E.3 | Principal inclusion criteria |
1. Individuals diagnosed with PN, defined as the presence of ≥ 20 pruriginous nodules, with predominantly nodular lesions overall.
2. Generalized PN, defined as PN lesions with a nodular component involving 2 distinct anatomical areas: for example, either 2 limbs; or a single limb and some axial portion of the body. Individuals with only axial lesions with 2 distinct anatomical areas of involvement, that have no peripheral nervous system overlap, are also eligible: for example, lesions involving a portion of the cranium and a portion of the trunk of the body. For purposes of this study, the axial portion will be defined as any non-appendicular portion of the body.
3. WINRS score, recorded daily over the 7 contiguous days prior to the baseline visit via electronic diary, must have at least 5 measurements recorded and all individual measurements must be ≥ 6. The arithmetic mean value of the measurements must be ≥ 7. If necessary, the last WINRS value used in the calculation may be recorded on the day of the baseline visit as long as it occurs prior to dosing.
4. Subjects using antidepressant and/or neuroleptic medications must be on a stable dose for a minimum of 8 weeks prior to signing consent and must be willing to remain on their stable dose for the entire duration of the study.
5. Subjects who are human immunodeficiency virus (HIV) positive may enroll if they meet the following criteria: (a) currently on a stable (> 6 months stable use) and well tolerated highly active antiretroviral therapy regimen; (b) CD4 count > 500 cells/mL; and (c) HIV ribonucleic acid (RNA) < 50 copies/mL documented for at least 6 months prior to enrollment. If enrolled, these subjects should continue to have their CD4 and HIV RNA monitored by their HIV provider per their standard of care for the duration of the TR11 study, and the data should be reported and documented at the next study visit.
6. Males, non-fecund females, or females of childbearing potential using an acceptable method of birth control (if sexually active). All females of childbearing potential must have a negative pregnancy test at the screening and baseline visits.
For the purpose of this study, all females are considered to be of childbearing potential unless they are postmenopausal (i.e., at least 1 year since last menses and age > 50 years) or surgically sterile (i.e.,
tubal ligation, hysterectomy, and/or bilateral oophorectomy).
Sexually active female subjects of childbearing potential are required to use 1 barrier method (e.g., condom, cervical cap, or diaphragm) of contraception in addition to 1 other method (e.g., intrauterine device in place at least 1 month, stable hormonal contraception for at least 3 months, or Essure procedure, or spermicide). For female subjects using a barrier method plus spermicide, that method must be used for at least 14 days prior to screening.
Female subjects who are abstinent may participate in the study, however; they must be counseled on the requirement to use appropriate contraception should they become sexually active. This counseling should occur at each study visit and must be documented in source records.
7. Age 18 years and older at the time of consent, and a life expectancy of at least 18 months.
8. Willing and able to understand and provide written informed consent.
9. Willing and able to comply with study requirements and restrictions.
10. Agree to the confidential use and storage of all data and use of all anonymized data for publication including scientific publication. |
1.Diagnostic de PN (défini comme la présence de ≥ 20 nodules prurigineux, avec des lésions essentiellement nodulaires dans l’ensemble).
2.PN généralisé, défini comme des lésions de PN avec une composante nodulaire impliquant 2 régions anatomiques distinctes : par exemple, soit 2 membres, soit un seul membre et une partie axiale du corps. Les patients ne présentant que des lésions axiales avec atteinte de 2 régions anatomiques distinctes, sans chevauchement avec le système nerveux central, peuvent également participer : par exemple, lésions impliquant une partie du crâne et une partie du tronc. Pour les besoins de cette étude, la partie axiale sera définie comme toute partie non appendiculaire du corps.
3.Le score WINRS, enregistré quotidiennement au cours des 7 jours consécutifs précédant la visite d’entrée dans l’étude par le biais du carnet électronique, doit comporter au moins 5 mesures enregistrées et toutes les mesures individuelles doivent être ≥ 6. La valeur arithmétique moyenne des mesures doit être ≥7. Si nécessaire, la dernière valeur WINRS utilisée dans le calcul peut être enregistrée le jour de la visite d’entrée dans l’étude, tant que l’enregistrement a lieu avant l’administration de la dose.
4.Les patients ayant recours à des antidépresseurs et/ou neuroleptiques doivent recevoir une dose stable depuis au moins 8 semaines au moment de la signature du consentement et doivent vouloir rester sous cette dose stable pendant la durée totale de l’étude.
