    Evaluar la eficacia de ambos regímenes en términos de tasa de Control de la Enfermedad.
    E.2.2Secondary objectives of the trial
    - Evaluar la tasa de respuesta en ambas ramas.
    - Evaluar la duración del control de la enfermedad, la duración de la respuesta, y el tiempo transcurrido hasta el fracaso del tratamiento (TTF) en ambas ramas.
    - Evaluar la supervivencia libre de progresión y la supervivencia global en ambas ramas.
    - Evaluar la tolerabilidad en ambas ramas.
    - Evaluar la QOL utilizando la LCSS en ambas ramas.
    - Evaluar el Cuestionario de Satisfacción en ambas ramas.
    - Evaluar el aspecto fármaco-económico en ambas ramas.

    E.3Principal inclusion criteria
    - Pacientes nunca antes expuestos a quimioterapia de 18 a 75 años.
    - Estado funcional KPS > 80 (apéndice II).
    - Cáncer de pulmón de células no pequeñas con confirmación histológica o citológica (se acepta la aspiración con aguja fina).
    - Estadio IIIB (con metástasis nodal supraclavicular o derrame pleural), estadio IV o recurrencia (local o distante) después de tratamiento local. Pacientes que no son adecuados para tratamiento loco-regional.
    - Esperanza de vida superior a las 12 semanas.
    - Funciones adecuadas de la médula ósea, hígado y riñón:
    o Neutrófilos > 2,0 x 109/ , plaquetas > 100 x 109/l, hemoglobina > 11 g/dl o 6,8 mmol/l.
    o Bilirrubina total &#61603;&#61472; 1,5 x LSN, transaminasas < 2,5 x LSN, fosfatasas alcalinas < 5 x LSN.
    o Creatinina < LSN (si está en el valor límite, aclaramiento de creatinina > 60 ml/min).
    - Terapia anterior:
    o Cirugía: los pacientes pueden haber estado sometidos a cirugía por CPCNP.
    o Quimioterapia: los pacientes no deben haber recibido quimioterapia sistémica ni inmunoterapia.
    o Terapia de radiación: el paciente es elegible si las lesiones diana están fuera del área irradiada.
    - Presencia de al menos una lesión indicadora medible (criterios RECIST. Versión 1.1) que no ha sido previamente. Las lesiones medibles (medidas en al menos una dimensión) (diámetro mayor a ser registrado) son &#8805; 20 mm con técnicas convencionales o &#8805; 10 mm con TAC helicoidal.
    El examen físico y la ecografía no se considerarán como evaluaciones objetivas del tumor.
    - El paciente debe otorgar su consentimiento por escrito (firmado y fechado por él mismo) antes de realizarse cualquier procedimiento relacionado con el estudio.
    - Ausencia de cualquier condición psicológica, familiar, sociológica o geográfica que potencialmente puedan obstaculizar el cumplimiento del protocolo del estudio y esquema de seguimiento; dichas condiciones deben evaluarse con el paciente antes de la inclusión en el estudio.
    - Las mujeres en edad fértil deben utilizar un método anticonceptivo médicamente aceptado (es decir, anticonceptivos orales, dispositivos intrauterinos) para evitar el embarazo durante los 2 meses anteriores al inicio del tratamiento del estudio, durante todo el período de duración del estudio y durante 3 meses después de la administración de la última dosis del tratamiento del estudio de manera que el riesgo de embarazo sea mínimo. Las mujeres en edad fértil deben tener resultado negativo de la prueba de embarazo realizada en suero u orina dentro de las 72 horas previas al inicio del tratamiento del estudio.
    - Los hombres en edad fértil deben utilizar un método anticonceptivo efectivo si sus parejas son mujeres en edad fértil durante el período del estudio y hasta 3 meses después de la administración de la última dosis del tratamiento.
    - El paciente debe tener acceso al sistema sanitario público de acuerdo con la legislación local.
    E.4Principal exclusion criteria
    - Una mujer no se podrá incluir en el estudio si:
    - Está embarazada o en período de lactancia.
    - El resultado de la prueba de embarazo es positivo en el momento de la inclusión.
    - Hipersensibilidad conocida al/los fármaco(s) del estudio o a fármacos con estructura química similar.
    - Tratamiento con algún fármaco en investigación dentro de los 30 días anteriores a la randomización.
    - Cualquier factor importante que probablemente modifique la absorción del fármaco, p.ej. cirugía del GIT, síndrome de malabsorción en grado significativo o enfermedad que afecte la función del tracto gastrointestinal.
    - Pacientes con recurrencia local, que no puede de ser tratada con terapia de radiación.
    - Radioterapia previa en el único sitio utilizado para evaluar la respuesta.
    - Radioterapia dentro de las 4 semanas previas.
    - Enfermedad cardiovascular, hepática, neurológica u otra enfermedad sistémica clínicamente relevante que dificulta la implementación del protocolo.
    o Algún trastorno activo del sistema nervioso central, metástasis cerebral o que afecta la zona leptomeníngea.
    o Neuropatía sintomática (sensitiva) > grado 1 según los Criterios de Toxicidades Comunes del NCI (NCI – CTC versión 2).
    o Trastorno médico concomitante/ incontrolado (insuficiencia cardíaca o infarto de miocardio dentro de los 3 meses previos, hipertensión incontrolada o arritmia, hipercalcemia incontrolada, infección activa que requiere de antibióticos i.v. dentro de las 2 semanas previas al inicio del tratamiento).
    o Pérdida de peso > 10% en de los 3 meses anteriores.
    o Síndrome de vena cava superior.
    o Terapia con oxígeno a largo plazo.
    o Derrame pleural preexistente que requiera toracocentesis
    o Ascitis o derrame pericárdico.
    - Antecedentes de otra malignidad en los últimos cinco años, a excepción del carcinoma de células basales de la piel o del carcinoma in situ de útero.
