Inclusion Criteria - Double blind period 1. Male or female =18 years of age. 2. Diagnosed with active primary or secondary wAIHA, defined as having all of the following: a.Hemoglobin concentration <9 g/dL or <10 g/dL if symptomatic or on corticosteroids/immunosuppressants AND b.Signs of hemolysis, defined as: lactate dehydrogenase (LDH) levels above the upper limit of normal (ULN), or haptoglobin below the lower limit of normal, or indirect bilirubin above the ULN AND c.Serological evidence of anti-erythrocyte antibodies associated with a DAT that is either positive for IgG only or is positive for IgG and C3d (fragment of the third component of complement) at screening. 3.Has been diagnosed with wAIHA for at least 3 months, and currently receiving or has previously received treatment for wAIHA (treatment naive patients not eligible) 4.If on corticosteroids, the dose must be stable for at least 14 days prior to randomization. 5.If currently receiving immunosuppressants, treatment must be stable for 30 days prior to Screening. Allowed concomitant immunosuppressants are azathioprine, mycophenolate mofetil/mycophenolic acid, cyclosporine, cyclophosphamide. 6.Have a platelet count =30 × 109/L. 7.Have a negative QuantiFERON®-TB Gold test. 8.Patients who have undergone splenectomy must be at least 3 months post resection prior to screening and must be vaccinated as per the United States Center for Disease Control and Prevention (CDC) annual Recommended Immunization Schedule for Adults Aged 19 Years or Older, United States 9.Patients with other autoimmune disease (eg, systemic lupus erythematosus or rheumatoid arthritis) or lymphoproliferative disorders may be eligible if they are stable (no changes in concomitant disease-related medications and severity of disease for at least 3 months prior to screening). Patients with lymphoproliferative disease must have a low grade, be stable and be, in the opinion of the Investigator, unlikely to require chemotherapy or monoclonal antibody therapy during the study. Patients requiring change of treatment or new treatment (but not rescue therapy for wAIHA) during the study will be terminated from the study 10.Have sufficient venous access to allow drug administration by IV infusion and blood sampling as per the protocol. 11.Women of childbearing potential, defined as women physiologically capable of becoming pregnant, must have a negative serum pregnancy test at screening and a negative urine pregnancy test at Baseline. Menopausal women must have an elevated serum follicle-stimulating hormone (FSH) level at Screening; if the FSH is not elevated, they are considered to be of childbearing potential and must have a negative serum pregnancy test at screening and a negative urine pregnancy test at Baseline to be eligible. 12.Women of childbearing potential (including menopausal women who do not have elevated FSH) must agree to remain totally abstinent (ie, refrain from sexual intercourse during the study) or to consistently use a reliable and highly effective method of contraception (eg, condom plus diaphragm, condom plus spermicide, diaphragm plus spermicide, or intrauterine device or oral/injectable/implanted hormonal contraceptive used in combination with an additional barrier method) during the study and for 30 days after the last study treatment. Inclusion criteria-Open-Label Extension 1.Have completed the double-blind period (through Week 24), or have required rescue therapy after Week 4 of the double-blind period, or failed to demonstrate an increase from baseline in Hgb of at least 1 g/dL at or after Week 16 of the double-blind period. 2.Are able to understand and voluntarily provide written informed consent to participate in the OLE period and comply with all study procedures. Inclusion Criteria #6, #9, #11, #12, and #13 from the Double-blind Period (Protocol Section 4.2.1) also apply to enrollment in the OLE |

