L'objectif principal de cet essai est de comparer l'immunogénicité au mois 6 (M6) de deux schémas de vaccination renforcée innovants contre le pneumocoque (une double ou quadruple doses de PCV13 suivies d'une dose de PPV23 au mois 5), au schéma standard (une dose de PCV13 suivie d'une dose de PPV23 au mois 5), chez des patients présentant une vascularite associée aux ANCA et recevant un traitement par rituximab. |
The primary objective of this trial is to compare immunogenicity at month 6 (M6) of two "reinforced" innovative pneumococcal vaccine regimens (a double or a quadruple dose of PCV13 followed by one dose of PPV23 at month 5), to the standard regimen (one dose of PCV13 followed by one dose of PPV23 at month 5), in patients with ANCA-associated vasculitides receiving rituximab therapy.

Les objectifs secondaires de cet essai sont les suivants: - Evaluer la tolérance clinique et biologique des stratégies vaccinales. - Evaluer et comparer la réponse immunitaire: oLa réponse immunitaire des différentes stratégies vaccinales à M6 en utilisant l'opsonophagocytose (OPA), oL'étendue de la couverture sérotypique après injection de PPV23 à M6, évaluée sur 3 sérotypes spécifiques : 10A, 12F and 15B, oLa durabilité et l'évolution de la réponse immunitaire après le début de chaque stratégie vaccinale. - Evaluer les déterminants de la réponse immunitaire aux différentes stratégies de vaccination anti-pneumococcique à M6 (dont la stratégie vaccinale, âge, sexe, antécédents de traitement par immunosuppresseurs ou immunomodulateurs, délai par rapport à une vaccination par un vaccin non conjugué, antécédent de vaccination par PPV23). - Décrire la fréquence de survenue d'infections invasives à pneumocoque selon les différentes stratégies vaccinales.

The secondary objectives of this trial are the following: - To evaluate clinical and biological safety of the vaccine strategies. - To evaluate and compare immune response: o Functional immune response of the different vaccine strategies at M6 using opsonophagocytosis (OPA), -Extension of serotype coverage after PPV23 injection at M6, assessed on 3 PPV23 specific serotypes : 10A, 12F and 15B serotypes, -Sustainability and evolution of immune response over time after the beginning of each vaccine strategy and an eventual prime-boost effect. - To assess determinants of immune response to pneumococcal vaccine strategies at M6 (including vaccine strategy, age, gender, previous immunosuppressive or immunomodulatory agents, time from previous unconjugated vaccination, previous PPV23 injections). - To describe the frequency of occurrence of invasive pneumococcal infections in the different vaccine strategies.

E.3 | Principal inclusion criteria |
1.Participants ayant un diagnostic de vascularite associée aux ANCA, en particulier GPA et PAM, définie par les critères de l'ACR de 1990 et/ou de la conférence de Chapel Hill et/ou l'algorithme de l'Agence Médicale Européenne, 2.Participants (hommes et femmes) âgés de 18 ans ou plus, 3.Participantes en âge de procréer ayant une méthode de contraception jugée efficace pendant toute la durée de l'étude, 4.Participants dont la maladie a été récemment diagnostiquée au moment de l'inclusion ou présentant une rechute de la maladie. Pour les patients en rechute, un traitement de maintenance à dose stable pendant les 3 derniers mois sera accepté: dose de prednisone <10 mg/jour, dose d'azathioprine dose <3 mg/kg/jour, dose de methotrexate <25 mg/semaine, ou dose de mycophenolate mofetil <3 g/j. 5.Participants ayant une maladie active définie par un BVAS ? 3, 6.Participants devant recevoir du rituximab en traitement d'induction selon le schéma recommandé (soit 375 mg/m2/semaine pendant 4 semaines consécutives) 7.Participants capables d'exprimer leur consentement par écrit avant toute participation à l'étude, 8.Participants affiliés ou bénéficiaires d'un régime de sécurité sociale.

1.Participants with a diagnosis of ANCA-associated vasculitis, in particular GPA and MPA, according to ACR 1990 criteria and/or revised Chapel Hill Consensus Conference definitions and/or European Medical Agency algorithm, 2.Participants (males and females) aged of 18 years or older, 3.Participants with childbearing potential having reliable contraception for the all duration of the study, 4.Participants with newly-diagnosed disease at the time of inclusion or presenting with a relapse of the disease. For relapsing patients, maintenance therapy at stable dose during the last 3 months will be admitted: prednisone dose <10 mg/day, azathioprine dose <3 mg/kg/day, methotrexate dose <25 mg/week, or mycophenolate mofetil dose <3 g/j. 5.Participants with an active disease defined as a BVAS ? 3, 6.Participants planned to receive rituximab as induction therapy using the recommended regimen (i.e. 375 mg/m2/week for 4 consecutive weeks), 7. Participants able to give written informed consent prior to participation in the study, 8. Participants covered by social security regimen or equivalent

