    E.2.1Main objective of the trial
    To evaluate the efficacy of oral administration of Simvastatin plus Rifaximin in halting the progression of decompensated cirrhosis as assessed by the time to first incidence of ACLF during treatment period.

    E.3Principal inclusion criteria
    Patients included into the study must meet all the following criteria:
    1. Age ≥ 18 years old.
    2. Cirrhosis defined by standard clinical criteria, ultrasonographic findings and/or histology. Cirrhosis of any aetiology may be included. However, patients with cirrhosis due to autoimmune hepatitis must be on stable corticosteroid dose for ≥3-month period before study inclusion.
    3. Child-Pugh B patients or Child-Pugh C patients (up to 12 points).
    4. Women of child-bearing potential* must have a negative pregnancy test in serum before the inclusion in the study and agree to use highly effective contraceptive methods during the study. Highly effective contraceptive methods will include: intrauterine device, bilateral tubal occlusion, vasectomized partner and sexual abstinence. Hormonal contraceptive methods will be avoided due to the risk of adverse events and impairment of liver function.
    Los pacientes incluidos en el estudio deberán cumplir los siguientes criterios:
    1. Edad ≥ 18 años.
    2. Cirrosis definida según criterios clínicos estándar y resultados de ultrasonográficos y/o histología. Pueden incluirse pacientes con cirrosis de cualquier etiología. Los pacientes con cirrosis de etiología autoinmune en tratamiento con corticosteroides deberán estar con dosis estable de corticosteroides por ≥ 3 meses, previa inclusión en el estudio.
    3. Pacientes con Child-Pugh B y C (hasta 12 puntos).
    4. Las mujeres en edad fértil* deben tener una prueba negativa de embarazo en suero antes de la inclusión en el estudio y deben comprometerse a utilizar métodos anticonceptivos altamente eficaces (dispositivo intrauterino, oclusión tubárica bilateral, pareja vasectomizada y abstinencia sexual).
    Se evitarán métodos anticonceptivos hormonales para evitar el riesgo de acontecimientos adversos y deterioro de la función hepática.
    E.4Principal exclusion criteria
    Potential participants who meet any of the following criteria will be excluded from participation in this study:
    1. Patients on treatment with statins or rifaximin up to one month before study inclusion.
    2. Patients with contraindications for statins or rifaximin therapy.
    3. Known hypersensitivity to simvastatin or rifaximin (or rifamycin derivatives).
    4. Patients with CK elevation of 50% or more above the upper limit of normal at study inclusion.
    5. Patients on treatment with potent inhibitors of CYP3A4 enzyme (see section 5.2: Concomitant, nonpermitted and permitted medication).
    6. Patients on treatment with drugs with potential interactions with simvastatin (see section 5.2: Concomitant, nonpermitted and permitted medication).
    7. Patients with previous history of myopathy.
    8. Patients with previous history of intestinal obstruction or those who are at increased risk of this complication.
    9. Patients with ACLF according to the criteria published by Moreau et al. (see appendix 2).
    10. Serum creatinine ≥2 mg/dL (176.8 μmol/L).
    11. Serum bilirubin>5 mg/dL (85.5 μmol/L).
    12. INR ≥2.5.
    13. Bacterial infection within 10 days before study inclusion.
    14. Gastrointestinal bleeding within 10 days before study inclusion.
    15. Current overt hepatic encephalopathy, defined as grade II-IV hepatic encephalopathy according to the New-Haven classification.
    16. Patients with active hepatocellular carcinoma or history of hepatocellular carcinoma that is in remission for less than six months for uninodular HCC or for less than 12 months for multinodular HCC within Milan criteria.
    17. Patients on antiviral therapy for HCV or those who have received it within the last 6 months.
    18. Severe alcoholic hepatitis requiring corticosteroid therapy (Maddrey’s Discriminant function ≥ 32 and/or ABIC score > 6.7).
    19. Patients with active alcohol consumption of more than 21 units per week.
    20. HIV infection.
    21. Patients with a history of significant extra hepatic disease with impaired short-term prognosis, including congestive heart failure New York Heart Association Grade III/IV, COPD GOLD >2, chronic kidney disease with serum creatinine >2mg/dL or under renal replacement therapy.
    22. Patients with current extra hepatic malignancies including solid tumours and hematologic disorders.
    23. Pregnancy or breastfeeding.
    24. Patients included in other clinical trials in the month before inclusion.
    25. Patients with mental incapacity, language barrier, bad social support or any other reason considered by the investigator precluding adequate understanding, cooperation or compliance in the study.
