    Presence of analyzable tissue for the analysis of activating mutation of EGFR
    inoperable and non irradiable stage III or stage IV
    No prior or just one line of chemotherapy for patient with EGFR mutation. 1 or 2 lines of chemotherapy for patient EGFR wild type

    Les patientes porteuses de mutation de l’EGFR doivent être chimionaïve ou être évolutive après une seule ligne de chimiothérapie antérieure (incluant la maintenance). Les patientes sans mutation de l’EGFR doivent avoir reçu une ou deux lignes de chimiothérapie auparavant. La chimiothérapie de maintenance n’est pas considérée comme une ligne de traitement. La chimiothérapie adjuvante n’est pas considérée comme une première ligne de traitement si elle date de plus de un an.

    Analyse de la toxicité de l’association EGFR-TKI et fulvestrant :
    Les toxicités seront évaluées dès le début du traitement selon l’échelle NCI-CTC v4.0. Leur causalité sera recueillie en détail dans le CRF. Le nombre de patients pour lesquels au moins un évènement indésirable aura été reporté, sera présenté, ainsi que le nombre d’évènements lui-même, selon la relation au traitement, l’intensité, et le cycle d’apparition.
    Analyse du taux de réponse :
    Les malades sont évaluables pour la réponse après deux cycles. La réponse sera évaluée par mesure planimétrique des cibles unidimensionnelles selon les critères RECIST (version 1.1 Eur J Cancer 2009;45:228-247) à chaque évaluation.
    Analyse de la survie globale :
    La survie globale est définie comme le délai entre la date d’inclusion et le décès toute cause. Elle sera calculée selon la méthode de Kaplan-Meier. Les taux de survie globale à 6 mois, 9, 12 et 18 mois, ainsi que la médiane de survie et leur intervalle de confiance à 95% seront calculés.
    Analyse de la qualité de vie EORTC-QLQ30 et LCSS :
    Le questionnaire EORTC QLQ C30 comprend 30 questions explorant 15 domaines (physique, social, fonctionnement personnel et psychologique, état de santé global, fatigue, nausées, vomissements, douleur, dyspnée, insomnie, appétit, constipation, diarrhée et problèmes financiers). L’EORTC QLQ C30 est donc un questionnaire qui prend largement en compte les symptômes (9 échelles symptomatiques) par rapport aux fonctions (5 échelles symptomatiques). Le module poumon de l’EORTC QLQ, Lung Cancer 13 (LC13), compte 13 questions, explorant notamment l’hémoptysie, la dyspnée et la toux.
    Le questionnaire LCSS évalue six symptômes majeurs du cancer du poumon et leurs effets sur la détresse symptomatique, sur les activités fonctionnelles et sur la qualité de vie globale.
    Les questionnaires seront réalisés tous les 2 cycles concomitamment aux évaluations tumorales.
