E.3 | Principal inclusion criteria |
Screening part I: Participants eligible for inclusion must fulfill all of the following criteria prior to scheduling the genetic disclosure.
1. Written informed consent must be obtained before any assessment is performed as part of the study, including consent to receive disclosure of their risk estimates to develop clinical symptoms of AD based on
their APOE genotype and, if HTs, with evidence of elevated brain amyloid.
2. Male or female, age 60-75 years inclusive, at the time of signing the informed consent. To ensure that no more than 20% of participants in the age group 60-64 years are randomized across the whole recruitment period, a site level process will be implemented.
3. Females must be considered post-menopausal and not of child bearing potential i.e. they have had 12 months of natural (spontaneous) amenorrhea with an appropriate clinical profile (e.g. history of vasomotor symptoms), or have had surgical bilateral oophorectomy (with or without hysterectomy), total hysterectomy, or tubal ligation.
4. Intellectually, visually and auditorily capable, fluent in, and able to read, the language in which study assessments are administered (e.g. completion of at least 6 years of regular schooling or sustained employment or equivalent local level of knowledge).
5. Mini-Mental State Examination total score ≥24.
6. Willing to have a study partner throughout the study.
N.B. Buccal swab and genetic disclosure do not need to be repeated if already performed within the API015A2201J study.
Screening part II: Participants eligible for inclusion must fulfill all of the following criteria prior to randomization based on the results from the screening test procedures.
7. Carrier of at least one ε4 allele of the APOE gene: HMs with elevated or not elevated brain amyloid OR HTs with elevated brain amyloid (as measured by CSF Aβ or amyloid PET imaging).
Evidence of elevated brain amyloid can be waived if images are available from a previous amyloid PET scan and can be sent for visual (qualitative) assessment by the central reader.
Note: In cases where both CSF Aβ and amyloid PET imaging tests are performed, at least one should be indicative of elevated brain amyloid.
8. Cognitively unimpaired at screening visit as defined by: Score of 85 or greater on the RBANS delayed memory index score AND CDR global score of 0 with two special exceptions: a) If the RBANS delayed memory index score is between 70 and 84 (inclusive) AND the global CDR = 0, the participant may be allowed to continue ONLY if the investigator judges that cognition is unimpaired following review of the MCI/dementia criteria; b) If the global CDR score = 0.5 AND the RBANS delayed memory index score is 85 or greater, the participant may be allowed to continue ONLY if the investigator judges that cognition is unimpaired following review of the MCI/dementia criteria.
9. Having a study partner who agrees to participate in the study and who is intellectually, visually, and auditorily capable, and fluent in, and able to read, the language in which study assessments are administered.
Additionally, the study partner must be capable of and willing to: a) Accompany the participant to visits that requires the input of the study partner; b) Meet the definition of a “study partner” as described in protocol Appendix 2. |
Sélection partie I: les critères suivants doivent être satisfaits avant l’annonce diagnostique
1. Un consentement éclairé écrit doit être obtenu avant la réalisation de toute évaluation, y compris le consentement à recevoir l’annonce de leur génotype APOE et, si HTS, de leur statut amyloïde élevé.
2. Les participants doivent être des hommes ou femmes de 60 à 75 ans (inclus), à la signature du consentement. Pour s’assurer que le groupe d'âge 60-64 ans ne dépasse pas 20%, une procédure sera mise en place au niveau des centres.
3. Les femmes doivent être considérées comme ménopausées et non en capacité d'avoir des enfants, c.-à-d. qu'elles doivent justifier de 12 mois d'aménorrhée naturelle (spontanée) avec un profil clinique adapté (notamment, âge adéquat, antécédent de symptômes vasomoteurs) ou avoir subi une ovariectomie bilatérale (avec ou sans hystérectomie), une hystérectomie totale ou une ligature des trompes,
4. Le participant doit présenter un fonctionnement adéquat (notamment intellectuel, visuel et auditif) et posséder une maîtrise orale courante de la langue utilisée lors des évaluations de l'étude et pouvoir lire cette même langue (p.ex. avoir suivi une scolarité d’au moins 6 ans, ou un emploi régulier ou l’équivalent local de ce niveau de connaissance).
5. Mini-Mental State Examination (MMSE) score ≥ 24.

Absence de trouble cognitif défini par: un score de 85 ou plus au questionnaire RBANS pour l'indice de mémoire différée ET un Score total CDR = 0 Avec deux exceptions: o Si le score de l'indice de mémoire différée RBANS est compris entre 70 et 84 (inclus) ET que le score CDR global = 0, le participant pourra être admis à poursuivre UNIQUEMENT si l'investigateur juge qu'il n'y a pas de trouble cognitif en fonction des critères de MCI/démence. o Si le score CDR global = 0,5 ET que le score de l'indice de mémoire différée RBANS est égal ou supérieur à 85, le participant pourra être admis à poursuivre UNIQUEMENT si l'investigateur juge qu'il n'y a pas de trouble cognitif en fonction des critères de MCI/démence.
