    E.2.1Main objective of the trial
    Comparer dans les néphroses corticodépendantes de l'enfant, l'efficacité respective d'un traitement par du cyclophosphamide pendant 12 semaines et par du MMF pendant 18 mois sur la survenue de la première rechute de néphrose au cours des 24 mois suivant le début du traitement
    E.2.2Secondary objectives of the trial
    1) Evaluer le délai de la première rechute.
    2) Comparer la corticothérapie (dose cumulée et dose seuil nécessaire au maintien de la rémission) entre les 2 bras de traitement, de l'inclusion à la fin de l'étude.
    3) Evaluer l'influence de l'âge et de la corticodépendance sur l'efficacité de chaque traitement.
    4) Evaluer et comparer l'évolution auxologique des patients dans les 2 groupes.
    5) Etudier la pharmacocinétique du MMF chez les patients néphrotiques, étudier la relation entre l'aire sous la courbe et les échecs de traitement et valider la dose de 1200 mg/m²/jour chez les patients atteints de néphrose.
    6) Evaluer les effets secondaires de chaque produit dans cette indication

    E.3Principal inclusion criteria
    - Examen médical préalable du patient.
    - Pour les jeunes filles pubères, absence de grossesse (test de grossesse négatif)
    - Enfants âgés de 2 à 16 ans, atteints de néphrose corticodépendante requérant un traitement immunosuppresseur.
    - Survenue d'une rechute (définie par l'existence d'une protéinurie 0.25 g/mmol (ou > 2g/g) de créatininurie entraînant une hypoalbuminémie = 30 g/l) sous une dose de prednisone > 30 mg/m²/48h.
    - OU survenue de rechutes sous une dose de corticoïdes 15 mg/m²/48h et au moins une des complications de la corticothérapie :
    * vitesse de croissance < 4 cm/an ou < -1 DS (déviation standard), et/ou perte de 1 DS de taille ou plus sous corticothérapie discontinue.
    * poids 2 DS par rapport au poids théorique pour la taille sous corticothérapie discontinue.
    * complication osseuse de la corticothérapie.
    * survenue d'une complication grave (thrombose, collapsus, infection sévère) lors d'une rechute sous une corticothérapie 15 mg/m²/48h (0,5 mg/kg/48h).
    - Enfant dont le ou les titulaires de l'autorité parentale ont signé un consentement.éclairé
    - Enfant dont les titulaires de l'autorité parentale sont affiliés à un régime de sécurité sociale ou CMU (couverture médicale universelle)
    E.4Principal exclusion criteria
    - Patients atteints de néphrose initialement corticorésistante même celles devenues corticosensibles et corticodépendantes.
    - Patients atteints de néphrose partiellement corticosensible définie par la persistance d'une protéinurie résiduelle > 0,05 g/mmol de créatininurie à l'issue du traitement initial de la première poussée.
    - Traitement antérieur par cyclophosphamide, MMF ou cyclosporine.
    - Jeunes filles susceptibles de mettre en route une grossesse ou n'utilisant pas de contraception efficace.
    - Refus d'un ou des 2 titulaires de l'autorité parentale de signer le consentement éclairé.
    - Impossibilité de suivi du patient à pendant la durée de l'étude.
    - Allergie ou intolérance à l'un des composants du cyclophosphamide ou du MMF.
    - Infection sévère non contrôlée (septicémie, …)
    - Patient présentant une tumeur maligne.
    - Insuffisance hépatique ou rénale.
    - Patient présentant ou ayant présenté une insuffisance médullaire.
    - Infection urinaire aigue, cystite hémorragique préexistante
    - Patiente allaitante
    - Association avec des vaccins vivants (notamment contre la fièvre jaune)
    - Patients traités, au moment de l'inclusion, par les médicaments interdits dans le cadre du protocole :
    _Tout autre immunosuppresseur ou immunostimulant, en particulier, inhibiteur des calcineurines, lévamisole, inhibiteurs de la mTOR et anticorps poly - ou monoclonaux à visée immunitaire.
    _ Anti-inflammatoires non stéroïdiens
    _ Inhibiteurs de l'enzyme de conversion, en raison de leur action anti-protéinurique
    _ Antagoniste du récepteur AT1 de l'angiotensine II, en raison de leur action antiprotéinurique.
    - Patient participant à une autre recherche thérapeutique

    Critère d'évaluation principal
    Le critère principal sera la survenue d'une rechute de la néphrose définie par des critères biologiques objectifs (présence d'une protéinurie > 0.25 g/mmol de créatininurie (ou > 2g/g) et d'une hypo albuminémie < 30 g/l) au cours des 24 mois suivant le début du traitement de l'étude.

    Critères d'évaluation secondaires

    1. Le délai de la 1ère rechute après le début du traitement et le nombre de rechutes pendant les 2 ans de suivi de l'étude comparé au nombre de rechutes pendant l'année précédente.

    2. La dose de corticoïdes reçue :
    - Pour les patients ayant rechuté, la dose seuil de corticoïdes nécessaire à maintenir la rémission pendant l'étude comparée à la dose seuil de corticoïdes reçue au moment de la rechute motivant l'inclusion.
    - Pour tous, la dose cumulée/an de corticoïdes pendant les 2 ans suivant le début du traitement de l'étude comparée à celle de l'année avant traitement.

    3. Evolution des données auxologiques (poids, taille, IMC).
    Comparaison de la vitesse de croissance (nombre de cm pris par an) pendant l'année sous MMF/Cyclophosphamide, comparée à la vitesse de croissance pendant l'année avant traitement ainsi que la variation du score de DS de taille et de poids un an avant l'inclusion, lors de l'inclusion, puis tous les 6 mois pendant l'étude.
    Le stade pubertaire et l'âge osseux seront donc enregistrés pour les filles de plus de 9 ans et pour les garçons de plus 11 ans à M0, M12 et à M24.

    4. Pour les patients du groupe MMF, la détermination des paramètres pharmacocinétiques du MMF et l'étude de la relation entre l'exposition au MMF (mesurée par l'aire sous la courbe des concentrations plasmatiques en fonction du temps, à l'équilibre) et la réponse au traitement (échec de traitement défini par la survenue des rechutes).