In caso di trattamento con corticosteroidi, la dose deve essere stabile per almeno 14 giorni prima della randomizzazione. 5. In caso di trattamento con immunosoppressori, il trattamento deve essere stabile per 30 giorni prima dello screening. I farmaci immunosoppressori concomitanti consentiti sono azatioprina, micofenolato mofetile/acido micofenolico, ciclosporina, ciclofosfamide. 6. Conta piastrinica =30 x 10P9P/l. 7. Risultato negativo del test QuantiFERONP®P-TB Gold. 8. I pazienti splenectomizzati devono aver superato almeno 3 mesi dalla resezione prima dello screening ed essere vaccinati secondo il Programma annuale di immunizzazione raccomandato dal Centro nazionale per il controllo e la prevenzione delle malattie (CDC) degli Stati Uniti per gli adulti di età uguale o superiore a 19 anni, Stati Uniti. 9. I pazienti con un’altra malattia autoimmune (es. lupus eritematoso sistemico o artrite reumatoide) o disturbi linfoproliferativi possono essere eleggibili se stabili (nessuna modifica dei farmaci concomitanti correlati alla malattia e variazione della gravità della malattia da almeno 3 mesi prima dello screening). I pazienti con sindrome linfoproliferativa devono presentare malattia a basso grado, essere stabili e, secondo il parere dello sperimentatore, deve essere improbabile nel corso dello studio la necessità di chemioterapia o terapia con anticorpi monoclonali. I pazienti che richiedono una modifica del trattamento o un nuovo trattamento (ma non la terapia di salvataggio per wAIHA) durante lo studio lo interromperanno. 10. Presentare un accesso venoso sufficiente a consentire la somministrazione del farmaco mediante infusione EV e i prelievi dei campioni di sangue secondo il protocollo. 11. Criteri di inclusione, estensione in aperto 1. Hanno completato il periodo in doppio cieco (fino alla Settimana 24) o hanno avuto la necessità di una terapia di salvataggio dopo la Settimana 4 del periodo in doppio cieco o non sono riusciti a dimostrare un aumento dalla baseline dell’Hgb di almeno 1 g/dl alla o dopo la Settimana 16 del periodo in doppio cieco. 2. Sono in grado di comprendere e fornire volontariamente un consenso informato scritto per partecipare al periodo OLE e rispettare tutte le procedure dello studio. Exclusion Criteria - Double blind period 1. Are currently taking IgG Fc-related protein therapeutics 2.Have received a transfusion within 30 days prior to randomization. 3.Have any other associated cause of hereditary or acquired hemolytic anemia. 4.Have received rituximab within 3 months prior to screening. 5.Have received IVIg within 6 weeks prior to screening. 6.Have cold antibody AIHA, cold agglutinin syndrome, mixed type (ie, warm and cold) AIHA, or paroxysmal cold hemoglobinuria. 7.Have a severe infection (eg, pneumonia, biliary tract infection, diverticulitis, Clostridium difficile infection) that requires parenteral anti-infectives and/or hospitalization, and/or is assessed as serious/clinically significant by the Investigator, within 8 weeks prior to screening. Any patient with an infection requiring oral antiinfectives (eg, sinusitis, bronchitis, uncomplicated urinary tract infection) within 4 weeks prior to screening will be excluded. 8.Have a chronic infection (eg, bronchiectasis, chronic osteomyelitis, chronic pyelonephritis) or require chronic treatment with anti-infectives (eg, antibiotics, antivirals). 9.Have received a live vaccine within 3 months prior to screening, or have a known need to receive a live vaccine during the study or within at least 3 months after the last dose of study drug. 10. Have any confirmed or suspected clinical immunodeficiency syndrome not related to treatment of his/her wAIHA, or has a family history of congenital or hereditary immunodeficiency unless confirmed absent in the patient. 11.Have a known history of, or positive test result for human immunodeficiency virus-1 (HIV 1) and HIV 2 antibodies, hepatitis B virus (HBV core antigen), or hepatitis C virus (HCV) at screening. Note: Patients with past HCV may be included in the study if they have a documented negative HCV ribonucleic acid level in the serum at 12 weeks or longer after the completion of HCV therapy and at screening. Patients with spontaneous resolution of HCV (as confirmed by specialist) may be included in the study if they demonstrate a negative serum HCV ribonucleic acid level 12.Are currently breastfeeding, pregnant, intend to become pregnant during the study, or are planning egg donation during the study or within 30 days after the last dose of study drug. 13.Have current alcohol/substance abuse/dependence, a history of alcohol/substance abuse/dependence within the 12 months prior to screening, or, in the Investigator's opinion, show evidence of ongoing alcohol/substance abuse/dependence. 14.Are currently participating in another interventional clinical trial or have received any investigational drug within the past 3 months prior to screening. 15.Have had any major surgery within 3 months prior to screening or have plans for or have been scheduled for any elective surgery or major dental procedure during the study. 