Phase I
The primary objective of the Phase I portion of the study is to determine the MTD of the combination of melflufen and dexamethasone in patients with relapsed and/or relapsed-refractory multiple myeloma.

E.3 | Principal inclusion criteria |
Eligible patients will be considered for inclusion in this study if they meet all of the following criteria:
1. Male or female, age 18 years or older.
2. Patient has a diagnosis of multiple myeloma with documented relapsed and/or relapsed-refractory disease.
3. Patient has measurable disease defined as any of the following:
• Serum monoclonal protein ≥ 0.5 g/dL by protein electrophoresis.
• ≥200 mg of monoclonal protein in the urine on 24-hour electrophoresis
• Serum immunoglobulin free light chain ≥10 mg/dL AND abnormal serum immunoglobulin kappa to lambda free light chain ratio.
• If no monoclonal protein is detected (non-secretory disease), then ≥ 30% monoclonal bone marrow plasma cells.
4. Patient has had at least 2 or more prior lines of therapy. (see appendix D for the definition of lines of therapy);
5. Life expectancy of ≥6 months;
6. Patient has an ECOG performance status ≤ 2. (Patients with lower performance status based solely on bone pain secondary to multiple myeloma will be eligible);
7. Females of childbearing potential (FCBP)† must have a negative serum or urine pregnancy test prior to starting therapy;
8. Female patients of child bearing potential and non-vasectomized male patients agree to practice appropriate methods of birth control;
9. Ability to understand the purpose and risks of the study and provide signed and dated informed consent and authorization to use protected health information;
10. The patient has or, accepts to have, an acceptable infusion device for infusion of melflufen (see Study Reference Manual for acceptable devices);
11. 12 lead ECG with QTcF interval of ≤470 msec;
12. The following laboratory results must be met within 21 days, or as specified in the table of assessments, of patient registration:
• Absolute neutrophil count (ANC) ≥ 1,000 cells/mm3 (1.0 x 109/L) (Growth factors cannot be used within 14 days before patient registration).
• Platelet count ≥ 75,000 cells/ mm3 (75 x 109/L) (platelet count ≥ 50,000 cells/ mm3 for patients in whom ≥ 50% of bone marrow nucleated cells are plasma cells (without transfusion during the previous 14 days to patient registration).
• Hemoglobin ≥ 8.0 g/dL (RBC transfusions are permitted)
• Total Bilirubin ≤ 1.5 X upper limit of normal (ULN);
• Renal function: Estimated creatinine clearance ≥ 45 ml/min and/or serum creatinine ≤ 2.0 mg/dL;
• AST (SGOT) and ALT (SGPT) ≤ 3.0 x ULN.
† (FCBP) is any sexually mature female who: 1) has not undergone a hysterectomy or bilateral oophorectomy or 2) has not been naturally postmenopausal (not having menstrual cycles due to cancer therapy does not rule out childbearing potential) for at least 24 consecutive months.

1. Maschio o femmina di almeno 18 anni.
2. Paziente con una diagnosi di mieloma multiplo con malattia recidivante e/o refrattaria documentata.
3. Il/la paziente ha una malattia misurabile, definita come:
• proteina monoclonale sierica > 0,5 g/dl mediante elettroforesi proteica;
• oltre 200 mg di proteina monoclonale nelle urine nell’elettroforesi delle 24 ore;
• catena leggera libera delle immunoglobuline sieriche > 10 mg/dl E rapporto anomalo kappa-lambda della catena libera delle immunoglobuline sieriche;
• se non viene rilevata alcuna proteina monoclonale (malattia non secretoria), > 30% di cellule plasmatiche del midollo spinale monoclonali.
4. Il paziente è stato sottoposto ad almeno 2 linee terapeutiche precedenti (v. l’appendice D per la definizione delle linee terapeutiche);
5. Aspettativa di vita ≥ 6 mesi.
6. Il/la paziente ha uno stato prestazionale ECOG ≤ 2 (saranno idonei i pazienti con uno stato prestazionale inferiore basato unicamente sul dolore osseo secondario al mieloma multiplo).
