E.3 | Principal inclusion criteria |
1. Histologically confirmed diagnosis of soft tissue sarcoma of high or intermediate grade with one of the following histological subtypes: a. Adipocytic sarcoma, including: i. Dedifferentiated ii. Myxoid iii. Round Cell iv. Pleomorphic. b. Leiomyosarcoma. 2. Documented evidence of advanced (locally recurrent, locally advanced and/or metastatic) adipocytic (restricted to subtypes listed in Inclusion 1) or leiomyosarcoma, incurable by surgery and/or radiotherapy. 3. Subjects should have received standard therapies for advanced soft tissue sarcoma which must have included an anthracycline (unless contraindicated) with or without ifosfamide and then at least one additional regimen after failure of the anthracycline. 4. Radiographic evidence of disease progression by RECIST criteria on or after the last anti-cancer therapy within the 6 months prior to randomization. 5. Presence of measurable disease meeting the following criteria: a. At least one lesion of ≥ 1.0 cm in long-axis diameter for non lymph nodes or ≥ 1.5 cm in short-axis diameter for lymph nodes which is serially measurable according to RECIST 1.1 using either computerized tomography or magnetic resonance imaging or panoramic and close-up color photography. b. Lesions that have had radiotherapy must show evidence of progressive disease based on RECIST 1.1 to be deemed a target lesion. 6. Eastern Cooperative Oncology Group, performance status of 0, 1 or 2. 7. Adequate renal function defined as calculated creatinine clearance > 50 mL/min per the Cockroft and Gault formula. 8. Adequate bone marrow function, defined as: a. Absolute neutrophil count (ANC) ≥ 1,500/mm3 or ≥ 1.5 x 10[9]/L. b. Platelet count ≥ 100,000/mm3 or ≥ 100 x 10[9]/L. c. Hemoglobin (Hb) ≥ 10g/dL at baseline (blood transfusions, hematopoietic growth factors and hematinics are allowed during the Prerandomization Phase to correct Hb values < 10g/dL). 9. Adequate liver function, defined as: a. Bilirubin ≤ 1.5 times the upper limit of normal (ULN) except for unconjugated hyperbilirubinemia of Gilbert’s syndrome. b. Alkaline phosphatase (ALP), alanine aminotransferase (ALT), and aspartate aminotransferase (AST) ≤ 3 times ULN. For total ALP >3 times ULN, the ALP liver isoenzyme must be ≤ 3 times ULN. 10. Female subjects of child-bearing potential must agree to use two forms of highly effective contraception from the last menstrual period prior to randomization (or use a double barrier method until they are on two forms of highly effective contraception for at least one menstrual cycle), during the study treatment, and for 3 months after the final dose of study treatment. 11. Male subjects and their female partner who are of child-bearing potential (as defined in Inclusion 10), and are not practicing total abstinence, must agree to use two forms of highly effective contraception from the last menstrual period of their female partner prior to randomization (or use a double barrier method until they are on two forms of highly effective contraception for at least one menstrual cycle), during study treatment, and for 3 months (or 6 months if they received dacarbazine) after the final dose of study treatment. 12. Voluntary agreement to provide written informed consent and the willingness and ability to comply with all aspects of the protocol. 13. Males or females aged ≥ 18 years at the time of informed consent. |
1.Diagnosi di sarcoma dei tessuti molli confermata istologicamente di grado elevato o intermedio con uno dei seguenti sottotipi istologici:
a.Liposarcoma, compresi:
i.Dedifferenziato
ii.Mixoide
iii.A cellule rotonde
iv.Pleomorfo.
b.Leiomiosarcoma.
2.Evidenza documentata di liposarcoma (ristretto ai sottotipi elencati in inclusione I) o leiomiosarcoma avanzato (localmente recidivante, localmente avanzato e/o metastatico), incurabile con chirurgia e/o radioterapia.
3.I soggetti devono aver ricevuto le terapie standard per il sarcoma dei tessuti molli avanzato che devono aver incluso un’antraciclina (a meno che non fosse controindicato) con o senza ifosfamide e quindi almeno un ulteriore regime dopo il fallimento dell’antraciclina.
4.Evidenza radiografica di progressione della malattia secondo i criteri RECIST durante o dopo la terapia anticancro entro i 6 mesi prima della randomizzazione.
5.Presenza di malattia misurabile che soddisfi i seguenti criteri:
a.Almeno una lesione di ≥ 1,0 cm di diametro nell’asse lungo per nodi non linfatici o ≥ 1,5 cm di diametro dell’asse breve per linfonodi che sia misurabile in serie secondo i RECIST 1.1 per mezzo di tomografia computerizzata o risonanza magnetica o fotografia a colori panoramica e di dettaglio.
b.Le lesioni che sono state sottoposte a radioterapia devono mostrare evidenza di malattia progressiva in base a RECIST 1.1 per essere considerate lesioni target.
6.Stato di performance secondo l’ECOG di 0, 1 o 2.
7.Funzione renale adeguata definita come clearance della creatinina calcolata > 50 ml/min secondo la formula Cockroft e Gault.
8.Funzione del midollo osseo adeguata, definita come:
a.Conta assoluta dei neutrofili (ANC) ≥ 1.500/mm3 o ≥ 1,5 x 109/l.
b.Conta piastrinica ≥ 100.000/mm3 o ≥ 100 x 109/L.
c.Emoglobina (Hb) ≥ 10g/dl al basale (trasfusioni di sangue, fattori di crescita ematopoietici ed ematinici sono consentiti durante la Fase di prerandomizzazione al fine di correggere i valori di Hb < 10g/dl)
9.Funzione epatica adeguata, definita come:
a.Bilirubina ≤ 1,5 volte il limite superiore della norma (ULN) con l’eccezione dell’ iperbilirubinemia non coniugata dovuta alla sindrome di Gilbert.
b.Fosfatasi alcalina (ALP), alanina aminotransferasi (ALT) e aspartato aminotransferasi (AST) ≤ 3 volte l’ULN. Per una ALP totale >3 volte l'ULN, l’isoenzima epatico ALP deve essere ≤ 3 volte l’ULN.
