    E.2.1Main objective of the trial
    Evaluar la eficacia de 4 dosis de AHU377 administrado una vez al día (50 mg, 100 mg, 200 mg y 400 mg) cuando se administran en combinación con valsartán 320 mg en pacientes con hipertensión esencial, para lo que se comprobará la hipótesis de que la reducción de la presión arterial sistólica media en sedestación (PASms) muestra una respuesta a la dosis en comparación con la monoterapia con valsartán 320 mg después de 8 semanas de tratamiento.
    E.2.2Secondary objectives of the trial
    • Evaluar la relación dosis-respuesta en la reducción de la presión arterial diastólica en sedestación (PADms) de dosis ascendentes de AHU377 en combinación con valsartán 320 mg

    E.3Principal inclusion criteria
    1. El consentimiento informado por escrito deberá obtenerse antes de que se realice ninguna evaluación
    2. Pacientes hombres y mujeres, de 18 años de edad y mayores.
    3. Pacientes con hipertensión sistólica leve o moderada, no tratados o que actualmente estén tomando tratamiento antihipertensivo.
    4. Los pacientes no tratados deben presentar una PASms en la consulta &#8805; 150 mmHg y < 180 mmHg y una PADms &#8805; 70 mmHg en la visita de aleatorización (Visita 3) y en la visita anterior (Visita 2).
    5. Pacientes con diagnóstico nuevo de hipertensión esencial o pacientes que tengan antecedentes de hipertensión pero que no hayan estado tomando ningún fármaco antihipertensivo (o sean pacientes no tratados según se define arriba) durante al menos 4 semanas antes de la Visita 1
    6. Los paciente pretratados deben tener una PASms &#8805; 150 mmHg y < 180 mmHg y una PADms &#8805; 70 mmHg en la visita basal (Visita 3)
    7. Los pacientes deben presentar una diferencia absoluta de &#8804;15 mmHg en la PASms durante las últimas dos visitas consecutivas (incluida la Visita 3)
    8. Capacidad para comunicar y cumplir todos los requisitos del estudio y demostrar un buen cumplimiento de la medicación (tasa de cumplimiento &#8805; 80%) durante el período de preinclusión.
    E.4Principal exclusion criteria
    1. Hipertensión severa (PADms &#8805;110 mmHg y/o PASms &#8805; 180 mmHg).
    2. Antecedentes de angioedema, relacionado con el fármaco o de cualquier otro modo, comunicado por el paciente.
    3. Mujeres embarazadas o en período de lactancia, donde el embarazo se define como el estado de una mujer tras la concepción y hasta el final de la gestación, confirmado mediante resultado positivo en la prueba de hCG realizada en laboratorio (> 5 mUI/ml).
    4. Mujeres en edad fértil (WOCBP), definidas como todas las mujeres fisiológicamente capaces de quedarse embarazadas, incluidas las mujeres cuya carrera profesional, estilo de vida u orientación sexual excluya el coito con un varón, y mujeres cuyas parejas hayan sido esterilizados mediante vasectomía u otros métodos, A NO SER QUE estén utilizando dos métodos anticonceptivos. Los dos métodos pueden ser un método de doble barrera (si el comité ético local lo acepta) o un método de barrera junto con un método hormonal.
    • Los métodos anticonceptivos de barrera adecuados incluyen: diafragma, preservativo (utilizado por la pareja), dispositivo intrauterino (cobre u hormonal), esponja o espermicida. Los anticonceptivos hormonales incluyen cualquier anticonceptivo comercializado que contenga un estrógeno y/o un progestágeno.

    • Se considera que una mujer es posmenopáusica y no fértil si lleva 12 meses de amenorrea natural (espontánea) con un perfil clínico apropiado (p. ej., edad apropiada, antecedentes de síntomas vasomotores) o seis meses de amenorrea espontánea con niveles de FSH séricos > 40 mUI/ml o ha sido sometida a ooforectomía bilateral quirúrgica (con o sin histerectomía) al menos seis meses antes. En caso de ooforectomía sola, únicamente cuando el estado reproductivo de la mujer se ha confirmado mediante la evaluación de seguimiento del nivel hormonal.
    5. Antecedentes o indicios de una forma secundaria de hipertensión, incluidas, pero sin limitarse a cualquiera de las siguientes: hipertensión parenquimatosa renal, hipertensión renovascular (estenosis de la arteria renal unilateral o bilateral), coartación aórtica, hiperaldosterismo primario, enfermedad de Cushing, feocromocitoma, enfermedad renal poliquística, e hipertensión inducida por medicamentos.
    6. Cualquier antecedente de encefalopatía hipertensiva o accidente cerebrovascular, o antecedentes de ataque isquémico transitorio (AIT), infarto de miocardio, derivación aortocoronaria, o cualquier intervención coronaria percutánea (PCI)
    7. Angina de pecho actual que requiera tratamiento farmacológico (se permitirá el uso de nitratos para el tratamiento de la angina de pecho).
    8. Pacientes con diabetes mellitus Tipo 1 o Tipo 2 que no se controle bien según el criterio clínico del investigador. Se recomienda que a los pacientes que actualmente están siendo tratados para la diabetes mellitus se les mantenga con una dosis estable de medicación antidiabética durante al menos 4 semanas antes de la Visita 1.
    9. Diagnóstico previo o actual de insuficiencia cardiaca (Clase II-IV de la NYHA).
    10. Cardiopatía valvular clínicamente significativa en la Visita 1.
    11. Antecedentes o diagnóstico actual de las siguientes anormalidades cardíacas:
    • Bloqueo AV de segundo o tercer grado sin marcapasos
    • Arritmias cardíacas clínicamente significativas, incluida fibrilación auricular activa
    • Antecedentes familiares de síndrome de QT largo o antecedentes familiares de torsade de pointes.
    12. Antecedentes de neoplasia maligna de cualquier sistema orgánico, tratada o no tratada, en los últimos 5 años, tanto si existen o no indicios de recidiva o metástasis local, excepto carcinoma basocelular cutáneo localizado.
    13. Hepatopatía severa tales como cirrosis o hepatitis activa.

    17. Cualquier contraindicación o antecedente de hipersensibilidad a cualquiera de las medicaciones del estudio o a medicaciones con estructuras químicas similares.
    18. Cualquiera de los siguientes valores anormales de laboratorio significativos:
    • Potasio sérico > 5,5 mEq/L en la Visita 1.
    • ALT o AST > 3 veces el límite superior de normalidad (LSN) en la Visita 1.
