1. Informed consent given according to local requirements before any trial-related activity. (A trial-related activity is any procedure that would not have been performed during the routine management of the patient.)
2. Legally competent male or female outpatient
3. Age between 18 to 65 years
4. IgE-mediated seasonal allergic rhinitis or rhinoconjunctivitis with or without bronchial asthma caused by birch pollen documented by:
4a) SPT wheal for birch pollen ≥ 3mm in diameter
4b) Histamine wheal ≥ 3mm
4c) NaCl control reaction < 2mm
5. IgE result ≥ 0.70 kU/L to birch pollen
6. At least 1 month with symptoms due to allergic rhinitis or rhinoconjunctivitis in the months March to May with birch pollen exposure
7. In case of bronchial asthma at entry: confirmed diagnosis of asthma
8. In case of a diagnosed asthma: asthma classified as being “well controlled” according to GINA guideline (GINA, 2015)
9. Treatment with anti-allergic medications for at least 2 years prior to enrolment

1. Der Patient hat vor der Durchführung jeglicher studienbezogener Verfahren seine Einwilligung gemäß den örtlichen Anforderungen erteilt. (Ein studienbezogenes Verfahren ist jedes Verfahren, das bei einer routinemäßigen Behandlung des Patienten nicht durchgeführt worden wäre.)
2. Der Patient ist im rechtlichen Sinne geschäftsfähig.
3. Der Patient ist zwischen 18 und 65 Jahre alt.
4. Der Patient leidet an einer IgE-vermittelten saisonalen allergischen Rhinokonjunktivi-tis mit oder ohne durch Birkenpollen verursachtem Asthma, belegt durch den Hautpricktest (HPT):
4 a) Birkenpollen-Allergen: Quaddel ≥ 3 mm Durchmesser
4 b) Histamin: Quaddel ≥ 3 mm
4 c) NaCl -Kontrollreaktion: Quaddel < 2 mm
5. IgE -Wert ≥ 0,70 kU/l für Birkenpollen-Allergene
6. Der Patient hat mindestens 1 Monat Symptome aufgrund von allergischer Rhinitis oder Rhinokonjunktivitis in der Zeit mit Birkenpollenexposition von März – Mai.
7. Im Falle von bronchialem Asthma bei Studienbeginn: Bei dem Patienten liegt eine bestätigte Asthmadiagnose vor.
8. Bei vorliegender Asthmadiagnose: Das Asthma ist „gut kontrolliert“ im Sinne der GINA Leitlinie (GINA, 2015).
9. Behandlung mit Antiallergika mindestens 2 Jahre vor Studienbeginn. |

Immunotherapy criteria:
14. AIT with birch pollen allergens within the last 5 years
15. Current treatment with any kind of immunotherapy
16. AIT with unknown allergen within the last 5 years
Diseases and health status:
17. Clinically relevant chronic rhinoconjunctival or respiratory symptoms related to other reasons than allergy
18. Peak expiratory flow (PEF) < 70% of predicted normal (European Coal and Steel Community) under adequate asthma treatment according to GINA guideline (GINA, 2015)
19. Uncontrolled or partly controlled asthma according to GINA guideline (GINA, 2015)
20. Rhinoconjunctival atopic symptoms for 20 years or longer
21. Severe acute or chronic diseases (e.g. diabetes mellitus type I, malignant neoplasia, chronic renal failure, active tuberculosis), severe inflammatory diseases (liver, kidneys)
22. Autoimmune diseases, immune-defects including immune-suppression, immune-complex-induced immunopathies (e.g. HIV, post-transplant patients, vitiligo, lupus erythematodes [SLE], Grave’s disease, multiple sclerosis)
23. Severe psychiatric and psychological disorders including impairment of cooperation (e.g. alcohol or drug abuse)
24. Recurrent seizures
25. Irreversible secondary alterations of the reactive organ (e.g. emphysema, bronchiectasis)
26. Laboratory value greater than grade 1 according to the FDA Guidance for Industry (Toxicity Grading Scale for Healthy Adult and Adolescent Volunteers Enrolled in Preventive Vaccine Clinical Trials) (FDA, 2007)
Medications:
27. Use of β-blockers (locally and systemically)
28. Contraindication for use of adrenalin (e.g. acute or chronic symptomatic coronary heart disease, severe hypertension)
29. Completion or ongoing treatment with anti-IgE-antibody
30. Completion or onging long-term treatment with tranquilizer or other psychoactive drugs