16.Have a history of a major organ transplant (eg, heart, lung, kidney, liver), or hematopoietic stem cell/marrow transplant. Exclusion Criteria - Opel Label Extension 1. Met any of the stopping criteria (see Section 6.4.1 of the protocol) or discontinued study drug during the double-blind period due to treatment-related AE. 2. Currently have a serious or clinically significant infection (eg, pneumonia, biliary tract infection, diverticulitis, C. difficile infection) requiring parenteral anti-infectives and/or hospitalization. The following exclusion criteria from the Double-blind Period (Protocol Section 4.2.2) also apply to enrollment in the OLE: Exclusion Criteria #8 and #19. |

Sono affetti da AIHA da anticorpi freddi, sindrome da agglutinine a frigore, AIHA di tipo misto (ovvero calda e fredda) o emoglobinuria parossistica a frigore. 7. Hanno un’infezione grave (ad esempio polmonite, infezione delle vie biliari, diverticolite, infezione da Clostridium difficile) che richiede la somministrazione di antinfettivi per via parenterale e/o il ricovero ospedaliero e/o è valutata come seria/clinicamente significativa dallo sperimentatore, entro 8 settimane prima dello screening. Qualsiasi paziente con un'infezione che richiede la somministrazione di antinfettivi per via orale (ad esempio sinusite, bronchite, infezione non complicata delle vie urinarie) entro 4 settimane prima dello screening sarà escluso. 8. Hanno un'infezione cronica (ad esempio bronchiectasia, osteomielite cronica, pielonefrite cronica) o necessitano di un trattamento cronico con antinfettivi (come antibiotici e antivirali). 9. Hanno ricevuto un vaccino vivo nei 3 mesi precedenti lo screening o hanno la necessità nota di ricevere un vaccino vivo durante lo studio o entro almeno 3 mesi dall'ultima dose di farmaco in studio. 10. Presentano sindrome da immunodeficienza clinica confermata o sospetta non correlata al trattamento della wAIHA, o presentano anamnesi familiare di immunodeficienza congenita o ereditaria, a meno che non ne sia confermata l’assenza nel paziente. 11. Presentano anamnesi nota o un risultato positivo ai test per il virus dell'immunodeficienza umana-1 (HIV) 1 e anticorpi HIV 2, virus dell'epatite B o virus dell'epatite C (HCV) allo screening. Nota: I pazienti con pregressa infezione da HCV possono essere inclusi nello studio se presentano un livello sierico di acido ribonucleico del virus HCV negativo documentato a 12 o più settimane successive al completamento della terapia per HCV e allo screening. I pazienti con risoluzione spontanea del HCV (confermata dallo specialista) possono essere inclusi nello studio se dimostrano un livello sierico di acido ribonucleico del virus HCV negativo. 12. Attualmente allattano al seno, sono in gravidanza, intendono iniziare una gravidanza durante lo studio o pianificano di donare gli ovuli durante lo studio o entro 30 giorni dall'ultima dose di farmaco in studio. 13. Presentano un’attuale dipendenza/abuso di alcol/sostanze stupefacenti, anamnesi di dipendenza/abuso di alcol/sostanze stupefacenti nei 12 mesi precedenti lo screening o, secondo il parere dello sperimentatore, mostrano evidenza di dipendenza/abuso di alcol/sostanze stupefacenti in corso. 14. Partecipano attualmente a un'altra sperimentazione clinica interventistica o hanno ricevuto un farmaco sperimentale negli ultimi 3 mesi prima dello screening. E.5.1 | Primary end point(s) |
The primary efficacy endpoint is durable response in improvement of Hgb, defined as the attainment of the following at 3 consecutive visits, where at least the first is at or before Week 16, without the need of rescue therapy: • Hgb concentration =10 g/dL AND • An increase from baseline in Hgb =2 g/dL The first of the three consecutive visits must be at or before week 16 in order to qualify for success in the primary efficacy endpoint. For example, if a patient attained the above criteria at weeks 18, 20, and 22 and not before, this patient would not be considered a success. If the patient attained the criteria at weeks 16, 18, and 20, the patient would be considered a success. In case 1 of the 3 consecutive Hgb observations is missing and the other 2 met the above criteria, the next non-missing Hgb observation must also meet the criteria in order for the patient to qualify as a success in the primary efficacy endpoint. No more than 1 missing observations is allowed. For