1. Written informed consent must be obtained prior to any screening procedures 2. Adult ≥ 18 years old at the time of informed consent 3. Histologically confirmed locally advanced/metastatic (stage IIIB or IV per AJCC/IASLC v. 8) NSCLC 4. Histologically or cytologically confirmed diagnosis of NSCLC that is both EGFR wt status and ALK- negative rearrangement status: • Patients with NSCLC of pure squamous cell histology can enter screening without EGFR mutation or ALK rearrangement testing or result; however, patients with pure squamous cell histology who are known to have EGFR mutations in exons 19 or 21 or ALK rearrangements will be excluded 5. Patients must have demonstrated progression of locally advanced/ metastatic NSCLC (stage IIIB, not amenable for definitive chemo-irradiation, or stage IV) following one prior platinum doublet and one prior PD-(L)1 checkpoint inhibitor (either alone or in combination) • Maintenance therapy given after first-line chemotherapy will be considered as part of the first-line therapy if given to patients with documented response or stable disease before starting the maintenance therapy. • Neo-adjuvant and adjuvant systematic therapies will count as one prior line of systemic treatment for the advanced stage if relapse occurred within 12 months from the end of the neoadjuvant or adjuvant systemic therapy. • The most recent line of therapy should include a PD-(L)1 checkpoint inhibitor (either alone or in combination) 6. Patients must be candidates for single agent chemotherapy with docetaxel 7. Patients must have recovered from all toxicities related to prior anticancer therapies to grade ≤ 1 (CTCAE v 5.0). Patients with any grade of alopecia are allowed to enter the study 8. At least one measurable lesion as defined by RECIST 1.1. A previously irradiated site lesion may only be counted as a target lesion if there is clear sign of progression since the irradiation 9. Patients must have adequate organ function including the following laboratory values at the screening visit: • Absolute neutrophil count (ANC) ≥ 1.5 x 109/L without growth factor support • Platelets ≥ 75 x 109/L • Hemoglobin (Hgb) ≥ 9 g/dL • Calculated creatinine clearance (using Cockcroft-Gault formula) ≥ 45 mL/min • Total bilirubin ≤ 1.5 x ULN • Aspartate transaminase (AST) ≤ 3 x ULN, except for patients with liver metastasis, who may only be included if AST ≤ 5 x ULN • Alanine transaminase (ALT) ≤ 3 x ULN, except for patients with liver metastasis, who may only be included if ALT ≤ 5 x ULN • Alkaline phosphatase (ALP) ≤ 5.0 x UL • Asymptomatic serum amylase ≤ Grade 2. Patients with Grade 1 or Grade 2 serum amylase at the beginning of the study must be confirmed to have no signs and/or symptoms suggesting pancreatitis or pancreatic injury (e.g., elevated P-amylase, abnormal imaging findings of pancreas, etc.) • Serum lipase ≤ ULN • Fasting plasma glucose ≤ 160 mg/dL (≤ 8.9 mmol/L) 10. ECOG performance status (PS) of 0 or 1 11. Willing and able to comply with scheduled visits, treatment plan and laboratory tests |
1. Consentement éclairé écrit avant toute procédure de sélection. 2. Adultes âgés ≥ 18 ans lors du recueil du consentement éclairé 3. Cancer bronchique non à petites cellules (CBNPC), localement avancé ou métastatique confirmé par histologie (stade IIIB ou IV selon l’AJCC/l’IASLC v.8) 4. CBNPC confirmé histologiquement ou cytologiquement, sans mutation EGFR (statut sauvage/wild type) ni réarrangement ALK (statut ALK négatif) • Les patients atteints d’un CBNPC à histologie purement épidermoïde peuvent entrer dans la phase de sélection sans que le statut EGFR ou ALK soit connu ; néanmoins ceux ayant des mutations EGFR dans l’exon 19 ou 21 ou des réarrangements ALK connus seront exclus de l’étude 5. Le CBNPC localement avancé/métastatique (stade IIIB, ne relevant pas d’une chimio/radiothérapie définitive ou de stade IV) doit avoir progressé après administration d’un doublet à base de platine et d’un inhibiteur de point de contrôle anti-PD-(L)1 (seul ou en association) • Un traitement de maintien reçu après une chimiothérapie de première ligne sera considéré comme faisant partie du traitement de première ligne s’il est administré à des patients disposant d’une réponse documentée ou ayant une maladie stable avant le début de ce traitement de maintien. • Les thérapies systématiques néo-adjuvantes et