1. Patient is 60-years of age or older at time of randomization 2. Histologically confirmed Ann Arbor stage II, III, or IV DLBCL; or FL Grade 3B according to the WHO classification 2008 (from initial diagnosis made prior to starting R-chemotherapy). 3. An H&E stained slide and unstained slides must be available for confirmatory pathology review, as per the separate Pathology Guidance document. Patients may be randomized based on the local diagnosis.. 4. Presence of at least one IPI risk factor. The aaIPI is defined by one point for each factor: a. LDH > upper limit of normal (ULN); b. Stage III or IV; and c. WHO/ECOG performance status >1. 5. First-line treatment must have been 6 cycles of standard R-CHOP or R-CHOP-like chemotherapy (e.g. R-CHOP21, R-CHOP14, or DA-EPOCH-R). Patients who received pre-phase therapy for the purpose of improving performance status prior to initiating R-CHOP are eligible. Standard dose reductions for toxicity are allowed. 6. Complete remission (CR) according to the Revised Response Criteria for Malignant Lymphoma [22] (Appendix 2) after first-line treatment. a. Diagnostic CT scans with contrast of chest, abdomen, and pelvis must have been performed within 8 weeks after the first dose of the last cycle of R chemotherapy. PET-CTs obtained elsewhere before and after R-CHOP are acceptable for evaluating response to R-CHOP, but a diagnostic CT pre-randomization is requested as reference for post randomization interval change. A neck CT will be applicable if the patient had involvement of the neck region by palpation / physical examination at initial diagnosis. The CT portion of an FDG PET that includes the neck will be acceptable if the neck had involvement. b. A negative FDG-PET scan performed within 8 weeks after the first dose of the last cycle of R-chemotherapy and confirming CR, with negative defined as a score of 1-3 on the Deauville 5-point scale (Appendix 3) used to quantify radionucleotide density in PET scans as determined locally [23]. PET positive/indeterminate lesions which are confirmed on biopsy to harbor no active lymphoma will be considered negative for determination of CR status. c. If positive bone marrow involvement at initial diagnosis the patient must have a negative bone marrow biopsy following R-chemotherapy to confirm the CR. 7. WHO/ECOG performance status of 0, 1 or 2. 8. Adequate hematopoietic functions unsupported by transfusion within the last 2 weeks: Absolute neutrophil count (ANC) ? 1.5 x 109/L, Hemoglobin (Hgb) ? 9 g/dL, Platelets ? 100 x 109/L. Patients with blood counts close to recovery towards these values after R-CHOP should be discussed with the study Medical Monitor prior to randomization, but blood counts must have met these thresholds prior to treatment with Y-90 Zevalin. 9. In patients who had a post R-chemotherapy bone marrow biopsy performed, the marrow must show cellularity > 15%. For patients without a post R-chemotherapy bone marrow biopsy (i.e. those patients with negative marrow at diagnosis), a repeat biopsy to assess bone marrow cellularity of > 15% will be required only for patients randomized to the Zevalin Regimen. 10. Life expectancy of 6 months or longer. 11. Written informed consent obtained according to local guidelines. |
1. El paciente tiene 60 años o más en el momento de la aleatorización. 2. LDCBG en estadio II, III o IV de la clasificación de Ann Arbor histológicamente confirmado; o LF de grado 3B de acuerdo con la clasificación de 2008 de la OMS (desde el diagnóstico inicial hecho antes de comenzar la quimioterapia con R). 3. Se debe disponer de un corte con tinción de hematoxilina y eosina y de cortes sin tinción para una revisión confirmatoria por anatomía patológica, tal y como se indica en el documento orientativo de patología aparte. Se podrá aleatorizar a los pacientes en base al diagnóstico local. 