example, if a patient met the criteria at weeks 14, 16, and 20, and with a missing value at week 18, this patient would be considered a success in the primary efficacy endpoint. If the patient met the criteria at weeks 14, 18, and 22, but with missing data at weeks 16 and 20, the patient would not be considered a success. Patients who took rescue therapy before reaching the criteria will be treated as having failed the primary efficacy outcome. If a patient has missing Hgb at both screening and baseline but is randomized by mistake, the patient will be excluded from the ITT analysis. This situation is unlikely to happen because the IVRS system requires screening Hgb for randomization to occur. |

Key secondary endpoints: -Change in the level of patient fatigue. Change from baseline in the total score from the FACIT-Fatigue Scale at the time of durable response or the end of the double-blind period. -Simultaneous attainment of normal LDH (below the ULN), AND haptoglobin (above the LLN), AND indirect bilirubin levels (below the ULN) at a minimum of 3 consecutive visits after baseline. -Any reduction in corticosteroid exposure. A patient is considered as having any reduction in corticosteroid exposure if each of the 3 following criteria are met. If a patient does not meet all of the below criteria, the patient will be considered as having no reduction in corticosteroid exposure: 1.Achieved durable response per the primary efficacy endpoint, AND 2.The corticosteroid exposure in term of average daily dose was reduced based on the Investigator's prescription after achieving durable response per the primary efficacy endpoint, AND 3.After corticosteroid dose reduction, the corticosteroid exposure in terms of average daily dose stays below the baseline level. Si ritiene che un paziente presenti una riduzione dell’esposizione a corticosteroidi se viene soddisfatto ciascuno dei seguenti 3 criteri. Se un paziente non soffisfa tutti i criteri indicate di seguito, si riterrà che il paziente non presenti riduzione dell’esposizione a corticosteroidi: 1. è stata ottenuta una risposta duratura secondo l’endpoint primario di efficacia, E 2. L’esposizione ai corticosteroidi in termini di dose giornaliera media è stata ridotta in base alla prescrizione dello sperimentatore dopo aver ottenuto una risposta duratura secondo l’endpoint primario di efficacia, E 3. Dopo la riduzione della dose di corticosteroidi, l’esposizione ai corticosteroidi in termini di dose giornaliera media rimane al di sotto del livello basale. Altri Endpoint Secondari: -Variazioni nell'uso di corticosteroidi orali: 1. Percentuale complessiva di pazienti che ottengono una riduzione della dose giornaliera media di corticosteroidi mantenendo la risposta dell’Hgb 2.Riduzione percentuale della dose media giornaliera di corticosteroidi in ciascun gruppo tra la baseline e la fine del periodo in doppio cieco - Associazione tra riduzione delle IgG e i parametri di Hgb/emolisi: Tasso di Hgb allo stato stazionario (stimato utilizzando un’analisi longitudinale basata su modello dei parametri di Hgb/emolisi in relazione al livello di IgG e al regime posologico) -Normalizzazione dei parametri ematologici ed emolitici: Concentrazione di emoglobina, conta dei reticolociti, marcatori emolitici e variazione rispetto alla baseline di questi parametri fino alla Settimana 16 del periodo in doppio cieco Concentrazione di emoglobina, conta dei reticolociti, marcatori emolitici e variazione rispetto alla baseline durante lo studio -Normalizzazione dei marcatori emolitici 1. The first of the three consecutive visits must be at or before week 16 in order to qualify for success in the primary efficacy endpoint. In case 1 of the 3 consecutive Hgb observations is missing and the other 2 met the above criteria, the next non-missing Hgb observation must also meet the criteria in order for the patient to qualify as a success in the primary efficacy endpoint. No more than 1 missing observations is allowed. For example, if a patient met the criteria at weeks 14, 16, and 20, and with a missing value at week 18, this patient would be considered a success in the primary efficacy endpoint. If the patient met the criteria at weeks 14, 18, and 22, but with missing data at weeks 16 and 20, the patient would not be considered a success. |
La prima delle tre visite consecutive deve essere effettuata alla settimana 16 o prima per poter essere considerato un successo nell'endpoint primario di efficacia.
