Phase I part: The primary objective is to evaluate the safety and tolerability of the combination of hypofractionated stereotactic radiation therapy and Durvalumab immunotherapy in recurrent glioblastoma.
Phase II part: The main primary objective of the phase II part is to evaluate the local control at 6 months of the combination of hypofractionated stereotactic radiation therapy and Durvalumab immunotherapy versus hypofractionated stereotactic radiation therapy alone for recurrent glioblastoma. |
Phase I : L’objectif principal est d’évaluer la sécurité de l’association de la radiothérapie hypofractionnée en conditions stéréotaxiques et du Durvalumab dans le glioblastome en récidive.
Phase II : L’objectif principal de la phase II est d’évaluer le contrôle local à 6 mois de l’association de la radiothérapie hypofractionnée en conditions stéréotaxiques et du Durvalumab versus radiothérapie hypofractionnée en conditions stéréotaxiques seule chez des patients porteurs de glioblastome en récidive.

Exploratory studies are included in the prospective STERIMGLI clinical trial (final version n°1 dated 2016-06-27).
The objectives of these exploratory studies are:
Biological Objectives: − To evaluate the correlation between PDL1 expression in primary GBM tumor with time to progression in patient with the combined treatment. − To identify specific biomarkers through proteomic and exosome analysis of response of the combined hFSRT and Durvalumab treatment by studying prediction of such biomarkers of clinical response to this combination (correlation with TTP) and to bring to light new biomarkers of sensitivity either to hFSRT or to hFSRT and Durvalumab combination.
Imaging Objectives: − To evaluate the capacity of multi-modal MR imaging to study the efficacy of this combination and to discriminate progression to pseudo-progression. − To evaluate by multi-modal MR imaging (MRSI, perfusion, diffusion) the correlation between pseudo-progression occurrence after hFRST and durvalumab treatment and TTP and overall survival. |

Les objectifs de ces études exploratoires sont les suivants :
Objectifs des études exploratoires biologiques : − Evaluer la corrélation entre le niveau d’expression de PD-L1 au niveau de la tumeur primitive et le temps jusqu’à progression chez les patients recevant le traitement combiné. − Identifier, via des analyses de protéomique et des analyses sur les exosomes, des biomarqueurs spécifiques de la réponse au traitement combiné et mettre en évidence de nouveaux biomarqueurs de sensibilité à l’hFSRT ou au traitement combiné hFSRT + durvalumab.
Objectifs des études exploratoires en imagerie : − Identifier par l’imagerie IRM multimodale des biomarqueurs d’efficacité du traitement combiné et permettant de discriminer la progression de la pseudo-progression. − Evaluer par l’imagerie IRM multimodale (perfusion, diffusion, spectroscopie) la corrélation entre l’apparition d’une pseudo-progression après traitement par hFSRT et durvalumab et le temps jusqu’à progression et la survie globale.

E.3 | Principal inclusion criteria |
INCLUSION CRITERIA: 1. Age ≥ 18 years at time of study entry. 2. Previous Histopathologic confirmation of glioblastoma. 3. Any line of recurrence of glioblastoma proven by contrast enhanced MRI within 28 days prior to the first fraction of RT, per modified RANO criteria (Wen et al JCO 2010). Note: Recurrence is defined as progression following therapy (i.e., chemotherapy, radiation, second surgery). 4. Recurrent nodule of an histologically confirmed diagnosis of World Health Organization (WHO) Grade IV malignant glioma (Glioblastoma) occurring in or out the previous irradiation fields. 5. Recurrent disease documented by MRI evidence with a size of the recurrence evaluated on T1 post-gadolinium sequence ≤35mm. 6. Patient for which a re-irradiation (by hFSRT) has been decided by the multidisciplinary medical board. 7. Patients with measurable disease. 8. Prior radiotherapy must be ended at least 12 weeks before the first fraction of RT (unless progressive disease outside of the radiation field or histopathologic confirmation of unequivocal tumor to eliminate pseudoprogression images according to RANO recommendations, Wen et al JCO 2010). 9. In case of previous anti-VEGF/VEGFR targeted therapy: at least 28 days between the last injection of anti-VEGF/VEGFR targeted therapy and the first fraction of RT. 10. Karnofsky performance status ≥70. 11. Adequate hematologic, renal and hepatic function, as defined below: - Absolute Neutrophil Count ≥ 1500/mm3 - Haemoglobin ≥ 9.0 g/dL - Platelet count ≥ 100,000/mm3 - Total bilirubin ≤ 1.5 x ULN (for patient with confirmed Gilbert’s syndrome, Total bilirubin ≤ 3 x ULN) - Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) ≤ 2.5 x ULN - Creatinine ≤ 1.5 x ULN or creatinine clearance (CrCl) ≥ 40 mL/min, using the Cockcroft-Gault formula: o Female CrCl = (140 - age in years) x weight in kg x 0.85 /72 x serum creatinine in mg/dL o Male CrCl = (140 - age in years) x weight in kg x 1.00/72 x serum creatinine in mg/dL 12. Female Patients must either be of non-reproductive potential (i.e., post-menopausal by history: ≥60 years old and no menses for ≥ 1 year without an alternative medical cause; OR history of hysterectomy, OR history of bilateral tubal ligation, OR history of bilateral oophorectomy) or must have a negative serum pregnancy test upon study entry. 13. Written informed consent and any locally required authorization (e.g., Social security for France (Health Insurance)) obtained from the patient prior performing any protocol-related procedures, including screening evaluations. 14. Patient willing and able to comply with the protocol for the duration of the study including undergoing treatment and scheduled visits and examinations including follow up. |
