- Patients of both sexes aged ≥18 and ≤ 75 years - IBS-D or IBS-M diagnostic confirmed according to Rome IV criteria - Recurrent abdominal pain in the last three months (six months prior to diagnosis) on average at least one day per week associated with at least one of the following factors: - associated with defecation - associated with a change in stool frequency - associated with a change in stool consistency - Evaluation of the IBS-D or IBS-M symptoms: o NRS pain > 3 points at visit 1 and visit 2; the value of NRS pain at visit 2 differs from that at visit 1 by max. 1 point o IBS-SSS > 75 points on visit 1 and visit 2; the value of the IBS-SSS on visit 2 differs from that on visit 1 by max. 25 points o stool consistency documented by the patient using the Bristol stool form scale. • Patienten beiderlei Geschlechts im Alter von ≥18 und ≤ 75 Jahren • Gesicherte IBS-D oder IBS-M Diagnose gemäß Rom-IV-Kriterien o wiederkehrende Bauchschmerzen in den letzten drei Monaten (sechs Monate vor der Diagnosestellung) im Durchschnitt mindestens einen Tag pro Woche, die mit mindestens einem der folgenden Faktoren in Verbindung gebracht wurden: -assoziiert mit der Stuhlentleerung -assoziiert mit einer Veränderung der Stuhlfrequenz -assoziiert mit einer Veränderung der Stuhlkonsistenz • Bewertung der IBS-D oder IBS-M Symptomatik: o NRS Schmerz > 3 Punkte an Visite 1 und Visite 2; der Wert des NRS Schmerz an Visite 2 unterscheidet sich von dem an Visite 1 um max. 1 Punkt o IBS-SSS > 75 Punkte an Visite 1 und Visite 2; der Wert des IBS-SSS an Visite 2 unterscheidet sich von dem an Visite 1 um max. 25 Punkte o Stuhlkonsistenz dokumentiert durch den Patienten anhand der Bristol- Stuhlformen Skala. Inadequate drug medication for known hyperthyroidism, hypothyroidism, Hashimoto's thyroiditis, or evidence of thyroid dysfunction according to visit 1 blood results and at the investigator's discretion. 19. known hypersensitivity to myrrh, chamomile flowers or any of the adjuvants of MYRRHINIL-INTEST® or placebo 20. Serious somatic/psychosomatic, neurological and/or psychiatric conditions diagnosed and making it difficult for the patient to make an informed decision about consent to participate in the clinical trial. This is assessed by the investigator treating the patient. b) Medication 1. taking neuroleptics up to 1 month before the start of the study and during participation in the study 2. taking antibiotics within the last 10 days before the start of the study and/or during participation in the study 3. taking of systemic corticosteroids up to 1 month before the start of the study and during participation in the study 4. taking medication to treat IBS-D or IBS-M (including herbal and probiotic medicines) at visit 1 and within the last 30 days prior to the start of the study 5. continued use of NSAIDs (non-steroidal anti-inflammatory drugs, e.g. ibuprofen) for more than 14 days (excluding NSAIDs used as low-dose thrombosis prevention medication, e.g. acetylsalicylic acid or to treat adverse events) 6. intake of iron preparations and cardiac glycosides 7. Intake of opioids c) General anamnesis 1. patients who participated or are participating (screening) in other clinical trials 30 days prior to the initial inclusion examination or during the clinical trial, or who previously participated in the same trial(randomization) 2. patients with a disease or in a situation which, in the opinion of the investigator, could expose the patient to a significant risk, affect the study results or significantly affect them 3. signs or symptoms of incipient bacterial or viral infection accompanied by fever (> 38,5 °C) 4. Sofern Stuhltests in den vorangegangen 6 Monaten vor Einschluss in die Studie durchgeführt wurden: o Ausschluss von Blut im Stuhl,außer: - Blutspuren wegen lokaler Irritation und/oder einer Analfissur aufgrund häufiger Defäkation bzw. harter Stuhlkonsistenz (festgestellt durch lokale Inspektion