The European Union Clinical Trials Register   allows you to search for protocol and results information on:
  • interventional clinical trials that were approved in the European Union (EU)/European Economic Area (EEA) under the Clinical Trials Directive 2001/20/EC
  • clinical trials conducted outside the EU/EEA that are linked to European paediatric-medicine development

  • EU/EEA interventional clinical trials approved under or transitioned to the Clinical Trial Regulation 536/2014 are publicly accessible through the
    Clinical Trials Information System (CTIS).


    The EU Clinical Trials Register currently displays   44313   clinical trials with a EudraCT protocol, of which   7357   are clinical trials conducted with subjects less than 18 years old.   The register also displays information on   18700   older paediatric trials (in scope of Article 45 of the Paediatric Regulation (EC) No 1901/2006).

    Phase 1 trials conducted solely on adults and that are not part of an agreed paediatric investigation plan (PIP) are not publicly available (see Frequently Asked Questions ).

    E.2 Objective of the trial
    E.2.1Main objective of the trial
    The primary objective of the first randomized question (R1) open for all LBL patients (pts) of the core study cohort, is to evaluate whether the cumulative incidence of relapses in the central nervous system can be decreased by substituting prednisone (60 mg/m²/d for 21 days plus a 9 day tapering) (standard arm, SA) by dexamethasone (10 mg/m²/d for 14 days without tapering) (experimental arm, EA) in induction therapy. The primary objective of the second randomized question (R2) open for high-risk pts of the core study cohort, is to test whether the probability of pEFS can be improved by an intensified treatment arm (EA) compared to the standard treatment arm (SA). In the EA pts receive 2 additional doses of PEG asparaginase during protocol Ib* and an intensified protocol M consisting of one course for high-risk (HR) ALL (HR-1’), followed by one standard high-dose methotrexate (MTX) course, followed by another intense course for HR ALL (HR-2’) and a second standard high-dose MTX course.

    Objectives: Identify molecular markers in primary pB-cell lymphoblastic lymphoma that might be prognostically relevant Patient specific parameters, like sex, age, stage of disease and tumor load will be extracted from the database. Candidate genes will be analyzed from primary patient material or lymphoma material using sanger sequencing. Results will be correlated to the clinical data to predict the prognostic impact of the candidate genes. Only residual material after diagnostic procedures will be used.

    3- Susceptibility for adverse events and role of polymorphisms
    Objectives: Identify single-nucleotide polymorphisms (SNPs) associated with adverse events in LBL patients in a genome-wide association study. Patient specific parameters, like sex, age, stage of disease and type of glucocorticoid will be extracted from the database. SNP genotyping will be performed on germline DNA from LBL patients. Results will be correlated to the clinical data to predict the prognostic impact of the SNPs. Only residual material after diagnostic procedures will be used.

    4- CNS involvement in NHL among children and adolescents
    Objectives: To explore: 1) The frequency of CNS involvement and the kinetics of disappearance of neoplastic cells in the spinal fluid until the 12th day of therapy for T-LBL We hypothesise that 20% of all patients with NHL have CNS involvement at levels below 5 x 106/L at diagnosis and that this can be detected if highly sensitive methods are applied 2) The prognostic significance of CNS involvement at diagnosis for event-free survival and types of relapse We hypothesise that patients with CNS involvement have a worse outcome than patients without neoplastic cells in the CNS

    5- Role of Minimal Residual Disease (MRD) assessment
    Objectives: By collecting and analyzing the available data on MRD analyses performed according to local standards in individual institutions or national groups we aim to enlighten the role of MRD as a monitoring tool to evaluate the response to treatment.

    Obiettivi: Raccogliendo e analizzando i dati disponibili di FDG-PET-(CT/MRI) eseguiti secondo gli standard locali nelle singole istituzioni, puntiamo ad evidenziare il valore di FDGPET-(CT) come strumento diagnostico e di monitoraggio per definire meglio il ruolo dell'imaging funzionale nella LBL pediatrica.

    2- Analisi del profilo molecolare di pB-LBL
    Obiettivi: Identificare marcatori molecolari di pB-LBL che potrebbero essere parametri paziente specifici prognosticamente rilevanti come il sesso, l'età, lo stadio di malattia e carico tumorale saranno estratti dal database. I geni candidati saranno analizzati da materiale primario del paziente o materiale linfoma utilizzando il sequenziamento di Sanger. I risultati saranno correlati ai dati clinici per prevedere l'impatto prognostico dei geni candidati. Solo il materiale residuo dopo le procedure diagnostiche sarà utilizzato.

    3- Suscettibilità per gli eventi avversi e ruolo dei polimorfismi
    Obiettivi: Identificare polimorfismi mononucleotidici (SNP) associati ad eventi avversi in pazienti con LBL in uno studio di associazione a livello genomico. Parametri specifici del paziente, come sesso, età, stadio di malattia e tipo di glucocorticoide saranno estratti dal database. La genotipizzazione SNP sarà eseguita sul DNA germinale di pazienti con LBL. I risultati saranno correlati ai dati clinici per prevedere l'impatto prognostico degli SNP. Solo il materiale residuo dopo le procedure diagnostiche sarà utilizzato.

