Patients eligible for inclusion in this study have to meet the following criteria:
1.Histologically confirmed metastatic RCC with a clear cell component..
2.Bone metastases upon bone scan and MRI performed any time within period of 4 weeks prior to study entry, with at least one evaluable unidimensional bone lesion (i.e., ≥1 malignant tumour mass that can be accurately measured in at least 1 dimension ≥ 10 mm on T1-weighted Magnetic Resonance Imaging [MRI]).
Group A: bone metastases (lymph nodes and/or adrenal metastases, and/or ≤ 5 lung metastases of less than 1 cm each, are allowed) .
Group B: bone metastases AND visceral metastases upon MRI or CT-scan (according to revised RECIST 1.1 criteria).
3.Naïve patient (first line setting) or patient with disease progression on last therapy (second or third line setting).
4.Male or female, age ≥18 years at ICF signature time.
5.ECOG performance status of 0 or 1.
6.Good or Intermediate prognostic group according to the International Metastatic Database Consortium (IMDC).
7.At least 4 weeks from the end of a previous systemic treatment, if any, with resolution of all treatment-related toxicity according to NCI CTCAE Version 4.03 grade ≤ 1 except for alopecia.
8.Palliative local treatment allowed if performed ≥ 2 weeks prior to study entry for radiotherapy, cimentoplasty or minor surgery; ≥ 4 weeks prior to study entry for major surgery.
9.Adequate organ function defined by the following criteria:
a.Absolute Neutrophils count (ANC) ≥1 500 cells/mm3
b.Platelets ≥100 000 cells/mm3
c.Haemoglobin ≥ 9.0 g/dL
d.AST and ALT ≤ 2.5 x upper limit of normal (ULN), unless there are liver metastases in which case AST and ALT ≤5.0 x ULN
e.Total bilirubin ≤ 1.5 x ULN
f.Serum creatinine ≤ 1.5 x ULN or calculated creatinine clearance ≥ 50 mL/min
g.Urinary protein < 2+ by urine dipstick. If dipstick is ≥ 2+ then a 24-hour urine collection can be done and the patient may enter only if urinary protein is < 2 g per 24 hours
h.Corrected calcium ≤ 2.8 mmol/L.
10.Women of childbearing potential must have a negative serum pregnancy test within 7days prior to treatment initiation.
11.Signed and dated informed consent document indicating that the patient (or legally acceptable representative) has been informed of all pertinent aspects of the trial prior to enrolment.
12.Willingness to comply with protocol requirements, including effective contraception during study treatment and for 6 months after last dose of study drug.

Les patients éligibles pour l'inclusion dans cette étude doivent satisfaire aux critères suivants:
1.Cancer du rein métastatique avec une composante à cellules claires histologiquement confirmé.
2.Métastases osseuses à la scintigraphie osseuse, et/ou à l’IRM avec au moins une lésion osseuse évaluable dans une dimension (i.e. - ≥1 masse tumorale maligne pouvant être mesurée avec précision dans au moins 1 dimension ≥ 10 mm par IRM pondérée en T1),
Groupe A: métastases osseuses (tolérés : ganglions lymphatiques et/ou métastases surrénales, et/ou ≤ 5 métastases pulmonaires de moins de 1 cm chacune) sur scintigraphie osseuse réalisée dans un délai de 4 semaines avant l'entrée dans l'étude,
Groupe B : métastases osseuses à la scintigraphie osseuse ET métastases viscérales à l’IRM ou au scanner (selon critères RECIST 1.1 révisés), réalisé dans un délai de 4 semaines avant l'entrée dans l'étude.
3.Patient naïf (première ligne) ou patient avec progression de la maladie sous la dernière ligne de traitement (deuxième ou troisième ligne).
4.Homme ou femme, âge ≥ 18 ans à la signature du consentement.
5.ECOG-PS : 0 ou 1.
6.Groupe de bon pronostique ou intermédiaire selon les critères de l'International Metastatic Database Consortium (IMDC).
7.A distance d’au moins 4 semaines de la fin du traitement systémique précédent (si applicable) et résolution de toute toxicité liée à ce traitement à un Grade ≤ 1 (NCI CTCAE version 4.03), excepté pour l'alopécie.
8.Traitement local palliatif autorisé si réalisé depuis ≥ 2 semaines avant l'entrée dans l'étude pour radiothérapie, cimentoplastie ou chirurgie mineure; ≥ 4 semaines avant l'entrée dans l'étude pour une intervention chirurgicale majeure.
9.Fonctions organiques définies par les critères suivants :
a.Polynucléaires Neutrophiles ≥ 1500/mm3
b.Plaquettes ≥ 100,000/mm3

