1. Written informed consent prior to any study procedures (outcomes of standard-of-care procedures performed before signing of informed consent by the patient but within the allowed screening period can be used for patient screening). 2. Female. 3. ≥ 18 years of age. 4a. EITHER: (Post)menopausal status at the time of initiation of adjuvant study medication • patient underwent bilateral oophorectomy, or • age ≥ 60, or • age < 60 and amenorrhea for 12 or more months (in the absence of chemotherapy, tamoxifen, or ovarian suppression) and/or FSH and estradiol in the postmenopausal range per local normal range. 4b. OR: Pre-menopausal patients: • confirmed negative serum or urine pregnancy test (β-hCG) before starting study treatment, or • patient has had a hysterectomy. 5. Histologically confirmed diagnosis of primary estrogen-receptor positive and/or progesterone-receptor positive (>1%) early breast cancer by local laboratory. In case the receptor status from local pathology is unclear a central pathology review is obligatory. Results must be known prior to randomization. 6. Patient has HER2-negative breast cancer defined as • a negative in-situ hybridization test or an IHC status of 0, 1+, or 2+, • if IHC is 2+, a negative in-situ hybridization (FISH, CISH, or SISH) test is required (based on the analyzed tissue sample at initial diagnosis by a local laboratory). 7. Patients are eligible • with completed (i.e., 5 years according to SoC), planned or ongoing adjuvant endocrine therapy, without any signs of distant relapse or secondary malignancy AND • if primary diagnosis was 6 years or less before enrollment 8a. Known high clinical risk, in case of neoadjuvant chemotherapy, defined as either one of the following criteria: see protocol Known high clinical risk, in case of adjuvant treatment (chemotherapy and/or ET), defined as either one of the following criteria: see protocol OR 8b. Patient has available tumor tissue from primary diagnostic biopsy. 12. No contraindication for adjuvant ET. 13. Eastern Cooperative Oncology Group (ECOG) performance status 0-1. 14. Patient has adequate bone marrow and organ function as defined by the following laboratory values: see protocol 15. Ability to swallow abemaciclib tablets or to administer other study medication, respectively. 16. Ability to communicate with the investigator and comply with study procedures. 17. 1. Weiblich. 3. ≥ 18 Jahre alt. 4a. ENTWEDER: (Post)menopausaler Status zum Zeitpunkt des Beginns der adjuvanten Studienbehandlung: • die Patientin weist eine beidseitige Adnektomie oder • ein Alter ≥ 60 Jahre oder • ein Alter < 60 Jahre mit Amenorrhoe über 12 oder mehr Monate (in Abwesenheit von Chemotherapie, Tamoxifen oder der ovarieller Suppressionstherapie) und/oder FSH bzw. Estradiol im postmenopausalen Bereich gemäß der gültigen Normalwerte, auf. 4b. ODER: Prämenopausale Patientinnen mit: • bestätigtem, negativem Serum-Schwangerschaftstest (β-hCG) vor Start der Studienbehandlung oder • bestätigter Hysterektomie. 5. Histologisch bestätigte Diagnose eines primären Östrogen-Rezeptor positiven und/oder Progesteron-Rezeptor positiven (>1%) frühen Mammakarzinoms gemäß lokaler Laborwerte. Wenn der Rezeptorstatus aus der lokalen Pathologie unklar ist, ist eine Überprüfung durch die zentrale Pathologie obligatorisch. Die Ergebnisse müssen vor der Randomisierung bekannt sein. 6. Die Patientin hat ein HER2-negatives Mammakarzinom, definiert als • einen negativen in-situ Hybridisationstest oder einen IHC Status von 0, 1+, oder 2+, • bei IHC 2+ ist ein negativer in-situ Hybridisationstest (FISH, CISH, oder SISH) nötig (basierend auf den jüngsten analysierten Gewebeproben, gemessen durch das lokale Labor). 7. Patientinnen können eingeschlossen werden • mit abgeschlossener (d. h. 5 Jahre entsprechend Standardbehandlung), geplanter oder laufender adjuvanter endokriner Therapie ohne Anzeichen eines Fernrezidivs sowie einer sekundären Malignität UND • falls die Primärdiagnose 6 Jahre oder weniger vor Studieneinschluss stattfand. 8a. Bekanntes hohes klinisches Risiko, im Falle neoadjuvanter Chemotherapie definiert als eines der folgenden Kriterien: siehe Protokoll Bekanntes hohes klinisches Risiko, im Falle adjuvanter Behandlung (Chemotherapie und/oder ET) definiert als eines der folgenden Kriterien: siehe Protokoll ODER 8b. Kein Anhalt für Fernmetastasierung (bestätigt vorzugsweise durch Thorax- bzw. Abdomen-CT, Thorax-Röntgen, Sonografie der Leber, Knochenszintigrafie oder PET-CT) 11. Es steht Tumorgewebe aus der diagnostischen Biopsie zur Verfügung 12. Keine Kontraindikation für adjuvante ET 13. “Eastern Cooperative Oncology Group (ECOG) performance status” 0 oder 1 14. E.4 | Principal exclusion criteria |
