E.3 | Principal inclusion criteria |
1. Histologically or cytologically confirmed diagnosis of metastatic NSCLC according to the 7th edition of the AJCC Cancer Staging Manual. In addition, the NSCLC must harbor an ALK rearrangement, assessed at a Novartis designated central laboratory using the FDA approved Vysis ALK Break Apart FISH Probe Kit (Abbott Molecular Inc.) test and scoring algorithm (including positivity criteria). Patients must wait for the central result of the ALK rearrangement status before initiating treatment with ceritinib. 2. All patients must have a tumor tissue sample available as an archival sample (if possible obtained after the completion of the patient?s last therapeutic regimen) or as a new biopsy to send to the Novartis designated central laboratory. If that is not possible, any tumor biopsy obtained at or since the time of diagnosis can be used. 3. At least one extracranial measurable lesion as defined by RECIST 1.1. A previously irradiated site lesion may only be counted as a target lesion if there is clear sign of progression since the irradiation. 4. Patient is 18 years of age or older at the time of informed consent. 5. Patients may or may not have neurological symptoms but must: ? Be able to swallow and retain oral medication. ? Be neurologically stable within at least 1 week prior to the first dose of study drug. Neurologically stable is defined as improved or stable neurological examination without increased doses of steroids to manage CNS symptoms within the last 5 days.. 6. Patients may have received prior chemotherapy, crizotinib, biologic therapy or other investigational agents including other ALK inhibitors. Patients must have recovered from all toxicities related to prior anticancer therapies to grade ? 1 (CTCAE v 4.03). Patients with any grade of alopecia are allowed to enter the study. ? Patients who have been treated with chemotherapy, with biological therapy or other investigational agent (except ALK inhibitors) must have discontinued the treatment at least 2 weeks (14 days) prior to starting study drug. In case last chemotherapy contains nitroso-urea or mitomycin C, the treatment must be discontinued at least 6 weeks prior to the first dose of study drug. ? Patients, if previously treated with ALK inhibitors (including crizotinib) must discontinue treatment at least 1 week (7 days) prior to the first dose of study drug.

1. Diagnóstico histológicamente o citológicamente confirmado de CPCNP metastásico de acuerdo con la 7ª edición del Manual de estadios de cáncer del AJCC. Además, el CPCNP deberá ser portador de reordenación de ALK, evaluada en un laboratorio central designado por Novartis, utilizando el test Vysis ALK Break Apart FISH Probe Kit (Abbott Molecular Inc.) aprobado por la FDA y el algoritmo de puntuación (incluyendo criterios de positividad). Se deberá esperar el resultado del laboratorio central del estado de reordenación de ALK de los pacientes antes de iniciar el tratamiento con ceritinib. 2. Se deberá disponer de una muestra de tejido tumoral de todos los pacientes como una muestra de archivo (si es posible obtenida tras la finalización del último régimen terapéutico del paciente) o como una nueva biopsia para enviar al laboratorio central designado por Novartis. Si esto no es posible, puede utilizarse cualquier biopsia tumoral obtenida en o desde el momento del diagnóstico. 3.Por lo menos una lesión extracraneal medible definida con los RECIST 1.1. Una lesión previamente irradiada sólo puede contabilizarse como una lesión diana, si existen signos evidentes de progresión desde la irradiación. 4.Pacientes de 18 años de edad o mayores en el momento del consentimiento informado. 5.Los pacientes pueden o no tener síntomas neurológicos, pero deberán: ?Ser capaces de tragar y retener medicación oral. ?Estar neurológicamente estable durante al menos 1 semana antes de la primera dosis de la medicación del estudio. Neurológicamente estable se define como un mejor o estable examen neurológico sin aumento de las dosis de esteroides para tratar los síntomas en el SNC en los últimos 5 días. 6. Los pacientes pueden haber recibido quimioterapia previa, crizotinib, bioterapia u otros fármacos en investigación incluyendo otros inhibidores de ALK. Los pacientes deberán haberse recuperado de todas las toxicidades relacionadas con terapias antineoplásicas previas a grado ? 1 (CTCAE v 4.03). Los pacientes con cualquier grado de alopecia podrán ser incluidos en el estudio. ?A los pacientes que hayan sido tratados con quimioterapia, con bioterapia u otros fármacos en investigación (excepto inhibidores de ALK) se les deberá retirar el tratamiento al menos 2 semanas (14 días) antes de comenzar la medicación del estudio. E.4 | Principal exclusion criteria |