7. Le donne in età fertile (females of childbearing potential, FCBP)† devono avere un test di gravidanza sul siero o sulle urine negativo prima di iniziare la terapia.
8. Le pazienti in età fertile e i pazienti non vasectomizzati acconsentono a praticare dei metodi contraccettivi adeguati.
9. Capacità di capire lo scopo e i rischi dello studio e di fornire un consenso informato datato e firmato e autorizzato a utilizzare le informazioni sanitarie protette.
10. Il/la paziente ha o accetta di avere un dispositivo adeguato per l’infusione di melflufen (consultare il manuale di riferimento dello studio per i dispositivi accettabili).
11. ECG a 12 derivazioni con un intervallo QTcF ≤ 470 msec.
12. I seguenti risultati di laboratorio devono essere raggiunti entro 21 giorni dalla registrazione del paziente oppure secondo quanto specificato nella tabella delle valutazioni:
• conta assoluta dei neutrofili (absolute neutrophil count, ANC) ≥ 1000 cellule/dl (1,0 x 109/l) (i fattori di crescita non possono essere utilizzati nei 14 giorni precedenti alla registrazione del paziente);
• conta piastrinica ≥ 75000 cellule/dl (75 x 109/l) (conta piastrinica ≥ 50000 cellule/dl per i pazienti ≥ 50% delle cellule nucleate del midollo osseo sono cellule plasmatiche (senza trasfusione nei 14 giorni precedenti alla registrazione del paziente);
• emoglobina ≥ 8,0 g/dl (sono consentite le trasfusioni di globuli rossi);
• bilirubina totale ≤ 1,5 X il limite superiore al normale (upper limit of normal, ULN);
• funzione renale: clearance della creatinina stimata ≥ 30 ml/min o creatinina sierica ≤ 2,5 mg/dl;
• AST (SGOT) e ALT (SGPT) ≤ 3,0 x ULN.
† Per (FCBP) si intende qualsiasi donna che abbia raggiunto la maturità sessuale e che: 1) non sia stata sottoposta a isterectomia o a ooforectomia bilaterale oppure 2) che non sia in menopausa naturale (chi non ha il ciclo mestruale a causa della terapia antitumorale resta comunque potenzialmente fertile) da almeno 24 mesi consecutivi.

E.4 | Principal exclusion criteria |
Patients will be ineligible for this study if they meet any one of the following criteria:
1. Patient has evidence of mucosal or internal bleeding and/or platelet transfusion refractory (i.e., unable to maintain a platelet count ≥50,000 cells/mm3);
2. Any medical conditions that, in the Investigator’s opinion, would impose excessive risk to the patient or would adversely affect his/her participating in this study. Examples of such conditions are: a significant history of cardiovascular disease (e.g., myocardial infarction, significant conduction system abnormalities, uncontrolled hypertension, ≥ grade 3 thromboembolic event in the last 6 months), renal insufficiency (unless felt to be secondary to MM);
3. Known active infection requiring parenteral or oral anti-infective treatment;
4. Other malignancy within the past 3 years with the exception of adequately treated basal cell carcinoma, squamous cell skin cancer, carcinoma in-situ of the cervix;
5. Other ongoing anti-myeloma therapy. Patients may be receiving concomitant therapy with bisphosphonates and low dose corticosteroids (e.g., prednisone up to but no more than 10 mg PO q.d. or its equivalent) for symptom management and comorbid conditions. Doses of corticosteroid should be stable for at least 7 days prior to study treatment;
6. Pregnant or breast-feeding females;
7. Serious psychiatric illness, active alcoholism, or drug addiction that may hinder or confuse compliance or follow-up evaluation;
8. Known HIV or hepatitis B or C viral infection;
9. Patient has concurrent symptomatic amyloidosis or plasma cell leukemia;
10. POEMS syndrome (plasma cell dyscrasia with polyneuropathy, organomegaly, endocrinopathy, monoclonal protein and skin changes);
11. Previous cytotoxic therapies, including cytotoxic investigational agents, for multiple myeloma within 3 weeks (6 weeks for nitrosoureas) prior to patient registration. Biologic, novel therapy (including investigational agents in this class) or corticosteroids within 2 weeks prior to start of patient registration. Patient has side effects of the previous therapy > grade 1 or previous baseline.