9. Le partenaire d'étude qui accepte de participer à l'étude doit présenter un fonctionnement suffisant (notamment intellectuel, visuel et auditif), ainsi que la maîtrise courante et la capacité à lire la langue dans laquelle les évaluations de l'étude seront réalisées.
En outre, le partenaire d'étude doit être capable de et disposé à : accompagner le participant à toutes les visites qui demandent sa participation aux évaluations ET correspondre à la définition d’un partenaire d’étude : passer suffisamment de temps avec le participant pour connaître son fonctionnement global et ses comportements, et être en mesure de fournir des informations suffisantes au sujet du participant, notamment : (a) connaissance des activités à la maison, des centres d’intérêts, des habitudes de vie, des aptitudes sociales et des activités de base de la vie quotidienne ; (b) vie professionnelle et parcours scolaire ; (c) performances cognitives, y compris les capacités de mémoire, les capacités linguistiques, l'orientation temporelle et spatiale, le jugement et la résolution de problèmes ; (d) état émotionnel et psychologique ; et (e) état de santé général |

E.4 | Principal exclusion criteria |
Screening part I: Participants will be excluded if they fulfill any of the following criteria prior to scheduling the genetic disclosure.
1. Current medical or neurological condition that might impact cognition or performance on cognitive assessments e.g. MCI, dementia, Huntington’s or Parkinson’s disease etc.
2. Advanced, severe progressive or unstable disease that may interfere with the safety, tolerability and study assessments, or put the participant at special risk e.g. active hepatitis, HIV infection, severe renal impairment, severe hepatic impairment etc.
3. History of malignancy of any organ system, treated or untreated, within the past 60 months, regardless of whether there is evidence of local recurrence or metastases. However, localized nonmalignant tumors not requiring systemic chemo- or radio-therapy, localized basal or squamous cell carcinoma of the skin, or in-situ cervical cancer are permitted.
4. Current treatment with Cholinesterase Inhibitors and/or another AD treatment.
5. Clinically relevant depigmenting or hypopigmenting conditions or active/history of chronic urticaria in the past year.
6. Score “yes” on item 4 or 5 of the Suicidal Ideation section of the C-SSRS, if this ideation occurred in the past 6 months, or “yes” on any item of the Suicidal Behavior section, except for the “Non-Suicidal Self-Injurious Behavior”, if this behavior occurred in the past 2 years prior to screening.
7. Lacking psychological readiness to receive APOE genotype/amyloid status results, as assessed based on investigator’s judgement supported by the pre-disclosure rating scales: Geriatric Depression Scale total score >6; Six Item Subset Inventory of the modified State Trait Anxiety Inventory total score >17.
8. Use of other investigational drugs prior to screening until: Small molecules: after 5 half-lives, or within 30 days until the expected pharmacodynamic effect has returned to baseline, whichever is longer; Biologicals: blood concentration has returned to baseline (or below serological responder threshold) for antibodies induced by active immunotherapy; or 5 half-lives for monoclonal antibodies or other biologicals.
9. Treatment in the 4 weeks prior to randomization with any drug or treatment known for the potential to cause major organ system toxicity i.e. drugs that may require periodic safety monitoring of a specific organ or body fluid. Examples include but are not limited to clozapine, cancer medical treatment like tamoxifen, immunosuppressive drugs like methotrexate or interferon, or other biological medicines for rheumatic diseases or multiple sclerosis or associated with Torsades de pointes.
10. Current chronic treatment (>3 months) with: Strong CYP3A4 inducers or inhibitors; Drugs with a narrow therapeutic index known to be primarily metabolized by CYP2C or CYP3A isoenzymes and sensitive P-gp substrates.
11. Violations of concomitant medication restrictions.
12. Donation or loss of 400 mL or more of blood within 8 weeks prior to randomization.
13. Contraindication or intolerance to MRI investigations.
Screening part II: Participants fulfilling any of the following criteria based on results from the screening test procedures will be excluded.
14. A positive drug screen, if, in the investigator’s opinion, this is due to drug abuse. Participants with a positive drug screen not believed to be related to drug abuse can be re-screened.
15. Previous participation in a CNP520 study with >3-month exposure to active treatment.
16. Significantly abnormal laboratory results at screening, meeting the exclusionary alert values specified in the Laboratory Manual. If, in the opinion of the investigator, an abnormal finding is the result of a temporary condition, the laboratory test can be repeated.
17. Clinically significant ECG findings from central reader (e.g. sustained ventricular tachycardia, significant second or third degree atrioventricular block without a pacemaker, prolonged QT syndrome).
18. Brain MRI results from the central reading showing findings unrelated to AD that, in the opinion of the investigator might be a leading cause of future cognitive decline, might pose a risk to the participant, or might confound MRI assessment for safety monitoring (e.g. extensive white matter lesions, stroke, cerebrovascular disease as evidenced by multiple lacunar infarcts ≤20 mm or single infarct >20 mm, evidence of cerebral contusion etc.).
19. If PET scans are scheduled for this participant: Total dosimetry above the acceptable exposure in the country when combining the previous or planned Nuclear Medicine Radiology exposure and the scheduled study PET scan(s).
