    The EU Clinical Trials Register currently displays   40995   clinical trials with a EudraCT protocol, of which   6701   are clinical trials conducted with subjects less than 18 years old.
    The register also displays information on   18700   older paediatric trials (in scope of Article 45 of the Paediatric Regulation (EC) No 1901/2006).


    Phase 1 trials conducted solely in adults and that are not part of an agreed PIP are not public in the EU CTR (refer to European Guidance 2008/C 168/02   Art. 3 par. 2 and   Commission Guideline 2012/C 302/03,   Art. 5) .

    E.2 Objective of the trial
    E.2.1Main objective of the trial
    El principal objetivo es evaluar la efectividad, seguridad y tolerabilidad de dosis en aumento de tapentadol PR (300 mg/día tras un periodo de preinclusión y aumento de la titulación a 500 mg/día) frente a una combinación de tapentadol PR (300 mg/día tras el periodo de preinclusión) y pregabalina (titulada a 300 mg/día) en sujetos que requieren analgesia adicional tras titulación de tapentadol PR 300 mg/día
    E.2.2Secondary objectives of the trial
    . Evaluar el impacto de tapentadol PR y una combinación de tapentadol PR y pregabalina sobre los parámetros de función y calidad de vida (resultados comunicados por los sujetos) en pacientes con lumbalgia severa con un componente de dolor neuropático.
    . Evaluar a un subgrupo de sujetos aliviados con dosis moderadas de tapentadol PR (300 mg al día).
    . Evaluar los perfiles de respondedores en base a sub-perfiles "pain DETECT"