    26. Refusal to give informed consent.
    Los potenciales participantes que cumplan algunos de los siguientes criterios serán excluidos del estudio:
    1. Pacientes en tratamiento con estatinas o rifaximina durante el mes antes de la inclusión en el estudio.
    2. Pacientes con contraindicaciones para la rifaximina o la simvastatina.
    3. Hipersensibilidad conocida a la rifaximina (o derivados de la rifamicina) o a la simvastatina.
    4. Pacientes con aumento de los niveles de CK del 50% o superiores del límite alto de la normalidad en el momento de la inclusión en el estudio.
    5. Pacientes que reciban tratamiento con inhibidores potentes de la enzima CYP3A4 (ver sección 5.2: Medicación concomitante no permitida y permitida).
    6. Pacientes que reciban tratamiento con fármacos con interacciones potenciales con la simvastatina (ver sección 5.2: Medicación concomitante no permitida y permitida).
    7. Pacientes con antecedentes de miopatía.
    8. Pacientes con antecedente o con riesgo aumentado de obstrucción intestinal.
    9. Pacientes con ACLF de acuerdo con los criterios de Moreau et al. (ver Anexo 2).
    10. Creatinina sérica ≥ 2 mg/dL (176,8 μmol/L).
    11. Bilirrubina sérica > 5 mg/dL (85,5 μmol/L).
    12. INR ≥ 2,5.
    13. Infección bacteriana en los 10 días previos a la inclusión.
    14. Hemorragia gastrointestinal en los 10 días previos a la inclusión.
    15. Encefalopatía hepática clínica en el momento de la inclusión, definida como grado II-IV según la clasificación New-Haven.
    16. Pacientes con carcinoma hepatocelular activo o antecedentes de carcinoma hepatocelular en remisión durante menos de seis meses por HCC uninodular o por menos de 12 meses por HCC multinodular según los criterios de Milán.
    17. Pacientes en tratamiento antiviral para el VHC o tratados en los últimos 6 meses.
    18. Hepatitis alcohólica severa que requiera tratamiento con corticoides (Índice de Maddrey ≥ 32 y/o score ABIC > 6,7).
    19. Pacientes con consumo activo de alcohol de más de 21 unidades de bebida a la semana.
    20. Infección por VIH.
    21. Pacientes con historia de enfermedad extrahepática significativa con mal pronóstico a corto plazo, incluyendo insuficiencia cardiaca congestiva grado III/IV de la clasificación de New York Heart Association, enfermedad pulmonar obstructiva crónica GOLD >2, enfermedad renal crónica con creatinina sérica > 2 mg/dL o en tratamiento con hemodiálisis.
    22. Pacientes con cáncer actual extrahepático, incluyendo tumores sólidos o enfermedades hematológicas.
    23. Embarazo o lactancia.
    24. Pacientes incluidos en otros ensayos clínicos en el último mes.
    25. Pacientes con incapacidad mental, barrera lingüística mayor, pobre soporte social o alguna otra razón que el investigador considere que no permita una adecuada comprensión, cooperación o seguimiento del estudio.

    10. Cambios respecto a la visita basal en la función hepática evaluada mediante MELD score, CLIF-AD score y Child Pugh Score (ver Anexo 4) al mes 1 y a los 3, 6, 9 y 12 meses.
    11. Calidad de vida, evaluación funcional y encefalopatía hepática mínima durante el período de tratamiento, mediante la evaluación de los cuestionarios CLDQ (Cuestionario para pacientes con enfermedad hepática crónica), Liver Frailty score y PHES (Test de encefalopatía hepática mínima) en la visita basal, al mes 1 y a los 3, 6, 9 y 12 meses (ver anexos 5, 6 y 7).
    12. Cambios respecto a la visita basal en la estigmatización en los pacientes con cirrosis descompensada, evaluada mediante un cuestionario específico (ver Anexo 8) a los 3, 6 y 12 meses.
    13. Aparición de toxicidad muscular en la visita basal, mes 1 y a los 3, 6, 9 y 12 meses mediante el cuestionario específico de miopatía relacionado con el uso de estatinas (ver Anexo 3).
    14. Evaluación del polimorfismo genético del transportador de membrana de estatinas, OATP1B1, en todos los pacientes incluidos en el estudio.
