E.3 | Principal inclusion criteria |
1) Diagnosis of large duct PSC based on cholangiogram (magnetic resonance cholangiopancreatography [MRCP], endoscopic retrograde cholangiopancreatography [ERCP], or percutaneous transhepatic cholangiogram [PTC]) 2) Liver biopsy at Screening that is deemed acceptable for interpretation and demonstrates stage F0 – F3 fibrosis (according to the Ludwig classification) in the opinion of the central reader a. A historical liver biopsy within 6 months of the Screening visit may be accepted as the Screening biopsy if the sample is deemed acceptable for interpretation by the central reader. 3) Subject has the following laboratory parameters at the Screening visit, as determined by the central laboratory: a. Platelet count ≥ 150,000/mm3 b. Estimated glomerular filtration rate (eGFR) ≥ 30 milliliter/minute (mL/min), as calculated by the Cockcroft-Gault equation c. ALT ≤ 8 x ULN d. Total bilirubin < 2 mg/dL, unless the subject is known to have Gilbert’s syndrome or hemolytic anemia e. International normalized ratio (INR) ≤ 1.4, unless due to therapeutic anticoagulation f. Negative anti-mitochondrial antibody 4) For subjects on ursodeoxycholic acid (UDCA), the dose of UDCA must have been stable for at least 6 months before Screening and anticipated to remain stable throughout the trial. For subjects not on UDCA, no UDCA use for at least 6 months prior to Screening. |
1) Diagnóstico de CEP del conducto grande en base a colangiografía (colangiopancreatografía por resonancia magnética [CPRM], colangiopancreatografía retrógrada endoscópica [CPRE] o colangiografía transhepática percutánea [CTP]). 2) Biopsia hepática durante la selección que se considere aceptable para su interpretación y muestre una fibrosis en estadio F0 a F3 (según la clasificación de Ludwig) en opinión del evaluador central. a. Recuento de plaquetas ≥150 000/mm3. b. Tasa de filtración glomerular estimada (TFGe) ≥30 mililitros/minuto (ml/min), según el cálculo mediante la ecuación de Cockcroft-Gault. c. ALT ≤8 veces el límite superior de la normalidad (LSN). d. Bilirrubina total <2 mg/dl, a menos que se sepa que el paciente padece síndrome de Gilbert o anemia hemolítica. e. Índice internacional normalizado (INR) ≤1,4 a menos que se deba a tratamiento anticoagulante. f. E.4 | Principal exclusion criteria |
Current or prior history of any of the following: a. Cirrhosis as defined by any of the following: i. Liver biopsy demonstrating stage F4 fibrosis according to the Ludwig classification (or equivalent) ii. Decompensated liver disease, including ascites, hepatic encephalopathy (HE), or variceal hemorrhage iii. Liver stiffness > 14.4 kPa by FibroScan® b. Liver transplantation c. Cholangiocarcinoma or hepatocellular carcinoma (HCC). If a dominant stricture has been identified, cholangiocarcinoma must be adequately excluded in the opinion of the investigator prior to Day 1. d. Ascending cholangitis within 30 days of Screening 2) Presence of a percutaneous drain or biliary stent 3) Other causes of cholangitis including IgG4-related sclerosing cholangitis, autoimmune hepatitis/PSC overlap syndrome, secondary sclerosing cholangitis, small duct PSC (histologic evidence of PSC with normal bile ducts on cholangiography), and viral, metabolic, alcoholic, and other autoimmune conditions. Subjects with hepatic steatosis may be included if there is no evidence of nonalcoholic steatohepatitis (NASH) on liver biopsy in the opinion of the central reader 4) Current or prior history of any of the following: a. Malignancy within 5 years of Screening with the following exceptions: i. Adequately treated carcinoma in situ of the cervix ii. Adequately treated basal or squamous cell cancer or other localized non-melanoma skin cancer. Subjects under evaluation for possible malignancy are not eligible. b. Unstable cardiovascular disease as defined by any of the following: i. Unstable angina, myocardial infarction, coronary artery bypass graft surgery or coronary angioplasty within 6 months prior to Screening ii. Transient ischemic attack or cerebrovascular accident within 6 months prior to Screening iii. Symptomatic obstructive valvular heart disease or hypertrophic cardiomyopathy iv. Symptomatic congestive heart failure v. Uncontrolled or recurrent ventricular tachycardia or other arrhythmia requiring an automatic implantable cardioverter defibrillator (AICD). Stable, controlled atrial fibrillation is allowed. c. Hypercoagulable condition or venous or arterial thromboembolic disease d. Intestinal resection or malabsorptive condition that may limit the absorption of GS-9674. Prior cholecystectomy and appendectomy are permitted. 