Detectar un aumento en la tasa de supervivencia libre de progresión (PFS*, ver capítulo de protocolo 7.3.6) en 1 año en cada grupo experimental (mFOLFOX6 + bevacizumab o panitumumab) comparado con el grupo mFOLFOX6 solo como tratamiento perioperatorio en metástasis hepática extirpable del cáncer colorrectal (CRC, por su sigla en inglés) tipo silvestre del homólogo del oncogén viral del sarcoma de rata de Kirsten (KRAS, por su sigla en inglés).
The primary objective is to detect an increase in progression free survival (PFS*, see chapter 7.3.6) rate at 1 year in each experimental arm (mFOLFOX6 + bevacizumab or panitumumab) compared to mFOLFOX6 alone arm as perioperative treatment for resectable liver metastasis from wild type Kirsten rat sarcoma viral oncogene homolog (KRAS) colorectal cancer (CRC). |

E.3 | Principal inclusion criteria |
• Cáncer colorrectal histológicamente comprobado con una a ocho metástasis hepáticas metacrónicas o sincrónicas consideradas totalmente extirpables.
• El tumor primario (o metástasis hepática) de cáncer colorrectal debe ser del “tipo silvestre” de KRAS.
• Los pacientes tienen que haber sido sometidos a la extirpación completa (R0) del tumor primario al menos 4 semanas antes de la asignación aleatoria. Para aquellos pacientes con metástasis sincrónica, el tumor primario puede haber sido extirpado (R0) al mismo tiempo que la metástasis hepática si: el paciente presenta un tumor primario no obstructivo y puede recibir quimioterapia preoperatoria (de 3 a 4 meses) antes de someterse a la cirugía.
• Enfermedad hepática apreciable mediante los Criterios de Evaluación de Respuesta en Tumores Sólidos
• Los pacientes deben ser menores de 18.
• Estado funcional según la Organización Mundial de la Salud (OMS) de 0 o 1.
• Se permite la quimioterapia adyuvante previa para CRC primario si la misma se completa al menos 12 meses antes de que el paciente sea incluido en este estudio.
• Los siguientes análisis se realizarán dentro de las 4 semanas previas a la asignación aleatoria del paciente al estudio:
• Recuento absoluto de neutrófilos ≥ 1,5 x 109/L, plaquetas ≥ 100 x 109/L, hemoglobina ≥ 9g/dL y recuento de leucocitos (WBC) ≥ 3 x 109/L.
• Creatinina del suero ≤ 1,5 veces el límite superior normal (ULN, por su sigla en inglés) y depuración de creatinina ≥ 30 ml/min, calculada por la fórmula de Cockroft y Gault (para excluir diversos trastornos renales); ausencia de proteinuria significativa (proteína en la orina < 1g/24 h desde que se recoge la muestra de orina) O relación entre proteínas/creatinina en una muestra de orina < 1.0 O 1+ proteinuria con tira reactiva.
• Magnesio ≥ límite inferior normal (LLN, por su sigla en inglés)
• Se aceptan pacientes que presenten un rango buffer de los valores normales de +/- 5% para hematología y +/- 10% para bioquímica. Esto no se aplicará a la Función Renal, incluyendo Creatinina.
• Las mujeres físicamente fértiles (WOCBP, por su sigla en inglés) deben tener una prueba de embarazo negativa (sangre u orina) realizada dentro de los 14 días previos a la primera dosis del tratamiento del estudio.
• Los hombres y mujeres fértiles/con capacidad de reproducción deben emplear las medidas anticonceptivas definidas por el investigador durante el período de tratamiento y durante al menos 6 meses después del último tratamiento del estudio. Un método anticonceptivo altamente eficaz se define como aquél que tiene una tasa mínima de error (es decir, menor al 1% al año) cuando se usa de forma coherente y correcta.
• Las mujeres que estén amantando deben dejar de hacerlo antes de recibir la primera dosis del tratamiento y hasta 6 meses después del último tratamiento del estudio.
• Antes de la inscripción/ asignación aleatoria del paciente, debe brindar su consentimiento informado por escrito según las normas emitidas por ICH/GCP y las autoridades nacionales y municipales.
-Histologically proven CRC with 1 to 8 metachronous or synchronous liver metastases considered to be completely resectable.
-Primary tumor (or liver metastasis) of CRC must be KRAS status ?wild type?.
-Patients must have undergone complete resection (R0) of the primary tumor at least 4 weeks before randomization. Or for patients with synchronous metastases the primary tumor can be resected (R0) at the same time as the liver metastases if: the patient has a non-obstructive primary tumor and is able to receive preoperative chemotherapy (3-4 months) before surgery.
-Measurable hepatic disease by RECIST version 1.1.
-Patients must be 18 years old or older.
-A WHO performance status of 0 or 1. Radiotherapy alone is allowed if given pre or post protocol treatment.
-Previous adjuvant chemotherapy for primary CRC is allowed if completed at least 12 months before inclusion in this study.
-Women of child bearing potential (WOCBP) must have a negative serum (or urine) pregnancy test within 14 days prior to the first dose of study treatment.
-Patients of childbearing / reproductive potential should use adequate birth control measures, as defined by the investigator, during the study treatment period and for at least 6 months after the last study treatment. A highly effective method of birth control is defined as those which result in low failure rate (i.e. less than 1% per year) when used consistently and correctly.
-Female subjects who are breast feeding should discontinue nursing prior to the first dose of study treatment and until 6 months after the last study treatment.
-Before patient registration/randomization, written informed consent must be given according to ICH/GCP, and national/local regulations. |