adjuvantes seront considérées comme des thérapies de première ligne en cas de rechute dans les 12 mois après la fin de celles-ci. • La thérapie la plus récente doit avoir inclus un inhibiteur de point de contrôle anti-PD-(L)1 (seul ou en association) • Les patients doivent avoir au moins une réponse documentée à la ligne de traitement contenant le point de contrôle PD- (L)1 6. Les patients doivent être éligibles à une mono-chimiothérapie par docétaxel 7. Les patients doivent avoir récupéré de toute toxicité liée aux traitements anticancéreux antérieurs, jusqu’à atteindre le grade ≤ 1 sur l’échelle CTCAE, (version 5.0). Les patients ayant une alopécie, quel qu’en soit le grade, peuvent participer à l’étude. 8. Présence d’au moins une lésion tumorale mesurable selon RECIST 1.1. Une lésion préalablement irradiée ne pourra être prise en compte comme lésion cible que s’il existe un signe clair de progression depuis l’irradiation. 9. Lors de la visite de sélection, les patients doivent avoir une fonction organique adéquate y compris les valeurs des paramètres biologiques : • Nombre absolu de neutrophiles ≥ 1,5 x 109/l sans stimulation par des facteurs de croissance • Numération plaquettaire ≥ 75 x 109/l • Hémoglobine ≥ 9 g/dl • Clairance de la créatinine sérique, calculée par la formule de Cockcroft-Gault ≥ 45 ml/mn • Bilirubine totale ≤ 1,5 x LSN (la limite supérieure de la normale) • ASAT ≤ 3 x LSN, sauf pour les patients présentant des métastases hépatiques qui ne pourront être inclus dans l’étude que si les valeurs de l’ASAT sont ≤ 5 x LSN • ALAT ≤ 3 x LSN, sauf pour les patients présentant des métastases hépatiques qui ne pourront être inclus dans l’étude si les valeurs de l’ALAT sont ≤ 5 x LSN • Phosphatases alcalines ≤ 5,0 x LSN • Amylase sérique asymptomatique ≤ Grade 2. Les patients dont les valeurs de l’amylase sérique sont de grade 1 ou 2 au début de l’étude doivent présenter une absence confirmée de signes et/ou symptômes évoquant une pancréatite ou une atteinte du pancréas (élévation du taux d’amylase P; anomalies du pancréas révélées par imageries) • Lipase sérique ≤ LSN • Glycémie à jeun ≤ 160 mg/dl (≤ 8,9 mmol/l) 10. 1. Prior treatment with a MET inhibitor or HGF-targeting therapy 2. Any untreated central nervous system (CNS) lesion. However, patients are eligible if all known CNS lesions have been treated with radiotherapy or surgery and remained stable for ≥ 4 weeks after treatment. Patients must be off corticosteroid therapy for ≥ 2 weeks 3. Carcinomatous meningitis. 4. Use of hematopoietic colony-stimulating growth factors (e.g. G-CSF, GMCSF, M-CSF), thrombopoietin mimetics or erythroid stimulating agents ≤ 2 weeks prior start of study treatment. If erythroid stimulating agents were initiated more than 2 weeks prior to the first dose of study treatment and the patient is on a stable dose, they can be maintained 5. Use of any live vaccines against infectious diseases within 3 months of initiation of study treatment. Patients randomized to the spartalizumab containing arm will need to comply with this criterion for the whole duration of the study treatment 6. Patients receiving treatment with any enzyme-inducing anticonvulsant that cannot be discontinued at least 1 week before first dose of study treatment, and for the duration of the study. Patients on non-enzyme-inducing anticonvulsants are eligible 7. Systemic chronic steroid therapy (≥ 10 mg/day prednisone or equivalent) or any immunosuppressive therapy 7 days prior to planned date of first dose of study treatment. Topical, inhaled, nasal and ophthalmic steroids are allowed. Steroid premedication for docetaxel infusion does not apply 8. Thoracic radiotherapy to lung fields ≤ 4 weeks prior to starting Cycle 1 Day 1 or patients who have not recovered from radiotherapy-related toxicities. For all other anatomic sites (including radiotherapy to thoracic vertebrae and ribs), radiotherapy ≤ 2 weeks prior to Cycle 1 Day 1, or patients who have not recovered from radiotherapy-related toxicities. 