1. Patient’s signed informed consent 2. Age ≥ 18 years at time of signing Informed Consent Form 3. Ability to comply with the study protocol, according to investigator's judgement 4. Life expectancy of at least 12 weeks 5. HCC with histologically confirmed diagnosis 6. Disease that is not amenable to curative surgical and/or local ablation treatment according to consensus resolution of the multidisciplinary tumor board of the trial center but eligible for TACE, with tumor burden below 50% of liver volume. 7. At least one measurable (per RECIST 1.1) untreated lesion 8. ECOG Performance Status of 0 or 1 9. Child-Pugh class A or B7 10. Adequate hematologic and end-organ function, defined by the following laboratory test results, obtained within 14 days prior to randomization, unless otherwise specified: – ANC ≥1.5 x 109/L (1500/µL) without granulocyte colony-stimulating factor support – Lymphocyte count ≥ 0.5 x 109/L (500/µL) – Platelet count ≥ 75 x109/L (75,000/µL) without transfusion – Hemoglobin ≥ 90 g/L (9 g/dL); patients may be transfused to meet this criterion. – AST, ALT, and alkaline phosphatase ≤ 5 x upper limit of normal (ULN) – Serum bilirubin ≤ 3 x ULN – Serum creatinine ≤ 1.5 x ULN or creatinine clearance ≥ 50 mL/min (calculated using the Cockcroft-Gault formula) – Serum albumin ≥ 28 g/L (2.8 g/dL) – For patients not receiving therapeutic anticoagulation: INR <1.25 – Urine dipstick for proteinuria < 2+ (within 14 days prior to initiation of study treatment), unless a subsequent 24-hour urine collection demonstrates < 1 g of protein in 24 hours. 11. Negative HIV test at screening 12. Documented virology status of hepatitis, as confirmed by screening HBV and HCV serology test. For patients with active hepatitis B virus (HBV): HBV DNA < 500 IU/mL obtained within 28 days prior to randomization, and Anti-HBV treatment (per local standard of care) for a minimum of 14 days prior to randomization and willingness to continue treatment for the length of the study 13. For females of childbearing potential (FCBP): negative pregnancy test within 14 days before randomization and agreement to remain abstinent (refrain from heterosexual intercourse) or use contraceptive methods with a failure rate of <1% per year during the treatment period and for at least 5 months after the last dose of atezolizumab or 6 months after the last dose of bevacizumab. A woman is considered to be of childbearing potential if she is postmenarcheal, has not reached a postmenopausal state (≥ 12 continuous months of amenorrhea with no identified cause other than menopause), and has not undergone surgical sterilization (removal of ovaries and/or uterus). Examples of contraceptive methods with a failure rate of < 1% per year include bilateral tubal ligation, male partner’s sterilization, hormonal contraceptives that inhibit ovulation, hormone-releasing intrauterine devices, and copper intrauterine devices. The reliability of sexual abstinence should be evaluated in relation to the duration of the clinical trial and the preferred and usual lifestyle of the patient. Periodic abstinence (e.g., calendar, ovulation, symptothermal, or postovulation methods) and withdrawal are not acceptable methods of contraception. 14. For men: agreement to remain abstinent (refrain from heterosexual intercourse) or use contraceptive measures, and agreement to refrain from donating sperm, as defined below: With female partners of childbearing potential, men must remain abstinent or use a condom plus an additional contraceptive method that together result in a failure rate of <1% per year during the treatment period and for 6 months after the last dose of bevacizumab. Men must refrain from donating sperm during this same period. With pregnant female partners, men must remain abstinent or use a condom during the treatment period and for 6 months after the last dose of bevacizumab to avoid exposing the embryo. Wirksamkeit • Gesamtüberleben (OS) • Progressionsfreies Überleben (PFS) • Überlebensrate nach 24 Monaten • Vollständige Ansprechrate (CRR) • Krankheitskontrollrate (DCR) • Zeit bis zur Verschlechterung der Leberfunktion, definiert als die Zeit von der Randomisierung bis zur Verschlechterung des CTCAE Grades im Vergleich mit Baseline und fortbestehend für ≥ 30 Tage für einen der folgenden Parameter: Aspartat-Aminotransferase (AST), Alanin-Aminotransferase (ALT), Gesamtbilirubin, Albumin und International Normalized Ratio (INR); gesondert für Patienten in Arm A und Arm B • Zeitdauer bis zur lokal nicht mehr behandelbaren Progression (TTuP), definiert als die Zeit von der Randomisierung bis zum Eintreten eines der folgenden Zustände: Progression, welche nach der letzten Staging-Untersuchung die gezielte Behandlung von mehr als drei bereits vorbehandelten Läsionen erfordern würde, Progression auf Grund von diffusem Tumorwachstum, Auftreten eines Gefäßeinbruchs, Auftreten von extrahepatischer Tumorausbreitung, Verschlechterung der Leberfunktion auf einen Child-Pugh Score von 8 oder höher als 8 • Zeitdauer bis zum Fortschreiten zum nächsten Tumorstadium, definiert als Zeit von Randomisierung bis zum Fortschreiten der Krankheit zu BCLC C • Dauer bis zur ersten TACE (nur in Arm A)
Ansprechen und Progression werden nach RECIST 1.1 durch den Prüfarzt bestimmt.
Lebensqualität • Lebensqualität bewertet mit EORTC QLQ-C30- und EORTC QLQ-HCC18-Fragenbögen
Sicherheit • Typ, Häufigkeit und Schwere von unerwünschten Ereignissen, Schwere bewertet gemäß NCI CTCAE v5.0
