E.3 | Principal inclusion criteria |
I1.Male or female patients aged ≥ 18 years at time of inform consent signature. I2.Histologically confirmed soft tissue sarcoma including liposarcoma, leiomyosarcoma, myxofibrosarcoma, UPS, angiosarcoma, all translocation sarcoma except Ewing, rhabdomyosarcoma (RMS), and myxoid liposarcoma (LPS). I3.Soft tissue sarcoma suitable for neoadjuvant RT and amenable to surgery with curative intent (high-grade non-metastatic tumors, intermediate and low-grade tumors greater than 5 cm). Note: Patients with local relapsing disease amenable to surgery are eligible. I4.Presence of at least one tumor lesion with a diameter ≥10 mm, visible by medical imaging and accessible to percutaneous or endoscopic sampling that permit core needle biopsy without unacceptable risk and suitable for retrieval of a minimum of three, but ideally 4, cores using a biopsy needle of at least 16-gauge. I5.Eastern Cooperative Oncology Group performance status (ECOG PS) of 0 or 1 (Appendix 1). I6.Adequate end organs and bone marrow functions as defined below according to lab tests performed within 7 days before W1D1: Bone marrow (without transfusion within 2 weeks before W1D1): o Hemoglobin ≥ 9.0 g/dL, oAbsolute neutrophil count ≥ 1.5 x 109/L, oPlatelets ≥ 100 x 109/L, oLymphocyte count ≥ 0.5 x 109/L. Renal function: o Serum creatinine clearance ≥ 30 mL/min/1.73m2 (MDRD or CKD-EPI formula - Appendix 3) Hepatic function o Serum bilirubin < 1.5 × Upper Limit of Normal (ULN), with the following exception: Patients with known Gilbert disease who have serum bilirubin level ≤ 3 ULN may be enrolled. o Aspartate aminotransferase (AST), alanine aminotransferase (ALT) and alkaline phosphatase ≤ 2.5 ULN. Coagulation o INR and aPTT ≤ 1.5 ULN Note: This is applicable only for patients not receiving an anticoagulant treatment: patients receiving therapeutic anticoagulation must be on stable dose. I7.Minimal wash-out period for prior treatments (minimal time required from the end date of prior treatment to W1D1): Immunosuppressive medication > 28 days, with the exceptions of intranasal and inhaled corticosteroids or systemic corticosteroids at physiological doses, which are not to exceed 10 mg/day of prednisone, or an equivalent corticosteroid, Live attenuated vaccines > 30 days, Note: Seasonal influenza vaccines for injection are generally inactivated flu vaccines and are allowed; however intranasal influenza vaccines (e.g., Flu-Mist®) are live attenuated vaccines, and are not allowed. Major surgical procedure, open biopsy (excluding skin cancer resection and screening tumor biopsy), or significant traumatic injury > 14 days (the wound must have healed), Any approved or investigational anti-cancer therapy, including chemotherapy, hormonal therapy or targeted therapy > 21 days, Systemic immunostimulatory agents > 28 days or five half-lives of the drug, whichever is longer, Oral or IV antibiotics > 14 days. I8.Women of child-bearing potential must have a negative serum pregnancy test within 7 days before randomisation and must agree to use an effective form of contraception from the time of the negative pregnancy test up to 5 months after the last dose of atezolizumab (See Appendix 5). I9.Fertile men must agree to use an effective method of birth control during the study and for up to 5 months after the last dose of atezolizumab. I10.Patient should understand, sign, and date the written voluntary informed consent form prior to any protocol-specific procedures performed. Patient should be able and willing to comply with study visits and procedures as per protocol. I11.Patients must be covered by a medical insurance in country where applicable.

