E.3 | Principal inclusion criteria |
1. Signed informed consent (IC) obtained before any study specific procedures. Subjects must be able to understand and willing to sign a written informed consent. 2. Male or female subjects 18 years of age. 3. Life expectancy of at least 3 months. 4. Histological or cytological documentation of adenocarcinoma of the colon or rectum. All other histological types are excluded. 5. Measurable metastatic stage IV disease with at least 1 measurable metastatic lesion following RECIST criteria v 1.1 and be documented by radiological evaluation. 6. Subjects with metastatic colorectal cancer (Stage IV). 7. Progression during or within 3 months following the last administration of approved standard therapies which must include fluoropyrimidine, oxaliplatin, irinotecan, an anti-VEGF and an anti-EGFR (if RAS WT). 8. Subjects treated with oxaliplatin in an adjuvant setting should have progressed during or within 6 months of completion of adjuvant therapy. 9. Subjects who progress more than 6 months after completion of oxaliplatin containing adjuvant treatment must be retreated with oxaliplatin-based therapy to be eligible. Subjects who have withdrawn from standard treatment due to unacceptable toxicity warranting discontinuation of treatment and precluding retreatment with the same agent prior to progression of disease will also be allowed into the study. 10. ECOG Performance Status of 0 or 1(within 14 days prior to the initiation of study treatment). 11. Adequate bone marrow, liver and renal function as assessed by the following laboratory requirements: a. Total bilirubin ≤ 1.5 x the upper limit of normal (ULN). b. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) ≤ 2.5 x ULN (5 x ULN for subjects with liver involvement of their cancer). c. Alkaline phosphatase limit ≤ 2.5 x ULN (5 x ULN for subjects with liver and/or bone involvement of their cancer). d. Lipase ≤ 1.5 x the ULN. e. Serum creatinine 1.5 x the ULN or ≥ 30 mL/min as calculated using the Cockcroft-Gault equation. f. Platelet count >100000/mm3, hemoglobin >9 g/dL, absolute neutrophil count (ANC) >1500/mm3. g. International normalized ratio (INR)/ Partial thromboplastin time (PTT) 1.5 x ULN. (Subjects who are therapeutically treated with an agent such as warfarin or heparin will be allowed to participate provided that no prior evidence of underlying abnormality in coagulation parameters exists. Close monitoring of at least weekly evaluations will be performed until INR/PTT is stable based on a measurement that is pre-dose as defined by the local standard. h. Blood transfusion to meet the inclusion criteria will not be allowed. 12. Women of childbearing potential and men must agree to use adequate contraception before entering the program until at least 8 weeks after the last study drug administration. The investigator or a designated associate is requested to advise the subject on how to achieve an adequate birth control. Adequate contraception is defined in the study as any medically recommended method (or combination of methods) as per standard of care. Women of childbearing potential must have a blood or urine pregnancy test performed a maximum of 7 days before start of study treatment, and a negative result must be documented before start of study treatment. |
1. Obtention du consentement éclairé (CÉ) signé avant toute procédure spécifique à l'étude. Les patients doivent disposer des facultés nécessaires à la compréhension et à la volonté de signer un formulaire de consentement éclairé écrit. 2. Patients de sexe masculin ou féminin âgés d’au moins 18 ans. 3. Espérance de vie d’au moins 3 mois. 4. Adénocarcinome du colon ou du rectum confirmé par histologie ou cytologie Tous les autres types histologiques sont exclus. 5. Maladie métastatique mesurable de stade IV avec au moins une lésion métastatique mesurable d'après la v 1.1 des critères RECIST et confirmée par examen radiologique. 6. Patients atteints d'un cancer colorectal métastatique (de stade IV). 7. Progression pendant ou dans les trois mois suivant la dernière administration de traitements conventionnels approuvés incluant nécessairement la fluoropyrimidine, l'oxaliplatine, l'irinotécan, un anti-VEGF et un anti-EGFR (si RAS WT). 8. Les patients traités par oxaliplatine en tant que traitement adjuvant devront présenter une progression de la maladie pendant ou dans les 6 mois suivant la fin du traitement adjuvant. 