bzw. eine digitale rektale Untersuchung) - Blutspuren wegen Hämorrhoiden (festgestellt durch eine digitale rektale Untersuchung oder Proktoskopie o Positiver Test auf Parasiten und Wurmeier o klinisch relevant erhöhte Calprotectin-Werte (> 50 μg/g) im Stuhl 5. Diagnostizierte infektiöse Gastroenteritis 6. Bekannte Anomalitäten des Gastrointestinaltraktes (z. B. Immungeschwächte Patienten (Patienten mit Organtransplantation innerhalb der vorangegangenen 3 Jahre, Patienten mit bekannter HIV 16. Zustand nach Kolonteilresektionen 17. Bekannter Diabetes Mellitus, Typ I und/oder Typ II 18. Nicht angemessene medikamentöse Einstellung bei bekannter Hyper- oder Hypothyreose, Hashimoto-Thyreoiditis oder Anzeichen einer Schilddrüsenfunktionsstörung gemäß der Blutwerte von Visite 1 und nach Ermessen des Prüfarztes. 19. Bekannte Überempfindlichkeit gegen Myrrhe, Kamillenblüten oder einen der Hilfsstoffe von MYRRHINIL-INTEST® oder des Placebos 20. Diagnostizierte schwere psychosomatische, neurologische und/oder psychiatrische Erkrankungen b) Medikation 1. Einnahme von Neuroleptika bis 1 Monat vor Studienbeginn und während der Studienteilnahme 2. ASS oder zur Behandlung unerwünschter Ereignisse eingesetzt werden) 6. Einnahme von Eisenpräparaten und Herzglykosiden 7. Einnahme von Opioiden c) Allgemeine Anamnese 1. Patienten, welche 30 Tage vor erstmaliger Einschlussuntersuchung oder während der klinischen Prüfung an anderen Arzneimittelstudienteilnahmen oder teilnehmen (Screening), oder vorher in der gleichen Studie teilgenommen haben (Randomisierung) 2. Efficacy is assessed by a responder criterion, which is different for patients with IBS-D and IBS-M. For both subgroups a response of abdominal pain (NRS pain) is required: • A responder is defined as a patient who has a decrease in abdominal pain by at least 30% from the value in the 2-week run-in phase before randomization in the 2-week control period at the end of the treatment phase. For patients of group IBS-D an additional assessment of stool consistency is required (Bristol stool scale): • A stool consistency responder is defined as a patient who has a decrease in days with at least one stool consistency 6 or 7 by at least 50% over the run-in phase during the 2-week control period at the end of the treatment phase. In addition, for patients in the IBS-M group, an overall assessment of improvement in gut health based on the PGI-I scale is required: • Responder for improvement in gut state of health is defined as a patient who, during the 2-week control period at the end of the treatment phase has an improvement state 1 or 2 (much better or better) according to the PGI-I scale |
Die Wirksamkeit wird durch ein Responderkriterium bewertet, das für Patienten mit IBS-D und IBS-M unterschiedlich vorgegeben wird. Für beide Subgruppen wird eine Response des abdominellen Schmerzes verlangt (NRS Schmerz): • Ein Responder ist definiert als Patient, der in der 2-wöchigen Kontrollperiode am Ende der Behandlungsphase eine Abnahme der abdominellen Schmerzen um wenigstens 30% vom Wert in der 2- wöchigen Run-In Phase vor Randomisierung aufweist. Für Patienten der Gruppe IBS-D wird zusätzlich eine Bewertung der Stuhlkonsistenz verlangt (Bristol-Stuhlformen-Skala): • Ein Responder bzgl. der Stuhlkonsistenz ist definiert als Patient, der in der 2-wöchigen Kontrollperiode am Ende der Behandlungsphase eine Abnahme der Tage mit wenigstens einem Stuhl der Konsistenz 6 oder 7 um wenigstens 50% gegenüber der Run-In Phase aufweist. Für Patienten der Gruppe IBS-M wird zusätzlich eine Gesamtbeurteilung der Änderung des Gesundheitszustands des Darms anhand der PGIISkala verlangt: Ein Responder bzgl. der Änderung des Gesundheitszustands des Darms ist definiert als Patient, der in der 2-wöchigen Kontrollperiode am Ende der Behandlungsphase eine Zustandsänderung der Stufe 1 oder 2 (sehr viel besseren Zustand oder viel besseren Zustand) der PGI-I-Skala aufweist. |
