    E.2.1Main objective of the trial
    To evaluate the efficacy of oral administration of Simvastatin plus Rifaximin in halting the progression of decompensated cirrhosis as assessed by the time to first incidence of ACLF during treatment period.

    E.2.3Trial contains a sub-study No
    E.3Principal inclusion criteria
    Patients included into the study must meet all the following criteria:
    1. Age ≥ 18 years old.
    2. Cirrhosis defined by standard clinical criteria, ultrasonographic findings and/or histology. Cirrhosis of any aetiology may be included. However, patients with cirrhosis due to autoimmune hepatitis must be on stable corticosteroid dose for ≥3-month period before study inclusion.
    3. Child-Pugh B patients or Child-Pugh C patients (up to 12 points).
    4. Women of child-bearing potential* must have a negative pregnancy test in serum before the inclusion in the study and agree to use highly effective contraceptive methods during the study. Highly effective contraceptive methods will include: intrauterine device, bilateral tubal occlusion, vasectomized partner and sexual abstinence. Hormonal contraceptive methods will be avoided due to the risk of adverse events and impairment of liver function.
    1. Alter ≥ 18 Jahre alt.
    2. Zirrhose definiert nach klinischen Standardkriterien, Ultraschallbefunde und/oder Histologie. Eine Zirrhose jeglicher Ätiologie kann eingeschlossen werden. Patienten mit einer Zirrhose aufgrund von Autoimmunhepatitis müssen jedoch auf einer stabilen Kortikosteroiddosis für ≥3 Monate vor der Studie sein.
    3. Child-Pugh B-Patienten oder Child-Pugh C-Patienten (bis zu 12 Punkte).
    4. Frauen im gebärfähigen Alter* müssen einen negativen Schwangerschaft im Serum vor der Aufnahme in die Studie nachweisen und stimmen der Verwendung von hochwirksamen Verhütungsmethoden während der Studie zu. Hochwirksame Verhütungsmethoden beinhalten: Intrauterinpessar, bilaterale Eileiterokklusion, vasektomierter Partner und sexuelle Abstinenz. Hormonelle Verhütungsmethoden werden vermieden, da ein Risiko von Nebenwirkungen und Beeinträchtigung der Leberfunktion besteht.
    E.4Principal exclusion criteria
    Potential participants who meet any of the following criteria will be excluded from participation in this study:
    1. Patients on treatment with statins or rifaximin up to one month before study inclusion.
    2. Patients with contraindications for statins or rifaximin therapy.
    3. Known hypersensitivity to simvastatin or rifaximin (or rifamycin derivatives).
    4. Patients with CK elevation of 50% or more above the upper limit of normal at study inclusion.
    5. Patients on treatment with potent inhibitors of CYP3A4 enzyme (see section 5.2: Concomitant, nonpermitted and permitted medication).
    6. Patients on treatment with drugs with potential interactions with simvastatin (see section 5.2: Concomitant, nonpermitted and permitted medication).
    7. Patients with previous history of myopathy.
    8. Patients with previous history of intestinal obstruction or those who are at increased risk of this complication.
    9. Patients with ACLF according to the criteria published by Moreau et al. (see appendix 2).
    10. Serum creatinine ≥2 mg/dL (176.8 μmol/L).
    11. Serum bilirubin>5 mg/dL (85.5 μmol/L).
    12. INR ≥2.5. Patients under anticoagulant therapy will be excluded if the INR valuares are ≥3.5
    13. Bacterial infection within 10 days before study inclusion.
    14. Gastrointestinal bleeding within 10 days before study inclusion.
    15. Current overt hepatic encephalopathy, defined as grade II-IV hepatic encephalopathy according to the New-Haven classification.
    16. Patients with active hepatocellular carcinoma or history of hepatocellular carcinoma that is in remission for less than six months for uninodular HCC or for less than 12 months for multinodular HCC within Milan criteria.
