E.3 | Principal inclusion criteria |
Participants are eligible to be included in the study only if all of the following criteria apply:
1. Signed informed consent.
2. Men and women, aged 18 or older (or as applicable per local country requirements).
3. Diagnosis of MCC with distant metastatic disease as a component of tumor burden and no more than 3 prior systemic treatments, inclusive of
systemic adjuvant therapy.
4. ECOG performance status of 0 to 1.
5. Measurable disease according to RECIST v1.1.
6. Availability of tumor tissue (fresh or archival) for central pathology review.
7. Willingness to avoid pregnancy or fathering children based on the criteria below:
a. Men must agree to take appropriate precautions to avoid fathering children (with at least 99% certainty) from screening through 6 months after the last dose of study treatment and must refrain from donating sperm during this period. Permitted methods that are at least 99% effective in preventing pregnancy should be communicated to the participants and their understanding confirmed.
b. Women of childbearing potential must have a negative serum pregnancy test at screening and before the first dose on Day 1 and must agree to take appropriate precautions to avoid pregnancy (with at least 99% certainty) from screening through safety follow-up. Permitted methods that are at least 99% effective in preventing pregnancy should be communicated to the participants and their understanding confirmed.
c. Women of nonchildbearing potential (ie, surgically sterile with a hysterectomy and/or bilateral oophorectomy OR ≥ 12 months of amenorrhea and at least 50 years of age) are eligible. |
Seuls les patients qui satisfont à l'ensemble des critères suivants sont éligibles à l'étude :
1. Signature du consentement éclairé.
2. Hommes et femmes, âgés de 18 ans ou plus (ou en fonction des exigences du pays concerné).
3. Diagnostic de CCM avec maladie métastatique distante en tant que composante de la charge tumorale, et pas plus de 3 traitements systémiques antérieurs, y compris les traitements d'appoint systémiques.
4. Indice de performance ECOG de 0 à 1.
5. Maladie mesurable selon les critères RECIST 1.1.
6. Disponibilité de tissu tumoral (frais ou archivé) pour examen pathologique central.
7. Accepter d'éviter une grossesse ou de concevoir un enfant, conformément aux critères suivants :
a. Les hommes doivent accepter de prendre des précautions appropriées pour éviter de concevoir un enfant (avec une certitude d'au moins 99 %)
depuis la sélection jusqu'à 6 mois après la dernière dose du traitement à l'étude, et ils doivent s'abstenir de faire des dons de sperme pendant cette période. Les méthodes autorisées qui sont efficaces à au moins 99% dans la prévention de la grossesse doivent être communiquées aux patients, qui doivent confirmer les avoir comprises.
b. Les femmes aptes à procréer doivent présenter un test de grossesse sérique négatif lors de la sélection et avant la première dose du jour 1, et doivent accepter de prendre des précautions appropriées pour éviter une grossesse (avec une certitude d'au moins 99 %) depuis la selection et jusqu'à 120 jours après la dernière dose du traitement à l'étude. c. Les femmes qui ne sont pas aptes à procréer (c.-à-d., chirurgicalement stériles ayant subi une hystérectomie et/ou une oophorectomie bilatérale OU ayant observé ≥ 12 mois d'aménorrhée et âgées d'au moins 50 ans) sont éligibles. |

E.4 | Principal exclusion criteria |
Participants are excluded from the study if any of the following criteria apply:
1. Prior PD-1 or PD-L1–directed therapy.
2. Treatment with anticancer drugs or participation in another interventional clinical study within 21 days before the first administration of study drug.
3. Participant has not recovered to ≤ Grade 1 or baseline from toxic effects of prior therapy (with the exceptions for anemia not requiring transfusion support and any grade of alopecia) and/or complications from prior surgical intervention within 7 days before starting study treatment.
4. Radiation therapy administered within 2 weeks of first dose of study treatment or radiation therapy in the thoracic region that is > 30 Gy within 6 months of the first dose of study treatment.
5. Known CNS metastases and/or carcinomatous meningitis
6. Any known additional malignancy that is progressing or requires active treatment, or history of other malignancy within 3 years of study entry with the exception of cured basal cell or squamous cell carcinoma of the skin, superficial bladder cancer, prostate intraepithelial neoplasm, carcinoma in situ of the cervix, or other noninvasive or indolent malignancy, or cancers from which the participant has been disease-free for > 1 year, after treatment with curative intent.
7. Participants with laboratory values at screening as defined in protocol.
8. Evidence of interstitial lung disease or active, noninfectious pneumonitis.
9. Participants with impaired cardiac function or clinically significant cardiac disease:
• New York Heart Association Class III or IV cardiac disease, including preexisting clinically significant ventricular arrhythmia, congestive heart failure, or cardiomyopathy.
• Unstable angina pectoris ≤ 6 months before study participation.
• Acute myocardial infarction ≤ 6 months before study participation.
• Other clinically significant heart disease (ie, ≥ Grade 3 hypertension, history of labile hypertension, or poor compliance with an antihypertensive regimen). Must have recovered (to baseline or ≤ Grade 1) from toxicity associated with prior treatment.
10. Active autoimmune disease requiring systemic immunosuppression in excess of physiologic maintenance doses of corticosteroids.
