    The EU Clinical Trials Register currently displays   43871   clinical trials with a EudraCT protocol, of which   7290   are clinical trials conducted with subjects less than 18 years old.   The register also displays information on   18700   older paediatric trials (in scope of Article 45 of the Paediatric Regulation (EC) No 1901/2006).

    E.2 Objective of the trial
    E.2.1Main objective of the trial
    Comparer la survie sans récidive à 3 ans après chirurgie en territoire irradié suivie de réirradiation
    - soit par radiothérapie monofractionnée avec 5FU et Hydoxycarbamide concomitants, réalisée une semaine sur deux (protocole de Vokes : bras de référence)
    - soit par radiothérapie hyperfractionnée avec Cetuximab concomitant, administrée de façon continue (bras expérimental)
    E.2.2Secondary objectives of the trial
    Evaluer dans les deux bras, à 3 ans de la randomisation la survie globale et le contrôle loco-régional.
    La toxicité aiguë (échelle NCI-CTCAE) sera évaluée pendant la réirradiation et à 3 mois de la fin de la réirradiation.
    La toxicité tardive sera évaluée selon l'échelle NCI-CTCAE. On étudiera en particulier la courbe actuarielle d'apparition des toxicités de grade 3 ou 4 dans les deux bras.
    Qualité de vie (questionnaire EQ-5D (score et échelle d’évaluation)) à 18 et 36 mois

    E.3Principal inclusion criteria
    •Récidive ou deuxième localisation des carcinomes des VADS survenant en territoire déjà irradié à une dose >= 50 Gys
    •Carcinome épidermoïde
    •Absence de séquelles grade III ou IV liées à la première irradiation (sauf séquelles salivaires)
    •Récidive ou deuxième localisation cliniquement infiltrante et/ou récidive ganglionnaire >= 3 cm et/ou association d'une récidive locale et ganglionnaire
    •Cavité buccale, pharynx, larynx (si rT4), région cervicale (si >3cm)
    •Absence de métastase à distance, confirmée par TDM thoracique, échographie abdominale (ou TDM) en cas d'anomalie de la fonction hépatique, et scintigraphie osseuse en cas de symptômes locaux.
    •Chirurgie en territoire irradié, ayant permis une exérèse macroscopiquement suffisante
    •Chirurgie comportant une protection vasculaire par un lambeau myo-cutané ou libre
    •Volume de recouvrement ≥50% du volume de la première irradiation
    •Délai > = 6 mois entre la fin de la première irradiation et la chirurgie en territoire irradié
    •Cicatrisation dans un délai permettant une réirradiation dans un délai de 8 semaines après la chirurgie en territoire irradié
    •Absence de toute participation à un essai thérapeutique dans les 30 jours précédant l’inclusion.
    •Âge compris entre 18 et 70 ans.
    •Echelle de karnofsky ≥80%
    •Fonction hématologique : neutrophiles * 2 x 106/l, plaquettes : * 100 x 106/l, hémoglobine : * 10 g/dl (ou 6,2 mmol/l)
    •Fonction hépatique : bilirubine totale normale ; ASAT (SGOT) et ALAT (SGPT) * 2.5 * la limite supérieure normale (LNS) de chaque centre ; phosphatases alcalines * 5 * LNS. Les patients avec des ASAT ou ALAT > 1.5 * LNS associés à des phosphatases alcalines * 2.5 * LNS ne seront pas éligibles pour l’essai.
    •Fonction rénale : créatinine sérique * 120 *mol/l (1.4 mg/dl) ; si la créatinine est > 120 *mol/l, la clairance de la créatinine doit être * 60 ml/min.
    •Patient ayant donné son consentement écrit
    E.4Principal exclusion criteria
    •Récidive superficielle et non associée à une récidive ganglionnaire, récidive ganglionnaire isolée et inférieure à 3 cm
    •Métastases à distance
    •dossier technique de la première irradiation inaccessible
    •Séquelles grade 3 ou 4 de la première irradiation (en dehors des séquelles salivaires)
    •Chirurgie macroscopiquement insuffisante
    •Retard de cicatrisation imposant un délai de plus de 8 semaines pour la réirradiation
    •Toxicité induite par une chimiothérapie reçue lors d’un traitement antérieur > au grade 3
    •Hypersensibilité au cetuximab
    •Pathologies médicales sérieuses concomitantes (liste non exhaustive)
    •Pathologie cardiaque instable malgré traitement.
    •Antécédents neurologiques ou psychiatriques tels que démence, convulsions.
    •Infection sévère non contrôlée.
    •Broncho-pneumopathie obstructive ayant nécessité une hospitalisation dans l'année précédant l’inclusion.
    •Présence, à la sélection, de facteurs d’ordre psychologique, familial, social ou géographique susceptibles de modifier l’observance du patient avec le protocole à l’étude et le suivi, constitue un critère de non-inclusion. Ces facteurs doivent être discutés avec le patient avant inclusion dans l’essai.
    •Femmes enceintes, allaitant ou en âge de procréer sans contraception efficace