5.Les patients positifs au virus de l’immunodéficience humaine (VIH) peuvent être inclus dans l’étude s’ils remplissent les critères suivants : (a) actuellement sous traitement antirétroviral hautement actif stable (traitement stable > 6 mois) et bien toléré ; (b) numération des CD4 > 500 lymphocytes/ml ; et (c) acide ribonucléique (ARN) du VIH < 50 copies/ml documenté depuis au moins 6 mois au moment de l'inclusion. Les patientes sexuellement actives en capacité de procréer doivent obligatoirement utiliser 1 méthode de contraception barrière (par ex. préservatif, cape cervicale ou diaphragme) en plus d’une autre méthode (par ex. dispositif intra-utérin en place depuis au moins 1 mois, contraception hormonale stable depuis au moins 3 mois, procédure Essure, ou spermicide). Pour les patientes utilisant une méthode barrière plus un spermicide, cette méthode doit être utilisée depuis au moins 14 jours au moment de la sélection.
Les patientes abstinentes peuvent participer à l’étude ; néanmoins, elles doivent recevoir une information sur l’obligation de recourir à une contraception appropriée au cas où elles deviendraient sexuellement actives. 1. Pruritus due to localized PN (only 1 body part affected, for example only 1 arm).
2. Active, uncontrolled, pruritic dermatoses in need of treatment (such as atopic dermatitis, or bullous pemphigoid for example) or other dermatologic conditions that in the opinion of the Investigator could confound the ability to assess PN-related itch.
3. Prurigo Nodularis associated with a history of atopic dermatitis is excluded if acute eczematous lesions are present, as characterized by erythematous, active-predominant lichenified plaques with oozing and crusting.
4. Major psychiatric disorder, which in the opinion of the Investigator, could interfere with the assessment of anti-pruritic efficacy and/or safety events during the study or with the ability of the subject to cooperate with study requirements.
5. Serum bilirubin > 2.5 × upper limit of normal range at screening unless explained by a clinical diagnosis of Gilbert's Syndrome.
6. Serum hepatic alanine aminotransferase or aspartate aminotransferase enzymes > 100 U/L at screening.
7. Estimated glomerular filtration rate ≤ 44 mL/min/1.73 m^2 at screening.
8. Significant medical condition or other factors that in the opinion of the Investigator may interfere with the conduct of the study.
9. Subjects who have an active malignancy (either solid tumor or hematologic) are excluded. Subjects who have a past history of malignancy and who have no evidence of active disease, but who continue on therapy to prevent disease recurrence (i.e., tamoxifen for breast cancer, testosterone blockade for prostate cancer, etc.), may be eligible if approved by the Medical Monitor.
10. History of substance abuse which as determined by the Investigator may interfere with the conduct of the study.
11. Known intolerance of or hypersensitivity or allergy to nalbuphine or vehicle components.
12. Pregnant or lactating females.
13. Concurrent enrollment in an ongoing clinical trial or anticipated enrollment in a concurrent clinical trial.
Medication-related Exclusions:
14. Known intolerance (gastrointestinal, central nervous system symptoms) or hypersensitivity/drug allergy to opioids.
15. Potential subjects taking monoamine oxidase inhibitors are excluded, as concomitant opiate use may increase the risk for serotonin syndrome.
16. Potential subjects taking cyclosporin A are excluded unless they undergo a 6-week washout prior to beginning the screening period.
Washout should occur after signing informed consent, if done for study participation purposes, and prior to electronic diary (e-diary) WINRS collection. WINRS collection should not take place prior to 6 weeks after discontinuation of cyclosporin A. Subjects are prohibited from using cyclosporin during the study.
17. Potential subjects taking biologics (including monoclonal antibodies), which modify the immune system, are excluded unless they undergo a 3-month washout prior to beginning the screening period.
Washout should occur after signing informed consent, if done for study participation purposes, and prior to e-diary WINRS collection.
18. Potential subjects who have previously received dupilumab or nemolizumab are excluded.
19. Exposure to any investigational medication, including placebo, within
4 weeks (3 months for biologics) prior to e-diary WINRS collection during the screening period.
20. Potential subjects receiving UV-therapy (PUVA, UVA, UVB, Excimer) are excluded unless they have discontinued > 4 weeks prior to e-diary WINRS collection during the screening period. Discontinuation should
occur after signing informed consent, if done for study participation purposes, and prior to e diary WINRS collection. Subjects are prohibited from using UV-therapy for the duration of the study.
21. Potential subjects cannot have received opiates within 14 days prior to the screening period. Subjects are prohibited from using opioids, including naltrexone, for the duration of the study.
22. Potential subjects cannot have received gabapentin, pregabalin, calcineurin inhibitors, cannabinoid agonists, capsaicin, cryosurgery, topical doxepin, thalidomide or methotrexate, antihistamines (systemic or topical), and topical corticosteroids within 14 days prior to the screening period. These medications are prohibited for the duration of
the study.
23. Potential subjects cannot have received systemic corticosteroids within 4 weeks prior to the screening period. These medications are prohibited for the duration of the study.
Cardiac-related Exclusions:
24. Subjects with a history of congestive heart failure of Class 2 or higher as graded using the New York Heart Association scale.
25. Subjects with a history of angina pectoris Grade 2 or higher as graded using the Canadian Cardiovascular Society grading scale.
