    progression radiographique durant l'année ^précédant l'inclusion dans l'étude ; contre indication au traitement anti-TNF; intervention chirurgicale programmée ans les 18 mois à venir; incapacité à comprendre la langue française, absence de consentement éclairé signé.

    Evolution de l'activité inflammatoire de la PR évaluée par DAS 28 en mesures répétées tous les 3 mois sur 18 mois, à l'aide d'une calculatrice spécifique et reposant sur:
    - le compte des articulations douloureuses à la pression sur sites différents,
    - le compte des articulations gonflées (synovites) sur les mêmes sites,
    - la vitesse de sédimentation,
    - l'évaluation de l'activité globale de la PR par le patient sur EVA

    Secondaires:
    1) Rapport coût-efficacité incrémentiel, c'est à dire Delta (coût maintien - coût décroissance) / Delta (Efficacité maintien - Efficacité Décroissance)
    2) - Activité de la maladie mesurée par le DAS 44
    - Rechute pendant ls 18 mois de suivi
    - Incapacité fonctionnelle selon l'indice HAQ
    - Qualité de vie selon le SF-36 à 18 mois et sur la période des 18 mois