Patients eligible for inclusion in this study must not meet any of the following criteria: 1. Patient with distant metastases of breast cancer beyond regional lymph nodes. 2. Previously received CDK 4/6 inhibitor or patient with an indication for abemaciclib in the clinical routine per respective country: • N 2-3 or • N 1 and at least one of the following criteria: G3 or T3 and • <14 months after primary diagnosis 3. Patient with a known hypersensitivity to any of the excipients of abemaciclib or standard-of-care endocrine therapy. 4. Patient has had major surgery within 14 days prior to starting study drug or has not recovered from major side effects. 5. Patient has not recovered from clinical and laboratory acute toxicities related to prior anticancer therapies to NCI CTCAE version 5.0 Grade ≤ 1 (polyneuropathy ≤ 2 or residual alopecia is allowed). 6. Patient has a concurrent malignancy or non-breast malignancy within 5 years prior to randomization. 7. Patient has impairment of gastrointestinal (GI) function or GI disease that may significantly alter the absorption of the study drugs (e.g., uncontrolled ulcerative diseases, uncontrolled nausea, vomiting, diarrhea, malabsorption syndrome, or small-bowel resection). 8. Patient has any active systemic bacterial infection (requiring intravenous antibiotics at time of initiating study treatment), fungal infection, or detectable viral infection (such as known human immunodeficiency virus positivity or with known active hepatitis B or C [for example, hepatitis B surface antigen positive]. Screening is not required for enrollment. 9. Patient has any other concurrent severe and/or uncontrolled medical condition that, in the investigator´s judgment, could cause unacceptable safety risks, contraindicate patient participation in the clinical study, or compromise compliance with the protocol (e.g., interstitial lung disease, severe dyspnea at rest or requiring oxygen therapy, severe renal impairment [e.g. estimated creatinine clearance <30ml/min], history of major surgical resection involving the stomach or small bowel, or preexisting Crohn’s disease or ulcerative colitis or a preexisting chronic condition resulting in baseline Grade 2 or higher diarrhea, etc.). 10. Patient has a personal history of any of the following conditions: syncope of cardiovascular etiology, ventricular arrhythmia of pathological origin (including, but not limited to, ventricular tachycardia and ventricular fibrillation), or sudden cardiac arrest. 11. Patient is currently receiving any of the following substances, which cannot be discontinued 7 days prior to day 1 of study treatment: - concomitant medications and herbal supplements, that are strong inducers or inhibitors of CYP3A4. 12. Participation in a prior investigational study within 30 days prior to enrollment.. 13. Not able to understand and to comply with study instructions and requirements. 14. Pregnant or nursing (lactating) woman. 15. Woman of child-bearing potential defined as woman physiologically capable of becoming pregnant, unless she is using highly effective methods of contraception during the study treatment and for 21 days after stopping the treatment: a. total abstinence (when this is in line with the preferred and usual lifestyle of the patient), b. female sterilization (have had surgical bilateral oophorectomy with or without hysterectomy), total hysterectomy, or tubal ligation at least 6 weeks before taking study treatment, c. male partner sterilization (at least 6 months prior to study screening). For female patients on the study, the vasectomized male partner should be the sole partner for that patient, d. placement of a non-hormonal intrauterine device (IUD). e. Use of condom + spermicide. 16. Use of oral (estrogen and progesterone), transdermal, injected, or implanted hormonal methods of contraception as well as hormone replacement therapy. |