1.Participants ayant une granulomatose éosinophilique à polyangéite ou une autre vascularite non associée aux ANCA, 2.Participants présentant une thrombocytopenie ou d'autres troubles de la coagulation 3.Participants ayant une infection aigüe ou une infection chronique active à la visite d'inclusion (notamment sérologie positive documentée pour le VIH, VHB (Ag HBs) ou VHC à l'inclusion). 4.Participants présentant une maladie associée à une réponse immunitaire diminuée (splénectomie, transplantation de cellules souches hématopoïétiques, déficience immune primaire telle que le déficit immunitaire commun variable, cancer dans les 5 années précédentes, drépanocytose), 5.Participants traités par rituximab dans les 12 mois précédents, 6.Participants ayant reçus du sang, des produits sanguins, et/ou des produits dérivés du plasma tels que des préparations parentérales d'immunoglobulines dans les 12 semaines précédant l'inclusion, 7.Participants traités avec un autre agent immunosuppresseur ou immunomodulateur dans les 3 mois précédents (notamment cyclophosphamide, anti-TNF-alpha, immunoglobulines en intraveineuse, abatacept), 8.Participants traités avec une dose >10 mg/jour de prednisone pour une durée supérieure ou égale à 15 jours avant l'inclusion, 9.Participants ayant reçu une injection de vaccin anti-pneumococcique conjugué, à n'importe quel moment, 10. Le critère principal d'évaluation de cet essai est la proportion de patients répondeurs à M6 contre 12 sérotypes, selon les 4 catégories de réponse suivantes, c'est-à-dire le nombre de patients ayant une réponse positive à 0-3, 4-6, 7-9, or 10-12 sérotypes communs aux vaccins PCV13 et PPV23. Selon les critères internationaux, un participant sera considéré comme répondeur à un sérotype donné s'il a, à M6, c'est à dire 1 mois après l'injection de PPV23, un titre d'IgG spécifiques ?1 µg/mL par ELISA, et une augmentation de 2 fois du titre en anticorps comparé au titre à J0. Les critères secondaires d'évaluation de cet essai sont: - Tolérance: -Réactions sollicitées locales et/ou générales survenues dans les 7 jours suivant chaque vaccination : proportion de participants ayant eu un événement, nature, nombre, degré et délai de survenue. oTout événement indésirable survenu pendant l'étude lié ou possiblement lié à la vaccination: proportion de participants ayant eu un événement, nature, nombre, degré et délai de survenue. -Tout événement indésirable survenu pendant l'étude lié à la vaccination et conduisant à un arrêt du schéma vaccinal: proportion de participants ayant eu un événement, nature, nombre, degré et délai de survenue. -Tout événement indésirable grave survenu pendant l'étude, quel que soit le lien avec la vaccination : proportion, nombre, nature, degré et délai de survenue. -Proportion de patients ayant une poussée, selon les critères EULAR, pendant la durée de l'étude et le délai de rechute de la maladie.
- Réponse immunitaire: - Titres d'IgG spécifiques dirigés contre les 12 sérotypes communs aux deux vaccins, conjugué et non conjugué, (1, 3, 4, 5, 6B, 7F, 9V, 14, 18C, 19 A, 19F and 23F) évalués par OPA au jour 0 (J0) et à M6, et proportion des patients répondeurs à M6 (selon les 4 catégories, prédéfinies en fonction du nombre de réponses positives, utilisées pour le critère principal) avec une réponse positive par sérotype définie par un titre >8 et une augmentation d'un facteur 4 du titre en anticorps à M6 en comparaison au titre à J0. -Titres d'IgG spécifiques dirigés contre les 3 sérotypes de PPV23 (10A, 12F et 15B) à J0 et M6, et proportion de participants répondeurs par sérotype individuel et par nombre de réponses à ces trois sérotypes à M6 (avec une réponse positive par sérotype définie par un titre d'IgG spécifiques ?1 µg/mL par ELISA et un titre en anticorps multiplié par deux par rapport au titre à J0) -Titres d'IgG spécifiques dirigés contre les 12 sérotypes communs aux deux vaccins, conjugué et non conjugué, (1, 3, 4, 5, 6B, 7F, 9V, 14, 18C, 19 A, 19F and 23F) évalués par ELISA à M1 et M5 (titres et proportion de patients répondeurs, selon les critères de réponse définis ci-dessus). -Titres d'IgG spécifiques dirigés contre les 12 sérotypes communs aux deux vaccins, conjugué et non conjugué, (1, 3, 4, 5, 6B, 7F, 9V, 14, 18C, 19 A, 19F and 23F) évalués par ELISA à M12 et M18 (titres et proportion de patients répondeurs, selon les critères de réponse définis ci-dessus). -Titres d'IgG spécifiques dirigés contre les 12 sérotypes communs aux deux vaccins, conjugué et non conjugué, (1, 3, 4, 5, 6B, 7F, 9V, 14, 18C, 19 A, 19F and 23F) évalués par OPA à M12 et M18 (titres et proportion de patients répondeurs, selon les critères de réponse définis ci-dessus). - Modélisation des déterminants de l'immunogénicité: stratégie vaccinale, âge, sexe, antécédents de traitements par immunosuppresseurs ou immunomodulateurs, délai par rapport à une vaccination par un vaccin non conjugué, antécédent de vaccination par PPV23 - Nombre et délai de survenue d'infections sévères pendant la durée de l'étude, en particulier les infections invasives à pneumocoque. La description d'éventuelles infections invasives à pneumocoque et de pneumonies communautaires acquises survenant pendant l'étude sera effectuée par documentation des infections bactériennes invasives avec des analyses biologiques et radiologiques en cas de suspicion d'infection invasive.

The primary assessment criterion of this trial is immune response at M6 against 12 pneumococcal serotypes, according to four ordered categories of response: positive response to 0-3, 4-6, 7-9, or 10-12 serotypes common to the PCV13 and PPV23 vaccines. This endpoint will be analyzed as the number and proportion of participants in each of the four response categories According to international criteria , a participant will be considered as responder to given serotype if s/he has, at M6, i.e.one month after the PPV23 injection, a titer of specific IgG ?1 µg/mL using ELISA, and a two-fold increase in antibody titer at M6 compared to the titer at Day 0.
