E.2 Objective of the trial |

To evaluate the efficacy of dupilumab compared to placebo on a background of mometasone furoate nasal spray (MFNS) in reducing nasal congestion/obstruction (NC) severity and endoscopic nasal polyp score (NPS) in patients with bilateral nasal polyposis (NP). In addition for Japan, reduction in computed tomography (CT) scan opacification of the sinuses will be also a coprimary objective. |
Valutare l’efficacia di dupilumab 3 in confronto al placebo in pazienti con terapia di base con mometasone furoato spray nasale (mometasone furoate nasal spray, MFNS), nel ridurre la gravità della congestione/ostruzione nasale (CN) e il punteggio endoscopico dei polipi nasali (nasal polyp score, NPS) in pazienti con poliposi nasale bilaterale (PN). Inoltre, per il Giappone, costituirà un obiettivo coprimario anche la riduzione dell’opacizzazione dei seni alla tomografia computerizzata (TC). |

Valutare l’efficacia di dupilumab nel migliorare il punteggio totale dei sintomi ( TSS). Valutare l’efficacia di dupilumab nel migliorare la percezione olfattiva. Valutare l’efficacia di dupilumab nel ridurre l’opacizzazione dei seni alla TC (primario per Giappone). Valutare la capacità di dupilumab di ridurre la percentuale di pz che necessitano di un trattamento con corticosteroidi orali o di un intervento chirurgico per la PN. E.3 | Principal inclusion criteria |
Patients with bilateral sinonasal polyposis that despite prior treatment with systemic corticosteroids (SCS) anytime within the past 2 years; and/or have a medical contraindication / intolerance to SCS, and/or had prior surgery for NP at the screening visit, have:
-An endoscopic bilateral NPS of at least 5 out of a maximum score of 8 (with a minimum score of 2 in each nasal cavity).
-Ongoing symptoms (for at least 8 weeks prior to V1) of nasal congestion / blockage / obstruction with moderate or severe symptom severity (score 2 or 3) at V1 and a weekly average severity of greater than 1 at the time of randomization (V2), and another symptom such as loss of smell, rhinorrhea (anterior/posterior).
-Signed written informed consent. |
Pazienti con poliposi naso-sinusale bilaterale, i quali, nonostante siano stati sottoposti in precedenza a trattamento con corticosteroidi sistemici (systemic corticosteroids, SCS) in un qualsiasi momento dei 2 anni precedenti, e/o che mostrino una controindicazione medica/intolleranza ai SCS, e/o si siano precedentemente sottoposti ad intervento chirurgico per la PN, presentino alla visita di screening: • Un punteggio endoscopico bilaterale NPS di almeno 5 su un massimo di 8 (con un punteggio minimo di 2 in ciascuna cavità nasale). • Sintomi continui (per almeno 8 settimane prima della V1) di: - Congestione/blocco/ostruzione nasale con gravità del sintomo da moderata a grave (punteggio 2 o 3) alla V1, ed una gravità media settimanale superiore a 1 al momento della randomizzazione (V2), e un ulteriore sintomo, come perdita della percezione olfattiva o rinorrea (anteriore/posteriore). • Firma del consenso informato scritto.