• existencia de metástasis extrahepática (de CRC).
• El paciente ha sido sometido a quimioterapia para el tratamiento de la metástasis ni a un tratamiento quirúrgico (por ej. resección quirúrgica o ablación por radiofrecuencias) por la metástasis hepática.
• Se ha realizado un procedimiento quirúrgico, una biopsia abierta ni ha sufrido una lesión traumática dentro de las 4 semanas previas a la asignación aleatoria del paciente al estudio.
• insuficiencia hepática de importancia (bilirrubina ≤ 1.5 x ULN y aspartato aminotransferasa (AST) y alanina aminotransferasa (ALT) ≤ 5 x ULN).
• El paciente haber sido sometido a un tratamiento terapéutico con un receptor del factor de crecimiento epidérmico (EGFR, por su sigla en inglés) o un receptor del factor de crecimiento endotelial (VEGF/VEGFR, por su sigla en inglés) dentro de los últimos 12 meses.
• haber tomado aspirinas u otros medicamentos antiinflamatorios no esteroideos (AINES) periódicamente.
• diátesis hemorrágica (por ej. hemoptisis de ≥ ½ cucharadita o 2.5mL), coagulopatía o necesidad de administración de anticoagulantes a dosis plenas.
• neuropatía periférica > grado 1 (Criterios Terminológicos Comunes para la Evaluación de las Reacciones Adversas, v.4.0) complicaciones serias de heridas, úlceras o fracturas de huesos.
• enfermedades cardiovasculares significativas, incluyendo: Hipertensión no controlada, insuficiencia cardíaca clase II-IV según la New York Heart Association (NYHA), infarto del miocardio o angina de pecho inestable, accidente cerebrovascular o ataque isquémico transitorio en los últimos 12 meses, enfermedad vascular periférica ≥ grado 2, arritmia cardiaca seria que requiere medicación y otras enfermedades cardiovasculares con importancia clínica.
• diverticulitis sintomática o ulcera gastroduodenal activa o no controlada.
• antecedentes de enfermedad pulmonar intersticial (por ej. neumonitis, fibrosis pulmonar).

-No previous exposure to EGFR or VEGF/VEGFR targeting therapy within the last 12 months.
-No major surgical procedure, open biopsy, or significant traumatic injury within 4 weeks prior to randomization.
-No regular use of aspirin or other non-steroidal anti-inflammatory drugs (NSAIDs). No bleeding diathesis (e.g. hemoptysis of ? 1/2 teaspoon or 2.5mL), coagulopathy, or need for administration of full-dose anti-coagulant(s).
-Presence of peripheral neuropathy > grade 1 (Common Terminology Criteria for Adverse Events, v4.0) serious wound complications, ulcers, or bone fractures.
-Clinically significant cardiovascular disease, including: uncontrolled hypertension, New York Heart Association (NYHA) class II-IV heart failure, myocardial infarction or unstable angina pectoris, cerebrovascular accident or transient ischemic attack within the past 12 months, peripheral vascular disease ? grade 2, serious cardiac arrhythmia requiring medication and other clinically significant cardiovascular disease.
-Presence of symptomatic diverticulitis or active or uncontrolled gastroduodenal ulceration.
-History or evidence of interstitial lung disease (e.g. pneumonitis, pulmonary fibrosis).
-Any psychological, familial, sociological, or geographical condition potentially hampering compliance with the study protocol and follow-up schedule; those conditions should be discussed with the patient before registration in the trial.
-Significant disease that, in the investigator?s opinion, would exclude the patient from the study. Including known allergy or any other adverse reaction to any of the study drugs (including any of the excipients) or to any related compound, including hypersensitivity to Chinese hamster ovary (CHO) cell products or other recombinant human or humanized antibodies.
-Participation in another clinical study (except sub studies of this protocol) within the 30 days before randomization and during this study. |