CRITERES D’INCLUSION : 1. Age ≥ 18 ans à l’entrée dans l’étude. 2. Diagnostic de glioblastome préalablement confirmé par histologie. 3. Glioblastome en récidive (quelle que soit la ligne thérapeutique) prouvé par une IRM avec produit de contraste réalisée dans les 28 jours avant la première fraction de radiothérapie, selon les critères RANO modifiés (Wen et al JCO 2010). Note: la récidive est définie comme une progression avérée après un traitement (i.e. chimiothérapie, radiothérapie, 2e chirurgie). 4. Nodule récidivant d’un gliome malin de grade IV selon la classification OMS (glioblastome) histologiquement confirmé se présentant à l’intérieur ou en dehors de la précédente zone d’irradiation. 5. Maladie récidivante documentée par IRM et dont la taille de la récidive évaluée sur la séquence T1 post-gadolinium est ≤ 35mm. 6. Patients pour lesquels une ré-irradiation (par hFSRT) a été décidée en RCP. 7. Patients dont la maladie est mesurable. 8. En cas de radiothérapie antérieure, celle-ci devra avoir été terminée au minimum 12 semaines avant la première fraction de radiothérapie de l’étude (à l’exception d’une maladie progressive en dehors du champ d’irradiation ou de confirmation histopathologique sans équivoque de tumeur pour éliminer les images de pseudo progression, en accord avec les recommandations RANO, Wen et al JJCO 2010). 9. En cas de précèdent traitement par une thérapie ciblée anti-VEGF/VEGFR, un délai minimum de 28 jours entre la dernière injection de la thérapie ciblée anti-VEGF/VEGFR et la première fraction de radiothérapie devra être respecté. 10. Indice de performance Karnofsky ≥ 70. 11. Fonctions hématologique, rénale et hépatique adéquates, définies comme ci-après : - Nombre absolu de neutrophiles ≥ 1500/mm3 - Hémoglobine ≥ 9.0 g/dL - Numération plaquettaire ≥ 100,000/mm3 - Bilirubine totale ≤ 1.5 x LNS (pour les patients présentant un syndrome de Gilbert confirmé, Bilirubine totale ≤ 3 x LNS) - Alanine aminotransférase (ALAT) et aspartate aminotransférase (ASAT) ≤ 2.5 x LNS - Créatinine sérique ≤ 1.5 x LNS ou clairance à la créatinine (CrCl) ≥ 40 mL/min, selon la formule de Cockcroft et Gault : o Femmes : CrCl = (140 - âge en années) x poids en kg x 0.85 /72 x créatinine sérique en mg/dL o Hommes : CrCl = (140 - âge en années) x poids en kg x 1.00/72 x créatinine sérique en mg/dL 12. EXCLUSION CRITERIA 1. Multifocal GBM recurrence. 2. Distance between tumor and optic ways including chiasma or brainstem <1 cm. 3. Prior re-irradiation. 4. Prior exposure to Durvalumab or other anti-PD-1, anti-PD-L1, anti-CTLA4 antibodies. 5. Patient who received a live vaccine within 30 days prior to the first fraction of RT. 6. Receipt of the last dose of anti-cancer therapy (chemotherapy, immunotherapy, endocrine therapy, targeted therapy, biologic therapy,tumor embolization, monoclonal antibodies, other investigational agent) within 28 days prior to the first fraction of RT. 7. Current or prior use of immunosuppressive medication within 28 days before the first fraction of RT (exception: systemic corticosteroids at physiologic doses not exceeding 10 mg/day of prednisone or equivalent are allowed as well as steroids as premedication for hypersensitivity reactions (eg, CT scan premedication) - Topical, inhaled, nasal and ophthalmic steroids are not prohibited). 8. Mean QT interval corrected for heart rate (QTc) ≥470 ms calculated from 3 electrocardiograms (ECGs) using Fridericia’s Correction 9. Presence of diffuse leptomeningeal disease or extracranial disease. 10. Active suspected or prior documented autoimmune disease (including inflammatory bowel disease, celiac disease, irritable bowel syndrome, Wegner's granulomatosis and Hashimoto's thyroiditis). Note: participants with vitiligo, type I diabetes mellitus, residual hypothyroidism due to autoimmune condition only requiring hormone replacement, psoriasis not requiring systemic treatment, or conditions not expected to recur in the absence of an external trigger, are permitted to enroll. 11. Known primary immunodeficiency or active HIV. 12. Known active or chronic viral hepatitis or history of any type of hepatitis within the last 6 months indicated by positive test for hepatitis B surface antigen (HBV sAG) or hepatitis C antibody. 13. History of organ transplant requiring use of immunosuppressive medication. 14. History of active tuberculosis. 