    E.3Principal inclusion criteria
    Principales criterios de inclusión (general) en la Visita de Incorporación
    1. Los sujetos deben firmar un Formulario de Consentimiento Informado indicando que comprenden la finalidad y los procedimientos exigidos para el ensayo y desean participar.
    2. Hombres o mujeres de más de 18 años.
    3. Las mujeres en edad fértil deben presentar una prueba negativa de embarazo en la Visita de Incorporación.
    4. Las mujeres sexualmente activas deben ser posmenopáusicas, quirúrgicamente estériles o utilizar un método anticonceptivo eficaz (p. ej. prescripción de anticonceptivos orales, inyecciones anticonceptivas, dispositivo intrauterino, parche anticonceptivo, esterilización de pareja masculina) antes de entrar y durante el ensayo.
    5. Los sujetos deben poder comunicar adecuadamente y diferenciar la localización e intensidad del dolor, y cumplimentar los cuestionarios utilizados en el ensayo.
    Principales criterios de inclusión (específicos del ensayo) en la Visita de Incorporación:
    6. Los sujetos deben tener diagnóstico de lumbalgia crónica, definida como dolor de al menos 3 meses de duración.
    7. El dolor debe requerir un analgésico potente (definido como de Escalón III por la OMS) determinado por el Investigador.
    8. La puntuación del cuestionario de diagnóstico "painDETECT" debe ser:
    - "positiva" o
    - "dudosa"
    o
    Si el sujeto está tratado con un régimen estable de analgésicos de acción central (opiáceos) y/o coanalgésicos, se aceptará incluso una puntuación "negativa" de painDETECT (pero de 9 como mínimo) en la Visita de Incorporación.
    9. Si recibe un pretratamiento diario regular con un analgésico opioide de Escalón II/III de la OMS y/o un coanalgésico de acción central:
    - Los sujetos deben estar tomando un analgésico de Escalón II o III o coanalgésico diariamente durante al menos 2 semanas antes de la Visita de Incorporación.
    - Los sujetos pretratados con un analgésico opioide de Escalón II de la OMS y/o un coanalgésico de acción central deben haber comunicado una puntuación de intensidad media de dolor de al menos 5 puntos (NRS-3-5) durante los últimos 3 días anteriores a la Visita de Incorporación.
    o
    Si reciben un pretratamiento diario regular con monoterapia analgésica de Escalón I de la OMS o si no se comunica un pretratamiento analgésico regular:
    - Los sujetos deben presentar una puntuación de intensidad media de dolor de al menos 6 puntos (NRS-3-6) durante los últimos 3 días anteriores a la Visita de Incorporación.
    Principales criterios de incorporación en la Visita Inicial
    10. Para poder participar en el ensayo, los sujetos deben reunir los criterios de inclusión y no cumplir ninguno de los criterios de exclusión en la Visita de Incorporación. Deben confirmarse los siguientes criterios en la Visita Inicial tras 3 días a 2 semanas de "lavado" de opioides y coanalgésicos de acción central y tras recepción de los resultados de los análisis de laboratorio.
    - Los sujetos deben tener una puntuación de intensidad media de dolor (NRS-3) de al menos 6 puntos (NRS-3-6)
    - Los sujetos deben puntuar "positivo" o "dudoso" en el cuestionario de diagnóstico "painDETECT" en la Visita Inicial.
    - Los sujetos deben experimentar un aumento de al menos 1 punto en el NRS-3 con relación a la Visita de Incorporación si recibieron un tratamiento analgésico regular anterior con un opiáceo de Escalón II o III de la OMS (post "lavado").
    Los sujetos sólo recibirán el MI si se dispone de documentación que muestre que reúnen estos criterios de inclusión.
    E.4Principal exclusion criteria
    Principales criterios de exclusión (general) en la Visita de Incorporación
    Cualquiera de las siguientes circunstancias excluirá a los sujetos del ensayo:
    1. Presencia de enfermedad clínicamente significativa o resultado de laboratorio que el Investigador considere que puede afectar las evaluaciones de eficacia o seguridad.
    2. Presencia de infección sistémica o local activa que el Investigador considere que puede afectar las evaluaciones de eficacia, calidad de vida/función, o seguridad.
    3. Historial de alcoholismo o drogadicción, o sospecha por parte del Investigador.
    4. Presencia de afección inflamatoria autoinmune concomitante.
    5. Insuficiencia renal moderada (aclaramiento de creatinina <60 mL/min).
    6. Historial conocido de valores de laboratorio que reflejen insuficiencia hepática actual moderada o severa.
    7. Historial de convulsión o epilepsia.
    8. Cualquiera de las siguientes dolencias durante el primer año de Incorporación: lesión cerebral traumática leve/moderada, embolia, isquemia transitoria, o neoplasia cerebral. Lesión cerebral traumática severa dentro de los 15 años de la incorporación (consistente en 1 o más de los casos siguientes: contusión cerebral, hematoma intracraneal, amnesia inconsciente o post-traumática de más de 24 horas de duración) o secuelas residuales que sugieran cambios transitorios de consciencia.
    9. Mujeres embarazadas o lactantes.
    10. Historial de alergia o hipersensibilidad a tapentadol, pregabalina, paracetamol o sus excipientes.
    11. Contraindicaciones y advertencias relativas a tapentadol, pregabalina o paracetamol no cubiertas por otros criterios de incorporación, incluyendo:
    - Asma bronquial agudo o severo.
    - Depresión respiratoria severa con hipoxia y/o hipercapnia.
    - Cor pulmonale.
    - Enfermedad pulmonar obstructiva crónica severa
    - Presencia o sospecha de íleo paralítico inducido por no opiáceos
    -12. Sujetos con insuficiencia cardiaca severa, por ejemplo, infarto de miocardio de clase >3 de la New York Heart Association menos de 6 meses antes de la incorporación, y/o angina pectoris inestable.
    13. Empleados del Investigador o del lugar del ensayo, que participen directamente en este u otros ensayos bajo la dirección del Investigador o del lugar del ensayo, así como familiares o empleados del Investigador.
    14. Participación en otro ensayo simultáneamente, o durante las 4 semanas anteriores a la Visita de Incorporación.
    15. Sujetos que se sabe o se sospecha no pueden cumplir el protocolo ni utilizar los MI.
    16. Utilización de inhibidores de la monoaminooxidada durante los 14 días anteriores a la Visita de Incorporación.
    17. Administración no estable de inhibidores selectivos de recaptación de serotonina durante los 30 días anteriores a la Visita de Incorporación (las dosis deben mantenerse estables durante el ensayo).

    Principales criterios de exclusión (específicos del ensayo) en la Visita de Incorporación
    18. Presencia de manifestaciones dolorosas concomitantes distintas de la lumbalgia que pudieran confundir las evaluaciones de los sujetos del ensayo o la autoevaluación del índice de dolor, como síndromes con dolor extendido tales como fibromialgia.
    19. Lumbalgia causada por cáncer y/o metástasis.
    20. Todo procedimiento doloroso previsto durante el periodo del ensayo (como cirugía mayor) que pudiera, en opinión del Investigador, afectar las evaluaciones de eficacia o seguridad del MI.
    21. Juicio pendiente o solicitud de prestaciones de seguros/estatales por dolor crónico o incapacidad y si, de concederse, las prestaciones pudieran verse influidas por una participación con éxito en el ensayo.
    22. Problemas hereditarios raros de intolerancia a la galactosa, deficiencia de lactasa Lapp o malabsorción de glucosa-galactosa, intolerancia a la lactosa.

    Los sujetos sólo recibirán el MI si se dispone de documentación que muestre que no reúnen estos criterios de exclusión.