    4- Il coinvolgimento del SNC nei LNH tra bambini e adolescenti
    Obiettivi: Esplorare: 1) La frequenza del coinvolgimento del sistema nervoso centrale e la cinetica della scomparsa delle cellule neoplastiche nel fluido spinale fino al 12 º giorno di terapia per T-LBL Ipotizziamo che il 20% di tutti i pazienti con NHL hanno coinvolgimento del SNC a livelli inferiori a 5 x 106/L alla diagnosi e che questo può essere rilevato se i metodi altamente sensibili sono applicati 2) L'importanza prognostica del coinvolgimento del SNC alla diagnosi per event-free survival e tipi di ricaduta. Ipotizziamo che i pazienti con coinvolgimento SNC hanno un outcame peggiore rispetto ai pazienti senza cellule neoplastiche nel SNC

    5- Ruolo della valutazione della malattia residua minima (MRM)
    Obiettivi: Raccogliendo e analizzando i dati disponibili sulle analisi MRD eseguite secondo gli standard locali in singole istituzioni o gruppi nazionali, puntiamo a chiarire il ruolo della MRD come strumento di monitoraggio per valutare la risposta al trattamento.

    Patients meeting the following criteria are eligible to the study (inclusion criteria):
    •newly diagnosed lymphoblastic lymphoma
    •age <18 years at diagnosis
    •patient enrolled in a participating center
    •written informed consent of patient (>14 years of age or according to local law and regulation) and parents to trial participation and transfer and processing of data
    •Willingness of patients and the investigator/pathologist to provide adequate slides/blocks for reference (molecular)pathology and international pathology panel and/or fresh or fresh frozen samples for genetic risk group stratification if these samples are available after standard diagnostic procedures

    Patients meeting the following criteria are not eligible to the study (exclusion criteria):
    •lymphoblastic lymphoma as secondary malignancy
    •non-lymphoma related relevant medical, psychiatric or social conditions incompatible with trial treatment including among others :
    - prior organ transplant
    - severe immunodeficiency
    - demyelinating Charcot-Marie Tooth syndrome
    - serious acute or chronic infections, such as HIV, VZV and
    tuberculosis
    - urinary tract infection, cystitis, urinary outflow obstruction, severe
    renal impairment (creatinine clearance less than 20 ml/min)
    - severe hepatic impairment (bilirubin >3 times ULN, transaminases
    >10 times ULN)
    - myocardial insufficiency, severe arrhythmias
    - ulcers of the oral cavity and known active gastrointestinal ulcer
    disease
    - known hypersensitivity to any IMP and to any excipient
    •steroid pre-treatment with = 1 mg/kg/d for more than two weeks during the last month before diagnosis
    •vaccination with live vaccines within 2 weeks before start of protocol Treatment
    •treatment started according to another protocol or pre-treatment with cytostatic drugs
    •participation in another clinical trial that interferes with the protocol, except NHL-BFM Registry 2012 and trials with different endpoints, involving aspects of supportive treatment, which can run parallel to LBL 2018 without influencing the outcome of this trial (e.g. trials on antiemetics, antibiotics, strategies for psychosocial support)
    •evidence of pregnancy or lactation period
    •sexually active adolescents not willing to use highly effective contraceptive method (pearl index < 1) until 12 months after end of cytostatic therapy

    pretrattamento steroideo con = 1 mg/kg/d per più di due settimane durante l'ultimo mese prima della diagnosi;
    - vaccinazione con vaccini vivi entro 2 settimane prima dell'inizio del protocollo di trattamento;

    -partecipazione ad un altro studio clinico che interferisce con il protocollo, tranne il Registro NHL-BFM 2012 e le prove con diversi endpoint, che coinvolgono aspetti del trattamento di sostegno, che possono andare in parallelo a LBL 2018 senza influenzare l'esito di questo processo (ad es. prove su antiemetici, antibiotici, strategie di sostegno psicosociale);
    - gravidanza o periodo di allattamento;
    - adolescenti sessualmente attivi non disposti a utilizzare metodo contraccettivo (indice di perl < 1) altamente efficace fino a 12 mesi dopo la fine del terapia citostatica

    Primary endpoints of randomization 1:
    For the randomized question 1 the cumulative incidence of relapse with involvement of the CNS (CNS-relapse, pCICR) is the primary endpoint. The time to relapse is the time from randomization to the first relapse or the date of last follow-up. Other events will be taken into account as competing events.

    CNS-relapse is defined as follows:
    •=5/µl nucleated cells in CSF and morphologically unequivocal evidence of lymphoblasts
    •histologically proven LBL in cases of intracranial mass without evidence of blasts in CSF

    Primary endpoints of randomization 2
    For the randomized question 2 the estimated probability of event-free survival (pEFS) is the primary endpoint. The pEFS is the time from randomization to the first event or date of last follow-up.

    The following occurrences are defined as an event
    • non-response
    • disease progression/relapse
    • secondary malignancy
    • death from any cause