E.4 | Principal exclusion criteria |
-Patients <18 years of age.
-Patient who has previously been treated in dupilumab studies.
-Patient who has taken:
-Biologic therapy/systemic immunosuppressant to treat inflammatory disease or autoimmune disease (eg, rheumatoid arthritis, inflammatory bowel disease, primary biliary cirrhosis, systemic lupus erythematosus, multiple sclerosis, etc.) within 2 months before V1 or 5 half-lives, whichever is longer.
-Any experimental monoclonal antibody (mAB) within 5 half-lives or within 6 months before V1 if the half-life is unknown.
-Anti-immunoglobulin E (IgE) therapy (omalizumab) within 130 days prior to V1.
-Leukotriene antagonists/modifiers unless patient is on a continuous treatment for at least 30 days prior to V1.
-Initiation of allergen immunotherapy within 3 months prior to V1 or a plan to begin therapy or change its dose during the run-in period or the randomized treatment period.
-Patients who have undergone any intranasal and/or sinus surgery (including polypectomy) within 6 months prior to V1.
-Patients who have had a sinonasal or sinus surgery changing the lateral wall structure of the nose making impossible the evaluation of NPS.
-Patients with conditions/concomitant diseases making them nonevaluable at V1 or for the primary efficacy endpoint such as:
-Antrochoanal polyps.
-Nasal septal deviation that would occlude at least one nostril.
-Acute sinusitis, nasal infection or upper respiratory infection.
-Ongoing rhinitis medicamentosa.
-Allergic granulomatous angiitis (Churg-Strauss syndrome), granulomatosis with polyangiitis (Wegener’s granulomatosis), Young’s syndrome, Kartagener’s syndrome or other dyskinetic ciliary syndromes, concomitant cystic fibrosis.
-Radiologic suspicion, or confirmed invasive or expansive fungal rhinosinusitis.
-Patients with nasal cavity malignant tumor and benign tumors (eg, papilloma, blood boil etc).
-Patients with forced expiratory volume in 1 second (FEV1) 50% or less (of predicted normal).
-Patients receiving concomitant treatment prohibited in the study.
-Treatment with a live (attenuated) vaccine within 12 weeks before the baseline visit.
-History of human immunodeficiency virus (HIV) infection or positive HIV serology at screening.
-Positive with hepatitis B surface antigen (HBsAg) or hepatitis C antibody at the screening visit.
-Active chronic or acute infection requiring systemic treatment within 2 weeks before the baseline visit.
-Known or suspected history of immunosuppression.
-Pregnant or breastfeeding women, or women planning to become pregnant or breastfeed during the study.
-Women unwilling to use adequate birth control, if of reproductive potential and sexually active.

• Pz <18 anni di età. • Pz precedentemente trattati in studi con dupilumab. • Pazienti che hanno assunto: - Una terapia biologica/immunosoppressori sistemici per trattare una malattia infiammatoria o autoimmune (ad es. artrite reumatoide, malattia infiammatoria intestinale, cirrosi biliare primaria, lupus sistemico eritematoso, sclerosi multipla, ecc.) nei 2 mesi precedenti alla V1, o 5 emivite, in base quale abbia la maggiore durata. - Un qualsiasi anticorpo monoclonale sperimentale (monoclonal antibody, mAB) in un periodo che corrisponda a 5 emivite o nei 6 mesi precedenti alla V1, se l’emivita non è nota. - Una terapia anti-immunoglobulina E (IgE) (omalizumab) nei 130 giorni precedenti alla V1. - Antagonisti/modificatori del leucotriene, a meno che il pz non sia sottoposto a trattamento stabile per almeno 30 giorni prima della V1. • Inizio di una immunoterapia specifica per l’allergia nei 3 mesi precedenti alla V1, o per cui sia pianificato l’inizio o una modifica nella dose durante il periodo di run-in o di trattamento randomizzato. • Pazienti sottoposti a chirurgia intranasale e/o sinusale (inclusa polipectomia) nei 6 mesi precedenti alla V1. • Pazienti sottoposti a chirurgia naso-sinusale o sinusale con modificazione della struttura laterale del naso, che rende impossibile la valutazione dei NPS. • Pazienti con disturbi/malattie concomitanti che li rendono non valutabili alla V1 o per l’endpoint di efficacia primario, come ad es.: - Polipi antrocoanali. - Deviazione del setto nasale che occluderebbe almeno una narice. - Sinusite acuta, infezione nasale o infezione delle vie aeree superiori. - Rinite medicamentosa in atto. - Angioite granulomatosa allergica (sindrome di Churg-Strauss), granulomatosi con poliangioite (granulomatosi di Wegener), sindrome di Young, sindrome di Kartagener o altre sindromi ciliari discinetiche, fibrosi cistica concomitante. - Sospetto radiologico o conferma di rinosinusite fungina invasiva o espansiva. • Pazienti con tumori maligni e benigni delle cavità nasali (ad es. papilloma, emangioma, ecc.). • Pazienti con volume espiratorio massimo nel 1°secondo (FEV1) del 50% o inferiore (rispetto al valore normale previsto).
Pz che stanno ricevendo terapie concomitanti vietate nel corso dello studio - Trattamento con vaccino vivo (attenuato) nelle 12 settimane prima della visita basale; - Anamnesi di HIV o positività sierologica all'HIV allo screening Positività all'antigene di superficie dell'Epatire B (HBsAg) o all'anticorpo dell'epatite C alla visita di screening Infezione cronica o acuta che richieda un trattamento sistemico nelle 2 settimane prima della visita basale Nota o sospetta anamnesi di immunosoppressione Donne incinta o in allattamento, o donne che stiano pianificando una gravidanza o allattamento nel corso dello studio.
