    Definir la seguridad y la eficacia de pregabalina en sujetos con TAG en dosis bajas y altas con respecto a placebo y lorazepam después de 3 y 6 meses de tratamiento

    E.3Principal inclusion criteria
    1. Varones y mujeres de 18 a 65 años de edad. Las mujeres en edad fértil deberán emplear un método fiable de control de la natalidad durante el período de estudio.
    2. Diagnóstico primario de TAG según el DSM IV (DSM IV, 300.02)8 según lo determinado por un psiquiatra o psicólogo clínico autorizado y confirmado mediante la entrevista estructurada MINI (Entrevista neuropsiquiátrica internacional abreviada)9.
    3. Puntuación HAM A total >= 14 y apartado nº 1 de la HAM D con una puntuación de estado de ánimo deprimido <= 2 en las visitas de selección (visita 1) y basal (visita 2) para garantizar un predominio de los síntomas de ansiedad sobre los de depresión.
    4. Sujetos que precisan tratamiento farmacológico por TAG.
    5. Capacidad de entender y cooperar con los procedimientos del estudio y de otorgar un consentimiento informado.
    6. Prueba de un documento de consentimiento informado, firmado y fechado personalmente, en el que se indique que el paciente (o su representante legal) ha sido informado de todos los aspectos pertinentes del estudio.
    7. Sujetos dispuestos y capacitados para cumplir con las visitas programadas, el plan de tratamiento, los análisis clínicos y otros procedimientos del estudio
    E.4Principal exclusion criteria
    1. Diagnóstico activo o pasado de cualquier otro trastornos en el eje I del DSM IV (incluido un trastorno disfórico premenstrual activo) con las excepciones siguientes:
    • Depresión activa o pasada no especificada de otro modo, fobia específica o trastorno de somatización y
    • Antecedentes de un trastorno depresivo mayor, fobia social, trastorno de angustia o trastorno de los hábitos alimentarios.
    2. Abuso o dependencia de sustancias psicoactivas del DSM IV (salvo nicotina y cafeína) durante los 6 últimos meses.
    3. Presencia de trastornos de personalidad limítrofe y antisocial coexistentes, basándose en los criterios DSM IV según el criterio clínico del investigador.
    4. Riesgo de suicidio según los antecedentes de los 2 últimos años, notificación del propio sujeto o consideración clínica de que existe un riesgo grave de suicidio u homicidio.
    5. Interrupción o inicio de psicoterapia por TAG en el período 2 ó 3. Se permitirá participar en el estudio a los sujetos con psicoterapia de intensidad estable durante al menos 6 semanas o más.
    6. Uso diario de benzodiacepinas para tratar el TAG (>= 5 días/semana) durante los 3 meses anteriores a la selección. Se permitirá el uso ocasional de benzodiacepinas para tratar el insomnio, pero no durante las 2 semanas anteriores al período basal.
    7. Uso activo de psicofármacos que no pueden interrumpirse 2 semanas antes del período basal (5 semanas en el caso de fluoxetina).
    8. Antecedentes de un fracaso del tratamiento con lorazepam según lo determinado por el criterio clínico del investigador y teniendo en cuenta la idoneidad de la dosis y la duración del tratamiento.
    9. Sensibilidad conocida a lorazepam u otras benzodiacepinas.
    10. Exposición previa a pregabalina.
    11. Prueba positiva de detección de los siguientes fármacos y drogas en orina en la visita de selección: anfetaminas, barbitúricos, benzodiazepinas, narcóticos, sedantes e hipnóticos, cocaína, fenciclidina, cannabinoides u otras drogas ilegales o ilícitas.
    12. Prueba de alcoholemia positiva en la visita de selección.
    13. Cualquier trastorno hematológico, autoinmunitario, endocrino, cardiovascular, renal, hepático, digestivo o neurológico con importancia clínica, grave o inestable, entre los que figuran, a título de ejemplo, los siguientes:
    • Trastorno convulsivo de cualquier tipo.
    • Hipotiroidismo o hipertiroidismo no corregido. Los sujetos deberán recibir un tratamiento de reposición tiroidea estable por hipotiroidismo durante al menos 6 semanas antes de la selección.
    • Antecedentes de neoplasias potencialmente mortales tratadas durante los 5 últimos años, con la excepción de un carcinoma in situ del cuello uterino o carcinoma basocelular de la piel.
    • Antecedentes de migraña, con episodios que aparecen más de una vez al mes.
    • Glaucoma de ángulo estrecho.
    14. Concentración de alanina aminotransferasa (ALT) o aspartato aminotransferasa (AST) > 3 veces el límite superior de la normalidad.
    15. Recuento de plaquetas < 100.000/mm3.
    16. Natremia > 150 o < 130 mEq/l.
    17. Aclaramiento de creatinina (CLcr) < 45 ml/min.
    18. Anomalías en el electrocardiograma (ECG) con importancia clínica.
    19. Embarazo o lactancia.
    20. Participación en un estudio de un fármaco experimental durante los 30 últimos días.
    21. Cualquier otro trastorno médico o psiquiátrico grave, agudo o crónico, o cualquier alteración analítica que pueda aumentar el riesgo asociado a la participación en el estudio o a la administración del producto en investigación o interferir en la interpretación de los resultados del estudio y, en opinión del investigador o promotor, impedir la participación en este estudio.

    Criterios de valoración de la eficacia:

    • Escala de evaluación de la ansiedad de Hamilton (HAM A total): escala de evaluación de 14 preguntas que evalúa los síntomas de ansiedad y que se medirá en el período basal (antes del tratamiento), así como 3 y 6 meses después del tratamiento.
    • Impresión clínica global intensidad y mejoría (CGI S y CGI I): se obtendrá en los mismos momentos que la escala HAM A.

    Criterios de valoración de la seguridad:

    • Se vigilarán los acontecimientos adversos notificados espontáneamente durante todo el ensayo. Los acontecimientos adversos se resumirán según el grupo de tratamiento después de 3 y 6 meses de tratamiento del TAG.
    • Retirada del fármaco y ansiedad de rebote:
    • Lista de verificación de los síntomas de abstinencia por el médico (PWC): cuestionario que recopila información para determinar si los sujetos experimentan síntomas de abstinencia tras retirar la medicación del estudio.
    • Ansiedad de rebote: se utilizará la escala HAM A para determinar la ansiedad de rebote, definida como una reaparición rápida de los síntomas originales del sujeto después de retirar el fármaco, que son peores en comparación con el período basal.
    • Los signos y síntomas de aparición con la retirada (SSAR) asociados a la retirada del fármaco se determinarán a los 3 y 6 meses: los SSAR se determinarán a partir de los acontecimientos adversos de aparición con el tratamiento y notificados de manera espontánea que surjan de novo durante la reducción gradual o bien que ya existan, pero que empeoren durante el período de reducción gradual.
