E.3 | Principal inclusion criteria |
1. Histologically confirmed NSCLC with metastatic or unresectable, locally advanced disease. 2. Receipt of at least one prior platinum-containing chemotherapy or licensed immunotherapy regimen in the advanced disease setting. 3. Molecular analysis of patient-derived samples using a Sponsor approved method and/or laboratory that demonstrates a genetic alteration activating MET in tumor tissue and/or ctDNA. If eligibility is established using a Sponsor-approved local laboratory, a tumor tissue specimen and/or blood sample is expected to be available for retrospective sequencing in the central laboratory selected by the Sponsor 4. Measurable disease, as per RECIST version 1.1. 5. Eastern Cooperative Oncology Group (ECOG) performance status 0, 1, or 2. 6. Adequate bone marrow and organ function demonstrated by: -Absolute neutrophil count >=1,000/mm3 (>= 1.0 × 109/L) -Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) <=2.5 × Upper Limit of Normal (ULN), or <= 5.0 x ULN for patients with documented liver metastases. Alkaline phosphatase levels <= 2.5 × ULN. In the presence of extensive bone metastases, there is no upper limit for alkaline phosphatase. -Total bilirubin <= 1.5 x ULN or <=3.0 x ULN for patients with Gilbert Syndrome or documented liver metastases. 7. >= 18 years of age. 8. Women of child-bearing potential (WOCBP) or men whose partner is a WOCBP agrees to use contraception while participating in this study, and for a period of 6 months following termination of study treatment. 9. Completed informed consent process, including signing IRB/EC-approved informed consent form. 10. Willing to comply with clinical trial instructions and requirements. |
1.CPNMC confirmado histológicamente y metastásico o no operable, y localmente avanzado. 2.Haber recibido como mínimo un regimen de quimioterapia con platino o de inmunoterapia autorizada anterior estando en situación de enfermedad avanzada. 3.Disponer de un análisis molecular de muestras derivadas de los pacientes mediante un método y un laboratorio aprobados por el promotor, que demuestren una alteración genética activadora del factor de transición epitelio-mesenquimal en tejido tumoral y/o en ADNtc. Si se determina la idoneidad mediante un laboratorio local aprobado por el promotor, se deberá reservar una muestra de tejido tumoral y/o muestra de sangre para análisis de secuenciación retrospectivo en el laboratorio central elegido por el Promotor. 4.Enfermedadcuantificable según RECIST, versión 1.1. 5.Estado funcional del Grupo Oncológico Cooperativo de la Costa Este (ECOG) 0, 1 o 2. 6.Actividad suficiente de la médula ósea y del organismo, en función de lo siguiente: a.Recuento absoluto de neutrófilos >= 1.000/mm3 (>= 1,0 × 109/l). b.Aspartato aminotransferasa (AST) y alanina aminotransferasa (ALT) <= 2,5 × límite superior de la normalidad (LSN), o <= 5,0 × LSN en pacientes con metástasis hepática confirmada. Niveles de fosfatasa alcalina <= 2,5 × LSN. Si existen metástasis óseas extensas, no hay límite superior para el nivel de fosfatasa alcalina. c.Bilirrubina total <= 1,5 x LSN o <= 3,0 x LSN en pacientes con síndrome de Gilbert o metástasis hepáticas confirmadas. 7.>= 18 años de edad. 8.Pacientes u hombres cuyas parejas sean capaces de procrear que acepten utilizar métodos anticonceptivos mientras dure la participación en este estudio y durante un periodo de 6 meses después de la finalización del tratamiento del estudio. 9.Haber completado el proceso de dar el consentimiento informado, lo que incluye firmar un formulario de consentimiento informado aprobado por una JRI/CE. 10.Aceptar cumplir con las instrucciones y requisitos del ensayo clínico. |

