E.2 Objective of the trial |
E.2.1 | Main objective of the trial |
The primary objective is to demonstrate the superiority of Monoprost® versus Lumigan® 0.01% and Lumigan® 0.03% UD in term of safety with respect to the assessment of conjunctival hyperaemia in the worse eye at Day 84.
The conjunctival hyperaemia will be scored using the photographic McMonnies scale (0 to 5). The worse eye is defined as the eligible eye with the highest hyperaemia score on the McMonnies scale at D0. |
El objetivo principal consiste en demostrar la superioridad de Monoprost® frente a Lumigan® al 0,01 % y Lumigan® al 0,03 % en DU en relación con la evaluación de la hiperemia conjuntival en el ojo más grave en el día 84.
La hiperemia conjuntival se evaluará mediante la puntuación (de 0 a 5) en la escala fotográfica de McMonnies.
El ojo más grave se define como el ojo idóneo con la puntuación de hiperemia más alta en la escala de McMonnies en el día 0. |

? Mean IOP ? 18 mm Hg And ? Conjunctival hyperaemia decrease: reduction of at least 1 point on the McMonnies score compared to the score at the inclusion. |
El objetivo secundario consiste en demostrar la no inferioridad de Monoprost® frente a Lumigan® al 0,01 % y Lumigan® al 0,03 % en DU en relación con la respuesta al tratamiento del ojo más grave en el día 84, definida como:

Patients will be eligible for inclusion if all these criteria are respected:
[1.1] Male or female aged ?18 years old.
[1.2] Signed and dated informed consent.
[1.3] Association of the 3 following criteria: [1.3.1] Both eyes have primary open angle glaucoma or ocular hypertension, already treated and controlled by mono-therapy of Lumigan® 0.01% since at least 3 months (according to European Glaucoma Society guidelines).*. [1.3.2] IOP ? 18 mm Hg in BOTH eyes. [1.3.3] With local intolerance signs in at least one eye defined by the association of: [1.3.3.1] Hyperaemia = Grade (2) or (3) or (4) following the photographic McMonnies scale (0 to 5). And [1.3.3.2.1] Presence of at least 2 symptoms with a level of severity ? 1 (= mild or moderate or severe) among the following 5 symptoms: irritation / burning, itching, tearing, eye dryness sensation, foreign body sensation. And/Or [1.3.3.2.2] Presence of at least 2 signs with a level of severity ? 1 (= mild or moderate or severe) among the following 3 signs: superficial punctate keratitis, blepharitis, eyelid skin darkness.
*?Chronic, progressive optic neuropathies, that have in common characteristics morphological changes at the optic nerve head and retinal nerve fibre layer in the absence of other ocular disease or congenital anomalies. Progressive retinal ganglion cells death and visual field loss are associated with these changes? |
Los pacientes podrán incluirse en este estudio si cumplen todos estos criterios:
[1.1] Hombre o mujer de ?18 o más años de edad.
[1.2] Consentimiento informado por escrito.
[1.3] Combinación de los 3 criterios siguientes: [1.3.1] Glaucoma primario de ángulo abierto bilateral o hipertensión ocular bilateral ya tratada y controlada mediante Lumigan® al 0,01 % en monoterapia durante al menos 3 meses (de acuerdo con las guías clínicas de la Sociedad Europea de Glaucoma). [1.3.2] TIO ? 18 mmHg en ambos ojos. [1.3.3] Con signos de intolerancia local por lo menos en un ojo, definidos por una combinación de: [1.3.3.1] Hiperemia = grado (2), (3) o (4) de acuerdo con la escala fotográfica de McMonnies. Y [1.3.3.2.1] Presencia de al menos 2 síntomas con un nivel de gravedad ? 1 (= leve o moderado o grave) entre los 5 síntomas siguientes: irritación/ardor,prurito, ojos llorosos, sensación de sequedad ocular y sensación de cuerpo extraño. Y/O [1.3.3.2.2] Presencia de al menos 2 signos oculares con un nivel de gravedad ? 1 (= leve o moderado o grave) entre los 3 signos siguientes: queratitis punteada superficial, blefaritis y oscurecimiento cutáneo de los párpados. |

