In pazienti affetti da acromegalia:
•Valutare la sicurezza e la tollerabilità di CAM2029.
•Confrontare l'effetto del trattamento e dell'esposizione a CAM2029 sul tasso di risposta del fattore di crescita insulino-simile 1 (insulin-like growth factor 1, IGF-1) al Giorno 84 rispetto al trattamento basale con Sandostatina LAR (ossia normalizzazione dell'IGF-1 corretta per l'età e il sesso di appartenenza).
•Confrontare l'effetto del trattamento e dell'esposizione a CAM2029 sulla riduzione del livello medio di ormone della crescita (growth hormone, GH) a meno di 2,5 µg/l al Giorno 84 rispetto al trattamento basale con Sandostatina LAR.

Inclusion criteria for patients with Acromegaly;
1.Patients from whom written informed consent to participate in the study has been obtained prior to any screening procedures.
2.Male or female patients ≥18 years of age.
3.Patients with acromegaly currently being treated with no medical treatment for acromegaly other than Sandostatin LAR.
4.Patients currently receiving Sandostatin LAR at a stable dose of 10 mg, 20 mg or 30 mg i.m. given every 28 days for at least 2 months before the start of the Last Sandostatin LAR Dose Assessment Phase (Day -28)
Inclusion criteria for patients with NETs;
1.Patients from whom written informed consent to participate in the study has been obtained prior to any screening procedures.
2.Male or female patients ≥18 years of age.
3.Patients with functional, well-differentiated (G1 or G2) neuroendocrine tumour (NET) with symptoms of carcinoid (bowel movements and/or flushing).
4.Patients currently receiving medical treatment with Sandostatin LAR for symptom control at stable doses of 10 mg, 20 mg or 30 mg i.m. given every 28 days for at least 2 months before the start of the Last Sandostatin LAR Dose Assessment Phase (Day -28).
5.Patients with controlled symptoms, i.e., who do not require rescue medication with Sandostatin IR s.c. during Screening.
6.Patients not on treatment with any other agents for the treatment of NET (e.g., interferon, chemotherapy, radiotherapy, targeted agents).

Criteri di inclusione per i pazienti affetti da acromegalia:
1.Pazienti per i quali è stato ottenuto il consenso informato scritto alla partecipazione allo studio prima di qualsiasi procedura di screening.
2.Pazienti di sesso maschile o femminile di età ≥18 anni.
3.Pazienti affetti da acromegalia attualmente non sottoposti ad alcun trattamento medico per l'acromegalia ad eccezione di Sandostatina LAR.
4.Pazienti attualmente sottoposti a trattamento medico con Sandostatina LAR a una dose stabile di 10 mg, 20 mg o 30 mg somministrata per via intramuscolare (im) ogni 28 giorni per almeno 2 mesi prima dell'inizio della fase di valutazione dell'ultima dose di Sandostatina LAR (Giorno -28).
Criteri di inclusione per i pazienti affetti da NET:
1.Pazienti per i quali è stato ottenuto il consenso informato scritto alla partecipazione allo studio prima di qualsiasi procedura di screening
2.Pazienti di sesso maschile o femminile di età ≥18 anni
3.Pazienti affetti da NET funzionante e ben differenziato (di Grado 1 o 2) con sintomi di sindrome da carcinoide (numero di evacuazioni e/o vampate).
4.Pazienti attualmente sottoposti a trattamento medico con Sandostatina LAR per il controllo dei sintomi a dosi stabili di 10 mg, 20 mg o 30 mg somministrate per via intramuscolare ogni 28 giorni per almeno 2 mesi prima dell'inizio della fase di valutazione dell'ultima dose di Sandostatina LAR (Giorno -28).
5.Pazienti con sintomi controllati, ossia che non necessitano del farmaco di soccorso Sandostatina IR per via sottocutenea durante lo screening.
Nota: i pazienti arruolati a cui viene somministrato il farmaco di soccorso Sandostatina IR per via sottocutenea durante il Periodo 0 devono interrompere anticipatamente lo studio.
6.Pazienti non in terapia con altri agenti per il trattamento dei NET (ad es. interferone, chemioterapia, radioterapia o agenti mirati).