10.Femmes en âge de procréer doivent avoir un test de grossesse sanguin négatif dans les 7 jours avant le début du traitement.
11.Consentement éclairé signé et daté indiquant que le patient (ou son représentant légal) a été informé de tous les aspects pertinents de l'essai avant de rentrer dans l’étude.
12.Volonté de se conformer aux exigences du protocole, dont l’utilisation d’une contraception efficace pendant toute la durée de traitement et pendant 6 mois après la dernière dose de traitement.

E.4 | Principal exclusion criteria |
1. Poor prognostic group according to the IMDC.
2.Prior radiotherapy to ≥ 40% of bone marrow, whole pelvic irradiation and/or prior isotope therapy whatever the isotope (any α- or β-emitters).
3.Active secondary cancer including prior malignancy from which the subject has been disease-free for ≤ 3 years (however, adequately treated superficial basal cell skin or cervical carcinoma in situ before 4 weeks prior to entry are eligible to the study).
4. Known brain or leptomeningeal involvement.
5. Other concurrent serious illness or medical conditions.
6. Uncontrolled hypertension.
7. Uncontrolled cardiac arrhythmias, angina pectoris, and/or hypertension. History of congestive heart failure, or myocardial infarction within the last 6 months.
8. QTc interval (QTc) assessed by local device > 500ms in the 7 days prior to inclusion.
9. Biphosphonates, denosumab and/or vitamin D supplementation received within 2 weeks before the first injection of XOFIGO®.
10. Active infection requiring systemic antibiotic or anti-fungal medication.
11. Participation in another clinical trial with any investigational drug within 30 days prior to study enrolment.

1. Groupe de mauvais pronostic selon l'IMDC.
2. Radiothérapie antérieure ≥ 40% de la moelle osseuse ou de l’ensemble du pelvis, et/ou traitement préalable par isotopes (quel qu'ils soient émetteurs α- ou β).
3. Autre cancer actif, incluant une maladie tumorale pour laquelle le patient a été en rémission moins de 3 ans (cependant, un patient présentant un carcinome baso-cellulaire superficiel traité, ou un carcinome cervical in situ traité présent 4 semaines avant l'entrée dans l’étude peut être éligible).
4. Métastases cérébrales ou leptoméningées connues.
5. Autre maladies ou comorbidités sévères.
6. Arythmie cardiaque, angine de poitrine, et/ou hypertension non contrôlées. Des antécédents d'insuffisance cardiaque congestive, ou d’infarctus du myocarde dans les 6 derniers mois.
7. Intervalle QTc > 500 ms sur ECG local réalisé au cours des 7 jours avant l'inclusion.
8. Biphosphonates, denosumab et/ou supplémentation en vitamine D dans les 2 semaines précédant la première injection de XOFIGO®.
9. Infection active nécessitant un antibiotique systémique ou un traitement antifongique.
10. Participation à un autre essai clinique avec médicament expérimental dans les 30 jours précédant l'inclusion dans l'étude.

I.Primary endpoint:
Phase I (Group A and B):
Dose-limiting toxicities (DLT) within the first 6 weeks to determine the MTD.
DLT is defined as any XOFIGO® related adverse event occurring between C1D1 and C2D15 and listed below:
1.Haematological toxicity:
•Neutrophils < 0.5 x 109/L for ≥ 7 days or < 0.1x109/L for ≥ 3 days (grade 4)
•Febrile neutropenia defined as absolute granulocyte count < 1.0 x 109/L and fever ≥ 38.5°C or microbiologically documentation of unknown origin
•Platelets < 25 x 109/L OR <50 x 109/L with bleeding or requiring platelet transfusion
•Anemia grade 3 (<8g/dl)
2.Non-haematological toxicity:
•Any grade ≥ 3 major organ toxicity according to the CTC Version 4.03 (except alopecia or non-premedicated nausea/vomiting)
•Grade ≥2 diarrhea for ≥2 weeks despite adequate corrective treatment
3.Second administration of XOFIGO® (C2D1) delayed by more than 2 weeks due to an unresolved toxicity grade >1 related to first administration of XOFIGO®
Phase II (Group A and B):
Dose-limiting toxicities (DLT) during the phase II DLT period AND efficacy upon MRI +/- FNa-PET to determine the MSD.
DLT in phase II: to be determined as in phase I (same definition and causality assessment method). After completion of Phase I, the 45 days,time-window for DLT evaluation (DLT period) may be re-evaluated for phase II based on late-onset toxicities observed in phase I.
MSD: to be determined jointly from DLT rate and efficacy results upon WB-MRI +/- FNa-PET. Based on observed efficacy data (MRI +/- FNa-PET) in phase I, time-window for efficacy (efficacy period) will be defined for phase II.
Bone response criteria upon Whole body-MRI.
The benchmarks will be a revised RECIST criteria 1.1 which takes into account only bone lesions.
The number of targets to be evaluated is:
≤ 5 bone target lesions measuring ≥10 mm on contrast enhanced T1-weighted images on MRI
The standard thresholds for the RECIST 1.1-based method will be applied to bone lesions (decrease of 30% for a response and increase of 20% for a progression).