4. Presencia de al menos un factor de riesgo del índice pronóstico internacional (IPI). Se define el IPI ajustado a la edad sumando un punto para cada factor: a. LDH > límite superior de la normalidad (LSN); b. Estadio III o IV; y c. Estado funcional > 1 de OMS/ECOG. 5. El tratamiento de primera línea debe haber consistido en 6 ciclos de quimioterapia estándar con R-CHOP o similar a R-CHOP (por ej.: R-CHOP21, R-CHOP14 o DA-EPOCH-R). Los pacientes que recibieron tratamiento previo a la fase con el fin de mejorar el estado funcional antes de iniciar R-CHOP son elegibles. Se permiten realizar reducciones estándar de la dosis por motivos de toxicidad. 6. Remisión completa (RC) de acuerdo con los criterios revisados de respuesta para el linfoma maligno [22] (Apéndice 2) después del tratamiento de primera línea. a. Las TC diagnósticas con contraste de tórax, abdomen y pelvis se deben haber realizado en las 8 semanas siguientes a la primera dosis del último ciclo de la quimioterapia con R. Las PET-TC obtenidas en otro lugar antes y después del tratamiento con R-CHOP son aceptables para evaluar la respuesta a R-CHOP; sin embargo, es necesario realizar una TC diagnóstica antes de la aleatorización como referencia para el cambio en el intervalo posterior a la aleatorización. Procederá una TC de cuello si el paciente tiene afectación de la región del cuello mediante palpación/exploración física en el diagnóstico inicial. La parte de la TC de una FDG-PET que incluya el cuello será aceptable si había afectación en el cuello. b. Una FDG-PET negativa realizada en las 8 semanas siguientes a la primera dosis del último ciclo de la quimioterapia con R y que confirme la RC, definiéndose negativa como una puntuación de 1-3 en la escala de 5 puntos de Deauville (Apéndice 3) utilizada para confirmar la densidad de los radionucleótidos de las PET de la forma determinada localmente [23]. Las lesiones indeterminadas/positivas con PET que mediante biopsia se confirma que no contienen linfoma activo se considerarán negativas para la determinación del estado de la RC. c. Si hay afectación de médula ósea positiva en el diagnóstico inicial, el paciente debe tener un resultado negativo en la biopsia de médula ósea después de la quimioterapia con R para confirmar la RC. 7. Estado funcional de OMS/ECOG de 0, 1 o 2. 8. Funciones hematopoyéticas adecuadas sin ayuda de transfusiones en las últimas 2 semanas: recuento absoluto de neutrófilos (RAN) ? 1,5 x 109/l, hemoglobina (Hgb) ? 9 g/dl, plaquetas ? 100 x 109/l. En el caso de los pacientes con recuentos sanguíneos cercanos a la recuperación de estos valores después de R-CHOP, se deberá consultar con el monitor médico del estudio antes de la aleatorización, aunque los recuentos sanguíneos deben estar dentro de estos intervalos antes del tratamiento con Y 90 Zevalin. 9. En los pacientes que se sometieron a una biopsia de médula ósea después de la quimioterapia con R, la médula debe mostrar una celularidad > 15%. En los pacientes que no se sometieron a una biopsia de médula ósea después de la quimioterapia con R (es decir, los pacientes con medula negativa en el diagnóstico), se requerirá otra biopsia para evaluar la celularidad de la médula ósea de > 15% únicamente en los pacientes aleatorizados al régimen de Zevalin. 10. Esperanza de vida de 6 meses o mayor. 11. Obtención del documento de consentimiento informado conforme a las directrices locales. |

E.4 | Principal exclusion criteria |
1. Presence of any other malignancy or history of prior malignancy within 5 years of study entry. Within 5 years, patients treated with curative intent for Stage I or II cancers are eligible provided they have a life expectancy of > 5 years. The 5-year exclusion rule does not apply to-non melanoma skin tumors and in situ cervical cancer. 2. Prior radioimmunotherapy, including radiation therapy for NHL, or any other NHL therapy. 3. Presence of central nervous system (CNS) involvement, or testicular lymphoma at first diagnosis. 4. DLBCLas histological transformation of previously diagnosed indolent B-cell lymphoma. Patients with De Novo Transformed DLBCL, defined as DLBCL on lymph node biopsy and a ?discordant marrow? with small cells at initial diagnosis, are eligible. 