20. If CSF sampling is scheduled for this participant: Contraindication to lumbar puncture e.g. low platelet count, abnormal prothrombin time international normalized ratio, history of back surgery (except microdiscectomy or laminectomy over one level), signs or symptoms of intracranial pressure etc. |

2. Maladie avancée, évolutive grave ou instable pouvant perturber la sécurité, la tolérance et les évaluations de l'étude, ou exposer le participant à un risque spécifique (ex:hépatite active, infection au VIH, insuffisance rénale sévère, insuffisance hépatique sévère…)
3. Antécédents de cancer (tout système d'organe) traité ou non au cours des 60 derniers mois avec ou sans preuves de récidive locale ou métastases. Les tumeurs non malignes localisées ne nécessitant pas de chimiothérapie ni radiothérapie systémique, le carcinome basocellulaire ou spinocellulaire de la peau localisé ou le cancer du col utérin in situ sont acceptés
4. Traitement en cours par inhibiteurs de la cholinestérase et/ou autre traitement de la MA
5. Dépigmentation significative ou conditions d’hypopigmentation ou urticaire actuel ou antécédent d’urticaire chronique dans l’année précédente
6. Réponse «oui» aux questions 4 ou 5 de la section Idéation suicidaire du C-SSRS (si apparue au cours des 6 derniers mois) ou réponse «oui» à une des questions de la section Comportement suicidaire sauf le «comportement d'automutilation non suicidaire» (si survenu au cours des 2 dernières années)
7. Les participants doivent être jugés capables de recevoir l’information sur leur génotype APOE avec le support des échelles d’évaluation (ex :Score total à l'échelle de dépression gériatrique >6, Score total à l'inventaire des sous-ensembles à 6 questions du STAI-AD >17)
8. Utilisation d'autres médicaments expérimentaux avant la sélection: Petites molécules: dans les 30 jours ou les 5 demi-vies (selon le plus long) jusqu’à ce que l’effet pharmacodynamique soit revenu à la valeur de base. Biologiques: tant que les concentrations sanguines ne sont pas revenues aux valeurs initiales (ou sous les seuils sérologiques de réponse) pour les anticorps induits par immunothérapie active ou 5 demies vies pour les anticorps monoclonaux ou autres dérivés biologiques
9. Traitement dans les 4 semaines avant la randomisation par tout médicament ou thérapie pouvant entrainer une toxicité majeure dans un système d'organe c-à-d médicaments nécessitant la surveillance régulière d'un organe ou d'un fluide corporel spécifique(ex : clozapine et traitements anticancéreux comme tamoxifène, immunosuppresseurs, méthotrexate, interférons ou autres agents biologiques indiqués dans les maladies rhumatismales, la sclérose en plaques ou associés à des torsades de pointes)
10. Traitement chronique en cours (>3 mois) par inducteurs puissants du CYP3A4, inhibiteurs puissants du CYP3A4, médicaments métabolisés par les isoenzymes CYP2C et 3A et les substrats de la PGP
11. Violations des restrictions concernant les médicaments associés
12. Don ou perte de sang de 400ml ou plus dans les 8 semaines avant la randomisation
13. Contre-indication ou intolérance aux IRM
Sélection partie II: les participants répondant à un des critères suivants seront exclus
14. Dépistage positif de drogue s’il correspond à un abus de drogue. Les participants avec un résultat positif dont on pense qu'il n'est pas dû à un abus de drogues pourront faire un nouveau dépistage
15. Participation antérieure à une étude avec le CNP520 avec plus de 3 mois d’exposition
16. Anomalies significatives des analyses biologiques à la sélection (valeurs d'alerte d'exclusion selon le manuel du laboratoire). Si de l'avis de l'investigateur ceci est le résultat d'une affection temporaire, l'analyse pourra être répétée
17. Anomalies significatives à l’ECG selon le lecteur central (ex : tachycardie ventriculaire soutenue, bloc atrioventriculaire significatif de 2e ou 3e degré sans stimulateur cardiaque, syndrome du QT long)
18. Résultats d'IRM cérébrale (lecture centralisée) montrant des éléments non liés à la MA qui de l'avis de l'investigateur pourraient être la cause d'un déclin cognitif, exposer le participant à un risque, ou interférer dans l'évaluation IRM réalisée pour le suivi de la sécurité (ex: lésions étendues de la substance blanche, AVC ischémique ou hémorragique récent révélé par de multiples infarctus lacunaires ≤ 20mm ou infarctus antérieur unique > 20mm, signes d'une contusion cérébrale…)
19. Si des TEP sont programmées : dosimétrie supérieure à l'exposition admise dans le pays avec les TEP prévues dans l'étude et les explorations radiologiques de médecine nucléaire antérieures ou planifiées
20. Si des échantillons de LCR sont prévus : contre-indication à la ponction lombaire (ex: faible taux de plaquettes, anomalie du temps de Quick, antécédents de chirurgie lombaire à l'exception de microdiscectomie ou laminectomie sur un niveau, signes ou symptômes de pression intracrânienne…)
