E.3 | Principal inclusion criteria |
1. Subject (or legally authorized representative) voluntarily agrees to participate by giving written Informed Consent (and Assent as applicable) in accordance with ICH GCP guidelines and applicable local regulations.
2. Subject agrees to abide by all protocol required procedures including study related assessments, and management by the treating institution for the duration of the study including long term follow-up.
3. Age ≥16 and ≤75 years at the time the Pre-screening Informed Consent/Assent is signed.
4. Diagnosis of advanced (metastatic or inoperable) synovial sarcoma or myxoid liposarcoma / myxoid round cell liposarcoma confirmed cytogenetics. Inoperable refers to a tumor lesion in which clear surgical excision margins cannot be obtained without leading to significant functional compromise.
a. For Synovial Sarcoma: confirmation by the presence of a translocation between SYT on the X chromosome and SSX1, SSX2 or, SSX4 on chromosome 18 (may be presented in the pathology report as t (X; 18)).
b. For MRCLS: confirmation by the presence of the reciprocal chromosomal translocation t(12;16)(q13;p11) or t(12; 22) (q13;q12)
5. Must have previously received either an anthracycline or ifosfamide containing regimen. Subjects who are intolerant to both anthracycline and ifosfamide must have previously received at least one systemic therapy.
6. Measurable disease according to RECIST v1.1.
7. HLA-A*02 positive via Adaptimmune designated central laboratory testing.
8. Tumor (either an archival specimen or a fresh biopsy) shows MAGE-A4 expression of ≥2+ staining in ≥30% of the cells by immunohistochemistry. All samples must have been pathologically reviewed by an Adaptimmune designated central laboratory confirming expression.
9. Eastern Cooperative Oncology Group (ECOG) Performance Status of 0 or 1.
10. Left ventricular ejection fraction (LVEF) ≥40%.
11. Fit for leukapheresis and adequate venous access can be established for the cell collection.
12. Female subjects of childbearing potential (FCBP) must have a negative urine or serum pregnancy test AND must agree to use an effective method of contraception starting at the first dose of chemotherapy and continuing for at least 12 months, or 4 months after the gene modified cells are no longer detected in the blood, whichever is longer.
− OR Male subjects must be surgically sterile or agree to use a double barrier contraception method or abstain from heterosexual activity with a FCBP starting at the first dose of chemotherapy and continuing for 4 months thereafter (or longer if indicated in the country specific monograph/label for cyclophosphamide).
13. Must have adequate organ function as indicated by the laboratory values in the table (refer to protocol) |

3. Âge ≥ 16 et < 75 ans au moment où le consentement éclairé/l’accord de la pré-sélection est donné et signé.
4. Diagnostic de sarcome synovial avancé (métastatique ou inopérable) ouliposarcome myxoïde/liposarcome myxoïde à cellules rondes confirmé par les données cytogénétiques. Inopérable signifie qu’il est impossible d’effectuer une résection chirurgicale de la lésion tumorale avec des marges saines sans compromettre le fonctionnement de façon notable.
a. Pour le sarcome synovial : confirmation par la présence de translocation entre le gène SYT sur le chromosome X et les gènes SSX1, SSX2 ou SSX4 sur le chromosome 18 (peut être présenté dans le rapport pathologique comme t [X, 18]).
b. Pour le LSMCR : confirmation par la présence d’une translocation chromosomique réciproque t(12;16)(q13;p11) ou t(12; 22) (q13;q12)
5. Avoir reçu précédemment un protocole de traitement à base d’anthracycline ou d’ifosfamide. Les patients intolérants à la fois à l’anthracycline et à l’ifosfamide doivent avoir reçu précédemment au moins un traitement systémique.
6. Maladie mesurable selon les critères RECIST v1.1.
7. Sérotype positif au HLA-A*02 selon les tests du laboratoire central Adaptimmune désigné.
8. La tumeur (soit un échantillon archivé, soit une biopsie fraiche) présente une expression de l’antigène MAGE-A4 de coloration ≥ 2+ dans plus de 30 % des cellules grâce à l’immunohistochimie. Tous les échantillons doivent avoir subi l’examen pathologique d’un laboratoire central Adaptimmune désigné pour confirmer l’expression.
9. Indice de performance ECOG (Eastern Cooperative Oncology Group) de 0 sur 1.
10. Fraction d’éjection du ventricule gauche (FEVG) ≥ 40 %.
11. Des accès veineux adéquats et appropriés pour la leucaphérèse peuvent être établis pour le recueil des lymphocytes.
12. Les patientes en âge de procréer doivent obtenir un test de grossesse sanguin ou urinaire négatif ET doivent également accepter d’utiliser une méthode efficace de contraception à partir de la première dose de chimiothérapie et pendant au moins 12 mois, ou 4 mois après que les cellules modifiées génétiquement ne sont plus détectables dans le sang, selon l’échéance la plus longue.