9. Les patients dont la maladie a progressé plus de six mois après la fin du traitement adjuvant par oxaliplatine devront être de nouveau traités par oxaliplatine pour être admissibles. Les patients ayant interrompu leur traitement conventionnel en raison d'une toxicité inacceptable justifiant cet arrêt du traitement et refusant d'être de nouveau traités par le même agent avant la progression de la maladie seront également admissibles à l'étude. 10. Indice de performance ECOG de 0 à 1 (dans les 14 jours précédant le début du traitement de l'étude). 11. Fonctions de la moelle osseuse, hépatique et rénale adéquates, évaluées par les exigences de laboratoire suivantes: a. Bilirubine totale ≤ 1,5 x la limite supérieure de la normale (LSN). b. Alanine aminotransférase (ALT) et aspartate aminotransférase (AST) ≤ 2,5 x LNS (5 x LNS pour les patients présentant des problèmes hépatiques associés à leur cancer). c. Phosphatase alcaline max. ≤ 2,5 x LNS (5 x LNS pour les patients présentant des problèmes hépatiques et/ou osseux associés à leur cancer). d. Lipase ≤ 1,5 x la LNS. e. Créatinine sérique ≤ 1,5 x la LSN ou ≥ 30 mL/mL, calculée au moyen de la formule de Cockcroft-Gault. f. Numération plaquettaire >100 000/mm³, hémoglobine >9 g/dL, nombre absolu de neutrophiles (ANC) >1 500/mm³. g. Rapport international normalisé (INR)/temps de thromboplastine partielle (TTP) 1,5 x LNS. Les patients traités par un agent tel que la warfarine ou l'héparine seront autorisés à participer étant donné qu'aucun résultat antérieur n'a montré d'anomalie sous-jacente dans les paramètres de coagulation. Une surveillance étroite consistant en des examens au moins hebdomadaires sera mise en place jusqu'à ce que l'INR/le TTP soit stable, selon une mesure prise avant l'administration de la dose, telle que définie par les normes locales. h. La transfusion sanguine pour répondre aux critères d'inclusion ne sera pas autorisée. 12. Les femmes en âge de procréer et les hommes doivent accepter d'utiliser un moyen de contraception adaptée avant d'entrer dans l'étude et jusqu'à 8 semaines après la dernière administration du médicament de l’étude. L'investigateur ou un associé désigné doit donner des conseils au patient quant à la manière d'utiliser correctement un moyen de contraception adapté. Un moyen de contraception adapté est défini dans l'étude comme toute méthode médicalement recommandée (ou combinaison de méthodes) dans le cadre des normes de soins. Les femmes en âge de procréer doivent passer un test de grossesse sanguin ou urinaire qui sera effectué au maximum 7 jours avant le début du traitement de l'étude. Son résultat devra être négatif et être renseigné avant le début de l'étude. |

1. Prior treatment with regorafenib. 2. Major surgical procedure, open biopsy, or significant traumatic injury within 28 days before start of study drug. 3. Pregnant or breast-feeding subjects. 4. Congestive heart failure ≥ New York Heart Association (NYHA) class 2. 5. Unstable angina (angina symptoms at rest), new-onset angina (begun within the last 3 months). 6. Myocardial infarction less than 6 months before start of study drug. 7. Cardiac arrhythmias requiring anti-arrhythmic therapy (beta blockers or digoxin are permitted). 8. Uncontrolled hypertension. (Systolic blood pressure > 140 mmHg or diastolic pressure > 90 mmHg despite optimal medical management). 9. Arterial or venous thromboembolism within 6 months prior to randomization. 10. Pleural effusion or ascites that causes respiratory compromise (CTCAE Grade 2 dyspnea). 11. Ongoing infection > Grade 2 CTCAE v. 4.0. 12. Known history of human immunodeficiency virus (HIV) infection. 13. Known history of active hepatitis B or C, or chronic hepatitis B or C requiring treatment with antiviral therapy. 14. Subjects with seizure disorder requiring medication. 15. History of organ allograft. 16. Subjects with evidence or history of any bleeding diathesis, irrespective of severity. 17. Any hemorrhage or bleeding event ≥ CTCAE Grade 3 within 4 weeks prior to the start of study medication. 18. Non-healing wound, ulcer, or bone fracture. 19. Renal failure requiring hemo-or peritoneal dialysis. 20. Dehydration CTCAE v. 4.0 Grade ≥ 1. 21. Substance abuse, medical, psychological or social conditions that may interfere with the subject's participation in the study or evaluation of the study results. 