1. Each subject must be >18 to <45 years of age.
2. Each subject must have a physician?s diagnosis of active axial SpA with disease duration ?5 years, and chronic back pain of ? 3 month duration.
3. Each subject must meet either criterion ?a? or ?b? as adopted from ASAS classification criteria:
a. Active inflammation on MRI highly suggestive of sacroiliitis associated with spondyloarthropathy (as evidenced by the central reader) and 1 or more of the following spondyloarthritis characteristics OR
b. HLA-B27+ gene and 2 or more of the following spondylarthritis characteristics (not including HLA-B27):
? Inflammatory back pain, defined as having at least 4 out of the 5 following parameters:
? age at onset < 40 years;
? insidious onset;
? improvement with exercise;
? no improvement with rest;
? pain at night (with improvement upon getting up);
? Arthritis diagnosed by a physician;
? Enthesitis diagnosed by a physician;
? Dactylitis diagnosed by a physician;
? Psoriasis diagnosed by a physician;
? History of inflammatory bowel disease (IBD) diagnosed by a physician;
? History of uveitis confirmed by an ophthalmologist;
? Good response to NSAIDs
? Family history for SpA;
? Elevated CRP (> upper limit of normal based on central lab values);
? HLA-B27+ gene;
4. Each subject must show high disease activity at Screening and Baseline of both a total back pain evaluation of ?40 mm on a VAS of 0-100 mm and a BASDAI score of ? 40 mm.
5. Each subject must have either an inadequate response (as assessed by the investigator) to 30 days of continuous therapy with maximal recommended daily doses of at least one non-steroidal anti-inflammatory drug (NSAID) or must be unable to receive a maximal dose of NSAID therapy for a full 30 days because of intolerance, toxicity, or contraindications to NSAIDs. |
1. El sujeto debe tener entre ≥ 18 y ≤ 45 años de edad.
2. El sujeto debe tener un diagnóstico de EspA axial activa efectuado por un médico con una duración de la enfermedad ≤ 5 años y con dolor de espalda crónico de ≥ 3 meses de duración.
3. El sujeto debe cumplir el criterio “a” o “b” adoptado a partir de los criterios de clasificación de la ASAS:
a. Inflamación activa en la RM sumamente indicativa de sacroilitis acompañada de espondiloartropatía (a juzgar por el evaluador central) y 1 o más de las siguientes características de espondiloartritis O BIEN
b. Gen HLA-B27+ y 2 o más de las siguientes características de espondiloartritis (sin contar HLA-B27):
◦ Dolor de espalda inflamatorio, definido por la presencia de al menos cuatro de los cinco parámetros siguientes:
edad en el momento de comienzo < 40 años
inicio gradual
mejoría con el ejercicio
sin mejoría con el reposo
dolor nocturno (que mejora al levantarse)
◦ Artritis diagnosticada por un médico.
◦ Entesitis diagnosticada por un médico.
◦ Dactilitis diagnosticada por un médico.
◦ Psoriasis diagnosticada por un médico.
◦ Antecedentes de enfermedad inflamatoria intestinal (EII) diagnosticada por un médico.
◦ Antecedentes de uveítis confirmada por un oftalmólogo.
◦ Buena respuesta a los AINE.
Nota: se define buena respuesta como “24-48 h después de administrar una dosis plena de un AINE, el dolor de espalda ya no está presente o se encuentra mucho mejor”.
◦ Antecedentes familiares de EA.
◦ PCR elevada (> límite superior de la normalidad según los valores del laboratorio central).
◦ Gen HLA-B27+.
4. El sujeto debe presentar una actividad elevada de la enfermedad en los momentos de selección y basal según una evaluación del dolor de espalda total ≥ 40 mm en una EAV de 0-100 mm y una puntuación BASDAI ≥ 40 mm.
5. El sujeto debe presentar una respuesta insuficiente (evaluada por el investigador) al cabo de 30 días de tratamiento continuo con dosis diarias recomendadas máximas de al menos un antiinflamatorio no esteroideo (AINE) o ha de ser incapaz de recibir una dosis máxima de tratamiento con AINE durante 30 días completos por intolerancia, toxicidad o contraindicaciones de los AINE.
Nota: es posible que un sujeto presente una buena respuesta inicial a los AINE, pero que, posteriormente, presente una respuesta insuficiente durante 30 días o manifieste intolerancia al tratamiento con AINE.

La participación será de 66 semanas aprox, desde momento de firma de documento consentimiento informado (DCI) hasta contacto final.
