The secondary objectives are to investigate the effects of SAR153191 (REGN88) when added to DMARD therapy, in patients with active RA who are inadequate responders or intolerant to TNF-α antagonists, for: • reduction of signs and symptoms at 12 weeks, • improvement in physical function at 12 weeks, • improvement in disease activity as measured by other ACR derived components at Weeks 12 and 24, and • improvement in quality of life as measured by patient reported outcomes (PROs) at intermediate visits and Week 24. To assess the safety of sarilumab in this population. To assess the exposure of sarilumab added to DMARD therapy in this population |
Gli obiettivi secondari sono di ricercare gli effetti di SAR153191 (REGN88) in aggiunta ad una terapia con DIMARD, in pazienti con AR attiva che non rispondono adeguatamente o sono intolleranti agli antagonisti del TNF-α al fine di: - ridurre i segni e i sintomi alla 12 settimana, - migliorarare la funzionalità fisica alla 12 settimana, - migliorare il punteggio dell'attività di malattia come risulta da altri componenti ACR riscontrati alla 12 e 24 settimana e - migliorare la qualità della vita in seguito a misurazione degli esiti riferiti dal paziente (patient reported outcomes PRO) alle visite intermedie alla settimana 24. - Valutare la tollerabilità di Sarilumab in questa popolazione. - Valutare l'esposizione a Sarilumab in aggiunta ad una terapia con DIMARD in questa popolazione. |

Diagnosis of rheumatoid arthritis ≥6 months duration, according to the American College of Rheumatology (ACR)/European League against Rheumatism (EULAR) 2010 Rheumatoid Arthritis Classification Criteria ACR Class I-III functional status, based on 1991 revised criteria Anti-TNF therapy failures, defined by the investigator as patients with an inadequate clinical response, after being treated for at least 3 consecutive months, and/or intolerance to at least 1 anti-TNF blocker(s), resulting in or requiring their discontinuation: TNF-blockers may include, but are not limited to, etanercept, infliximab, adalimumab, golimumab and/or certolizumab. Moderate-to-severely active rheumatoid arthritis. Continuous treatment with one or a combination of DMARDs (except for simultaneous combination use of leflunomide and methotrexate) for at least 12 weeks prior to baseline and on a stable dose(s) for at least 6 weeks prior to screening: Methotrexate – 6 to 25 mg/wk orally or intramuscular Leflunomide – 10 to 20 mg orally daily Sulfasalazine – 1000 to 3000 mg orally daily. Hydroxychloroquine - 200 to 400 mg orally daily |
Diagnosi di artrite reumatoide di durata ≥6 mesi, in base ai criteri di classificazione dell’artrite reumatoide dell’American College of Rheumatology (ACR)/dell’ European League Against Rheumatism (EULAR) del 2010. Stato funzionale ACR di classe I-III, in base ai criteri del 1991 modificati sui fallimenti della terapia Anti-TNF, definito dallo Sperimentatore come pazienti con una risposta clinica inadeguata, dopo essere stati trattati per almeno 3 mesi consecutivi, e/o che mostrano intolleranza ad almeno 1 antagonista del TNF, che causi o richieda la loro interruzione: gli antagonisti del TNF possono includere, ma non sono limitati a, etanercept, infliximab, adalimumab, golimumab and/or certolizumab. Artrite reumatoide in fase attiva da moderata a severa. Trattamento continuativo con un o con una combinazione di DMARDs (fatta eccezione per l’uso combinato e simultaneo di leflunomide e metotressato) per almeno 12 settimane prima della visita di baseline e a dose/i stabile/i per almeno 6 settimane prima dello screening: Metotressato – da 6 a 25 mg/settimana per via orale o intramuscolare Leflunomide – da 10 a 20 mg/giorno per via orale Sulfasalazine – da 1000 a 3000 mg/giorno per via orale Idrossiclorochina – da 200 a 400 mg/giorno per via orale |

E.4 | Principal exclusion criteria |