22. Known hypersensitivity to any of the study drugs, study drug classes, or excipients in the formulation. 23. Any illness or medical conditions that are unstable or could jeopardize the safety of the subject and his/her compliance in the study. 24. Interstitial lung disease with ongoing signs and symptoms. 25. Persistent proteinuria of CTCAE Grade 3 (>3.5g/24 hours). 26. Subjects unable to swallow oral medications. 27. Any malabsorption condition. 28. Unresolved toxicity higher than CTCAE (v. 4.0) > Grade 1 attributed to any prior therapy/procedure excluding alopecia, hypothyroidism and oxaliplatin induced neurotoxicity > Grade 2. 29. Subjects treated with strong CYP3A4 inhibitors or inducers (refer to appendix 8 and to section 6.3.8. Prohibited concomitant medication). 30. Subjects receiving G-CSF within 3 weeks prior to signing the ICF. 31. Concomitant participation or participation within the last 30 days in another clinical trial. 32. Systemic anticancer therapy including cytotoxic therapy, signal transduction inhibitors, immunotherapy, and hormonal therapy during this trial or within 4 weeks (or within 6 weeks for mitomycin C) before starting to receive study medication. |

Intervention chirurgicale majeure, biopsie ouverte ou lésion traumatique importante dans les 28 jours précédant la première administration du médicament de l’étude. 3. Patientes enceintes ou allaitantes. 4. Insuffisance cardiaque congestive ≥ classe 2 de la NYHA (New York Heart Association). 5. Angor instable (symptômes d'angine de poitrine au repos), nouvelle apparition d'angor (dans les 3 mois précédents). 6. Infarctus du myocarde moins de 6 mois avant la première administration du médicament de l’étude. 7. Arythmie cardiaque nécessitant un traitement antiarythmique (bêtabloquants ou digoxine sont autorisés). 8. Hypertension non contrôlée. (Tension artérielle systolique >140 mmHg ou tension artérielle diastolique >90 mmHg malgré une prise en charge médicale optimale). 9. Thromboembolie artérielle ou veineuse dans les 6 mois précédant la randomisation. 10. Épanchement pleural ou ascite entraînant une insuffisance respiratoire (dyspnée de degré 2 sur l’échelle CTCAE). 11. Infection en cours >degré 2 sur l'échelle CTCAE v. 4.0. 12. Antécédents connus d’infection par le virus de l’immunodéficience humaine (VIH). 13. Antécédents connus d'hépatite B ou C active, ou d'hépatite B ou C chronique nécessitant un traitement antiviral. 14. Patients épileptiques sous traitement. 15. Antécédents d'allogreffe d'organe. 16. Les patients ayant une diathèse hémorragique connue ou des antécédents de diathèse hémorragique, quelle qu'en soit la gravité. 17. Tout événement hémorragique ≥ degré 3 sur l'échelle CTCAE dans les 4 semaines précédant la première administration du médicament de l'étude. 18. Plaie, ulcère ou fracture osseuse ne guérissant pas. 19. Insuffisance rénale où une dialyse péritonéale ou une hémodialyse est nécessaire. 20. Déshydratation de degré ≥ 1 selon la v. 4.0. sur l'échelle CTCAE. 21. Toxicomanie, troubles médicaux, psychologiques ou sociaux susceptible d’interférer avec la participation du patient à l’étude ou avec l’évaluation de ses résultats. 22. Hypersensibilité connue à l'un des médicaments de l’étude, à l'une des classes de médicaments à l'étude ou à des excipients présents dans la formule du médicament de l'étude. 23. Toute maladie ou trouble médical instable ou pouvant menacer la sécurité du patient et sa capacité à suivre l'étude en respectant la conformité. 24. Pneumopathie interstitielle avec signes et symptômes présents pendant l'étude. 25. Protéinurie persistante de CTCAE de degré 3 sur l’échelle CTCAE (> 3,5 g/24 heures). 26. Patients incapables d'avaler des médicaments par voie orale. 27. Tout problème de malabsorption. 28. Toxicité supérieure au degré 1 de l'échelle CTCAE (v. 4.0) attribuée à tout traitement/procédure à l'exception de l'alopécie, de l'hypothyroïdie et de la neurotoxicité déclenchées par l'oxaliplatine > degré 2. 29. Patients traités par des inhibiteurs puissants ou des inducteurs du CYP3A4 (voir annexe 8 et section 6.3.8. Médicaments concomitants interdits). 30. Patients recevant des G-CSF dans les 3 semaines précédant la signature du FCE. 31. Participation concomitante ou participation dans les 30 jours précédents à un autre essai clinique. 32.
