E.2 Objective of the trial |
E.2.1 | Main objective of the trial |
The primary objective of the trial is to demonstrate that the test (diclofenac2% gel produced by Epifarma) is non-inferior to the reference(Voltaren®Emulgel®2% gel), and that both, test and reference, are superior to placebo gel, in improving pain symptoms in patients with acute painful and flogistic (inflammatory) disease due to acute traumatic events (injury/contusion) of joints, muscles, tendons and ligaments over a treatment period of maximum 1 week. Efficacy will be determined by a 100-mm Visual Analogic Scale (VAS) assessment of pain at rest. |
L’obiettivo primario dello studio è dimostrare che il prodotto Test (diclofenac2%gel prodotto da Epifarma) sia non inferiore al prodotto di Riferimento(Voltaren Emulgel®2%, gel), e che entrambi I prodotti Test e Riferimento siano superiori al Placebo nel miglioramento dei sintomi del dolore in pazienti con un infortunio/contusione traumatica acuta o dolore flogistico(infiammatorio) in articolazioni,muscoli,tendini e legamenti in un periodo di trattamento di 1 settimana. L’efficacia nella valutazione del dolore a riposo verrà determinata mediante l’utilizzo di una scala analogica visiva(Scala VAS)lunga 100mm. |

Gli Obiettivi secondari dello studio sono il confronto degli effetti, tollerabilità e sicurezza dei prodotti Diclofenac 2% gel, sia Test che di Riferimento e del placebo, rispetto:

E.3 | Principal inclusion criteria |
• Male and female patients aged 18-65 years; • Patients with acute painful and flogistic (inflammatory) disease due to acute traumatic events (injury/contusion) of, joints, muscles, tendons and ligaments. • Patient with pain at rest in only one body surface area affected by the soft tissue injury/contusion; • Presence of mild to moderate pain at rest, defined as a scored VAS = 20 mm and = 60 mm, in the injured area; • Female of child-bearing potential (i.e. not in menopausal status from at least one year or permanently sterilized) must have a negative urine pregnancy test prior the first administration. • Satisfactory general health status as determined by the investigator based on medical history and physical examination; • Patients must understand and provide written informed consent before He/She can participate in the study. He/She must understand the study procedures of the trial, and be willing to complete the required assessments.

• Maschi e femmine di età compresa tra i 18 e 65 anni; • Pazienti con sintomi di dolore acuto o flogistico (infiammatorio) dovuto a eventi traumatici (infortunio/contusione) ad articolazioni, muscoli, tendini e legamenti; • Pazienti con dolore a riposo,dovuto a un infortunio/contusione traumatica acuta in una sola parte della superficie corporea, • Presenza di dolore nell’area interessata indicato come da leggero a moderato, definito mediante un punteggio della scala VAS a riposo tra i 20 mm e i 60 mm • Se donna in età fertile (non in menopausa da almeno un anno) dovrà avere un test di gravidanza (urine) negativo prima della prima amministrazione del farmaco in studio. • Uno Stato di salute generale determinato come soddisfacente dallo Sperimentatore sulla base della storia medica e dall’analisi fisica del paziente. • Il/la paziente deve essere in grado di intendere e volere e deve fornire un consenso informato scritto prima della partecipazione allo studio. Il/la paziente dovrà intendere le procedure previste dallo studio e dovrà essere disposto a completare le valutazioni richieste.

E.4 | Principal exclusion criteria |
• Patients unwilling to give an informed consent approval; • Patients with presence of pain at rest in more than one body surface area affected by injury/contusion; • Patients with a chronic painful or flogistic (inflammatory) disease (from more than three months); • Patients with painful or flogistic (inflammatory) disease arising from fractures or severe trauma events; • Pregnancy or lactation period throughout the whole study duration; • Women with childbearing potential who are not using adequate methods to avoid pregnancy as double barrier or hormonal contraceptive methods; • Presence of concurrent skin disorders or open wounds in the area to be treated; • History of alcohol or drug abuse; • History of allergy or hypersensitivity or intolerance to diclofenac and/or to active or inactive excipients of formulation; • Known hypersensitivity to non-steroid anti-inflammatory drugs, and in particular to paracetamol; • Use of non-steroid anti-inflammatory drugs (e.g. acetyl salicylic acid) and analgesics (with the exception of paracetamol) in the week before the study. Chronic intake of small doses of acetylsalicylic acid (= 162 mg/daily) taken for at least 30 days prior to the first dose of study medication for non-analgesic reasons may be continued for the duration of the study; • Any other treatment or medication for the same or other indication that, according to its pharmacological properties and in the opinion of the investigator can alter the perception of pain (e.g. heparinoids, opioids, psychotropic agents, anti H1 agents or analgesics like glucocorticosteroids, NSAIDs, etc.); in the week before the study; • Any other concomitant treatment (e.g. cosmetics, ointments at the treated area) or medication that cannot be interrupted and interferes with the conduct of the trial; • History of active or suspected esophageal, gastric, pyloric channel, or duodenal ulceration or bleeding within 30 days preceding screening; • History of uncontrolled chronic or acute concomitant disease which, in the Investigator’s opinion, would contraindicate study participation or confound interpretation of the results; • Participation in any other clinical study within 30 days prior to the screening |
• Pazienti che non intendano dare l’approvazione mediante firma del consenso informato; • Pazienti con presenza di dolore a riposo dovuto a un infortunio/contusione in più di un’area della superficie corporea; • Pazienti con sintomi di dolore o di infiammazione cronica (da più di tre mesi); • Pazienti con sintomi di dolore o di infiammazione dovuti a fratture o a eventi traumatici severi; • Pazienti in gravidanza o in allattamento durante il periodo di studio; • Donne in età fertile che non stiano utilizzando un adeguato metodo di contraccezione come metodi di contraccezione a doppia barriera o ormonali; • Presenza di disturbi cutanei o ferite aperte in corrispondenza dell’area del trattamento; • Storia di abuso di Alcool o Droghe; • Conoscenza di allergia o ipersensibilità o intolleranza al diclofenac e/o agli eccipienti di formulazione attivi o inattivi; • Ipersensibilità conosciuta ai farmaci anti-infiammatori non-steroidei e in particolare al Paracetamolo; • Utilizzo di farmaci anti-infiammatori non-steroidei (es. Acido acetilsalicilico) e analgesici (con la sola eccezione del paracetamolo) nella settimana precedente lo studio. L’assunzione continuativa di piccole dosi di acido acetilsalicico (= 162 mg/giorno) per ragioni non analgesiche per almeno 30 giorni prima dell’assunzione della prima dose di farmaco di studio può essere continuata per tutta la durata dello studio; • Ogni altro trattamento o medicazione, per la stessa o altra indicazione, che in accordo alle proprie caratteristiche farmacologiche e a opinione dello sperimentatore possa alterare la percezione del dolore (Eparinoidi, oppioidi, agenti psicotropi, agenti anti-H1 o analgesici quali i glucocorticosteroidi; FANS) durante la settimana prima dello studio; • Qualsiasi altro trattamento concomitante (es. cosmetici, unguenti) nell’area del trattamento o medicazione che non possa essere interrotto e che interferisca con la conduzione dello studio; • Storia clinica di sospette o presenti ulcerazioni o sanguinamenti nei 30 giorni antecedenti lo sceening a livello dell’esofago, gastrico, pilorico o duodenale; • Storia clinica di patologie concomitanti acute o croniche non controllate che a giudizio dello sperimentatore possano pregiudicare la partecipazione allo studio o confondere l’interpretazione dei risultati; • Partecipazione ad altri studi clinici nei 30 giorni precedenti alla fase di screening |