Critère principal:
1) Phase I: (Groupes A et B) :
Toxicités limitantes (DLT) au cours des 6 premières semaines pour déterminer la MTD.
Une DLT est définie comme tout événement indésirable lié au XOFIGO® et survenant entre C1J1 et C2J15 parmi:
1 Toxicité hématologique:
• Neutrophiles < 0,5 x 109 / L pendant ≥ 7 jours ou < 0.1x109 / L pendant ≥ 3 jours (grade 4)
• Neutropénie fébrile : nombre absolu de granulocytes <1,0 x 109 / L et fièvre ≥ 38,5 °C quelle que soit l’origine bactériologique
• Plaquettes < 25 x 109 / L, ou < 50 x 109/ L avec saignement ou nécessitant une transfusion de plaquettes
• Anémie grade 3(< 8g/dl)
2. Toxicité non-hématologique:
• Toute toxicité de grade ≥ 3 sur un organe majeur (sauf alopécie ou nausées / vomissements sans prémédication)
• Diarrhée grade ≥ 2 pendant plus de 2 semaines malgré un traitement correcteur
3. Seconde administration de XOFIGO® retardée de plus de 2 semaines en raison d'une toxicité non résolue de grade > 1 liée à la 1ère administration de XOFIGO®
2) Phase II (Group A and B) :
< toxicités limitantes (DLT) au cours de la période de DLT ET réponse osseuse par imagerie au cours de la période d’efficacité pour déterminer la dose optimale (MSD).
.La DLT sera évaluée comme dans la Phase I (même définition et méthode d'évaluation de la causalité). Seule la fenêtre de temps des 45 jours (période DLT)pourra être révisée pour améliorer la sécurité d’utilisation lors de la phase II.
La MSD sera évaluée conjointement à partir du taux d DLT et du taux de réponse des métastases osseuses par IRM corps entier / TEP-FNa.
Critères IRM corps entier:
Ces critères reposent sur les critères RECIST 1.1 prenant en compte les lésions osseuses.
Nous proposons de définir :
- ≤ 5 lésions osseuses cibles mesurant≥ 10 mm sur les images pondérées en T1 à l'IRM
Ensuite, les seuils standards des critères RECIST 1.1 sont appliqués (diminution de 30% pour une réponse et augmentation de 20% pour une progression), et seront évaluées la réponse osseuse d'une part et la réponse extra-osseuse d'autre part.

•Diffusion weighted imaging (DWI): Apparent diffusion coefficient (ADC) values in the five target bone lesions
•Ultra-short echo time (UTE) sequence: T2 measurements in the five target bone lesions
•Perfusion weighted imaging (PWI): assessments of the area under the curve at 1min30, and of the focal lesion (FLE max) enhancement.
-Pain assessments will be performed at baseline, prior to each administration of XOFIGO® and 4 weeks after last administration, then every 12 weeks until end of study.
Specific to phase II: efficacy assessment:
Efficacy will be a binary criterion (YES/NO) and will be assessed upon WB-MRI.
The definition is:
-Confirmed PD: progression=YES
-Not confirmed PD (SD or PR/CR): progression=NO
In case of progression=NO, acceptation will rely on FNa-PET results for further confirmation.
The algorithm is as follows:
-in case of PD upon WB-MRI, a confirmation WB-MRI will be performed at 4 weeks and
if PD is confirmed then progression=YES and date of progression=date of first WB-MRI, otherwise progression=NO.
-in case of non-PD upon WB-MRI, if also non-PD upon FNa-PET, then progression=NO; else if PD upon FNa-PET, then a confirmation WB-MRI will be performed at 4 weeks and
if PD is confirmed then progression=YES and date of progression=date of FNa-PET, otherwise progression=NO.
The optimal efficacy period (time window) to be considered for phase II will be defined upon collegial central review of phase I images by the participating centres.