OU
Les patients hommes doivent avoir fait l’objet d’une stérilisation chirurgicale, ou accepter d’utiliser une méthode contraceptive à double barrière, ou encore pratiquer l’abstinence hétérosexuelle avec une partenaire féminine en âge de procréer, à partir de la première dose de chimiothérapie et pendant 4 mois (ou plus longtemps si la législation en vigueur requiert d’autres mesures de précaution concernant le cyclophosphamide).
13. Les patients doivent présenter une fonction organique adéquate comme indiqué par les valeurs biologiques du tableau (se référer au protocole)

E.4 | Principal exclusion criteria |
1. Any of the following HLA-A genotypes via Adaptimmune designated central laboratory testing:
• HLA-A*02:05 in either allele; HLA-A*02 alleles having the same protein sequence as HLA-A*02:05 in the antigen binding domains will also be excluded;
• HLA-A*02:07 (and alleles having the same protein sequence in the antigen binding domains as A*02:07) or any A*02 null allele (designated with an “N” suffix, e.g. A*02:32N) as the sole HLA A*02 allele (e.g. a subject with HLA alleles A*02:04 and A*02:07 is eligible).
2. Received or plans to receive therapy/treatment prior to leukaphereseis or lymphodepleting chemotherapy (refer to table in the protocol)
3. Toxicity from previous anti-cancer therapy must have recovered to ≤ Grade 1 prior to enrollment (except for non-clinically significant toxicities, e.g., alopecia, vitiligo). Subjects with Grade 2 toxicities that are deemed stable or irreversible (e.g. peripheral neuropathy) can be enrolled.
4. History of allergic reactions attributed to compounds of similar chemical or biologic composition to fludarabine, cyclophosphamide or other agents used in the study.
5. History of autoimmune or immune mediated disease. Subjects with hypothyroidism, diabetes, adrenal insufficiency or pituitary insufficiency that are stable on replacement therapy are eligible. Subjects with disorders such as asthma, psoriasis or atopic dermatitis that are well controlled without requiring systemic immunosuppression are also eligible.
6. Leptomeningeal disease, carcinomatous meningitis or symptomatic CNS metastases. Subjects with a prior history of symptomatic CNS metastases must have received treatment (i.e., stereotactic radiosurgery (SRS), whole brain radiation (WBRT) or surgery) and be neurologically stable for at least 1 month, not requiring anti-seizure medications and off of steroids for at least 14 days prior to leukapheresis and lymphodepletion. Antiseizure prophylaxis is permitted. Subjects who have asymptomatic CNS metastases without associated edema, shift, requirement for steroids or anti-seizure medications for the treatment of seizures are eligible.
7. Any other prior malignancy that is not in complete remission. Resectable squamous or basal cell carcinoma of the skin is acceptable. Prior malignancies that have been surgically resected and show no evidence of disease are acceptable.
8. Uncontrolled intercurrent illness including, but not limited to:
• Ongoing or active infection;
• Clinically significant cardiac disease defined by congestive heart failure New York Heart Association (NYHA) Class 3 or Class 4;
• Uncontrolled clinically significant arrhythmia;
• Acute Coronary Syndrome (ACS) (angina or MI) in last 6 months;
• Interstitial lung disease (subjects with existing pneumonitis as a result of radiation are not excluded, however, subjects must not be oxygen dependent);
• Congenital or family history of long QT syndrome;
• Current uncontrolled hypertension despite optimal medical therapy;
• History of stroke or central nervous system bleeding; transient ischemic attack (TIA) or reversible ischemic neurologic deficit (RIND) in last 6 months
9. Active infection with HIV, HBV, HCV or HTLV as defined below:
• Positive serology for HIV;
• Active hepatitis B infection as demonstrated by test for hepatitis B surface antigen. Subjects who are hepatitis B surface antigen negative but are hepatitis B core antibody positive must have undetectable hepatitis B DNA and receive prophylaxis against viral reactivation. Prophylaxis should be initiated prior to lymphodepleting therapy and continued for 6 months;
• Active hepatitis C infection as demonstrated by hepatitis C RNA test. Subjects who are HCV antibody positive will be screened for HCV RNA by any RT PCR or bDNA assay. If HCV antibody is positive, eligibility will be determined based on a negative screening RNA value;
• Positive serology for HTLV 1 or 2;
• Re-screening for infectious disease markers is not required at baseline (prior to lymphodepletion) unless > 6 months has elapsed.