6. Der Patient steht in irgendeiner Abhängigkeitsbeziehung zum Sponsor oder dem Prüfarzt oder beiden.
7. Der Patient wurde bereits früher in diese Studie aufgenommen bzw. randomisiert.
8. Der Patient ist geistig behindert.
9. Der Patient befindet sich aufgrund einer behördlichen oder richterlichen Anordnung in einer Anstalt.
Frauen, die schwanger werden können (d. h. Frauen, die nicht chemotherapeutisch verhüten, operativ steril oder, die nicht postmenopausal sind):
10. Die Patientin weist beim Screening einen positiven Schwangerschaftstest auf oder ist schwanger.
11. Die Patientin verwendet während der Studie nach Meinung des Prüfarztes eine inakzeptable und unzuverlässige Form der Empfängnisverhütung. (Eine verlässliche Methode der Empfängnisver¬hütung ist definiert durch eine niedrige Versagensrate von weniger als 1 % pro Jahr).
12. Die Patientin stillt oder möchte stillen.
13. Die Patientin plant während der Studie schwanger zu werden.
Kriterien der Immuntherapie:
14. Der Patient hat sich in den letzten 5 Jahren einer spezifischen Immuntherapie mit Birkenpollen-Allergenen unterzogen.
15. Der Patient wird gegenwärtig mit irgendeiner Immuntherapie behandelt.
16. Der Patient hatte eine Immuntherapie mit unbekanntem Allergen innerhalb der letzten 5 Jahre.
Krankheiten und Gesundheitszustand:
17. Der Patient hat klinisch relevante chronische rhinokonjunktivale oder respiratorische Symptome aufgrund anderer als allergischer Ursachen.
18. Der Patient hat einen PEF Wert < 70 % der Norm (ECSC) unter adäquater Asthmabehandlung gemäß GINA Leitlinie (GINA, 2015).
19. Der Patienten leidet an unkontrolliertem oder teilweise kontrolliertem Asthma gemäß GINA-Leitlinie (GINA, 2015).
20. Der Patient leidet seit 20 Jahren oder länger an rhinokonjunktivalen Atopiesymptomen?
21. Der Patient leidet an schweren akuten oder chronischen Krankheiten (z. B. Diabetes mellitus Typ I, maligne Neoplasie, chronische Niereninsuffizienz, aktive Tuberkulose) oder an schweren entzündlichen Erkrankungen (Leber, Niere).
22. Der Patient leidet an Autoimmunkrankheiten, Immundefekten mit Immunsuppression, immunkomplexinduzierten Immunopathien (z. B. HIV, Patienten mit Zustand nach Transplantation, Vitiligo, Lupus erythematodes [SLE], Morbus Basedow, Multiple Sklerose).
23. Der Patient leidet an schweren psychiatrischen oder psychologischen Störungen mit Kooperationsbeeinträchtigung (z. B. Alkohol- oder Drogenmissbrauch).
24. Der Patient leidet an rezidivierenden Krampfanfällen.
25. Der Patient leidet an irreversiblen Sekundärveränderungen am reaktiven Organ (z.B. Emphysem, Bronchiektase).

The aim of the trial is to obtain information about the safety and tolerability of dose escalation with 4 and 7 injections, respectively, of Allergovit birch.
To achieve this goal, the following safety variables will be considered:
• Number, incidence, type, and intensity of local AEs (local reactions at the injection site > 5 cm or leading to distinct discomfort interfering with the patient’s daily activities), AEs and serious adverse events (SAEs) by MedDRA primary System Organ Class and Preferred Term;
• AEs as described above but considered to be related to trial medication by the investigator;
• Incidence and intensity of anaphylactic reactions after injections according to the WAO grading system;
• Number of patients reaching the maintenance dose without dose adjustment;
• Change of laboratory values (hematology, clinical chemistry and urinalysis) measured before and after the treatment phase;
• Change of vital signs and lung function measured before, during (after each injection) and after treatment;
• Number of local reactions at the injection site ≤ 5 cm;
• Assessment of the tolerability by the investigator and the patient using a 5 point Likert scale (Likert, 1932)
For efficacy, the change of specific total IgG and specific IgG4 for Betula alba between screening visit (S1) and final visit (FV) will be evaluated. |
Das Ziel der Studie besteht darin, Informationen über die Sicherheit und Verträglichkeit einer Aufdosierung von Allergovit Birke mit jeweils 4 bzw. 7 Injektionen zu sammeln.
Um dieses Ziel zu erreichen, werden die folgenden Variablen bedacht:
• Anzahl, Inzidenz, Art und Intensität von lokalen UEs (lokale Reaktionen an der Injektionsstelle > 5 cm), UEs und schwerwiegende unerwünschte Ereignisse (SUEs) nach dem primären Organklassensystem (SOC) und dem bevorzugten Begriff aus MedDRA,
• UEs, wie oben beschrieben, die aber vom Prüfarzt als mit dem Prüfpräparat zusammen¬hängend eingestuft werden,
• Inzidenz und Intensität von anaphylaktischen Reaktionen nach den Injek¬tionen gemäß dem WAO (World Allgery Organisation) Einstufungssystem
• Die Anzahl von Patienten, die die Erhaltungsdosis ohne Rückdosierung erreichen,
• Änderung der vor und nach Behandlung ermittelten Laborwerte (Hämatologie, klini-sche Chemie) und Urinanalyse,
• Änderung der Vitalzeichen und Lungenfunktion, die vor, während und nach der Behandlung erhoben werden,
• Anzahl der lokalen Reaktionen an der Injektionsstelle ≤ 5 cm und
• Bewertung der Gesamtverträglichkeit durch den Prüfarzt und Patienten anhand der 5-Punkte-Skala nach Likert (Likert, 1932)
Zur Bewertung der Wirksamkeit wird die Veränderung des spezifischen gesamt-IgG und spezifischen IgG4 für Betula alba zwischen Screening (Visite S1) und Behandlungsende (Visite FV) bestimmt. |
