- Patient is currently enrolled in a Novartis GDD or GMA-sponsored or Incyte-sponsored clinical study (where Incyte can delegate the sponsorship to a preferred CRO, if applicable) that is approved to enroll into this rollover study, and are receiving either ruxolitinib or combination of ruxolitinib and panobinostat, and fulfilled all of the requirements of the parent protocol.. - Patient is currently benefiting from the treatment with ruxolitinib monotherapy or combination of ruxolitinib and panobinostat, as determined by the investigator - Patient has demonstrated compliance, as assessed by the investigator, with the parent study protocol requirements - Willingness and ability to comply with scheduled visits, treatment plans and any other study procedures - Patient currently has no evidence of progressive disease, as determined by the investigator, following previous treatment with ruxolitinib or combination of ruxolitinib and panobinostat - Written informed consent obtained prior to enrolling in roll-over study and receiving study medication. If consent cannot be expressed in writing, it must be formally documented and witnessed, ideally via an independent trusted witness. Note 1: If the patient is a minor, the parent who signs the informed consent for the minor must be a legally recognized parent or guardian. Where deemed appropriate by the clinician, and the child's parent or guardian, the child will also be included in the alldiscussions about the trials and the minor aged 12 and above assent will be obtained. The parent or guardian will sign on the designated line on the ICF attesting to the fact that the child had given consent. Note 2: if the minor is an adolescent female, she will be informed during the assent process that for safety purpose, a pregnancy test is required. She will also be told that if it is positive, she will be counseled and will be assisted in telling her parents. If the minor does not want to proceed, she will be advised not to sign consent and her enrollment in this protocol will end. |

Se il consenso non può essere fornito per iscritto, deve essere formalmente documentato e testimoniato, idealmente da un testimone imparziale. Nota 1: Se il paziente è un minore, il consenso informato per il minore deve essere firmato da entrambi i genitori o dal tutore. Se il medico dello studio e i genitori o il tutore lo ritenessero appropriato, il minore parteciperà a tutte le discussioni riguardo agli studi e sarà ottenuto l’assenso dei minori. I genitori o il tutore firmeranno nell’apposita riga del consenso informato attestando che il minore ha dato il proprio assenso. Nota 2: Se il minore è una femmina adolescente, durante il processo di assenso sarà informata che sarà necessario un test di gravidanza per motivi di sicurezza. - Patient has been permanently discontinued from study treatment in the parent study due to any reason. - Patient's indication is currently approved and reimbursed in the local country - Patient has participated in a combination trial other than the panobinostat and ruxolitinib combination trial (CLBH589X2106), where ruxolitinib was dispensed in combination with another study medication and the patient is still receiving combination therapy. - Pregnant or nursing (lactating) women, where pregnancy is defined as the state of a female after conception and until the termination of XML File Identifier: dAJ2riFq9Erf1HWhOKAc3E1ATu4= Page 20/31 gestation, confirmed by a positive hCG laboratory test. - Female patients between = 12 and < 18 years of age and of childbearing potential (e.g. are menstruating) who do not agree to abstinence or, if sexually active, do not agree to the use of highly effective contraception as defined below, throughout the study and for up to 30 days after stopping treatment Women of child-bearing potential, defined as all women physiologically capable of becoming pregnant, unless they are using highly effective methods of contraception throughout the study duration inclusive of the 30-day safety follow up. Il paziente è stato definitivamente sospeso dal trattamento nello studio di appartenenza per qualsiasi motivo. - L’indicazione alla terapia del paziente è al momento approvata e rimborsabile nel Paese in cui vive il paziente. - Il paziente ha partecipato a uno studio con un’associazione di farmaci diversa dall’associazione di ruxolitinib e panobinostat (CLBH589X2106) nel quale ruxolitinib è stato fornito in associazione a un altro farmaco in studio e il paziente sta ancora ricevendo la terapia con l’associazione. - Gravidanza e allattamento, ove la gravidanza viene definita come lo stato di una donna dal momento successivo al concepimento fino al termine della gestazione, confermata dalla presenza di positività del test hCG. - Pazienti di età compresa tra = 12 anni e < 18 anni e potenzialmente fertili (per esempio, mestruate) che non acconsentono a praticare astinenza dai rapporti o, se sessualmente attive, non acconsentono a utilizzare un metodo contraccettivo di efficacia elevata come definito di seguito, per l’intera durata dello studio e per un periodo fino a 30 giorni dopo la sospensione del trattamento. Le donne potenzialmente fertili, definite come tutte le donne fisiologicamente capaci di iniziare una gravidanza, a meno che non utilizzino metodi contraccettivi di efficacia elevata durante l’intera durata dello studio compresi i 30 giorni di follow-up per la valutazione della sopravvivenza. I metodi contraccettivi di efficacia elevata comprendono: - Astinenza completa dai rapporti sessuali (se in coerenza con lo stile di vita abituale e preferito della paziente). L’astinenza periodica (ad esempio da calendario, ovulatoria, sintotermica, metodi post-ovulatori) e il coito interrotto non sono considerati metodi contraccettivi accettabili - Sterilizzazione femminile (pazienti sottoposte a rimozione chirurgica bilaterale delle ovaie con o senza isterectomia), isterectomia totale o legatura delle tube almeno sei settimane prima della somministrazione del trattamento in studio. Nel caso della sola ovariectomia lo stato riproduttivo della donna deve essere confermato dal follow up del livello ormonale - Vasectomia (sterilizzazione maschile) (almeno 6 mesi prima dello screening). I soggetti di sesso femminile partecipanti allo studio devono avere rapporti sessuali solo con il partner vasectomizzato (sterilizzato) - Impiego di metodi di contraccezione ormonale, per via orale, iniezione o impianto, o posizionamento di un dispositivo intrauterino (IUD) o di un sistema intrauterino (IUS) o altre forme di contraccezione ormonale con efficacia comparabile (frequenza di insuccesso <1%), per esempio anello ormonale vaginale o contraccezione ormonale transdermica - Nel caso della contraccezione orale le donne devono aver utilizzato la stessa pillola anticoncezionale a dosaggio stabile per un minimo di 3 mesi prima dell’assunzione del trattamento in studio. - Le donne sono considerate in post-menopausa e quindi non potenzialmente fertili se hanno avuto 12 mesi di amenorrea spontanea, con un profilo clinico adeguato (ad esempio età appropriata, storia di sintomi vasomotori) o sono state sottoposte a ovariectomia bilaterale (con o senza isterectomia) o legatura delle tube almeno sei settimane prima dell’ingresso nello studio. Nel caso della sola ovariectomia le donne sono considerate non potenzialmente fertili solo se questo dato viene confermato da un follow up dei livelli ormonali. - 6. Nei pazienti con l’associazione di ruxolitinib e panobinostat: -- Donne potenzialmente fertili e uomini con partner sessuali potenzialmente fertili che non sono disposti a utilizzare metodi contraccettivi di efficacia elevata durante la somministrazione del trattamento e per 3 mesi (pazienti potenzialmente fertili) --Partner potenzialmente fertili di pazienti partecipanti che non sono disposti a utilizzare metodi contraccettivi di efficacia elevata durante la somministrazione del trattamento e per 6 mesi dopo la sospensione del trattamento. |