10. Pregnant or breastfeeding.
11. In the opinion of the Investigator, the subject is unlikely to fully comply with protocol requirements. |

Toute toxicité d’un traitement anticancéreux antérieur doit être revenue à un niveau inférieur à 1 avant l’inclusion (à l’exception des toxicités qui ne sont pas cliniquement significatives comme l’alopécie ou le vitiligo, par exemple). Les patients présentant des toxicités de niveau 2 considérées comme stables ou irréversibles (par exemple, une neuropathie périphérique) peuvent être inclus.
4. Antécédents de réactions allergiques attribuées à des composés de composition chimique ou biologique similaire à la fludarabine, la cyclophosphamide ou d’autres agents utilisés dans cette étude.
5. Antécédents de maladie auto-immune ou à médiation immunitaire. Les patients atteints d’hypothyroïdie, de diabète, d’insuffisance surrénalienne ou hypophysaire stabilisés et sous traitement de substitution sont éligibles. Les patients atteints de troubles tels que l’asthme, le psoriasis et la dermatite atopique contrôlés et ne nécessitant pas d’immunosuppression systémique sont également éligibles.
6. Maladie leptoméningée, méningite carcinomateuse ou métastases du SNC symptomatiques. Les patients avec des antécédents de métastases au niveau du SNC doivent avoir reçu un traitement (radiochirurgie stéréotaxique [RCS], radiothérapie cérébrale ou chirurgie, par exemple) et être stabilisés sur le plan neurologique depuis au moins 1 mois, sans nécessiter de traitement anticonvulsivant, et être également en sevrage des corticoïdes 14 jours au moins avant la leucaphérèse et la lymphodéplétion. Une prophylaxie anticonvulsivante est autorisée. Les patients présentant des métastases asymptomatiques au niveau du SNC, sans œdème associé, variation, ou besoin de corticoïdes, ou de traitement anticonvulsivant sont éligibles.
7. Tout cancer antérieur qui ne serait pas en rémission complète. Un carcinome basocellulaire ou épidermoïde résécable de la peau est acceptable. Les tumeurs antérieures réséquées chirurgicalement et ne montrant aucun signe de maladie sont acceptables.
8. Une maladie intercurrente non contrôlée comprenant, sans s’y limiter :
• une infection active ou en cours ;
• une maladie cardiaque cliniquement significative définie comme une insuffisance cardiaque congestive de classe 3 ou 4 selon les critères de la New York Heart Association ;
• une arythmie cliniquement significative non contrôlée ;
• un syndrome coronarien aigu (SCA) (angor ou MI) dans les 6 derniers mois ;
• une pneumopathie interstitielle (les patients présentant une pneumonie résultant des radiations ne sont pas exclus, cependant, ces patients ne doivent pas être dépendants d’apports en oxygène) ;
• Syndrome congénital ou antécédents familiaux de syndrome du QT long ;
• Hypertension actuellement non contrôlée malgré un traitement médical optimal ;
• Antécédents d’AVC ou d’hémorragie du système nerveux central ; accident ischémique transitoire (AIT) ou déficit neurologique ischémique réversible (RIND) au cours des 6 derniers mois ;
9. Une infection active par le VIH, le VHB, le VHC ou le HTLV définie comme suit :
• Une séropositivité au VIH ;
• Une hépatite B active attestée par le test de l’antigène de surface de l’hépatite B. Les patients qui sont négatifs aux antigènes de surface de l’hépatite B mais positifs aux anticorps de l’hépatite B, ne doivent pas avoir d’ADN de l’hépatite B détectable et doivent recevoir une prophylaxie contre des réactions virales. La prophylaxie doit être commencée avant le traitement par lymphodéplétion et continuée pendant 6 mois ;
• Une hépatite C active attestée par le test de l’ARN de l’hépatite C. Les patients qui sont positifs aux anticorps du VHC doivent faire l’objet d’un test de dépistage de l’ARN du VHC, soit le RT PCR, soit le bDNA. Si le patient est positif aux anticorps du VHC, il sera éligible si le résultat du dépistage de l’ARN est négatif.
• Une séropositivité au HTLV 1 ou 2 ;
• Un nouveau dépistage des marqueurs de maladies infectieuses n’est pas nécessaire lors de l’inclusion (avant la lymphodéplétion), sauf si plus de 6 mois se sont écoulés.
10. Femmes enceintes ou qui allaitent.
11. Selon l’investigateur, il est peu probable que le patient se conforme réellement au protocole.