Critères secondaires (évalués à l’issue de la phase II) :

• Délai jusqu’au 1er événement osseux (SRE) : fracture pathologique, nécessité de procéder à une radiothérapie, compression médullaire ou nécessité d'une chirurgie osseuse
• Délai jusqu'à progression (TTP), défini comme la durée entre la première administration du XOFIGO® et la progression, quel que soit le site de progression (selon RECIST 1.1 modifié incluant les lésions osseuses)
• Délai jusqu'à progression osseuse, défini comme la durée entre la première administration du XOFIGO® et la progression osseuse (selon RECIST 1.1 modifié pour les lésions osseuses)
• Taux de survie à 1 an défini comme le pourcentage de patients en vie 1 an après la première administration de XOFIGO® ;
• Qualité de vie à partir des questionnaires FKSI-15 et EQ-5D ;
• Evaluation des critères IRM suivants comme marqueurs prédictifs de la réponse, de la survie spécifique et de la survie globale:
o Imagerie de diffusion (DWI): coefficient de diffusion apparent (ADC) au sein des 5 lésions cibles osseuses
o Séquence d’écho ultra-courte (UTE): mesures en T2 pour es 5 lésions cibles osseuses
o Imagerie pondérée de perfusion (PWI): évaluations de l’aire sous la courbe à 1min30, et du rehaussement de la lésion focale (FLE max)
L’évaluation de la douleur à l’aide du questionnaire BPI sera effectuée en baseline, avant chaque administration de XOFIGO® jusqu’à 4 semaines après la dernière administration de XOFIGO®, puis toutes les 12 semaines jusqu’à la fin d’étude.
Spécifiques à la phase II: efficacité osseuse
L'efficacité sera un critère binaire (OUI / NON) et sera évaluée sur IRM corps entier.
La définition est:
- MP Confirmée: progression = OUI
- MP Non confirmée (SD ou PR / CR): la progression = NON
En cas de progression = NON, l'acceptation reposera sur le résultat du TEP-FNa pour une confirmation supplémentaire.
L'algorithme est le suivant:
- En cas de MP sur IRM-corps entier, une confirmation IRM-corps entier, sera effectuée à 4 semaines et
si MP est confirmée ensuite progression = OUI et la date de progression = date de la première IRM-corps entier,, sinon la progression = NON.
- En cas de non-MP sur IRM-corps entier, si également non-MP sur TEP-FNa, puis la progression = NON; d'autre si MP sur TEP-FNa, puis une confirmation IRM-corps entier sera effectuée à 4 semaines et
si MP est confirmée ensuite progression = OUI et la date de progression = date de TEP-FNa, sinon la progression = NON.
La période d'efficacité optimale (fenêtre de temps) considérée pour la phase II sera définie après lecture centralisée des images de phase I par les centres participants.

Bone markers will be assessed at Baseline, C1D1, C1D15, before each administration of XOFIGO® and 4 weeks after last administration of XOFIGO®. If XOFIGO® is discontinued due to disease progression, than every 2 months.
Specific to phase II: Bone efficacy
FNa-PET and MRI data will be analysed separately and jointly to define reliable efficacy endpoint as well as the optimal efficacy time window for phase II part. The efficacy endpoint will be a binary criterion (response vs no-response) taking into account possible dissociated responses upon the 2 exams.

L'évaluation de la douleur par BPI sera effectuée en baseline, et avant chaque administration de XOFIGO jusqu’à la visite de fin traitement puis tous les 2 mois.Les marqueurs osseux seront évalués en baseline,C1J1,CIJ15, avant chaque administration du XOFIGO et 4 semaines après la dernière administration de XOFIGO. Si le XOFIGO est interrompu en raison de la progression de la maladie tous les 2 mois jusqu'à progression.A l’issue de la phase I les données de TEP-FNa et IRM corps entier seront analysées séparément et conjointement afin de définir 1 critère d’efficacité fiable ainsi qu’une fenêtre de temps optimale pour la phase II. Le critère d’efficacité sera binaire (réponse vs non-réponse)et prendra en compte la possibilité de réponses dissociées avec les 2 types d’imagerie |