1. Patient with a history of treatment with ceritinib. Patient with known hypersensitivity to any of the excipients of ceritinib (microcrystalline cellulose, mannitol, crospovidone, colloidal silicon dioxide and magnesium stearate). 2. Patients who need whole brain radiation to control the brain metastases. Patients will not be eligible unless treated brain lesions are progressive or new brain lesions are observed since the post whole brain radiation therapy MRI. 3. In case active brain lesions (single or not) require local treatment but other active brain lesions do not and are not treated, patients will be excluded only if the local treatment (neurosurgical treatment or Stereotactic Radiosurgery ) for the brain metastases is conducted within 2 weeks prior to starting study drug. Patients must have recovered from relevant toxicities related to these procedures to grade ? 1(CTCAE v 4.03) prior to receiving the first dose of study drug. 4. Planning of any brain local treatment (including but not limited to surgery, stereotactic radiosurgery, whole brain radiation, intrathecal chemotherapy) following the administration of the first dose of study drug. 5. Patient who has received thoracic radiotherapy to lung fields ? 4 weeks prior to starting the study treatment or patients who have not recovered from radiotherapy-related toxicities. For all other anatomic sites (including radiotherapy to thoracic vertebrae and ribs) radiotherapy ? 2 weeks prior to starting the study treatment or has not recovered from radiotherapy-related toxicities. Palliative radiotherapy for bone lesions ? 2 weeks prior to the first dose of study drug is allowed. 6. Patient has had major surgery (e.g., intra-thoracic, intra-abdominal or intra-pelvic) within 4 weeks prior to the first dose of study drug or has not recovered from side effects of such procedure. Video-assisted thoracic surgery (VATS) and mediastinoscopy will not be counted as major surgery and patients can receive study treatment ?1 week after the procedure.

1. Paciente con antecedentes de tratamiento con ceritinib. Pacientes con hipersensibilidad conocida a cualquiera de los excipientes de ceritinib (celulosa microcristalina, manitol, crospovidona, dióxido de silicona coloidal y estearato de magnesio). 2. Pacientes que precisen radioterapia de todo el cerebro para controlar las metástasis cerebrales. Los pacientes no serán elegibles salvo que las lesiones cerebrales tratadas sean progresivas o se observen nuevas lesiones cerebrales desde la RM posterior a la radioterapia de todo el cerebro. 3. En caso de que las lesiones cerebrales activas (únicas o no) precisen tratamiento local, pero otras lesiones cerebrales activas no y no estén tratadas, los pacientes serán excluidos sólo si el tratamiento local (tratamiento neuroquirúrgico o radiocirugía estereotáctica) para las metástasis cerebrales se realiza dentro de las 2 semanas previas al inicio de la medicación del estudio. Los pacientes deberán haberse recuperado de toxicidades relevantes relacionadas con estos procedimientos hasta grado ? 1 (CTCAE v 4.03) antes de recibir la primera dosis de la medicación del estudio. 4. Planificación de cualquier tratamiento local para el cerebro (incluido, pero no limitado a cirugía, radiocirugía estereotáctica, radioterapia de todo el cerebro, quimioterapia intratecal) tras la administración de la primera dosis de la medicación del estudio. 5. Pacientes a quienes se les haya realizado cirugía mayor (por ejemplo intratorácica, intraabdominal o intrapélvica) dentro de las 4 semanas anteriores a la primera dosis de la medicación del estudio o que no se hayan recuperado de los efectos secundarios de dicho procedimiento. La cirugía torácica asistida por vídeo (VATS) y la mediastinoscopia no se contabilizarán como cirugía mayor y los pacientes pueden recibir el tratamiento del estudio ?1 semana después del procedimiento.
