E.3 | Principal inclusion criteria |
1. Have house dust mite (HDM)-associated allergic rhinitis (AR) (with or without asthma) for at least 1 year based on the presence of: • Symptoms for 4 or more consecutive weeks in the previous year and for at least 4 days per week during those weeks. • Symptoms requiring regular intake of symptomatic treatment(s). • Symptoms evaluated as "troublesome" by the patients or impairing their daily activities, leisure or sport, school or work or involving sleep disturbance. 2. Have given signed informed consent to participate, after having been informed of the nature and aims of the study, in accordance with local regulation and requirements. 3. Male or female outpatients 12 to 65 years of age. 4. Sensitized to D. pteronyssinus (D. pte) and/or D. farinae (D. far) defined as skin prick test wheal diameter at least 5 mm greater than the negative control and HDM-specific serum IgE =3.5 kU/L. 5. Willing to and capable of completing the e-diary, study questionnaires and scales. |
1. Presenza di rinite allergica (RA) associata agli acari della polvere domestica (HDM) (con o senza asma) da almeno 1 anno, in base alla presenza di: o Sintomi per 4 o più settimane consecutive nell’anno precedente e per almeno 4 giorni alla settimana nel corso di tali settimane. o Sintomi richiedenti un’assunzione regolare di uno o più trattamenti sintomatici. o Sintomi valutati dai pazienti come “problematici” o compromettenti le attività quotidiane, il tempo libero o lo sport, l’attività scolastica o lavorativa o che comportano disturbi del sonno. 2. Rilascio di un consenso informato firmato a partecipare, dopo aver ricevuto informazioni in merito alla natura e agli scopi dello studio, ai sensi delle leggi e dei regolamenti locali. 3. Pazientie di sesso maschile o femminile, non ricoveratio, di età compresa tra 12 e 65 anni. 4. Sensibilizzazione a D. pteronyssinus (D. pte) e/o D. farinae (D. far) definita come presenza di pomfo successivo al prick test cutaneo con un diametro di almeno 5 mm superiore al controllo negativo e IgE sieriche specifiche per gli HDM = 3,5 kU/l. 5. Evidenza di esposizione agli HDM definita come ¿2 ¿g di allergene degli acari del gruppo 1 per grammo di polvere, nel campione di polvere prelevato a casa del paziente. 6. Pazientie dispostio e in grado di compilare correttamente il diario elettronico, i questionari dello studio e le scale di valutazione.

E.4 | Principal exclusion criteria |
1. A history of rhinitis, rhino-conjunctivitis or asthma to allergens other than HDM, likely to result in rhinitis symptoms during the baseline and primary evaluation periods. Specifically, when the following are present: • documented sensitization (positive Skin Prick Test or allergen specific serum IgE >3.5kU/L) and history of clinically relevant symptoms to allergen(s) other than HDM • anticipated exposure to such allergen(s) during the baseline and primary evaluation periods (i.e., the 4 weeks of evaluation between September and January) For example, the following patients are to be excluded: - patients sensitized to cat or dog allergens and regularly exposed to these animals - patients sensitized to perennial allergens, such as aspergillus,cladosporium, alternaria, cockroach - patients sensitized to seasonal allergens such as parietaria, ragweed or mugwort, if these allergens are endemic in the region during the baseline and primary evaluation periods. 2. Any diagnosed nasal (other than HDM allergic rhinitis) or oral disease that could interfere with the efficacy or safety assessments, such as nasal polyposis, recurrent chronic rhino-sinusitis (at least 2 isolated episodes per year in the 2 previous years, each episode lasting more than 8 weeks) or a history of chronic oral inflammation or current active oral inflammation from any etiology (e.g., oral lichen planus, oral ulceration or oral mycosis) and/or oral wounds. 3. Recent nasal surgery (i.e., within the previous 6 months). 4. Partially controlled or uncontrolled asthma defined in the Global Initiative for Asthma 2014 guidelines (GINA 2014) as the presence of daytime asthma symptoms more than twice/week or nocturnal symptoms/awakening or need for reliever/rescue treatment more than twice/week or FEV1 <80% of predicted or personal best value. 5. Asthma therapies consistent with GINA treatment Step 3, Step 4 and Step 5 i.e., the preferred controller medication consists of inhaled corticosteroid (ICS) combined with long-acting beta (ß)-2 agonist (LABA) according to GINA classification 2014 (refer to Appendix II for the full details of other controller options). 6. Experienced a life-threatening asthma attack or an asthma exacerbation that resulted in Intensive Care Unit (ICU) hospitalization. 7. Requiring continuous treatment with systemic corticosteroids for any indication. 8. Requiring continuous treatment with ß-blockers or with Monoamine Oxidase Inhibitors (MAOIs). 9. Received an immunosuppressive treatment within 3 months prior to screening. 10. Received allergen specific immunotherapy (AIT) by any route: - for house dust mites: AIT for more than 1 month within the 5 years before screening - for other allergen(s): ongoing or recently stopped (within 6 months) AIT. 11. Any history of anaphylaxis after previous allergen immunotherapy, exposure to allergen(s) or of unknown cause. 12. Any history of hypersensitivity to STG320 or its excipients or contraindication to the use of rescue medications 13. Female with positive urine pregnancy test or lactating or expecting to conceive within the duration of the study. 14. Sexually active female of child-bearing potential without medicallyaccepted contraceptive method 15. Unable or unwilling to comply with the study protocol requirements,including those who anticipate significant changes in their daily environment in relation to HDM exposure or who are likely to travel for extended periods of time during the main efficacy assessment period. 