5) Child-Pugh (CP) score > 6 at Screening, unless due to an alternative etiology such as Gilbert’s syndrome or therapeutic anticoagulation 6) Model for End-stage Liver Disease (MELD) score > 12 at Screening, unless due to an alternate etiology such as therapeutic anticoagulation 7) HIV infection (HIV Ab and HIV ribonucleic acid [HIV RNA] positive) 8) Chronic HBV infection (HBsAg positive) 9) Chronic HCV infection (HCV Ab and HCV RNA positive). Subjects cured of HCV infection ≥ 2 years prior to Screening are eligible. 10) Current moderate to severely active inflammatory bowel disease (IBD) defined as a Screening visit Partial Mayo score of > 4 and/or a score in the Screening visit Rectal Bleeding domain > 1, unless bleeding is due to perianal disease 11) Habitual alcohol consumption greater than 21 oz/week for males or 14 oz/week for females (1 oz/30 mL of alcohol is present in one 12 oz/360 mL beer, one 4 oz/120 mL glass of wine, and a 1 oz/30 mL measure of 40% proof alcohol) 12) Use of antibiotics (eg, vancomycin, metronidazole, minocycline, etc.) for the treatment of PSC within 60 days of Screening. Antibiotic prophylaxis for ascending cholangitis is permitted if stable for at least 6 months prior to Screening 13) Use of any prohibited concomitant medications |

Si se ha identificado una estenosis dominante, deberá excluirse adecuadamente el colangiocarcinoma según el criterio del investigador antes del día 1. d)Colangitis ascendente en los 30 días previos a la selección. 2)Presencia de drenaje percutáneo o endoprótesis biliar. 3) Otras causas de enfermedad hepática como colangitis esclerosante relacionada con IgG4, hepatitis autoinmune/síndrome de superposición de CEP, colangitis esclerosante secundaria, CEP del conducto pequeño (indicios histológicos de CEP con conductos biliares normales en la colangiografía) y otras afecciones víricas, metabólicas, debidas al consumo de alcohol y de origen autoinmune. Puede incluirse a pacientes con esteatosis hepática si no muestran indicios de esteatohepatitis no alcohólica (EHNA) en biopsia hepática según el criterio del evaluador central.4)Presencia o antecedentes de cualquiera de las siguientes enfermedades: a)Neoplasia maligna en los 5 años anteriores a la selección, con las siguientes excepciones: i.Carcinoma localizado de cuello uterino tratado de forma adecuada.ii.Carcinoma basocelular o epidermoide u otro cáncer cutáneo no melanoma localizado tratado de forma adecuada. Los pacientes que estén siendo evaluados para detectar una posible neoplasia maligna no son aptos. b)Enfermedad cardiovascular inestable definida por cualquiera de los siguientes motivos: i.Angina inestable, infarto de miocardio, cirugía de revascularización coronaria o angioplastia coronaria en los 6 meses previos a la selección. ii.Accidente isquémico transitorio o accidente cerebrovascular en los 6 meses previos a la selección. iii.Cardiopatía valvular obstructiva o miocardiopatía hipertrófica sintomáticas. iv.Insuficiencia cardíaca congestiva sintomática. v.Taquicardia ventricular no controlada o recurrente u otro tipo de arritmia que requiera un desfibrilador cardioversor implantable automático (DCIA). Se permite la fibrilación auricular estable y controlada. c)Estado de hipercoagulabilidad y enfermedad tromboembólica venosa o arterial. d)Resección intestinal o estado de absorción insuficiente que pueda limitar la absorción de GS-9674. Se permiten la colecistectomía y apendectomía previas. 5)Puntuación de Child-Pugh (CP) >6 durante la selección, a menos que se deba a una etiología alternativa como síndrome de Gilbert o anticoagulación terapéutica. 6)Puntuación de incide pronóstico Modelo para la enfermedad hepática en estadio final (Model for End-stage Liver Disease, MELD) >12 durante la selección, a menos que se deba a una etiología alternativa como anticoagulación terapéutica. 7)Infección por VIH (resultado positivo para Ac frente a VIH y al ácido ribonucleico de VIH [ARN del VIH]). 8)Infección por VHB (resultado positivo para HbsAg). 9)Infección por VHC (resultado positivo para Ac frente VHC y al ARN de VHC). Se consideran aptos los pacientes que se hayan curado de a infección por VHC ≥2 años antes de la visita de selección. 10)Enfermedad intestinal inflamatoria (EII) en curso moderada a gravemente activa definida como una puntuación Mayo parcial >4 en la visita de selección y/o una puntuación del dominio de hemorragia rectal >1 en la visita de selección, a menos que el sangrado sea debido a una enfermedad perianal. 11)Consumo habitual de alcohol superior a 630 ml/semana en varones o 420 ml/semana en mujeres (30 ml de alcohol suponen una cerveza de 360 ml, 1 vaso de vino de 120 ml y una medida de 30 ml de alcohol de 20°). 12)Uso de antibióticos (p. ej., vancomicina, metronizadol, minociclina, etc.) para el tratamiento de la CEP durante los 60 días previos a la selección. Está permitida la profilaxis con antibióticos para la colangitis ascendente si se ha mantenido estable durante al menos los 6 meses previos a la selección. 13)Uso de algún medicamento concomitante prohibido según se describe en el protocolo. |
