    E.2.1Main objective of the trial
    To study the safety and efficacy of chemoimmunotherapy on conjunction with bloodbrain barrier disruption in the therapy of PCNSL as measured by number of patients alive at 2 anf 5 years.
    Tutkia kemoimmunoterapian tehoa ja turvallisuutta veriaivoesteen avauksen yhteydessä toteutettuna primaaria keskushermostolymfoomaa sairastavilla potilailla. Mittarina toimii 2 ja 5 vuoden kohdalla elossaolevien potilaiden määrä.

    Response rate. NUmber of patients disease free at 2 and 5 years.
    Treatment related adverse effects and long term neurological adverse events.

    E.3Principal inclusion criteria
    1) A Patients has PCNSL, either newly diagnosed or relapsed
    2) The disease had DLBCL histology
    3) Patients age is 18-70 years
    4) The patient has given an informed consent
    1) Potilaalla on primaarinen aivolymfooma, joko vasta todettu tauti tai Bonnin sytostaattiyhdistelmän jälkeen uusiutunut tai sille refraktaari tauti.
    2) Tauti edustaa histologialtaan diffuusia suurisoluista B-solulymfoomaa.
    3) Potilaan ikä on 18-70 vuotta
    4)Potilas on antanut kirjallisen suostumuksen tutkimukseen osallistumisesta.
    E.4Principal exclusion criteria
    1) Whole brain irradiation
    2) More than one lines of previous chemotherapy
    3) Leesion in the deep brain structures
    4) Patient is not able to understand or give informed consent
    5) Liver failure
    6) Bone marrow failure
    7) Severe renal failure
    8) Any medication that can not be doscontinued during opening and is a contraindication to opening
    9) Any other heath related issue that is a contraindication to hydration, anestesia or other mandatory supportive care
    10) WHO performance score 2 or more due to othet reasons than PCNSL
    11) Other malignancies unless treated curatively
    1) Aiemmin annettu pään alueen sädehoito.
    2) Aiemmin annettu useamman kuin yhden linjan solunsalpaajahoito.
    3) Syvissä aivojen osissa sijaitseva lymfoomamuutos, joka turvotessaan voi aiheuttaa liquor-kierron häiriön.
    4) Potilaan psyykkinen sairaus tai taudista johtuva tilanne, joka estää häntä antamasta tietoista suostumusta tai sitoutumasta hoitoon.
    5) Maksan vajaatoiminta.
    6) Heikentynyt luuytimen toiminta.
    7) Vaikea munuaisten vajaatoiminta. Lievässä vajaatoiminnassa hoitoa voidaan antaa ilman metotreksaattia.
    8) Sellainen lääkitys, joka on kontraindisoitu veriaivoesteen avauksen yhteydessä ja jota ei voida tauottaa tai muuttaa turvalliseen lääkitykseen.
    9) Muu yleissairaus, joka estää hoidon edellyttämien nesteytysten, anestesioiden tai muiden tukitoimien läpiviemisen.
    10) Muusta kuin aivolymfoomasta johtuva alentunut yleistila WHO 2 tai yli.
    11) Toinen malignooma, ellei sitä ole hoidettu kuratiivisella tavoitteella ja saavutettu hyvä hoitovaste.