11. Chronic or current active infectious disease requiring systemic antibiotics, antifungal, or antiviral treatment.
12. Known active hepatitis A, B, or C or active infections requiring systemic antibiotics.
13. Has received a live vaccine within 90 days of planned start of study therapy.
Note: Examples of live vaccines include, but are not limited to, the following: measles, mumps, rubella, chicken pox/zoster, yellow fever, rabies, Bacillus Calmette–Guérin, and typhoid vaccine. Seasonal influenza vaccines for injection are generally killed virus vaccines and are allowed; however, intranasal influenza vaccines (eg, FluMist®) are live attenuated vaccines and are not allowed.
14. Current use of prohibited medication as described in Section 6.2.2.
15. Known hypersensitivity to another monoclonal antibody, which cannot be controlled with standard measures (eg, antihistamines and corticosteroids).
16. Participant lacks the ability or is unlikely, in the opinion of the investigator, to comply with the Protocol requirements.
17. Participant who is pregnant or breastfeeding
18. Any condition that would, in the investigator's judgment, interfere with full participation in the study, including administration of study drug/treatment and attending required study visits; pose a significant risk to the participant; or interfere with interpretation of study data.
19. History of organ transplant, including allogeneic stem cell transplantation.
20. Known allergy or hypersensitivity to any component of the study drug formulation. |
Les patients ne peuvent participer à l'étude s'ils remplissent l'un quelconque des critères suivants :
1. Traitement antérieur ciblant PD-1 ou PD-L1.
2. Traitement par médicaments anticancéreux ou participation à une autre étude clinique interventionnelle dans les 21 jours précédant la première administration du produit à l'étude.
3. Le patient n'est pas revenu à un grade ≤ 1 ou à son niveau de référence des effets toxiques d'un traitement antérieur (à l'exception d'une anémie ne nécessitant pas de transfusion et de tout grade d'alopécie) et/ou des complications d'une intervention chirurgicale antérieure dans les 7 jours précédant le début du traitement à l'étude.
4. Une radiothérapie administrée dans les 2 semaines précédant la première dose du traitement à l'étude ou une radiothérapie dans la région thoracique > 30 Gy dans les 6 mois précédant la première dose du traitement à l'étude.
5. Présence connue de métastases du SNC et/ou d'une méningite carcinomateuse.
Remarque : Les patients avec des métastases cérébrales préalablement traitées peuvent participer à condition qu'ils aient été stabilisés (sans preuve de progression par imagerie pendant au moins 28 jours avant la première dose de produit à l'étude, et les symptômes neurologiques sont revenus à leur niveau de référence), qu'ils n'aient aucun signe de métastases cérébrales nouvelles ou croissantes ni d'oedème du SNC, et qu'ils n'aient pas besoin de prendre des stéroïdes pendant au moins 14 jours avant la première dose du produit à l'étude.
6. Toute tumeur maligne supplémentaire connue qui progresse ou nécessite un traitement actif, ou des antécédents d'autre tumeur maligne dans les 3 ans précédant l'entrée dans l'étude, à l'exception d'un carcinome basocellulaire ou épidermoïde, d'un cancer de la vessie superficiel, d'un néoplasme intra-épithéliale prostatique, d'un carcinoma in situ du col de l'utérus ou de toute autre tumeur maligne non invasive ou indolente guéris, ou de cancers pour lesquels le patient a été declaré indemne depuis > 1 an, après un traitement avec intention curative.
7. Les patients présentant les résultats d'analyses à la sélection précisés dans le Tableau 5.
8. Preuve de maladie pulmonaire interstitielle ou de pneumopathie non infectieuse active.
9. Patients souffrant d'une insuffisance cardiaque ou d'une maladie cardiaque cliniquement significative :
• Maladie cardiaque de classe III ou IV selon la New York Heart Association, notamment arythmie ventriculaire cliniquement significative préexistante, insuffisance cardiaque congestive ou cardiomyopathie.
• Angine de poitrine instable ≤ 6 mois avant la participation à l'étude.
• Infarctus aigu du myocarde ≤ 6 mois avant la participation à l'étude.
• Autre maladie cardiaque cliniquement significative (c.-à-d., hypertension ≥ grade 3, antécédents d'hypertension labile ou mauvaise observance d'un schéma thérapeutique antihypertenseur). Doit avoir récupéré (au niveau de référence ou à un grade ≤ 1) de toute toxicité associée à un traitement antérieur.
10. Maladie auto-immune active nécessitant une immunosuppression systémique au-delà de doses d'entretien physiologiques à base de corticoïdes.
11. Maladie infectieuse active chronique ou en cours nécessitant un traitement systémique antibiotique, antifongique ou antiviral.
12. Infection active par le virus de l'hépatite A, B, ou C ou autre infection active connue nécessitant des antibiotiques systémiques.
13. A reçu un vaccin vivant dans les 90 jours précédant le début prévu du traitement à l'étude.
Remarque : Des exemples de vaccins vivants concernent notamment les maladies suivantes : la rougeole, les oreillons, la rubéole, la varicelle/le zona, la fièvre jaune, la rage, le bacille de Calmette–Guérin et la typhoïde. Les vaccins par injection contre la grippe saisonnière sont généralement des vaccins à virus inactivé et sont autorisés. Toutefois, les vaccins contre la grippe intranasale (par ex. FluMist®) sont des
vaccins vivants atténués et ne sont pas autorisés.
14. Utilisation en cours de médicaments interdits tels que décrits à la Section 6.2.2.