Um in die Studie eingeschlossen zu werden, darf keines der folgenden Kriterien erfüllt werden: 1. Die Patientin hat Fernmetastasen des Mammakarzinoms. 2. Die Patientin wurde bereits mit einem CDK4/6-Inhibitor behandelt oder hat eine Indikation für Abemaciclib in der klinischen Routine im entsprechenden Land: • N 2-3 oder • N1 und mindestens eines der folgenden Kriterien: G3 oder T3 und • <14 Monate nach der Primärdiagnose. 3. Die Patientin hat eine bekannte Überempfindlichkeit gegen irgendeine der sonstigen Substanzen von Abemaciclib oder der Standard-ET. 4. Die Patientin hatte eine große Operation innerhalb von 14 Tagen vor Beginn der Studientherapie oder hat sich von schweren Nebenwirkungen noch nicht erholt. 5. Die Patientin hat sich noch nicht von klinischen oder Laborwert-basierten akuten Toxizitäten erholt, die mit vorherigen Antikrebstherapien in Verbindung standen und aktuell den Status Grad≤ 1 des NCI-CTCAE, Version 5.0, Katalogs erfüllen (Polyneuropathie ≤ 2 und anhaltende Alopezie erlaubt). 6. Die Patientin hat gleichzeitig oder hatte innerhalb der letzten 5 Jahre vor Randomisierung eine maligne Erkrankung (kein Brustkrebs). 7. Die Patientin hat eine Störung der gastrointestinalen (GI) Funktion oder eine GI-Erkrankung, die signifikant die Absorption des Studienmedikaments verändert (z.B. unkontrollierte Ulkuserkrankung, unkontrollierte Übelkeit, Erbrechen, Diarrhoe, Malabsorptions-Syndrom oder Dünndarmresektion). 8. Die Patientin hat eine aktive systemische bakterielle Infektion (die zum Zeitpunkt des Beginns der Studienbehandlung intravenöse Antibiotika erfordert), eine Pilzinfektion oder eine nachweisbare Virusinfektion (z. B. bekannte Human-Immunschwäche-Virus-Positivität oder bekannte aktive Hepatitis B oder C [z. B. Hepatitis-B-Oberflächenantigen-positiv]. Ein Screening ist für die Rekrutierung nicht erforderlich. 9. Die Patientin hat irgendeine andere schwere oder unkontrollierte Begleiterkrankung, die nach Beurteilung des Prüfarztes inakzeptable Sicherheitsrisiken birgt, eine Gegenanzeige zur Studienteilnahme darstellt oder der Bereitschaft, das Studienprotokoll zu befolgen, entgegensteht (z.B. interstitielle Lungenerkrankung, schwere Dyspnoe in Ruhe oder mit Sauerstofftherapie, schwere Nierenfunktionsstörung [z.B. geschätzte Kreatinin-Clearance <30 ml/min], Vorgeschichte einer größeren chirurgischen Resektion mit Beteiligung des Magens oder Dünndarms, oder vorbestehender Morbus Crohn oder Colitis ulcerosa oder ein vorbestehender chronischer Zustand, der zu einer Diarrhöe des Ausgangsgrades 2 oder höher führt, usw.). 10. Die Patientin hat eine persönliche Vorgeschichte mit einer der folgenden Erkrankungen: Synkope mit kardiovaskulärer Ätiologie, ventrikuläre Arrhythmie pathologischen Ursprungs (einschließlich, aber nicht beschränkt auf ventrikuläre Tachykardie und Kammerflimmern) oder plötzlicher Herzstillstand. 11. Die Patientin erhält aktuell irgendeine der folgenden Substanzen, die nicht 7 Tage vor Beginn des 1. Zyklus abgesetzt werden können: - Begleitmedikation und pflanzliche Präparate, die starke Induktoren oder Inhibitoren des CYP3A4/5 sind. 12. Teilnahme an einer klinischen Studie innerhalb von 30 Tagen vor Registrierung in die Studie. 13. Die Patientin ist nicht in der Lage, Studienanforderungen oder Studienanweisungen zu verstehen oder zu befolgen. 14. Die Patientin ist schwanger oder stillt. 15. Frauen im gebärfähigen Alter, definiert als Frauen, die physiologisch in der Lage wären, schwanger zu werden, es sei denn, sie nutzen eine hocheffektive Methode der Empfängnisverhütung während der Studienbehandlung und für 21 Tage darüber hinaus: a. totale Abstinenz (wenn dies dem bevorzugten und gewöhnlichen Lebensstil der Patientin entspricht), b. weibliche Sterilisation (stattgehabte chirurgische, beidseitige Adnektomie mit oder ohne Hysterektomie), totale Hysterektomie oder Tubenligatur mindestens 6 Wochen vor Beginn der Studienbehandlung, c. Sterilisation des männlichen Partners (mindestens 6 Monate vor Screening-Beginn, wobei der vasektomierte Partner der einzige Partner der Patientin sein sollte, d. Einsatz einer Intrauterin-Spirale (IUD). e. Verwendung von Kondom mit Spermiziden. 16.