16. Patients with past or current disease(s) which, as judged by the Investigator, may affect the patient's participation in or the outcome of this study. These diseases include, but are not limited to, cardiovascular disease, malignancy, active tuberculosis, hepatic disease, renal disease, hematologic disease, neurologic or psychiatric disease, severe autoimmune disorder, immunodeficiency or immunologic disease and endocrine disease. 17. Patients with a history of eosinophilic esophagitis or with current severe or persistent gastroesophageal symptoms including dysphagia or chest pain. 18. Contraindications to allergen specific immunotherapy. 19. Patients with history of drug or alcohol abuse. 20. Participation in any clinical study within 30 days prior to the selection visit. 21. Possible dependency of the patient on sponsor or investigators/subinvestigators or study personnel. |
1. Anamnesi di rinite, rinocongiuntivite o asma ad allergeni diversi dagli HDM, che possano dar luogo a sintomi di rinite nel corso del periodo di valutazione del basale e del periodo di valutazione principale. Nello specifico, in caso di presenza di una delle seguenti situazioni: • sensibilizzazione documentata (positività al prick test cutaneo [presenza di pomfo con un diametro di almeno 5 mm superiore al controllo negativo] o IgE sieriche specifiche per gli allergeni > 3,5 kU/l) e anamnesi di sintomi clinicamente rilevanti a uno o più allergeni diversi dagli HDM • prevista esposizione a uno o più dei suddetti allergeni nel corso dei periodi di valutazione del basale e principale (ovvero, nelle 4 settimane di valutazione tra settembre e gennaio). Per esempio, i seguenti pazienti devono essere esclusi: - pazienti sensibili agli allergeni di gatto o cane e con regolare esposizione a questi animali - pazienti sensibili agli allergeni perenni, come Aspergillus, Cladosporium, alternaria, scarafaggio - pazienti sensibili agli allergeni stagionali come la parietaria, l’ambrosia o artemisia, se questi allergeni sono endemici della regione, nel corso dei periodi di valutazione del basale e principale. 2. Qualsiasi diagnosi di malattia orale o nasale (diversa dalla rinite allergica da HDM) in grado di interferire con le valutazioni di efficacia e sicurezza, come la poliposi nasale, la rinosinusite cronica recidivante (almeno 2 episodi isolati all’anno nei 2 anni precedenti, con ciascun episodio che si protrae per più di 8 settimane) oppure anamnesi di infiammazione orale cronica o attuale infiammazione orale attiva di qualsiasi eziologia (ad es., lichen planus orale, ulcera orale o micosi orale) e/o lesioni orali. 3. Recente intervento chirurgico (ovvero, nei precedenti 6 mesi). 4. Asma non controllata o parzialmente controllata definita nelle linee guida 2014 del Global Initiative for Asthma (GINA 2014) come la presenza di sintomi asmatici diurni per più di due volte/settimana o sintomi notturni/al risveglio oppure necessità di un trattamento di soccorso/alleviamento per più di due volte/settimana o un FEV1 < 80% rispetto al previsto o al miglior valore personale. 5. Terapie per l’asma conformi ai punti terapeutici 3, 4 e 5 del GINA, ossia il farmaco di controllo preferito consta di corticosteroidi inalati (inhaled corticosteroid, ICS) in combinazione con (ß)-2 agonisti a lunga durata d’azione (long-acting beta 2-agonist, LABA) in base alla classificazione GINA 2014 (per informazioni dettagliate sulle altre opzioni di controllo fare riferimento all’Appendice II).
Saranno considerati pazienti asmatici idonei quelli con asma controllata con uno o più trattamenti che rientrano nei punti terapeutici 1 o 2 del GINA 2014 [ovvero trattamento alleviante con ß2-agonisti a breve durata d’azione, se necessario, con o senza trattamento di controllo consistente in corticosteroidi inalati a bassa dose (ovvero, <400 ¿g di budesonide/giorno o dose equivalente di altro corticosteroide) o antagonista dei recettori leucotrienici o teofillina a bassa dose]. 6. Evento di asma potenzialmente letale o un’esacerbazione dell’asma che ha comportato il ricovero in un’unità di terapia intensiva (UTI). 7. Necessità di un trattamento continuo con corticosteroidi sistemici per qualsiasi indicazione. 8. Necessità di un trattamento continuo con ß-bloccanti o con inibitori della monoamminossidasi (IMAO). 9. Precedente trattamento immunosoppressore nei 3 mesi che precedono lo screening. 10. Precedente immunoterapia specifica per gli allergeni (allergen specific immunotherapy, AIT) con qualsiasi via di somministrazione: - per gli acari della polvere domestica: AIT per più di 1 mese nei 5 anni prima dello screening; - per altri allergeni: AIT in corso o sospesa di recente (entro 6 mesi). 11. Qualsiasi anamnesi di anafilassi dovuta a precedente immunoterapia allergenica, a esposizione a uno o più allergeni oppure di causa ignota. 12. Anamnesi di ipersensibilità a STG320 o ai suoi eccipienti o controindicazione all’uso dei farmaci di soccorso (ovvero, antistaminici e corticosteroidi). 13. Paziente di sesso femminile con test di gravidanza sulle urine positivo oppure in allattamento o in attesa di concepimento nel corso dello studio. 14. Paziente di sesso femminile sessualmente attiva, in età fertile che non ha adottato un metodo contraccettivo medicalmente accettato: contraccettivo ormonale (orale, iniettabile o impiantabile, per almeno 2 mesi prima dell’arruolamento) dispositivo intrauterino, preservativo maschile o diaframma utilizzato insieme a uno spermicida, relazione monogama con un compagno vasectomizzato. Le donne sono ritenute non in età fertile prima del menarca o almeno 2 anni dopo la menopausa o in caso di legatura delle tube o di una isterectomia totale o di una ooforectomia oppure ovariectomia bilaterale. 15.