15. Current pneumonitis or interstitial lung disease. 16. Uncontrolled intercurrent illness including, but not limited to, ongoing or active infection, symptomatic congestive heart failure, uncontrolled hypertension, unstable angina pectoris, cardiac arrhythmia, active peptic ulcer disease or gastritis, active bleeding diatheses. 17. Other invasive malignancy within 2 years prior to entry into the study, except for those treated with surgical therapy only. 18. History of severe allergic reactions to any unknown allergens or any components of the study drug. 19. Any prior Grade ≥ 3 immune-related adverse event (irAE) or any prior corticosteroid-refractory irAE. 20. Participation in any other clinical trial involving another investigational product within 4 weeks prior to the first fraction of RT. 21. Participation in any other clinical trial which delivered a dose >60 Gy for the primo-treatment for glioblastoma. 22. Female patients who are pregnant, breast-feeding or male or female patients of reproductive potential who are not employing highly effective method of birth control. 23. Any condition that, in the clinical judgment of the investigator, is likely to prevent the patient from complying with any aspect of the protocol or that may put the patient at unacceptable risk. 24. Mental impairment (psychiatric illness/social situations) that may compromise the ability of the patient to give informed consent and comply with the requirements of the study. 25. Patient who has forfeited his/her freedom by administrative or legal award or who is under guardianship. |
CRITERES D’EXCLUSION : 1. Récidive de GBM multifocal. 2. Distance entre la tumeur et les voies optiques incluant le chiasma ou le tronc cérébral < 1 cm. 3. Patient ayant déjà reçu une ré-irradiation. 4. Exposition antérieure au Durvalumab ou à un autre anticorps anti-PD-1, anti-PD-L1, anti-CTLA4. 5. Administration d’un vaccin vivant au cours des 30 jours précédant la première fraction de radiothérapie. 6. Administration de la dernière dose d’un traitement anticancéreux (chimiothérapie, immunothérapie, hormonothérapie, thérapie ciblée, biothérapie, embolisation, anticorps monoclonaux, autre médicament expérimental) au cours des 28 jours précédant la première fraction de radiothérapie. 7. Utilisation de médicament immunosuppresseur au cours des 28 jours précédant la première fraction de radiothérapie (exceptions : sont autorisés les corticostéroïdes à action systémique à des doses physiologiques n’excédant pas 10 mg/jour de prednisone ou équivalent ainsi que les corticostéroïdes en traitement préventif des réactions d’hypersensibilité (c’est à dire traitement préventif lors d’une tomodensitométrie) – Les corticostéroïdes topiques, inhalés, intranasaux et ophtalmiques ne sont pas interdits). 8. Intervalle QT moyen corrigé de la fréquence cardiaque (QTc) ≥ 470 ms calculé à partir de 3 électrocardiogrammes (ECG) à l’aide de la correction de Fridericia. 9. Présence de maladie leptoméningée diffuse ou de maladie extra crânienne. 10. Affections auto-immunes actives suspectées ou préalablement documentées (y compris maladies inflammatoires de l’intestin, maladie cÅ“liaque, syndrome du côlon irritable, granulomatose de Wegener et thyroïdite d’Hashimoto). Note : peuvent être inclus les participants présentant un vitiligo, un diabète de type I, un hypothyroïdisme résiduel dû à une pathologie auto-immune ne nécessitant qu’une hormonothérapie substitutive, un psoriasis ne requérant pas de traitement systémique, ou toute condition ne pouvant réapparaitre en l’absence de facteurs extérieurs. 11. Immunodéficience primitive connue ou infection active à HIV. 12. Hépatite virale active ou chronique ou antécédent d’hépatite dans les 6 derniers mois indiqué par un test positif à l’antigène de surface de l’hépatite B (HBC sAG) ou à l’anticorps du virus de l’hépatite C. 13. Antécédent de transplantation d’organe nécessitant l’utilisation d’immunosuppresseurs. 14. Antécédent de tuberculose active. 15. Pneumopathie en cours ou affection pulmonaire interstitielle. 16. Maladie intercurrente non contrôlée y compris, mais non limité à, une infection en cours ou active, une insuffisance cardiaque congestive symptomatique, une hypertension non contrôlée, un angor instable, une arythmie cardiaque, un ulcère gastro-duodénal ou une gastrite, une diathèse hémorragique active. 17. Participation à tout autre essai clinique ayant délivré une dose > 60 Gy pour le primo-traitement du glioblastome. 22. Patientes enceintes, allaitantes, ou patients de sexe masculin ou féminin présentant un potentiel reproductif et qui ne sont pas disposés à utiliser une méthode contraceptive hautement efficace. 23. Toute condition qui, selon le jugement de l’investigateur, ne permettrait pas au patient de respecter un des aspects du protocole ou qui ferait courir un risque inacceptable au patient. 24. Toute condition psychologique (maladie psychiatrique/condition sociale) qui pourrait compromettre la capacité du patient à donner son consentement éclairé et respecter les exigences de l’essai. 25.