    17. Patients on antiviral therapy for HCV or those who have received it within the last 6 months.
    18. Severe alcoholic hepatitis requiring corticosteroid therapy (Maddrey’s Discriminant function ≥ 32 and/or ABIC score > 6.7).
    19. Patients with active alcohol consumption of more than 21 units per week.
    20. HIV infection.
    21. Patients with a history of significant extra hepatic disease with impaired short-term prognosis, including congestive heart failure New York Heart Association Grade III/IV, COPD GOLD >2, chronic kidney disease with serum creatinine >2mg/dL or under renal replacement therapy.
    22. Patients with current extra hepatic malignancies including solid tumours and hematologic disorders.
    23. Pregnancy or breastfeeding.
    24. Patients included in other clinical trials in the month before inclusion.
    25. Patients with mental incapacity, language barrier, bad social support or any other reason considered by the investigator precluding adequate understanding, cooperation or compliance in the study.
    26. Refusal to give informed consent.
    27. Creatinine clearance < 30 ml/min
    28. Patients with cirrhosis due to cholestatic liver disease can only be included in the study if they present clinical decompensation of cirrhosis (i.e. ascites).
    29. Patients with previous organ transplantation.
    1. Patienten, die bis zu einem Monat vor der Aufnahme in die Studie mit Statin oder Rifaximin behandelt wurden.
    2. Patienten mit Kontraindikationen für Statin oder Rifaximintherapie.

    6. Patienten, die mit Medikamenten behandelt werden, die potenzielle Wechselwirkungen mit Simvastatin haben (siehe Abschnitt 5.2: Begleitende, nichtzugelassen und zugelassene Medikamente).
    7. Patienten mit Vorgeschichte einer Myopathie.
    8. Patienten mit Vorgeschichte einer Darmverschlussstörung oder solche, die einem erhöhten Risiko dieser Komplikation ausgesetzt sind.
    9. Patienten mit ACLF nach den Kriterien von Moreau et al. (siehe Anlage 2).
    10. Serumkreatinin ≥2 mg/dL (176,8 μmol/L).
    11. Serum Bilirubin >5 mg/dL (85,5 μmol/L).
    12. INR ≥2,5. Patienten unter gerinnungshemmender Therapie werden ausgeschlossen, wenn die INR-Werte ≥ 3,5 sind.
    13. Bakterielle Infektion innerhalb von 10 Tagen vor Aufnahme der Studie.
    14. Gastrointestinale Blutungen innerhalb von 10 Tagen vor Aufnahme in die Studie.
    15. Aktuelle offene Leber-Enzephalopathie, definiert als Grad II-IV Leber-Enzephalopathie nach der New-Haven-Klassifikation.
    16. Patienten mit aktivem hepatozellulären Karzinom oder Vorgeschichte eines hepatozellulären Karzinoms, das sich weniger als 6 Monate in Remission befindet für uninodulares HCC oder für weniger als 12 Monate für multinodulares HCC innerhalb der Milan-Kriterien.
    17. Patienten mit antiviraler Therapie für HCV oder diejenigen, die sie innerhalb der letzten 6 Monate erhalten haben.
    18. Schwere alkoholische Hepatitis, die eine Kortikosteroidtherapie erfordert (Maddrey’s Discriminant function ≥ 32 und/oder ABIC-Wert > 6,7).

    25. Patienten mit geistiger Behinderung, Sprachbarriere, schlechter sozialen Unterstützung oder aus einem anderen Grund, der vom Prüfer als ausschließend angesehen wird, ein angemessenes Verständnis, Zusammenarbeit oder Konformität in der Studie zu erbringen.
    26. Verweigerung, eine Einwilligung nach Aufklärung zu erteilen.
    27. Kreatinin-Clearance < 30 ml/min.
    28. Patienten mit Zirrhose aufgrund einer cholestatischen Lebererkrankung können nur in die Studie eingeschlossen werden, wenn sie eine klinische Dekompensation der Zirrhose (d.h. Aszites) aufweisen.

