The primary objective of this study is to assess the efficacy and safety of 52 weeks once daily treatment with orally inhaled tiotropium + olodaterol fixed dose combination ((2.5 µg / 5 µg; 5 µg / 5 µg) (delivered by the Respimat® Inhaler) compared with the individual components (2.5 and 5 ?g tiotropium, 5 µg olodaterol) (delivered by the Respimat® Inhaler) in patients with Chronic Obstructive Pulmonary Disease (COPD). |
El objetivo principal de este estudio es evaluar la eficacia y seguridad de 52 semanas de tratamiento oral inhalado una vez al día con tiotropio + olodaterol FDC (2,5 µg / 5 µg; 5 µg / 5 µg) (administrado con el Inhalador RESPIMAT) en comparación con sus componentes individuales (2,5 μg y 5 μg de tiotropio, 5 µg de olodaterol) (administrados con el Inhalador RESPIMAT) en pacientes con EPOC. |

Pharmacogenomic evaluation: Pharmacogenetics investigates genetic variations in patients to explain and to predict their individual response to drugs. Participation in the Pharmacogenetics part is voluntary and not a prerequisite for participation in the study.
24-Hour Holter monitoring: In a subset of patients (75 patients per treatment group; pooling with data collected in replicate study will provide 150 patients per treatment group) at selected sites, Holter
monitoring will be performed over a 24 hour period prior to the randomisation visit (Visit 2). Holter monitoring over a period of 24 hours will be repeated after completion of all study related tests at Visit 5. |

. All patients must sign an informed consent consistent with ICH-GCP guidelines prior to participation in the trial, which includes medication washout and restrictions.
2. All patients must have a diagnosis of chronic obstructive pulmonary diseaseand must meet the following spirometric criteria:
Patients must have relatively stable airway obstruction with a post-bronchodilator FEV1< 80% of predicted normal and a post-bronchodilator FEV1/FVC <70% at Visit 1
3. Male or female patients, 40 years of age or older.
4.Patients must be current or ex-smokers with a smoking history of more than 10 pack years. |

1.Patients with a significant disease other than COPD;
2.Patients with a, in the opinion of the investigator, clinically relevant abnormal baseline haematology, blood chemistry, or urinalysis;
3.Patients with a history of asthma.
Patients with any of the following conditions:
4.A diagnosis of thyrotoxicosis (due to the known class side effect profile of ß2- agonists).
5.A diagnosis of (>100 beats per minute) (due to the known class side effect profile of ß2-agonists).
6.A history of myocardial infarction within 1 year of screening visit (Visit 1).
7.Unstable or life-threatening cardiac arrhythmia.
8.Hospitalization for heart failure within the past year.
9.Known active tuberculosis.
10.A malignancy for which patient has undergone resection, radiation therapy or chemotherapy within last five years (patients with treated basal cell carcinoma are allowed).
11.A history of life-threatening pulmonary obstruction.
12.A history of cystic fibrosis.
13.Clinically evident bronchiectasis.
14.A history of significant alcohol or drug abuse.
15.Patients who have undergone thoracotomy with pulmonary resection
16.Patients being treated with oral or patch ?-adrenergics.
17.Patients being treated with oral corticosteroid medication at unstable doses
18.Patients who regularly use daytime oxygen therapy for more than one hour per day and in the investigator?s opinion will be unable to abstain from the use of oxygen therapy during clinic visits.
19.Patients who have completed a pulmonary rehabilitation program in the six weeks prior to the screening visit (Visit 1) or patients who are currently in a pulmonary rehabilitation program.
20.Patients who have taken an investigational drug within one month or six half lives (whichever is greater) prior to screening visit (Visit 1).
21.Patients with known hypersensitivity to ?-adrenergic drugs, BAC, EDTA, or any other component of the RESPIMAT inhalation solution
22.Pregnant or nursing women.
23.Women of childbearing potential not using a highly effective method of birth control.
24.Patients who have previously been randomized in this study or are currently participating in another study.
25.Patients who are unable to comply with pulmonary medication restrictions prior to randomization. |

Diagnóstico de tirotoxicosis (debido al perfil conocido de efectos secundarios de clase de los agonistas ß2). 5. Diagnóstico de taquicardia paroxística (>100 latidos por minuto) (debido al perfil conocido de efectos secundarios de clase de los agonistas ß2). 6. Antecedentes de infarto de miocardio en el año anterior a la visita de selección (Visita 1). 7. Arritmia cardíaca inestable o potencialmente mortal 8. Hospitalización por insuficiencia cardíaca en el año anterior . 9. Tuberculosis activa conocida. 10. Un proceso neoplásico por el que el paciente haya sido sometido a algún tipo de resección, radioterapia o quimioterapia en los últimos cinco años (se permite la entrada en el estudio de pacientes con carcinoma de células basales ya tratado). 11. Antecedentes de obstrucción pulmonar potencialmente mortal. 12. Antecedentes de fibrosis quística. 13. Bronquiectasias clínicamente evidentes. 14. Antecedentes de abuso significativo de alcohol o drogas. 15. Pacientes que hayan sido sometidos a toracotomía con resección pulmonar ( 16. Pacientes que estén siendo tratados con β-adrenérgicos por vía oral o en parche. 17. Pacientes que estén siendo tratados con corticosteroides orales a dosis inestables 18. Pacientes que utilicen regularmente oxigenoterapia diurna durante más de una hora al día y que, en opinión del investigador, no sean capaces de abstenerse del uso de oxigenoterapia durante las visitas al centro. 19. Pacientes que hayan finalizado un programa de rehabilitación pulmonar en las seis semanas previas a la visita de selección (Visita 1) o aquellos que estén actualmente en un programa de rehabilitación pulmonar. 20. Pacientes que hayan tomado un fármaco en investigación un mes o seis semividas (lo que sea mayor) antes de la visita de selección (visita 1). 21. Pacientes con hipersensibilidad conocida a los fármacos β-adrenérgicos y/o anticolinérgicos, BAC, EDTA, o a cualquier otro componente de la solución RESPIMAT para inhalación. 22. Mujeres embarazadas o lactantes. 23. Mujeres en edad fértil que no estén utilizando un método anticonceptivo de alta eficacia. 24. Pacientes que hayan sido previamente aleatorizados en este estudio o estén actualmente participando en otro estudio. 25.
