The European Union Clinical Trials Register   allows you to search for protocol and results information on:
  • interventional clinical trials that were approved in the European Union (EU)/European Economic Area (EEA) under the Clinical Trials Directive 2001/20/EC
  • clinical trials conducted outside the EU/EEA that are linked to European paediatric-medicine development

  • EU/EEA interventional clinical trials approved under or transitioned to the Clinical Trial Regulation 536/2014 are publicly accessible through the
    Clinical Trials Information System (CTIS).


    The EU Clinical Trials Register currently displays   43845   clinical trials with a EudraCT protocol, of which   7282   are clinical trials conducted with subjects less than 18 years old.   The register also displays information on   18700   older paediatric trials (in scope of Article 45 of the Paediatric Regulation (EC) No 1901/2006).

    Phase 1 trials conducted solely on adults and that are not part of an agreed paediatric investigation plan (PIP) are not publicly available (see Frequently Asked Questions ).

    Patients with progressive disease (recurrent and/or metastatic) of breast cancer, metastatic gastro-intestinal tumors, non small cell lung cancer (NSCLC) or others. Patients should have completed at least 1 line of treatment and no more than 2 as defined by the current version of the AIOM guidelines. Patients are included if surgery is contraindicated.

    The main objective of our study is to evaluate the efficacy (meant as overall response rate ORR) of TT vs SoC.

    E.3Principal inclusion criteria
    1. Age > = 18 at time of signing Informed Consent Form
    2. Patients able and willing to provide a written informed consent to participate to the study
    3. Patients with recurrent/metastatic breast, gastrointestinal cancer, non small cell lung cancer or others
    4. Patients not treatable with potentially curative surgery ot other loco-regional treatments.
    5. Patients should have been completed at least 1 line of treatment for breast cancer, gastro-intestinal cancer, non small cell lung cancer or other cancer
    6. ECOG performance status from 0 to 1
    7. Molecular target not actionable with approved drugs identified during screening by profiling with Foundation One on biopsy and Foundation ACT on blood
    8. Biopsiable disease (tumor biopsy mandatory for tumor profiling). The biopsy must be performed during the screening period, when patients complete the conventional therapy for their recurrent/metastatic cancer. Historical samples will be considered for the study if collected within 3 months before the ICF signature of the patient. Samples older than 3 months, with a maximum timeframe of 6 months, will be considered upon clinical judgement of the Investigator.
    9. Measurable disease, eligible to standard treatment. Patients must have measurable or evaluable disease defined, per RECIST 1.1 or irCS (immune related Response Criteria), as at least one lesion that can be accurately measured in at least one dimension (longest diameter to be recorded for non-nodal lesions and short axis for nodal lesions) as > =20 mm with conventional techniques or as > =10 mm with spiral computed tomography (CT) scan, Magnetic Resonance Imaging (MRI), or a subcutaneous or superficial lesion that can be measured with calipers by clinical exam. For lymph nodes, the short axis must be > =15 mm. Patients who have assessable disease by physical or radiographic examination but do not fully meet the above definitions of measurable disease (but still remains measurable) are eligible and will be considered to have evaluable disease. Patient's whose disease cannot be objectively measured by physical or radiographic examination (e.g., elevated serum tumor marker only) are NOT eligible. PET scan could be performed, if clinically indicated. For PET response evaluation PERCIST criteria will be applied.
    10. Adequate renal function defined by a serum creatinine <1.5xUNL (upper normal limit).
    11. Adequate liver function test defined by SGOT & SGPT <3xUNL (5xUNL in case of liver metastases), and bilirubin level <1.5xUNL
    12. Adequate bone marrow function defined by platelets >100,000/mm3, hemoglobin >10 g/dL, and neutrophils >1,000/mm3
    13. For female of child-bearing potential and for all women < 1 years after the onset of menopause: a negative pregnancy test <72 hours before starting study treatment is required. If sexually active, female of childbearing potential must use "highly effective" methods of contraception for the study duration. Contraception should continue after the last treatment for 3 months or for longer periods according to what reported in the Appendix 1

