In this sub-study we aim to:
- assess all enrolled patients concerning the presence of metabolic Syndrome

- assess treatment-related changes in metabolic parameters at baseline versus end of metformin Treatment in relation to disease characteristics and clinical endpoints.
2. Translational Research-"Transcriptomic Analyses of Metformin Drug Transporters in Prostate Cancer Patients treated with Salvage Radiotherapy plus Metformin"
In this sub-study we aim to:
- determine Metformin transporter expression in prostatic tissue after prostatectomy and in sites of disease recurrence after treatment
- determine Metformin transporter SNP´s (Single nucleotide polymorphism)
- determine association between metformin transporter expression and/or SNPs in prostate tumors and clinical outcomes

In this sub-study we aim to:
- validate the most promising biomarkers/approaches
- predict the toxicity of patients to potentially enable individual dose prescription, which should improve survival, decrease morbidity, and improve quality of life. |

1. Written informed consent according to ICH/GCP regulations before registration and prior to any trial specific procedures
2. Histologically confirmed adenocarcinoma of the prostate without small cell features
3. Tumor stage pT2a-3b, pN0 or cN0, M0, R0-1 resection margins, according to UICC TNM 2009, Gleason score available
4. Radical prostatectomy (RP) at least 12 weeks before registration
5. PSA progression after RP defined as two consecutive rises with the final PSA > 0.1 ng/mL or three consecutive rises. The first value must be measured earliest 4 weeks after RP
6. PSA ≤ 2 ng/mL within 14 days prior to registration
7. Age ≥ 18 years at time of registration
8. WHO performance status 0-1
9. Adequate hepatic function within 14 days prior to registration: bilirubin ≤ 1.5 x ULN (exception if Gilbert’s syndrome ≤ 3 x ULN), AST and ALT ≤ 2.5 x ULN
10. Adequate renal function within 14 days prior to registration: calculated corrected creatinine clearance ≥ 60 mL/min, according to the formula of corrected Cockcroft-Gault
11. Patient agrees not to father a child and to use effective contraceptive methods during salvage radiotherapy and until 6 months after the last fraction of radiotherapy
12. Paraffin block of the surgical specimen containing representative tumor tissue and non-neoplastic prostatic tissue available for biobanking |

10. Ausreichende Nierenfunktion über einen Zeitraum von 14 Tagen vor Registrierung: kalkulierte korrigierte Kreatinin-Clearance ≥ 60 mL/min entsprechend der Cockcroft-Gault-Formel
11. Patient stimmt zu, während der Salvage-Radiotherapie sowie 6 Monate nach der zuletzt erfolgten Radiotherapiesitzung sichere Verhütungsmethoden anzuwenden und keine Kinder zu zeugen

E.4 | Principal exclusion criteria |
1. Persistent PSA (> 0.4 ng/mL) 4 to 20 weeks after RP
2. Pelvic lymph node enlargement > 0.8 cm in short axis diameter (cN positive) assessed by MRI or PET-CT within 12 weeks prior to registration, unless the enlarged lymph node is sampled and negative
3. If PET-CT scan was performed, any metabolic uptake outside the prostatic bed considered clinically suspicious for metastasis, unless the lesion is sampled and negative
4. Palpable prostatic fossa mass suggestive of recurrence, unless an ultrasound guided biopsy is negative for malignancy
5. Presence or history of prostate cancer metastases. In case of clinical suspicion (e.g. bone scan, PET-CT scan, whole-body MRI) must be performed. The imaging method is at the discretion of the investigator
6. Previous hematologic or primary solid malignancy within 3 years prior registration with the exception of curatively treated localized non-melanoma skin cancer
7. Patients diagnosed with diabetes mellitus
8. Treatment with metformin within the last 3 months prior to registration
9. Prior pelvic radiotherapy
10. Hormonal treatment in the form of bilateral orchiectomy prior or following RP
11. Usage of products known to affect PSA levels within 4 weeks prior to start of trial Treatment phase, including any form of androgen suppression agents and androgen deprivation therapy
12. Bilateral hip prosthesis
13. Severe or active co-morbidity likely to impact on the advisability of salvage RT, e.g.:
a. History of inflammatory bowel disease or any malabsorption syndrome or conditions that would interfere with enteral absorption
b. Acute bacterial or fungal infection requiring intravenous antibiotics at the time of registration
c. Unstable angina, myocardial infarction and/or congestive heart failure requiring hospitalization within the last 6 months
d. Transmural myocardial infarction within the last 6 months
e. Chronic obstructive pulmonary disease (COPD) exacerbation or other respiratory illness requiring hospitalization or precluding study therapy at the time of registration
14. Any condition associated with increased risk of lactic acidosis (e.g. alcohol abuse, congestive heart failure NYHA III or IV)
15. Clinically significant history of liver disease consistent with Child-Pugh Class B or C, including viral or hepatitis, current alcohol abuse, or cirrhosis for use with metformin according to the approved product information
16. Any acute or chronic condition that could cause tissue hypoxia (e.g. cardiac or respiratory insufficiency, shock)
17. Treatment with any experimental drug or participation within a clinical trial within 30 days prior to registration (exception: concurrent participation in the biobank study SAKK 63/12 is allowed)
18. Any concomitant drug contraindicated for use with metformin according to the approved product information
19. Known hypersensitivity to metformin or to any of its components
20. Inability or unwillingness to swallow oral medication
21. Any other serious underlying medical, psychiatric, psychological, familial or geographical condition, which in the judgment of the investigator may interfere with the planned staging, treatment and follow-up, affect patient compliance or place the patient at high risk from treatment-related complications.