12. Prior peripheral stem cell transplant within 12 weeks of patient registration;
13. Radiotherapy within 21 days prior to patient registration. However, if the radiation portal covered ≤ 5% of the bone marrow reserve, the patient may be enrolled irrespective of the end date of radiotherapy;
14. Known intolerance to steroid therapy. |
1. Il/la paziente presenta evidenza di emorragia mucosale o interna e/o è refrattario/a alle trasfusioni piastriniche (ovvero non in grado di mantenere una conta piastrinica ≥ 50.000 cellule/mm3).
2. Qualsiasi condizione medica che, a parere dello sperimentatore, costituirebbe un rischio eccessivo per il/la paziente o che inciderebbe negativamente sulla sua partecipazione allo studio, quali ad esempio: anamnesi significativa di malattia cardiovascolare (ad es. infarto miocardico, significative anomalie del sistema di conduzione, ipertensione incontrollata, eventi trombolitici di grado ≥ 3 negli ultimi 6 mesi), insufficienza renale (a meno che non sia secondaria al MM).
3. Infezione attiva nota che richiede una terapia antinfettiva attiva per via parenterale o orale.
4. Altri tumori maligni nel corso degli ultimi 3 anni, ad eccezione di carcinoma cellulare basale, tumore cutaneo a cellule squamose e carcinoma della cervice in-situ trattati adeguatamente.
5. Somministrazione contemporanea di altre terapie antimieloma. I pazienti possono ricevere una terapia concomitante con bifosfonati e dosi basse di corticosteroidi (ad es. fino a un massimo di 10 mg al giorno di prednisone o suo equivalente assunto oralmente) per la gestione dei sintomi e per le condizioni di comorbilità. Le dosi di corticosteroidi dovrebbero essere stabili da almeno 7 giorni prima della somministrazione del trattamento dello studio.
6. Donne in gravidanza o in allattamento.
7. Grave patologia psichiatrica, alcolismo attivo o tossicodipendenza che potrebbero impedire o confondere la conformità o la valutazione di follow up.
8. Infezione virale da epatite B o C o HIV nota.
9. Il/la paziente è affetto/a contemporaneamente da amiloidosi sintomatica o leucemia cellulare plasmatica.
10. Sindrome POEMS (discrasia plasma cellulare con polineuropatia, organomegalia, endocrinopatia, picco monoclonale e alterazioni cutanee).
11. Terapie citotossiche precedenti, compresi agenti investigativi citotossici, per il mieloma multiplo nelle 3 settimane (6 settimane in caso di nitrosurea) precedenti alla registrazione del/la paziente. Terapia biologica, nuova (compresi gli agenti investigativi di questa classe) oppure corticosteroidi nelle 2 settimane precedenti all’avvio della registrazione del/la paziente. Il/la paziente manifesta degli effetti collaterali causati dalla terapia precedente > grado 1 oppure al basale precedente.
12. Trapianto precedente di cellule staminali periferiche avvenuto nelle 12 settimane precedenti o successive alla registrazione del/la paziente.
13. Radioterapia nei 21 giorni precedenti alla registrazione del/la paziente. Tuttavia se il portale di radiazione ha coperto ≤ 5% della riserva di midollo osseo, il/la paziente può essere arruolato/a a prescindere dalla data del termine della radioterapia;