Patients <18 years of age. Past history of, or current, autoimmune or inflammatory systemic or localized joint disease(s) other than RA. History of juvenile idiopathic arthritis or arthritis onset prior to age 16. Severe active systemic RA, including but not limited to vasculitis, pulmonary fibrosis, and/or Felty’s syndrome. Treatment with anti-TNF agents, as follows: • Within 28 days prior to the baseline visit – etanercept • Within 42 days prior to the baseline visit – infliximab, adalimumab, golimumab, certolizumab pegol Treatment with previous RA-directed biologic agents with other than TNF antagonist mechanisms: • Within 28 days prior to the randomization (baseline) visit – anakinra • Within 42 days prior to the randomization (baseline) visit – abatacept Within 6 months prior to the randomization (baseline) visit – any cell depleting agents including but not limited to rituximab without a normal lymphocyte and CD 19+ lymphocyte count. Treatment with any DMARD other than those allowed per protocol and limited to the maximum specified dosage within 12 weeks prior to baseline. Treatment with prednisone >10 mg or equivalent per day, or change in dosage within 4 weeks prior to baseline visit. Any parenteral or intra-articular glucocorticoid injection within 4 weeks prior to baseline. Prior treatment with anti-IL-6 or IL-6R antagonist therapies, including tocilizumab or sarilumab, participation in a prior study of sarilumab, irrespective of treatment arm. Prior treatment with a Janus kinase inhibitor (such as tofacitinib). New treatment or dose-adjustment to ongoing statin medication within 6 weeks prior to randomization, ie, stable dose for at least 6 weeks prior to randomization. Participation in any clinical research study evaluating another investigational drug or therapy within 5 half-lives or 60 days of first investigational medicinal product (IMP) administration, whichever is longer. History of alcohol or drug abuse within 5 years prior to the screening visit. Patients with a history of malignancy other than adequately-treated carcinoma in-situ of the cervix, nonmetastatic squamous cell or basal cell carcinoma of the skin, within 5 years prior to the randomization (baseline) visit. Nonmalignant lymphoproliferative disorders are also excluded. Patients with active tuberculosis or latent tuberculosis infection |
Pazienti con età < 18 anni. Storia precedente o attuale di malattia autoimmune o infiammatoria sistemica o localizzata alle giunture oltre all’AR. Storia di artrite idiopatica giovanile o sviluppo dell’artrite prima dei 16 anni. AR sistemica severa in fase attiva, che include, ma non è limitata a, vasculite, fibrosi polmonare, e/o sindrome di Felty. Trattamento con agenti anti-TNF, nella seguente modalità: • Entro 28 giorni prima della visita di baseline-etanercept • Entro 42 giorni prima della visita di baseline– infliximab, adalimumab, golimumab, certolizumab pegol Trattamento con precedenti agenti biologici diretti contro l’AR con meccanismi diversi da quelli degli antagonisti del TNF: • Entro 28 giorni prima della visita di randomizzazione (baseline)- anakinra • Entro 42 giorni prima della visita di randomizzazione (baseline)– abatacept • Entro 6 mesi prima della visita di randomizzazione (baseline)- qualsiasi agente che induca deplezione cellulare, incluso ma non limitato al rituximab, senza conta linfocitaria totale e dei linfociti CD19+ normalizzata. Trattamento con qualsiasi DMARDs oltre a quelli consentiti dal protocollo e limitato al dosaggio massimo specificato entro 12 settimane prima della visita di baseline. Trattamento con prednisolone >10 mg al giorno o equivalente, o cambiamenti nel dosaggio entro 4 settimane prima della visita di baseline. Qualsiasi iniezione parenterale o intrarticolare di glucocorticoidi entro 4 settimane prima della visita di baseline. Precedente trattamento con anti-interleuchina-6 (anti-IL-6) oppure terapie con antagonisti del recettore dell’interleuchina-6 (IL-6R), inclusi tocilizumab e sarilumab, la partecipazione in uno studio precedente con sarilumab, indipendentemente dal braccio di trattamento. Trattamento precedente con inibitori della Janus chinasi (come per esempio tofacitinib) Nuovo trattamento o un aggiustamento di dose nel trattamento concomitante con statine entro 6 settimane prima della randomizzazione, cioè, a dose stabile per almeno 6 settimane prima della randomizzazione. Partecipazione a qualsiasi altro studio di ricerca clinica che valuti un altro farmaco sperimentale o terapia entro 5 emivite o 60 giorni dalla prima somministrazione del prodotto medicinale oggetto della sperimentazione (IMP), qualunque duri più a lungo. Storia di abuso di alcol o di droghe entro 5 anni prima della visita di screening. Pazienti con storia di tumori maligni eccetto il carcinoma della cervice in-situ adeguatamente trattato, carcinoma della cute a cellule basali o a cellule squamose non metastatico, entro 5 anni prima della visita di randomizzazione (baseline). Anche i disordini linfoproliferativi sono esclusi. Pazienti con tubercolosi attiva o infezione da tubercolosi latente. |