    -Häufigkeit der Krankenhauseinweisungen aufgrund von Zirrhosekomplikationen bewertet zur Ausgangslage, 1, 3, 6, 9 und 12 Monate
    -Entwicklung/Verschlimmerung individueller Komplikationen der Zirrhose (Aszites, akute Nierenschädigung, gastrointestinale Blutungen, hepatische Enzephalopathie (HE), bakterielle Infektionen) bewertet zu baseline, 1, 3, 6, 9 und 12 Monate
    -Veränderungen gegenüber dem Ausgangswert der systemischen Entzündungsreaktion, ausgewertet durch Messung in einer Vielzahl von Plasmazytokinstufen einschließlich, aber nicht beschränkt auf TNFα, IL-6, IL8, IL-10, IL-1β, IFN-ɣ, G-CSF, VCAM, VEGF, sowie eine oxidierte Form von Albumin, menschliche Non-Mercaptalbumin-2 (HNA2), Dimethylarginin (ADMA und SDMA) zu den Zeitpunkten 3 Monate, 6 Monate und 12 Monate.
    -Änderungen gegenüber Baseline bei Plasmabiomarkern (FABP4, sCD-163), von Willebrand-Faktor (vWF) und Urin-Biomarker (NGAL, MCP-1 und Albumin) zu den Zeitpunkten 3 Monaten, 6 Monaten und 12 Monaten.
    -Veränderungen gegenüber dem Ausgangswert der vasoaktiven Hormone: Plasma-Renin Konzentration, Plasma-Norepinephrin und Plasma-Copeptin zu den Zeitpunkten 3 Monate, 6 Monate und 12 Monate.
    -Veränderungen gegenüber dem Ausgangswert des Blutspiegels der bakteriellen DNA oder bakterieller Produkte zu den Zeitpunkten 3 Monaten, 6 Monaten und 12 Monaten.
    -Bewertung von Veränderungen in der Mikrobiomzusammensetzung durch Analyse von mikrobieller Gene und Signatur bei Patienten, die in die Studie einbezogen wurden, zu den Zeitpunkten baseline, 3 Monate, 6 Monate und 12 Monate.
    -Veränderungen gegenüber dem Ausgangswert der Leberfunktion, bewertet durch den MELD-Score, CLIF-AD Score und Child Pugh Score
    -Lebensqualität, funktionelle Beurteilung und bei minimalem HE während der Behandlungszeit, bewertet durch CLDQ (Chronic Liver Disease Questionnaire), Liver Frailty Index und PHES (Psychometric Hepatic Encephalopathy Score) Fragebögen zu den Zeitpunkten baseline, 1 Monat, 3 Monate, 6 Monate, 9 Monate und 12 Monate
    -Veränderungen gegenüber dem Ausgangswert der Stigmatisierung bei Patienten mit dekompensierte Zirrhose, der durch einen zuvor validierten spezifischen Fragebogen (siehe Anlage 8) erfasst wurde, nach 3 Monaten, 6 Monaten und 12 Monaten.
    -Auftreten von Muskeltoxizität zu Baseline, 1 Monat, 3 Monate, 6 Monate Monate, 9 Monate und 12 Monate
    -Bewertung genetischer Polymorphismen der Statinmembran-Transporter OATP1B1 bei allen in die Studie einbezogenen Patienten.
    -Beurteilung der Veränderung der Leber- oder Muskeltoxizität in Leber- oder Muskelenzymen (Transaminasen, alkalische Phosphatase und Kreatinkinase) nach 1 Monat, 3 Monaten, 6 Monaten, 9 Monaten und 12 Monaten, im Vergleich zur Ausgangslage
    -Anzahl der Patienten und Schwere der behandlungsbedingten unerwünschten Ereignisse während des Studienzeitraums.
    -Jährliche Inzidenz von ACLF.
    -Zeit des allgemeinen und Zeit des krankheitsbedingten Überleben.
