La téicoplanine administrée par voie sous cutanée, dans le cadre de la prise en charge des infections ostéo-articulaires, présente de nombreux avantages : elle préserve le capital veineux, réduit les risques de complications liés à l’utilisation d’un dispositif implantable, améliore le confort du patient, facilite la prise en charge hors milieu hospitalier et réduit les coûts de santé.

L’objectif principal de cette étude est d’évaluer la tolérance générale et locale (au site d’injection) à la téicoplanine administrée par voie sous-cutanée dans le cadre de la prise en charge des infections ostéo-articulaires. |

Les objectifs secondaires sont les suivants :
• Comparer les taux résiduels de téicoplanine entre la voie IV et SC (plus exactement le dernier taux sous administration IV et le premier taux de la voie SC).
• Rechercher les facteurs explicatifs de variation de posologie (modification du poids, lipodystrophie…).

E.3 | Principal inclusion criteria |
• Patient de plus de 18 ans
• Atteint d’une infection ostéo-articulaire documentée à bactérie gram + (staphylocoques doré, staphylocoques à coagulase négative, enterocoques, streptocoques)
• Bactérie résistante aux lincosamides, aux quinolones et à la rifampicine
• Bactérie sensible à la téicoplanine (CMI ≤ 4mg/L)
• Patient équilibré sous téicoplanine administrée par voie IV (2 dosages successifs entre 30 et 40 mg/mL)

• Patient non affilié à la sécurité sociale
• Femmes enceintes et allaitantes
• Antécédents d’hypersensibilité à la téicoplanine (éruption cutanée,…)
• Patient ayant un cathéter central ou une chambre implantable
• Patient hémodialysé
• Patient participant a une autre recherche biomédicale portant sur un médicament afin d’éviter les risques d’interactions médicamenteuses
• Patient mineur
• Patient majeur protégé (sauvegarde de justice, curatelle, tutelle)
• Patient admis en situation d’urgence ou hors d’état de consentir

Les fréquences des effets indésirables au site d’injection SC seront mesurées afin d’évaluer la tolérance locale de la téicoplanine par voie SC.
Chaque patient devra remplir un questionnaire d’auto-évaluation de la tolérance avant chaque consultation.
A chaque consultation, le médecin investigateur remplira un questionnaire d’évaluation de la tolérance locale et générale.

• Comparer les taux résiduels de téicoplanine entre la voie IV et SC
o Par comparaison du dernier taux résiduel sous administration IV (à la posologie d’entretien) avec le premier taux résiduel de la voie SC.
• Modification de la biodisponibilité :
o Pour cela nous allons étudier les variations de posologie nécessaires au maintient des Cmin dans la cible thérapeutique chez les patients dont la clairance de la créatinine est restée stable tout au long du traitement (variation de la clairance de la créatine à ± 10 mL/min).