Exclusion criteria for patients with Acromegaly:
1.Patients with inadequate bone marrow function as determined by WBC <3.0 × 109/L, and/or Platelets <100 × 109/L, and/ or Hgb <90% LLN.
2.Patients with abnormal coagulation (prothrombin time [PT] and/ or partial thromboplastin time [PTT] elevated by ≥30% above normal limits).
3.Patients with impaired liver function as determined by ALT and/ or AST more than 2 × upper limit of normal (ULN), total serum bilirubin >2.0 × ULN, or serum albumin <0.67 × lower limit of normal (LLN). Liver disease or history of liver disease
4.Patients with congestive heart failure, unstable angina, sustained ventricular tachycardia, ventricular fibrillation, clinically significant bradycardia, advanced heart block, or a history of acute myocardial infarction. Patients with a screening or baseline (Days -28 to -1) QTcF >450 msec for males, and QTcF >460 msec for females, and patients with sustained or clinically significant cardiac arrhythmias.
5.Patients with risk factors for Torsades de Pointes such as hypokalaemia, hypocalcaemia and hypomagnesemia, cardiac failure, clinically significant/ symptomatic bradycardia, and taking concomitant medication(s) known to prolong the QT interval.
6.Patients with impaired renal function as determined by serum creatinine >1.5 x ULN at screening.
7.Diabetic patients whose blood glucose is poorly controlled despite adequate therapy, as evidenced by glycosylated haemoglobin (HbA1C) >8.0% at screening.
8.Patients who require a surgical intervention for relief of any sign or symptom associated with tumour compression. Patients with compression of the optic chiasm causing acute clinically significant visual field defects.
9.Patients who have undergone major surgery/surgical therapy within 2 months before randomisation, or patients who are scheduled for any surgery within 6 months of starting this study.
10.Patients with active malignant disease within the last 5 years (with the exception of basal cell carcinoma or carcinoma in situ of the cervix).
Exclusion criteria for patients with NET:
1.Patients with poorly differentiated neuroendocrine carcinoma, high-grade neuroendocrine carcinoma, adenocarcinoid, pancreatic islet cell carcinoma, insulinoma, glucagonoma, gastrinoma, goblet cell carcinoid, typical and atypical lung carcinoids, large cell neuroendocrine carcinoma and small cell carcinoma.
2.Patients with carcinoid syndrome refractory to treatment with conventional doses of SSAs.
3.Patients with inadequate bone marrow function as determined by ANC <1.5 × 109/L, and /or WBC <3.0 × 109/L, and/or Platelets <100 × 109/L, and/ or Hgb <9 g/dL.
4.Patients with abnormal coagulation (prothrombin time [PT] and/ or partial thromboplastin time [PTT] elevated by ≥30% above normal limits).
5.Patients with impaired liver function as determined by ALT and/ or AST >2 × upper limit of normal (ULN) without liver metastases, or > 5 x ULN if documented liver metastases; total serum bilirubin >2.0 × ULN, or serum albumin <0.67 × lower limit of normal (LLN). Liver disease or history of liver disease such as cirrhosis, chronic active hepatitis B and C or chronic persistent hepatitis.
6.Patients with congestive heart failure, unstable angina, sustained ventricular tachycardia, ventricular fibrillation, clinically significant bradycardia, advanced heart block, or a history of acute myocardial infarction. Patients with a screening or baseline (Days -28 to -1) QTcF >450 msec for males, and QTcF >460 msec for females, and patients with sustained or clinically significant cardiac arrhythmias.

11.Patients who have undergone major surgery/surgical therapy within 2 months before randomisation, or patients who are scheduled for any surgery within 6 months of starting this study. Patients who have not recovered from any adverse effects from surgery/radiotherapy.
12.History of hepatic embolisation, hepatic arterial chemoembolisation, and/or selective internal radiation therapy (e.g., SIR-Spheres) within less than 6 months prior to Screening. Scheduled hepatic embolisation, hepatic arterial chemoembolisation, and/or selective internal radiation therapy (e.g., SIR-Spheres) within 6 months of starting this study.
13.Patients with additional active malignant disease within the last 5 years (with the exception of basal cell carcinoma or carcinoma in situ of the cervix).