10.I soggetti di sesso femminile potenzialmente fertili devono acconsentire a usare due forme di contraccezione altamente efficaci a partire dall'ultimo ciclo mestruale prima della randomizzazione (o usare un metodo a doppia barriera come descritto sotto fino a che non utilizzino due metodi contraccettivi altamente efficaci per almeno un ciclo mestruale), durante il trattamento dello studio e per 3 mesi dopo la dose finale del trattamento dello studio.
11.I soggetti di sesso maschile e le loro partner potenzialmente fertili (come definito in Inclusione 10), che non praticano l’astinenza totale, devono acconsentire a usare due forme di contraccezione altamente efficaci a partire dall'ultimo ciclo mestruale della partner prima della randomizzazione (o usare un metodo a doppia barriera fino a che non utilizzino due metodi contraccettivi altamente efficaci per almeno un ciclo mestruale), durante il trattamento dello studio e per 3 mesi (o 6 mesi se hanno ricevuto dacarbazina) dopo la dose finale del trattamento dello studio.

E.4 | Principal exclusion criteria |
1. Subjects who have received any anti-cancer therapy, including radiotherapy, cytotoxic, hormonal, biological (including humanized antibodies) and targeted agents within 21 days, or any investigational agent within 30 days, prior to randomization. 2. Subjects who have not recovered from acute toxicities as a result of prior anti-cancer therapy to ≤ Grade 1, according to Common Terminology Criteria for Adverse Events (CTCAE), except for peripheral neuropathy (see Exclusion 6) and alopecia. 3. Subjects that have previously been treated with dacarbazine or participated in a study with eribulin (whether treated with eribulin or not). 4. Radiation therapy encompassing > 30% of bone marrow. 5. Major surgery within 21 days prior to randomization. 6. Pre-existing peripheral neuropathy > CTCAE Grade 2. 7. Significant cardiovascular impairment, defined as: a. Cardiac failure > New York Heart Association (NYHA) Class II according to the NYHA Functional Classification, b. Unstable angina or myocardial infarction within 6 months of enrolment, c. Serious cardiac arrhythmia or cardiac arrhythmia requiring treatment. 8. Subjects with a high probability of Long QT Syndrome. 9. Subjects with known central nervous system metastases. 10. Any serious concomitant illness or infection requiring treatment. 11. Any malignancy that required treatment, or has shown evidence of recurrence (except for soft tissue sarcoma, non-melanoma skin cancer, or carcinoma in situ of the cervix) during the 5 years prior to randomization. 12. Female subjects must not be pregnant as documented by a negative beta-human chorionic gonadotropin (ß-hCG) test with a minimum sensitivity 25 IU/L or equivalent unit of ß-hCG at Screening and Baseline, or breastfeeding. 13. Hypersensitivity to either halichondrin A or halichondrin B chemical derivatives or both; or to dacarbazine, or to any of the dacarbazine excipients (please refer to the dacarbazine prescribing information). 14. Any medical or other condition which, in the opinion of the PI or designee, will preclude participation in a clinical trial. |

2.Soggetti che non si sono ristabiliti da tossicità acute in conseguenza di precedente terapia anticancro a un Grado ≤ 1, in base ai Criteri terminologici comuni per gli eventi avversi (CTCAE, Common Terminology Criteria for Adverse Events), ad eccezione della neuropatia periferica (ved. Esclusione 6) e dell’alopecia.
3.Soggetti che sono stati precedentemente trattati con dacarbazina o hanno partecipato a uno studio con eribulina (a prescindere che siano stati trattati con eribulina o meno).
4.Radioterapia che comprende > 30% di midollo osseo.
5.Intervento chirurgico significativo entro 21 giorni prima della randomizzazione.
6.Neuropatia periferica pre-esistente > grado CTCAE 2.
7.Insufficienza cardiovascolare significativa, definita come:
a.Insufficienza cardiaca > classe II della New York Heart Association (NYHA) secondo la classificazione funzionale della NYHA,
b.Angina instabile o infarto miocardico entro 6 mesi dall’arruolamento,
c.Aritmia cardiaca seria o aritmia cardiaca che richieda un trattamento.
8.Soggetti con un’elevata probabilità di sindrome del QT lungo.
9.Soggetti con metastasi note al sistema nervoso centrale.
10.Eventuali gravi malattie concomitanti o infezioni che richiedano un trattamento.
11.Eventuale neoplasia maligna che richieda un trattamento o che abbia mostrato evidenza di recidiva (ad eccezione del sarcoma dei tessuti molli, del tumore della pelle non melanoma o del carcinoma in situ della cervice) durante i 5 anni precedenti la randomizzazione.
12.I soggetti di sesso femminile non devono essere in gravidanza, come documentato da un test negativo per la subunità beta della gonadotropina corionica umana (ß-hCG) con una sensibilità minima di 25 IU/l o unità equivalente di ß-hCG allo Screening e alla Baseline, o allattare al seno.
13.Ipersensibilità ai derivati chimici dell’alicondrina A o dell’alicondrina B o di entrambe; oppure alla dacarbazina, o a qualsiasi eccipiente della dacarbazina (fare riferimento alle informazioni di prescrizione della dacarbazina).
14.Eventuali condizioni mediche o di altro tipo che, a giudizio dello sperimentatore principale o di un suo designato, precluderà la partecipazione a uno studio clinico. |