9. Major surgery within 4 weeks prior to starting study treatment (2 weeks for resection of brain metastases), or patients who have not recovered from the side effects of such a procedure. Video-assisted thoracic surgery (VATS) and mediastinoscopy will not be counted as major surgery and patients can be enrolled in the study ≥1 week after the procedure 10. Impairment of GI function or GI disease that may significantly alter the absorption of capmatinib 11. Active, known or suspected autoimmune disease or a documented history of autoimmune disease. Note: patients with vitiligo, controlled type I diabetes mellitus on stable insulin dose, residual autoimmune-related hypothyroidism only requiring hormone replacement, psoriasis not requiring systemic treatment, or conditions not expected to recur in the absence of an external trigger are permitted to enroll. 12. Previous anti-cancer and investigational agents within 4 weeks or ≤ 5 x half-life of the agent (whichever is longer) before first dose of study treatment. If previous treatment is a monoclonal antibody or an anti PD-(L)1 checkpoint inhibitor, then the treatment must be discontinued at least 4 weeks before first dose of study treatment. If previous treatment is an oral targeted agent, then the treatment must be discontinued at least 5 x half-life of the agent 13. History of allogenic bone marrow or solid organ transplant |
1. Traitement préalable avec un inhibiteur de MET ou un traitement ciblant le HGF 2. Toute lésion non traitée du système nerveux central (SNC). Utilisation de facteurs de croissance stimulant les colonies hématopoïétiques (par exemple, G-CSF, GMCSF, M-CSF), de mimétiques de la thrombopoïétine ou d'agents stimulant les érythroïdes ≤ 2 semaines avant le début de l'étude. Si des agents stimulants érythroïdes ont été initiés plus de 2 semaines avant la première dose du traitement à l’étude et que le patient prend une posologie stable, ils peuvent être maintenus. 5. Utilisation de tout vaccin vivant contre les maladies infectieuses dans les 3 mois suivant le début de l'étude. Les patients randomisés dans le bras contenant le spartalizumab devront se conformer à ce critère pendant toute la durée du traitement à l'étude 6. Patients recevant un traitement par un anticonvulsivant induisant une enzyme qui ne peut pas être interrompu au moins une semaine avant la première dose de l'étude et pendant la durée de l'étude. Les patients recevant des anticonvulsivants non induisant des enzymes sont éligibles 7. Traitement systémique chronique aux stéroïdes (prednisone ≥ 10 mg / jour ou l’équivalent) ou tout traitement immunosuppresseur 7 jours avant la date prévue de la première dose du traitement à l’étude. Les stéroïdes topiques, inhalés, nasaux et ophtalmiques sont autorisés. La prémédication stéroïde pour la perfusion de docétaxel ne s'applique pas 8. Radiothérapie thoracique aux champs pulmonaires ≤ 4 semaines avant le début du cycle 1 jour 1 ou chez les patients qui ne se sont pas remis de leurs toxicités liées à la radiothérapie. Pour tous les autres sites anatomiques (y compris la radiothérapie de l'abdomen thoracique et des côtes), radiothérapie ≤ 2 semaines avant le premier jour du cycle 1 ou les patients qui ne se sont pas remis de leurs toxicités liées à la radiothérapie. 9. Intervention majeure dans les 4 semaines précédant le début de l'étude (2 semaines pour la résection de métastases cérébrales) ou sur les patients qui ne se sont pas remis des effets secondaires d'une telle procédure. La chirurgie thoracique vidéo-assistée (VATS) et la médiastinoscopie ne seront pas considérées comme une chirurgie majeure et les patients peuvent être inclus dans l'étude ≥ 1 semaine après la procédure. 10. Altération de la fonction gastro-intestinale ou d'une maladie gastro-intestinale pouvant altérer de manière significative l'absorption du capmatinib 11. Maladie auto-immune active, connue ou soupçonnée ou antécédents documentés de maladie auto-immune. Remarque: les patients atteints de vitiligo, de diabète sucré de type I contrôlé avec une posologie stable d'insuline, d'hypothyroïdie auto-immune résiduelle nécessitant uniquement un remplacement hormonal, de psoriasis ne nécessitant pas de traitement systémique ou de conditions ne nécessitant pas de récidive en l'absence de déclencheur externe sont autorisés. 12. Agents anticancéreux et expérimentaux antérieurs dans les 4 semaines ou ≤ 5 x la demi-vie de l'agent (qui est plus longue) avant la première dose du traitement à l'étude. Si le traitement précédent est un anticorps monoclonal ou un inhibiteur de point de contrôle anti-PD (L) 1, le traitement doit être interrompu au moins 4 semaines avant la première dose du traitement à l'étude.