Criteri di esclusione per i pazienti affetti da acromegalia:
1.Pazienti con funzionalità midollare inadeguata, stabilita come segue:

2.Pazienti con coagulazione anomala (aumento del tempo di protrombina [prothrombin time, PT] e/o del tempo di tromboplastina parziale [partial thromboplastin time, PTT] ≥30% rispetto ai limiti della norma) o pazienti in trattamento cronico con warfarin o derivati cumarinici
3.Pazienti con compromissione della funzionalità epatica, determinata in base ad uno dei seguenti fattori:
a.ALT e/o AST superiori a 2 × limite superiore della norma (upper limit of normal, ULN), bilirubina sierica totale >2,0 × ULN o albumina sierica <0,67 × LLN
b.Malattia epatica o anamnesi positiva per malattia epatica quale cirrosi, epatite B e C cronica attiva o epatite cronica persistente
c.Presenza dell'antigene di superficie dell'epatite B (hepatitis B surface antigen, HbsAg).
d.Presenza di test positivo agli anticorpi dell'epatite C (anti-hepatitis C virus, anti-HCV).
4.Pazienti con compromissione della funzionalità cardiaca, determinata in base ad uno dei seguenti fattori:
a.Insufficienza cardiaca congestizia (Classe III o IV della New York Heart Association [NYHA]), angina instabile, tachicardia ventricolare sostenuta, fibrillazione ventricolare, bradicardia clinicamente significativa, blocco cardiaco avanzato o anamnesi positiva per infarto miocardico acuto nelle 6 settimane precedenti lo screening
b.Intervallo QTcF allo screening o al basale (dal Giorno -28 al Giorno -1) >450 msec per gli uomini e >460 msec per le donne.
c.Anamnesi positiva per sincope, anamnesi familiare positiva per morte improvvisa idiopatica, anamnesi familiare positiva per sindrome congenita del QT lungo o diagnosi di sindrome congenita del QT lungo
d.Aritmie cardiache sostenute o clinicamente significative
e.Fattori di rischio per la torsione di punta quali ipopotassiemia, ipocalcemia e ipomagnesemia (salvo nel caso in cui tali condizioni siano corrette con adeguata supplementazione continua prima e durante lo studio), insufficienza cardiaca, bradicardia clinicamente significativa/sintomatica o blocco AV >1
f.Malattia/e concomitante/i che potrebbero determinare un prolungamento dell'intervallo QT quali neuropatia autonomica (causata dal diabete o dal morbo di Parkinson), virus dell'immunodeficienza umana (HIV) e cirrosi
g.Medicinale/i concomitante/i che notoriamente prolungano l'intervallo QT
h.Uno qualsiasi dei seguenti eventi nei 6 mesi precedenti l'inizio del trattamento in studio: infarto miocardico (IM), angina pectoris, innesto di bypass aorto-coronarico (coronary artery bypass graft, CABG), accidente cerebrovascolare (cerebrovascular accident, CVA) o attacco ischemico transitorio (transient ischemic attack, TIA)
5.Pazienti con compromissione della funzionalità renale, determinata da creatinina sierica >1,5 × ULN allo screening
6.Pazienti con malattia nota della colecisti o delle vie biliari, o con pancreatite acuta o cronica (i pazienti con colelitiasi asintomatica e dilatazione asintomatica delle vie biliari possono essere inclusi nello studio)
7.Pazienti diabetici con glicemia scarsamente controllata nonostante un'adeguata terapia, come evidenziato da valori di emoglobina glicata
Criteri di esclusione per i pazienti affetti da NET
1.Pazienti con carcinoma neuroendocrino scarsamente differenziato, carcinoma neuroendocrino ad alto grado di malignità, adenocarcinoide, carcinoma delle isole pancreatiche, insulinoma, glucagonoma, gastrinoma, carcinoide a cellule caliciformi, carcinoidi polmonari tipici e atipici, carcinoma neuroendocrino a grandi cellule e carcinoma a piccole cellule
2.Pazienti con sindrome da carcinoide refrattaria al trattamento a dosi convenzionali di analoghi della somatostatina (somatostatin analogues, SSA).
3.Pazienti con funzionalità midollare inadeguata, stabilita come segue:
a.Conta assoluta dei neutrofili <1,5 × 109/l, e/o
b.Leucociti <3,0 × 109/l, e/o
c.Piastrine <100 × 109/l, e/o
d.Emoglobina 9 g/dl
4.Pazienti con coagulazione anomala (aumento del tempo di protrombina [PT] e/o del tempo di tromboplastina parziale [PTT] ≥30% rispetto ai limiti della norma) o pazienti in trattamento cronico con warfarin o derivati cumarinici