5. Known seropositivity for hepatitis C virus (HCV) or hepatitis B surface antigen (HbsAg). Patients who are positive for HbsAg but without active disease (Hep B PCR below the limits of detection) and who receive adequate prophylaxis may be enrolled, but should continue prophylaxis for at least 6 months after the last dose of rituximab or Zevalin. 6. Known history of HIV infection. 7. Abnormal liver function: total bilirubin > 2 × ULN unless secondary to Gilbert disease. 8. Abnormal renal function: serum creatinine > 2.0 × ULN. 9. Ongoing toxic effects of chemotherapy > grade 2 and expected to interfere with Zevalin treatment. 10. Known hypersensitivity to murine or chimeric antibodies or proteins. 11. Colony stimulating factor therapy administered more than 8 weeks after last dose of R-chemotherapy or within 4 weeks prior to planned administration of Zevalin. 12. Concurrent severe and/or medically uncontrolled disease (e.g. uncontrolled diabetes, congestive heart failure, myocardial infarction within 6 months of study, unstable and uncontrolled hypertension, chronic renal disease, or active uncontrolled infection) which could compromise participation in the study. 13. Treatment with investigational drugs less than 4 weeks prior to randomization. 14. Major surgery less than 4 weeks prior to randomization. 15. Concurrent systemic corticosteroid use for any reason except as premedication in case of known or suspected allergies to contrast media or as premedication for potential side effects of rituximab treatment. Patients on a chronic dose of prednisone for a medical condition (e.g. Asthma or autoimmune disease) less than or equal to 20mg daily, stable for 4 weeks, are permissible. 16. Unwillingness or inability to comply with the protocol. 17. Pregnant women or women who are breastfeeding |
1. Presencia de alguna otra neoplasia maligna o antecedentes de una neoplasia maligna previa en los 5 años anteriores a entrar en el estudio. Dentro de este periodo de 5 años, los pacientes con tumores en estadio I o II tratados con intención curativa son elegibles si tienen una esperanza de vida > 5 años. La regla de exclusión de 5 años no es aplicable en los tumores de piel del tipo no melanoma ni en el cáncer de cuello uterino in situ. 2. Radioinmunoterapia previa, que incluye radioterapia para el LNH o cualquier otra terapia para el LNH. 3. Presencia de afectación del sistema nervioso central (SNC) o linfoma testicular en el primer diagnóstico. 4. LDCBG como transformación histológica del linfoma de células B indolente diagnosticado previamente. Son elegibles los pacientes con un LDCBG transformado de novo, definido como un LDCBG en la biopsia de los ganglios linfáticos y una ?médula discordante? con células pequeñas en el diagnóstico inicial. 5. Seropositividad conocida para el virus de la hepatitis C (VHC) o el antígeno de superficie de la hepatitis B (HbsAg). Se podrá inscribir a los pacientes que den positivo a HbsAg pero que no tengan enfermedad activa (PCR de hepatitis B por debajo de los límites de detección) y que reciban tratamiento profiláctico adecuado, aunque deberán continuar con el tratamiento profiláctico durante al menos 6 meses después de la última dosis de rituximab o Zevalin. 6. Historia conocida de infección por VIH. 7. Función hepática anómala: bilirrubina total > 2 × LSN a menos que sea secundaria a la enfermedad de Gilbert. 8. Función renal anómala: creatinina sérica > 2,0 × LSN. 9. Efectos tóxicos presentes de la quimioterapia > grado 2 y que se espera que interfieran con el tratamiento con Zevalin. 10. Hipersensibilidad conocida a los anticuerpos o proteínas murinos o quiméricos. 11. Administración de tratamiento con factor estimulante de colonias más de 8 semanas después de la última dosis de la quimioterapia con R o en las 4 semanas anteriores a la administración prevista de Zevalin. 12.