I1. Homme ou femme âgé(e) d’au moins 18 ans à la date de signature du consentement. I2. Diagnostic confirmé par un examen histologique de sarcome des tissus mous dont liposarcome, leiomyosarcome, myxofibrosarcome, sarcome indifférencié pléomorphe, angiosarcome, sarcomes à translocation à l’exception des sarcomes d’Ewing, rhabdomyosarcome et liposarcome myxoïde. I3. Sarcomes des tissus mous pouvant bénéficier d’un traitement par radiothérapie et être traités par chirurgie à visée curative (incluant les tumeurs de haut grade non métastatiques, les tumeurs de grade intermédiaire et les tumeurs de bas grade de plus de 5 cm). Note : Les patients avec une tumeur en rechute locale pouvant être traitée par chirurgie sont éligibles. I4. Présence d'au moins une lésion tumorale avec un diamètre ≥ 10 mm, visible par imagerie médicale et accessible pour une biopsie par voie percutanée ou endoscopique sans risque inacceptable et permettant le prélèvement d'au minimum trois, mais idéalement 4, carottes à l'aide d'une aiguille de biopsie d'au moins 16-gauge. I5. Statut de performance (PS) selon l’échelle ECOG (Eastern Cooperative Oncology Group) de 0 ou 1. I6. Fonction normale des principaux organes et de la moelle osseuse selon les critères listés ci-après devant être confirmés par analyse biologique dans les 7 jours précédant le S1J1 : Moelle osseuse (sans transfusion dans les 2 semaines précédant le S1J1) : o Hémoglobine ≥ 9.0 g/dL, o Nombre absolu de neutrophiles ≥ 1.5 x109/L, o Nombre de plaquettes ≥ 100 x109/L, o Nombre de lymphocytes ≥ 0.5 x109/L. Fonction rénale : o Clairance de la créatinine ≥ 30 mL/min/1.73 m2.calculée selon les formules MDRD ou CKD-EPI. Fonction hépatique : o Bilirubine totale sérique <1.5 x LNS (Limite Normale Supérieure) à l’exception des patients atteints d’un syndrome de Gilbert pour lesquels un taux sérique de bilirubine totale ≤ 3 x LNS est acceptable, o Alanine aminotransférase (ALT), aspartate aminotransférase (AST) et phosphatase alcaline ≤ 2.5 x LNS. Coagulation : o Rapport normalisé international (International Normalized Ratio, INR) et temps de céphaline activé (TCA) ≤ 1,5 × LNS. Note : Cela s’applique uniquement aux patients qui ne reçoivent pas de traitement anticoagulant; les patients qui reçoivent un traitement anticoagulant doivent être traités par une dose stable. I7. I9. Les hommes fertiles doivent accepter d’utiliser une méthode efficace de contraception pendant toute la durée de l’étude et jusqu’à 5 mois après la dernière prise de traitement. I10. Patient capable de comprendre, de signer et de dater le consentement éclairé avant le début de toute procédure du protocole de l’étude. Patient acceptant de se conformer aux visites et aux procédures imposées par le protocole. I11. Patient affilié ou bénéficiaire d’un régime de sécurité sociale pour les patients des pays dans lesquels cela est obligatoire.

E.4 | Principal exclusion criteria |
E1.Patients with evidence of metastatic disease, defined by the presence of any of the followings: Lesions that are discontinuous from the primary tumor, Lesions that are not regional lymph nodes, Lesions that do not share a body cavity with the primary tumor, Evidence by medical imagining (eg CT-scan) of metastatic disease. E2.Patients with history of severe allergic or other hypersensitivity reactions to: Chimeric or humanized antibodies or fusion proteins, Biopharmaceuticals produced in Chinese hamster ovary cells, or Any component of the atezolizumab formulation. E3.Patients using or requirement to use while on the study of any not permitted concomitant medications : Any approved anti-cancer systemic treatment including chemotherapy, hormonotherapy, biological therapy, immunotherapy other than atezolizumab, Any investigational agents, Live vaccines. Examples of live vaccines include, but are not limited to, the following: measles, mumps, rubella, chicken pox, yellow fever, rabies, BCG, and typhoid (oral) vaccine. Seasonal influenza vaccines for injection are generally killed virus vaccines and are allowed; however intranasal influenza vaccines (e.g. Flu-Mist®) are live attenuated vaccines, and are not allowed during the study active period, Traditional herbal medicines since the ingredients of many herbal medicines are not fully studied and their use may result in unanticipated drug-drug interactions that may cause or confound assessment of toxicity, Immunostimulatory agents, including but not limited to IFN-α, IFN-γ, or IL-2, during the entire study. These agents, in combination with atezolizumab, could potentially increase the risk for autoimmune conditions. In addition, all patients (including those who discontinue the study early) should not receive other immunostimulatory agents for 10 weeks after the last dose of atezolizumab, Immunosuppressive medications, including but not limited to cyclophosphamide, azathioprine, methotrexate, and thalidomide. These agents could potentially alter the activity and the safety of atezolizumab. Systemic corticosteroids and anti−TNF-α agents may attenuate potential beneficial immunologic effects of treatment with atezolizumab but may be administered to manage irAE (See Safety Plan). If feasible, alternatives to these agents should be considered.