Ophthalmic exclusion criteria (in either eye) [2.1]
[2.1.1] Presence of at least one severe objective sign among the following: [2.1.1.1] Global ocular staining with Oxford (0-15) grading scheme >12. [2.1.1.2] Blepharitis (Grade 4: very severe, i.e. Eczematiform lesion). [2.1.2] Any ocular hypertension other than primary ocular hypertension or primary chronic open angle glaucoma (such as congenital, angle closure glaucoma, secondary glaucoma). [2.1.3] Visual field not performed or not available within the 6 months before inclusion visit. [2.1.4] Fundus not performed or not available within the 6 months before inclusion visit. [2.1.5] Advance stage of glaucoma: [2.1.5.1] Absolute defect in the 10 degrees central point of the visual field. [2.1.5.2] Severe visual field loss according to the investigator?s best judgement. [2.1.5.3] Risk of visual field worsening as a consequence of participation in the trial according to the investigator?s best judgement. [2.1.6] Best far corrected visual acuity ? 1/10. [2.1.7] History of traumatism, infection, inflammation within the 3 months before inclusion visit. [2.1.8] Ongoing or known history of ocular allergy and/or uveitis and / or viral infection. [2.1.9] Severe dry eye (defined by severe epithelial erosions of the cornea and / or use of dry eye medication with a frequency exceeding 8 instillations / day). [2.1.10] Corneal ulceration. [2.1.11] Palpebral abnormalities not related to medical treatment study and incompatible with a good evaluation. [2.1.12] Any abnormality preventing accurate assessment e.g. reliable tonometry measurement, visual field examination.
The protocol also defines other exclusion criteria such as systemic/non ophthalmic exclusion criteria, specific exclusion criteria for women, exclusion criteria related to general conditions and exclusion criteria related to previous and concomitant medications / non-product therapies. |
Criterios de exclusión de carácter oftálmico en cualquiera de los ojos
- Presencia de al menos uno de los siguientes signos objetivos graves: ? Puntuación global de la tinción de la superficie ocular en la escala de Oxford (0-15) >12. ? Blefaritis (Grado 4: muy grave, es decir, lesión eccematiforme).
- Cualquier hipertensión ocular que no sea una hipertensión ocular primaria o un glaucoma primario crónico de ángulo abierto (p. ej., glaucoma congénito, de ángulo cerrado, glaucoma secundario).
- Campo visual no medido o no disponible en los 6 meses previos a la visita de inclusión.
- Fondo de ojo no medido o no disponible en los 6 meses previos a la visita de inclusión.
- Glaucoma en estado avanzado: ? Déficit absoluto en los diez grados centrales del campo visual. ? Pérdida grave del campo visual según el mejor criterio del investigador. ? Riesgo de deterioro del campo visual como resultado de la participación en este ensayo, según el mejor criterio del investigador.
- Mejor agudeza visual corregida de lejos ? 1/10.
- Antecedentes de traumatismo, infección o inflamación en los 3 meses previos a la visita de inclusión.
- Alergia ocular, infección vírica o uveítis en curso o antecedentes confirmados.
- Forma grave del síndrome del ojo seco (definido por graves erosiones epiteliales en la córnea y/o el uso de medicamentos para tratar el ojo seco con una frecuencia mayor de 8 instilaciones/día).
- Úlcera corneal.
- Anomalía palpebral no relacionada con el tratamiento del estudio e incompatible con una buena evaluación.
- Anomalía que podría interferir con la fiabilidad de los exámenes, tales como, por ejemplo, la tonometría de aplanación o el examen del campo visual.
El protocolo también define otros criterios de exclusión, tales como : - Criterios de exclusión de carácter sistémico/no oftálmico - Criterios de exclusión específicos para las mujeres - Criterios de exclusión relativos a factores de carácter general - Criterios de exclusión relacionados con los tratamientos previos y concomitantes/tratamientos no farmacológicos. |