La biopsia deve essere eseguita durante la fase di screening, quando i pazienti completano la terapia convenzionale per il cancro recidivante/metastatico. I campioni storici saranno accettati se raccolti entro 3 mesi dalla firma dell’ICF. Campioni più vecchi di 3 mesi, con un limite massimo di 6 mesi, verranno presi in considerazione in base al giudizio clinico del medico.
    9. Malattia misurabile, candidabile alla terapia standard. I pazienti devono avere malattia misurabile o valutabile secondo i criteri RECIST 1.1 o irCS (immune related Response Criteria), in modo che almeno una lesione sia misurabile in almeno una dimensione (deve essere misurato il diametro più lungo per le lesioni non-nodali e quello più corto per le lesioni nodali) come > = 20 mm con tecniche convenzionali o come > =10 mm con tomografia computerizzata a spirale (TC), risonanza magnetica (MRI), o una lesione subcutanea o superficiale che possa essere misurata con il calibro durante l’esame fisico. Per i linfonodi, l’asse corto deve essere > = 15 mm. I pazienti che presentano malattia misurabile per mezzo di esame fisico o radiologico, ma che non soddisfano le precedenti definizioni di malattia misurabile (ma che comunque rimane misurabile) sono eleggibili e verranno considerati avere malattia misurabile. I pazienti la cui malattia non può essere misurata in modo oggettivo mediante esame fisico o radiologico (es. solo elevato marcatore tumorale nel siero) NON sono eleggibili. La scansione PET può essere eseguita se clinicamente indicata. Per la valutazione della risposta della PET verranno applicati i criteri PERCIST.
    10. Adeguata funzionalità renale definita da valori di laboratorio di creatinina sierica <1.5xUNL (limite superiore normale).
    11. Adeguata funzionalità epatica definita da valori di laboratorio di SGOT & SGPT <3xUNL (5xUNL in caso di metastasi epatiche) e livelli di bilirubina <1.5xUNL.
    12. Adeguata funzionalità midollare definita da valori di laboratorio di piastrine >100;000/mm3, emoglobina >10g/dl e neutrofili >1,000/mm3.
    13. Per le donne in età fertile e per tutte le donne con <1 anno di amenorrea: un test di gravidanza negativo deve essere disponibile <72 ore dall’inizio dello studio.     1. Patients who have only bone and/or brain metastases
    2. Patients treated with more than 2 lines for breast cancer, gastro-intestinal, non small cell lung cancer and other cancer
    3. Patients whose brain metastases have not been monitored for >2 months
    4. Patients with well-established actionable targets for which approved and marketed targeted drugs are available (i.e. lung cancer with EGFR mutation, or ALK translocation, B-RAF mutant melanoma, GIST with KIT mutations or breast cancer with HER2 amplification)
    5. Patient participating in another clinical trial with an experimental drug
    6. Anticoagulation with anti-vitamin K (Low Molecular Weight Heparin [LMWH] is allowed)
    7. Patients with other concurrent severe and/or uncontrolled medical disease which could compromise participation in the study, including uncontrolled diabetes, cardiac disease, uncontrolled hypertension, congestive cardiac failure, ventricular arrhythmias, active ischemic heart disease, myocardial infarction within one year, chronic liver or renal disease, active gastrointestinal tract ulceration, severely impaired lung function
    8. Pregnant and/or breastfeeding women
    9. Patients with any psychological, familial, sociological or geographical condition potentially hampering compliance with the study protocol and follow-up schedule
    10. HIV, HBV, or HCV infection as per specific test performed at the screening visit or known as per Medical History
    11. Patients with documented contraindication to any of the IMPs that will be used for the study, as reported in the respective SmPcs/IBs and in Appendix 2
    12. Patients treated with the following drugs, because of the risk of immunosuppression: Chronic or high-dose oral corticosteroid therapy, TNF-inhibitors and Anti-T cell antibodies
    1. Pazienti con solo metastasi ossee/cerebrali
    2. Pazienti sottoposti a più di due linee di trattamento per carcinoma mammario, carcinoma gastro-intestinale, carcinoma polmonare non a piccole cellule e altri tumori.
    3. Pazienti le cui metastasi cerebrali non vengano monitorate da >2 mesi.
    4. Pazienti con target azionabili ben stabiliti per i quali siano disponibili in commercio farmaci target approvati (per esempio: carcinoma polmonare con mutazione di EGFR o traslocazioni di ALK, melanoma con B-RAF mutante, GIST con mutazioni di KIT o carcinoma mammario con amplificazione di HER2).
    5. Pazienti che stanno partecipando ad un altro studio clinico con un farmaco sperimentale.
    6. Utilizzo di anticoagulanti con anti-vitamina K (ad eccezione dell’eparina a basso peso molecolare [LMWH]).
    7. Pazienti affetti da qualsiasi altra condizione medica concomitante severa e/o non controllata che potrebbe compromettere la partecipazione allo studio come diabete non controllato, patologie cardiache, ipertensione non controllata, insufficienza cardiaca congestizia, aritmie ventricolari, cardiopatia ischemica attiva, infarto miocardico entro 1 anno, patologia epatica o renale cronica, ulcerazione attiva del tratto gastrointestinale, funzionalità polmonare gravemente compromessa.
    8. Donne in gravidanza o allattamento.
    9. Pazienti con qualsiasi condizione psicologica, familiare, sociologica o geografica che potrebbe compromettere l’aderenza al protocollo ed al follow-up.
    10. Pazienti affetti da infezione da HIV, HBV o HCV con anamnesi nota o da test specifico eseguito alla visita di screening.
    11. Pazienti con controindicazione documentata a qualsiasi trattamento che verrà utilizzato durante lo studio, come riportato nella corrispondente RCP/IB e nell'Appendice 2.
    12. Pazienti in trattamento concomitante con i seguenti farmaci per il rischio di immunosoppressione: terapia con corticosteroidi orali cronica o ad alto dosaggio, inibitori di TNF ed anticorpi anti cellule T.
    E.5 End points
    E.5.1Primary end point(s)
    Evaluation of the OVERALL RESPONSE RATE (ORR) of the Treatment at choice of physicians, according to Standard of Care (SoC) or of the Tailored Treatment (TT). The ORR will be constructed according to the specific design of the study, therefore including also the Rescue Therapy Phase data.
    This means that the ORR will take into account 3 evaluations:
    • The ORR estimation on the original final population ( i.e 384 patients divided into the 4 groups of type of cancer)
    • The ORR estimation done on the TT patients, which will include the original randomized TT patients and the patients swithed from the standard of care therapy (SoC therapy) to the TT Therapy, this latter within the Rescue Therapy Phase (patients switching upon the first documented progression)
    • The ORR estimation done on the population composed by the original TT patients, the original SoC patients and the switched TT patients. This
    means that the total population analyzed will include the original 384 population data (as per randomization) and the additional switched TT patients.
    Valutazione del tasso di risposta globale (ORR) del trattamento scelto dal medico, secondo la cura standard (SoC) o il trattamento su misura (TT).
    Il ORR verrà costruito secondo lo specifico disegno dello studio, includendo quindi anche i dati della fase della terapia di salvataggio.