E4.Patients with a malignancy other than STS within 5 years prior to randomisation with the exception of those with a negligible risk of metastasis or death and treated with expected curative outcome (such as adequately treated in situ carcinoma of the cervix, basal or squamous cell skin cancer, localised prostate cancer or ductal in situ carcinoma treated surgically with curative intent). E5.Patients with severe infections within 4 weeks prior to randomisation including but not limited to hospitalization for complications of infection, bacteraemia, or severe pneumonia. E6.Patients with history of autoimmune disease including but not limited to myasthenia gravis, myositis, autoimmune hepatitis, systemic lupus erythematosus, rheumatoid arthritis, inflammatory bowel disease, vascular thrombosis associated with antiphospholipid syndrome, Wegener's granulomatosis, Sjögren's syndrome, Guillain-Barré syndrome, multiple sclerosis, vasculitis, or glomerulonephritis. E7.Patients with active B or C hepatitis infection. E8.Patients with active tuberculosis. E9.Patients with ongoing toxicities (Grade ≥1 according to CTCAE V4.03) from previous therapies, except for alopecia (any grades) and lab values defined in inclusion criteria. E10.Patients with evidence of significant uncontrolled concomitant disease that could affect compliance with the protocol or interpretation of results, including significant liver disease (such as cirrhosis, uncontrolled major seizure disorder, or superior vena cava syndrome). E11.Patients with significant cardiovascular disease, such as NYHA cardiac disease Class II or greater, myocardial infarction within 3 months prior to W1D1, unstable arrhythmias or unstable angina. E12.Patients with history of idiopathic pulmonary fibrosis (including pneumonitis), drug-induced pneumonitis, organizing pneumonia (i.e. bronchiolitis obliterans, cryptogenic organizing pneumonia), or evidence of active pneumonitis on imaging. E13.Pregnant or lactating women. |

Patients présentant des antécédents de réactions allergiques sévères ou toute réaction d’hypersensibilité : - des anticorps chimériques ou humanisés ou à des protéines de fusion, - des composés biopharmaceutiques produits dans des cellules ovariennes de hamster chinois, - l’un des composants de la formulation de l’atezolizumab. E3. Patients traités par ou nécessitant pendant l‘étude l’un des traitements non autorisés par le protocole incluant : Tout traitement anti-cancéreux standard incluant toute chimiothérapie, thérapie ciblée, immunothérapie, traitement biologique ou hormonal autre que l’atezolizumab, Tout traitement expérimental, Vaccin vivant incluant par exemple les vaccins contre la rougeole, les oreillons, la rubéole, la varicelle, la fièvre jaune, la rage, le BCG et le vaccin contre la typhoïde (par voie orale). Les vaccins antigrippaux saisonniers injectés sont généralement des vaccins antigrippaux inactivés et sont autorisés. Toutefois, les vaccins antigrippaux par voie intranasale sont des vaccins vivants atténués et ne sont pas autorisés, Tout médicament traditionnel à base de plante puisque les ingrédients de ces médicaments ne sont pas entièrement étudiés Médicaments immunostimulants incluant par exemple IFN-α, IFN-γ, ou IL-2. Ces médicaments, combinés à l’atezolizumab, pourraient potentiellement augmenter le risque de maladies auto-immunes. En outre, tous les patients traités par atezolizumab ne doivent pas recevoir d'autres agents immunostimulantes au cours des 10 semaines suivant la dernière dose d’atezolizumab, Médicaments immunosuppresseurs systémiques incluant par exemple le cyclophosphamide, l’azathioprine, le méthotrexate, et le thalidomide. Ces agents pourraient potentiellement altérer l'activité et la sécurité de l’atezolizumab. E6. Patients présentant une infection sévère dans les 4 semaines avant la randomisation incluant mais sans s’y limiter, les hospitalisations pour complications infectieuses, bactériémie ou pneumonie sévère. E7. Patients présentant des antécédents connus de maladie auto-immune incluant mais n’étant pas limité aux maladies suivantes : myasthénie auto-immune, myosite, hépatite auto-immune, lupus érythémateux disséminé, polyarthrite rhumatoïde, maladie inflammatoire de l’intestin, thrombose vasculaire associée à un syndrome des anti-phospholipides, granulomatose de Wegener, syndrome de Sjögren’s, syndrome de Guillain-Barré, sclérose en plaques, vascularite, ou glomérulonéphrite. E8. Patients avec une hépatite B ou C active. E9. Patients présentant une tuberculose active. E10. Patients présentant une toxicité persistante de Grade ≥ 1 selon la classification NCI CTCAE v4.03 relative à des traitements antérieurs à l’exception de l’alopécie (tout grade) et des valeurs biologiques définies dans les critères d’inclusion.