1. Prior treatment with a small molecule or antibody inhibitor of MET or HGF. 2. Prior positive test for EGFR mutation or ALK gene rearrangement. Testing and documentation is required for patients with adenocarcinoma histology Testing is not required for patients with non-adenocarcinoma histology; however if documentation of a positive test is available, the patient will not be eligible 3. Most recent prior systemic therapy(e.g. chemotherapy or immunotherapy)or investigational agent discontinued <=2 weeks before first dose date. 4. Absence of recovery from the adverse effects of prior therapy at the time of enrollment to <=Grade 2 (excluding alopecia). 5. History of stroke or transient ischemic attack within the previous 6 months. 6. Any of the following cardiac abnormalities: -Unstable angina pectoris, -Congestive heart failure >=NYHA Class 3, or -QTc > 480 msec. 7. Known or suspected presence of another malignancy that could be mistaken for metastatic NSCLC during disease assessments. 8. Symptomatic or uncontrolled brain metastases requiring current treatment (less than 4 weeks from last cranial radiation, 2 weeks from stereotactic radiosurgery [gamma knife], or 2 weeks from last steroids). Known conditions associated with significant risk of intracranial bleeding including but not limited to vascular malformations and pituitary adenoma. 9. Inability to swallow oral medications or pre-existing gastrointestinal disorders that might interfere with proper absorption of oral drugs. 10. Known hypersensitivity to any of the components of the MGCD265 Drug Product. 11. Pregnancy. WOCBP must have a negative serum or urine pregnancy test documented within the screening period prior start of study drug. 12. Breast-feeding or planning to breast feed during the study or within 6 months after study treatment. 13. Any serious illness, uncontrolled inter-current illness, active or uncontrolled infection, or other medical history, including laboratory results, which, in the Investigator's opinion, would be likely to interfere with the patient's participation in the study, or with the interpretation of the results. |
1.Tratamiento anterior con una molécula pequeña o anticuerpo con capacidad de inhibir el factor de transición epitelio-mesenquimal o HGF. 2.Prueba anterior con resultado positivo para mutación EGFR o para reordenación génica de ALK. La prueba y documentación son requeridas para pacientes con histologia de adenocarcinoma. El test no se requiere para pacientes con histologia no de adenocarcinoma; sin embargo si está disponible la documentación de un test positivo, el paciente no será elegible. 3.La terapia sistemica previa (ej.: quimioterapia o inmunoterapia) o terapia con un fármaco experimental más reciente, se interrumpió <= 2 semanas antes de la fecha prevista de la primera dosis del medicamento en estudio. 4.En el momento de la inclusión, el paciente aún no se ha repuesto de los efectos adversos del tratamiento anterior hasta un grado <= 2 (excepto para la alopecia). 5.Antecedentes de accidente cerebrovascular o accidente isquémico transitorio en los 6 meses anteriores. 6.Cualquiera de las anomalías cardíacas siguientes: a.angina de pecho inestable; b.insuficiencia cardiaca congestiva >= clase 3 según la clasificación de la NYHA; o c.QTc > 480 mseg. 7.Presencia, presunta o confirmada, de otra neoplasia que podría haberse confundido con CPNMC metastásico durante las evaluaciones de la enfermedad. 8.Metástasis cerebrales sintomáticas o no estabilizadas que exijan un tratamiento en el momento presente (menos de 4 semanas desde la última sesión de radioterapia craneal, 2 semanas desde radiocirugia estereotáctica [bisturí gamma] o 2 semanas desde la última administración de esteroides). Afecciones confirmadas, asociadas a un riesgo considerable de sangrado intracraneal como pueden ser, entre otras, malformaciones vasculares y adenoma hipofisario. 9.Incapacidad de tragar medicamentos por vía oral o padecer trastornos digestivos que pudieran interferir con la adecuada absorción de los medicamentos administrados por vía oral. 10.Hipersensibilidad confirmada a cualquiera de los compuestos del medicamento MGCD265. 11.Embarazo. Antes de iniciar el tratamiento con el medicamento en estudio, las mujeres con capacidad de procrear deberán disponer de una prueba confirmada de embarazo en suero u orina con resultado negativo en el periodo de la selección. 12.La paciente está amamantando o tiene previsto amamantar durante el estudio o en los 6 meses posteriores al tratamiento del estudio. 13.Cualquier afección grave, enfermedad intercurrente no estabilizada, infección activa o no controlada u otros antecedentes médicos tales como resultados analíticos que, en opinión del investigador, podrían interferir en la participación del paciente en el estudio o en la interpretación de los resultados. |