    • la stima di ORR sulla popolazione originale finale (ossia 384 pazienti divisi nei 4 gruppi in base alla tipologia di tumore)
    • la stima di ORR effettuata sui pazienti TT, che includerà i pazienti randomizzati inizialmente nel gruppo TT ed i pazienti passati dalla terapia standard (SoC) a quella TT, questi ultimi entro la fase della terapia di salvataggio (i pazienti che effettuano il passaggio al momento della prima progressione documentata)
    • la stima di ORR eseguita sulla popolazione composta dai pazienti TT iniziali, i pazienti SoC iniziali e quelli passati a TT. Ciò significa che la popolazione
    complessiva analizzata includerà i dati dei 384 pazienti della popolazione iniziale e, in aggiunta, i pazienti che sono passati a TT.

    The secondary efficacy variables are as follows:
    Progression free survival: PFS the time from randomization to the first documented radiographical progressive disease,
    using current RECIST (Therasse et al. 2000 [40]), or irRC (Hodi et al. 2016 [37]), or death from any cause, whichever
    occurs first. Carcinomatous meningitis diagnosed by cytological evaluation of cerebral spinal fluid will also define
    progressive disease. Medical photography will also be allowed to monitor chest wall recurrences of subcutaneous
    lesions.
    Time to treatment failure (TTF): Time from randomization to discontinuation of treatment for any reason, including disease progression, treatment toxicity, and death. (refer to the study Protocol)