26. History of ventricular tachycardia, Torsade de Pointes, or family history of sudden death.

5.Bilirubine sérique>2,5×LSN à la sélection sauf si cela s’explique par un diagnostic clinique de syndrome de Gilbert.
6.Enzymes hépatiques sériques (alanine aminotransférase ou aspartate aminotransférase) > 100U/l à la sélection.
7.Débit de filtration glomérulaire estimé ≤ 44 ml/min/1,73 m2 à la sélection.
8.Pathologie médicale significative ou autres facteurs qui, de l’avis de l’investigateur, pourraient compromettre la conduite de l’étude.
9.Présence d’une néoplasie active (tumeur solide ou processus néoplasique hématologique). Les patients présentant des antécédents de néoplasies sans signe actuel de maladie active, mais qui sont toujours sous traitement pour prévenir la rechute (càd, tamoxifène pour le cancer du sein, anti-androgènes pour le cancer de la prostate, etc.), peuvent participer à l’étude après approbation de l’attaché de recherche clinique.
10.Antécédents d’abus de substances psychoactives qui, de l’avis de l’investigateur, peuvent compromettre la conduite de l’étude.
11.Intolérance, hypersensibilité ou allergie connues à la nalbuphine ou à ses excipients.
12.Femmes enceintes ou qui allaitent.
13.Inclusion simultanée dans une étude clinique en cours ou inclusion prévue dans une étude clinique concomitante.
Exclusions liées au médicament :
14.Intolérance (symptômes gastro-intestinaux, du système nerveux central) ou hypersensibilité/allergie médicamenteuse connue aux opioïdes.
15.Les patients prenant des inhibiteurs de la monoamine oxydase sont exclus, car l’utilisation concomitante d’opiacés peut augmenter le risque de syndrome sérotoninergique.
16.Les patients prenant de la cyclosporine A sont exclus sauf s’ils ont fait l’objet d’un sevrage thérapeutique de 6 semaines avant le début de la phase de sélection. Le sevrage thérapeutique doit intervenir après la signature du consentement éclairé, s’il est entrepris en vue de participer à l’étude, et avant l’évaluation WINRS dans le carnet électronique. L’évaluation WINRS ne doit pas avoir lieu avant que se soient écoulées 6 semaines depuis l’arrêt de la cyclosporine A. Le recours à la cyclosporine est interdit aux patients pendant l’étude.
17.Les patients prenant des médicaments biologiques (y compris des anticorps monoclonaux) modifiant le système immunitaires sont exclus sauf s’ils ont fait l’objet d’un sevrage thérapeutique de 3 mois avant le début de la phase de sélection. Le sevrage thérapeutique doit intervenir après la signature du consentement éclairé, s’il est entrepris en vue de participer à l’étude, et avant l’évaluation WINRS dans le carnet électronique.
18.Les patients ayant reçu antérieurement du dupilumab ou du némolizumab sont exclus.
19.Exposition à un produit expérimental, quel qu’il soit, y compris un placebo, dans les 4 semaines (3 mois pour les médicaments biologiques) précédant l’évaluation WINRS dans le carnet électronique pendant la phase de sélection.
20.Les patients recevant un traitement par UV (PUVA, UVA, UVB, excimère) sont exclus sauf si ce traitement a été arrêté plus de 4 semaines avant l’évaluation WINRS dans le carnet électronique pendant la phase de sélection. Safety:
All on-treatment safety data will be assessed descriptively based on the number and rates of adverse events (AEs), Serious AEs (SAEs), clinical laboratory measurements, central cardiac core laboratory read12lead ECG, vital signs, and physical examinations. Subjects will also complete the Subjective Opiate Withdrawal Scale (SOWS) on a daily basis for the 2 weeks following the last dose of investigational product, whenever that occurs and regardless of the reason (unless consent is withdrawn). The totality of these data addresses the secondary objective of characterizing the overall safety and tolerability of NAL ER in subjects with PN.
An independent Data Safety Monitoring Board will periodically review safety data.

Sécurité : toutes les données de sécurité recueillies sous traitement seront évaluées de manière descriptive sur la base du nombre et des taux d’événements indésirables (EI) et d’EI graves (EIG), des résultats d’analyses de laboratoire, de la lecture centralisée d’ECG à 12 dérivations, des constantes vitales et des examens cliniques. Les patients rempliront également l’échelle d’évaluation subjective des symptômes de sevrage aux opiacés (Subjective Opiate Withdrawal Scale ou SOWS) chaque jour pendant les 2 semaines qui suivront la dernière administration du produit expérimental, quelle que soit la date de l’arrêt et indépendamment de son motif (sauf en cas de retrait du consentement). L’intégralité de ces données sert l’objectif secondaire de caractérisation de la sécurité et de la tolérance globales de la NAL LP chez les patients atteints de PN.
Un comité de surveillance indépendant passera en revue périodiquement les données relatives à la sécurité.
Pharmacocinétique :