3. Im Falle eines erfolgten PET-CT-Scans: Jede festgestellte metabolische Aufnahme außerhalb des Prostatabettes, bei der nach Ermessen des zuständigen Prüfarztes ein klinischer Verdacht auf eine Malignität besteht, außer eine Biopsie der Läsion erweist sich als negativ
4. Tastbare Masse in der Prostataloge, die auf ein Rezidiv hindeutet, es sei denn der Befund einer Ultraschall-geführten Biopsie ist negativ und deutet nicht auf ein malignes Wachstum hin
5. Vorhandensein oder früheres Auftreten von Prostatakrebs-Metastasen. Im Fall eines klinischen Verdachts (z.B. bei Knochenschmerzen) muss ein bildgebendes Verfahren (z.B. Skelettszintigraphie, PET-CT-Scan, Ganzkörper-MRT) durchgeführt werden. Die Auswahl des Verfahrens obliegt dem Prüfarzt.
6. Frühere hämatologische oder primäre solide maligne Tumore innerhalb der letzten 3 Jahre vor Registrierung mit Ausnahme von erfolgreich behandeltem örtlich begrenztem Nicht-Melanom-Hautkrebs
7. Patienten mit der Diagnose Diabetes Mellitus
8. Behandlung mit Metformin innerhalb der letzten 3 Monate vor Registrierung
9. Frühere Strahlentherapie des Beckens
10. Hormontherapie in Form von bilateraler Orchiektomie vor oder nach RP
11. Verwendung von Produkten, die eine Senkung des PSA-Spiegels verursachen, darunter jede Form von Androgen-supprimierender Substanzen oder einer Androgendeprivationstherapie, innerhalb von 4 Wochen vor Beginn der Studienbehandlung
12. Bilaterale Hüftprothese
13. Schwere oder aktive Komorbititäten, die voraussichtlich Auswirkungen auf die Zweckmäßigkeit der Salvage-Radiotherapie (SRT) haben, wie:
a. Krankheitsgeschichte einer entzündlichen Darmerkrankung oder eines Malabsorptions-Syndroms oder Bedingungen, die die enterale Aufnahme beeinträchtigen
b. Akute Bakterien- oder Pilzinfektion, die eine intravenöse Antibiotikatherapie zum Zeitpunkt der Registrierung erfordert
c.Instabile Angina, Myokardinfarkt und/oder Herzinsuffizienz, die eine Hospitalisierung innerhalb der letzten 6 Monate erforderlich machen
d. Transmuraler Myokardinfarkt innerhalb der letzten 6 Monate
e. Verschlimmerung einer Chronisch obstruktiven Lungenerkrankung (COPD) oder anderer Atemwegserkrankungen, die eine Hospitalisierung erforderlich machen oder eine Therapie im Rahmen der Studie zum Zeitpunkt der Registrierung ausschließen
14. Jede Erkrankung, die mit einem erhöhten Risiko für Laktatazidose verbunden ist (z.B. Alkoholmissbrauch, Herzinsuffizienz der Klasse III oder IV nach NYHA-Klassifizierung (siehe Appendix 5 des Prüfplans))
15. Klinisch relevante Krankheitsgeschichte einer Lebererkrankung im Stadium B oder C nach den Child-Pugh-Kriterien, inklusive viraler Erkrankungen oder Hepatitis, derzeitiger Alkoholmissbrauch oder Leberzirrose bei Behandlung mit Metformin gemäß der zugelassenen Produktinformation/Fachinformation
16. Jede akute oder chronische Erkrankung, die eine Gewebehypoxie verursachen könnte (z.B. Herz-oder Ateminsuffizienz, Schock)
17. Behandlung mit einem experimentellen Arzneimittel oder Teilnahme an einer anderen klinischen Studie innerhalb der letzten 30 Tage vor Registrierung
18. Jede Einnahme von Begleitmedikation die bei Behandlung mit Metformin gemäß der zugelassenen Produktinformation/Fachinformation kontraindiziert ist
19. Bekannte Überempfindlichkeit gegenüber Metformin oder einem seiner Bestandteile
20. Patienten, denen es nicht möglich ist oder die nicht gewillt sind, Studienmedikation oral einzunehmen
